EMPLOYEE WAS BENDING OVER BESIDE A ROOF BOLTER, BENDING ROOF BOLTS, WHEN A PIECE OF ROCK FELL OUT BETWEEN ROOF BOLTS STRIKING THE BACK OF HIS HEAD ON THE LEFT SIDE.
HE WAS SETTING ROOF JACKS ALONG BELTLINE IN 11' ROOF. WEATHERED SHALE FELL STRIKING VICTIM ON RIGHT FOREARM. SIZE OF SHALE PIECE ABOUT 12" WIDE, 1" THICK, CAUSING CUT ON ARM.
