IE was setting up to start loading with the CM. IE had adjusted IE's tubing for ventilation & as IE walked outby towards the end of the CM a rock hinged off of the last row of roof bolts & knocked the tubing down. The tubing knocked IE down & the rock flipped over onto IE's left ankle which caused the injury.

IE was helping on the longwall recovery face. EE was unhooking the shield to walking shield hoses. EE was standing between the shield & the crib when a 2'X4"X2"-4" thick rock fell on EE. EE had 1 small laceration on forehead that required stitches & another small laceration on the back of the head. No broken bones were found by the Dr.

Injured was changing bits on the longwall shearer when a draw rock fell and struck the injured in the hard hat. The injured struck against the shearer and suffered a laceration to the face requiring stitches and a fracture to the Maxilla.

A fireboss/pumper was allegedly struck in the head by a falling rock in the walkway leading to 239A seal.

Employee was scooping for belt move when a loose rock fell and hit his right forearm requiring 8 stitches.

Employee was standing in front of brattice chipping a cement block with an axe when a rock fell from between roof bolts striking employee in back of head, neck & shoulder.

injured had backed the miner our of the face and was cutting down some loose rock. a rock fell from between the roof bolt rows. rock struck injured on his right forearm and hand requiring in 4 stitches.

Employee was handling bullhose while mucking. Rock fell down shaft and hit right little finger. Contusion of finger.

Injured employee was changing bits in tailgate shearer drum. A rock fell from the roof line at the face and struck employee on his index finger & forefinger of his left hand, resulting in a laceration to both fingers.

THE INJURED WAS CHECKING BELT STRUCTURE IN A BELT CHANNEL. A ROCK 30" BY 23" BY 7" FELL FROM BETWEEN THE ROWS OF ROOF BOLTS AND STRUCK INJURED ON HIS SHOULDER AND HEAD.
