Mining in adverse geological conditions-low cover. A Roof fall occurred overnight and was discovered at 5:46 am on pre-shift exam. Fall was investigated by section foreman, DMS, and MSHA

A unintentional roof fall occurred on the #11 belt at spad # 5935. The belt line was the only equipment involved in the incident. No injuries occurred. The fall measured 24' wide 28' long and 6' high.

Employee was climbing up the steps on the 980H loader. EE reached for the door handle, the door handle was slick from the rain causing hand to slip off the handle as the door opened. This caused ee to fall off the loader hurting back, shoulder, neck and arm.

A pillar fall occurred on the 003 MMU 60' in by SS#8673 in the #2 entry resulting in 2 MRS units being trapped. The fall occurred on pillar section in pillar line. Continuation of the pillar fall was the cause.

Received a CWP diagnosis for employee. He voluntarily quit on 9/4/2015.

Received CWP claim for employee. He last worked on 3/25/15 and was termed on 9/21/15.

Coal dust filed a state and federal Black Lung claim

Employee was lifting battery lid when he felt a sharp pain in his groin area and back.

Employee was positioning an A-Leg at the staging on the job site. After he staged the A-Leg he turned and tripped over dirt/rocks, hitting his left knee on a piece channel in the staging area.

Man trip employee pulled over to let another man trip go by and the employee had part of his right foot sticking out of the ride. His foot got caught on the rib resulting in multiple fractures.

On 11/20/14 a roof fall occurred on the 003 MMU, #5 entry, 1 break in by SS#6763 resulting in 2 MRS units being trapped. Fall occurred on pillar section in pillar line. #3 MRS equipment failure was the cause of the fall.

Assisting another employee move miner cable and waterline. As employee lifted the cable he felt pain in his mid to upper back.

Employee was bolting in #6 entry, putting up the last row of bolts in the face. The hole was drilled and the employee was putting the glue in the hole when a piece of rock fell glancing him on the back of the head resulting in a laceration.

Employee was placing crib blocks under a diesel engine that was being held by a forklift. When he bent over to put the crib block in place he struck his head on one of the forks resulting in a laceration requiring staples in his head.

Employee stated he was replacing the main jack on ATRS roof bolter FB#21. The ATRS was secured to mine roof by chains, the main jack was secured to drill by bottom pin. Employee started machine at operator's station which position on end of ATRS bar, the ATRS bar fell striking him on front of hard hat and right shoulder.

Employee was putting bottom roller in on #1 belt when roller came out of hanger causing employee to fall backwards hitting mantrip. Employee felt pain in his lower back on the left side and down his left leg.

Employee was on his knees next to the miner at the control panel. Mechanic was working inside the contact panel checking voltage from the master panel to the contact panel. The master control panel and contact panel was swung out. When employee began to rise from his knees he inadvertently laid his right hand on the swing out portion of the contact panel which was energized.

Bates Contractor was recovering water line and loading it on water line sled. While loading the water line he smashed his middle finger on his right hand between sled and water line.

On September 13, 2013 employee filed a hearing loss claim with the Kentucky Labor Cabinet, Department of Workers' Claims. As per the audiologist's report, he suffers from a 20% hearing loss. His last day worked is December 5, 2011. **There are no accidents associated with this claim.**

On August 17, 2013, EE picked up a belt roller, twisted around and felt a pop in his back.

On July 10, 2013, widow of employee filed a Black Lung with the Office of Workers Compensation, Division of Caol Mine Workers Compensatin. ***THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.

A piece of drill steel that was stuck in the top, fell out and struck employee on the left thumb, smashing his thumb, resulting in a contusion and cut.

Employee put his left leg between the bumper of the shuttle car and the bucket of the scoop resulting in a fracture to his tibia.

While helping on the roof bolter, the roof bolter operator accidentally lowered the drill pot on IEE's right foot resulting in a fractured big toe.

EE filed a claim with the U.S. Department of Labor, Office of Worker's Compensation, under the Black Lung Benefits Act on July 13, 2012. There is no accident associated with this claim.

The EE was building a brattice on the MMU. No 002 Section. The EE bent over to pick up a single concrete block and felt a burning pain in his lower left side.

A roof fall was discovered by fireboss inby Survey Station # 155 on mine slope, size of fall 6" W X 18' L X 6' H. Roof fall will be cleaned up, does not affect ventilation

When putting belt structure in, employee stepped on a rock under rock dust and said he felt pain in his right foot.

On 6/13/13 EE filed a Cumulative trauma claim with the KY Labor Cabinet. As per his medical report he has a combined impairment of 11%. 8% in his left knee and 3% in his right knee. There is no accident associated with this claim.Note on form EE retired 4/13/12

Employee was assisting in the changing of the cutter drums on the continuous miner. He was standing about 8' and slightly in the front of the drums as they were using a port-a-power to push the drums off. The metal broke and it flew off striking the employee on the left leg. He was treated by M.E.T. on the section and transported to Hazard A.R.H. hospital.

A roof fall was observed in the #4 entry of the 2A Bleeder Panel. The roof fall is located in the intersection at survey station #999. The roof fall is approx. 20' Long X 30' Wide X 8' High.

EE was helping move continuous miner cable out of the roadway and stepped in a dip and sprained his left ankle. The ee continued to work, but his foot continued to hurt. On 11/7/11 additional x-rays of the ee foot were taken. At this time is was determined that the ee has a hairline fracture to bone in his left foot. The ee has not missed any work due to this injury.

The employee was running a centerline in the # 1 heading when a small piece of draw rock fell from the roof hitting the employee on the left wrist. This resulted in a cut to the left wrist requiring (4) stitches.

A roof fall was discovered on the MMU. No. 002-0 section at approximately 7:25pm. The roof fall was reported to MSHA. The roof fall is located in the #4 entry at survey station #3231. The roof fall is approximately 20'feet wide X 20' feet long X 10' feet high. There were no injuries associated with this roof fall.

A Roof Fall was discovered on the MMU. No. 002-0 Section. The roof fall is located in the # 2 entry at survey station # 3256. The roof fall extends outby approximately 130' feet. The roof fall is approximately 20' wide X 130' long x 8' feet high.

The contract employee was drilling a hole in the #1 entry when the drill steel fell out of the hole and mashed the end of the contract employees little finger on the right hand. The injury resulted in 8 stitches in the employee's finger.

The employee was roof bolting in the #3 entry on the MMU. 002 section. The employee was down on his knees and started to stand up when something popped in his right knee.

While removing a pogo stick to pull down curtain a rock approx. 14 inches long and 5 inches wide was dislodged cutting the left hand below the thumb. Resulted in three (3) stitches.

Employee was operating Roof Bolter as he was pulling a metal strap off the bolting machine, he cut his arm just above the left wrist requiring (13) stitches.

EE's were lifting belt stand out of the man trip. EE felt pain in his lower back. EE went to the doctor and went back to work but he has had constant pain and just started to miss work on 11/21/10.

Employee was diagnosed with coal miners Pneumoconiosis (black lung)

Roof fall discovered on 2A Bleeder Panel at SS# 825 # 3 entry by Ronnie Maggard. Size of fall 40' long x 10' high. Area is timbered off. Fall does not affect ventilation or travel. Fall discovered at 9:05 AM. Fall will not be cleaned up.

Employee was cutting a zip tie that was holding a cable and struck his right hand against the top. He received a laceration that required stitches.

A roof fall was found at 1 South Mains 40 feet outby SS# 923, in the #5 entry. Size of the fall is 70' wide X 60' long X height unknown. Ventilation and travel is not affected and the fall will not be cleaned up.

An unintentional, non-injury roof fall was discovered at 03:25 pm one break inby SS 960 #5 entry. Size of fall measured 35' X 19' X 7' and does not effect ventilation or travel. The area has been timbered and dangered off and will not be cleaned up.

Carrying pieces of metal, tripped on rock, falling and cutting thumbs. Stitches in one hand, damaged tendon in other. Tendon repair 9/18/09. Admitted to hospital, discharged 9/19/09 with return to work on light duty.

Employee was setting bits in the miner cutter drum when he struck his left thumb with a hammer causing a laceration that required stitches.

Employee cut his right forearm on a band that had been cut while reaching for a roof strap in the supply hole. Stitches were required.

Employee tripped and fell, fracturing his right arm.

A non-injury roof fall was discovered at 12:10am on 07/15/09. Size of the fall is 75' X 8' X 19'. Fall will not be cleaned up and ventilation is not affected or travel. All areas around the fall have been cribbed off.

Employee was unloading head drive boom when the boom flipped upward mashing his finger against the top.

Rock fell from the top and struck the employee on his right foot while he was bolting top. Resulted in a cut to his foot requiring stitches.

Employee moved a pogo stick on corner in #5 entry when a piece of drawrock fell and cut his right wrist.

Loading cable bolts onto scoop, when one bounced up and hit employee on forehead. Resulted in a cut that required 3 stitches.

A non-injury roof fall occurred on the 001 MMU in the #7 entry inby SS#410. Size of the fall measured 10' - 12' high x 19' wide x 30' long. No equipment involved. Fall will be cleaned up and additional supports added. Ventilation and travel will not be impeded.

Employee was struck by a piece of drawrock on left arm and elbow. He continued working until 1/16/09 when the doctor removed him from work.

Employee was installing a 4 inch water line, when he fell against the water line resulting in a fracture of three Ribs on the left side.

Employee was repairing miner when the panel board cover slipped and caused a laceration to the little finger, left hand.

Employee's fingers were pinched, in a spring loaded concrete chute, while cleaning.
