A ROOF FALL OCCURRED IN THE SIC RIGHT XCUT ON THE NO 2 SECTION RESULTING IN THE DEATH OF EMPLOYEE. THE FALL MEASURE 18' LONG, 15' WIDE AND 12" THICK.
Employee states that while jacking up the motor and flatcar, the motor slipped off the jack pinching his (R) ring finger between the jack bar and mine floor.
Employee was riding on the passenger side of a rubber tired man trip, when they rode over a 2" discharge line causing the end to flip up and strike him in the face. It caused a laceration to his lip.
Employee states that while making a belt splice he was using channel locks to pull on the belt, he felt pain in his right shoulder. After a couple of days, the pain and discomfort continued and he went to the Dr. and he was taken off work.
Employee was getting ready to operate a battery powered mantrip. As he was setting down he slipped, causing his elbow to strike a can of WD-40.
As employee was exiting the bonnie car he was crossing at the front end of the car, he dropped his tools. As he turned to pick them up, he tripped on the bonnie car rope falling, landing on his left elbow.
Employee states that he was installing a new bolt next to a bent bolt. His hand got caught between the pizza pan and plate of the new bolt and the bent one.
Employee was loading in the #5 entry on the left side of the entry when a piece of rock fell from the top of the rib and struck him on the left leg.
An unintentional roof fall occurred on the 002 MMU. The fall occurred in the #5 face, approx. 100 ft. inby spad #14204. It measured approx. 40' x 20' x 8' to 9' thick.
Employee states that while bolting top, he went to put the drill steel in the pot when a piece of draw rock fell from the top and struck him in the left forearm.
Employee was walking across the section when he slipped in the #4 entry twisting his (R) knee. The area was damp but no tripping hazards were present. He reported this incident to his supervisor after his knee began to swell. He completed this shift and continued to work until 06/05/2013 when he began to miss work.
Employee states that while adjusting the leveling chain on the pot he had placed his (R) hand on the boom. His (R) middle finger got caught between short arm and pot arm. This caused a laceration to his (R) middle finger. Employee was wearing leather gloves at the time of the incident.
Employee was shoveling belt and was standing in mud. As he turned and twisted his (L) knee popped. He continued to work and finished his shift. He went to the Dr. 5/16/2013 when he was given days off.
Employee was using a bucket to scoop up mud and water. As he turned to pass off the bucket, he felt something in his lower back pop and had pain go down his right leg. This incident remains under investigation.
Employee states that he was hanging the miner cable and as he was lifting the cable he felt a pop in his left shoulder and neck. This report was filed by the contractor P177 on 05/21/2013. It was determined thru a Part 50 audit the operator, 4608763 was required to also report this incident. Citation #7166777
Employee states that while starting down the steps on the 3a overcast, the steps were loose causing me to loose my balance and fall. This incident remains under investigation.
Employee was making a splice on the CM when the utility knife slipped and struck him in the right hand causing a laceration.
Employee was bolting #2 right, as he was placing a bolt in the hole he felt pain in his right shoulder.
Employee was working around the C1 belt head, as he bent over to pick up a rock he felt a pain in his lower back. Employee continued to work until 02/01/2013 when he went to the Doctor. He was then placed off work.
Employe was lifting on a piece of metal on a tail piece when he felt 3 places in his groin area start to burn. Employee continued to work until 03-25-2013 when he had surgery to repair the hernia, he then began to miss work.
EE went up to the #2 C.M. to ask the operator if it was running properly. EE walked back towards the operators right, a rock measuring 6'4" L.X 8'4" W. came down 9in a swinging motion) striking EE on his right hip.
Employee states that while dusting the track entry on 1 belt he was walking thru the overcast when he raised up and struck his head on a metal beam. Employee did not report this incident until 10/28/12 to a man bar of mgt at home. A incident report was filled out on the 27 when mgt returned to work.
Employee states that while changing the tram motor on the continuous miner, a piece of rock fell from the top and struck him on the back of the right leg. He was diagnosed on the 9th of Aug with a Fracture of the right fibula. On August 14th mgt. became aware of the diagnosis. Employee has not missed any work from this injury.
The 003 section was pillaring in the #5 entry when the roof set down on the #2 JH Fletcher Mobile Roof Support (MRS) causing it to get stuck. We spot bolted with 8ft. bolts from the outby intersection to in front of the MRS, and set timbers before removal.
An unplanned roof fall occurred in the #7 entry of 4th Right 1-B southwest mains return. The fall has since been cribbed off.
At the end of the shift on 2/18/12, employee states the ride to outside derailed, he helped lift the ride back onto the rail and strained his back. He continued working until 2/22/12 and went to the doctor....He was off 2 days and returned to work 2/25/12.
Employee was pulling rail out of mud when he twisted his right knee.
1/13/12 Coal River Safety reported to me an alleged injury occurring 1/3/12. EE was terminated 1/6/12 when EE was notified he did not report an injury. EE states to C.R.on 1/13/12 that while unloading top sections of the belt structure, he turned and slipped twisting his left knee. He finished his shift and informed his section supervisor of his incident. No reports made till 1/13.
While jacking up a derailed rail car, the employee was using a lifting jack, the jack slipped out and the rail car fell on both feet.
While performing a regular rope inspection using non-destructive testing, the hoist rope was found to be out of compliance. The rope was removed from service and replaced with a new rope.
A unplanned roof fall occurred in the #1 return, entry 3, 200 ft. from the D-Section face. The size of the fall is 60 ft. x 18 ft. x 6 to 8 ft thick (thru intersection). The fall was breakered off with 3 heinzman jacks in each entry leading to the fall.
Employee states that while working on the conveyor chain he was cutting a metal snap ring off with a hammer and chisel, the ring broke and a piece went into his chin.
Employee states that while setting the ATRS, a piece of draw rock fell and struck him on the neck, shoulder and lower back.
EE states that while changing the pot on the roof bolter, a piece of rock fell from the top striking him on his upper back.
B-Section was pillaring in the #6 entry when the MRS became stuck and required assistance to become free.
Employee states that while bolting top in the #8 entry a piece of draw rock fell from the top striking him on the arm causing an laceration.
An unintentional fall of roof occurred in the #5 track entry, at or near spad #5502 on the 2nd southwest mains near break 27. The fall measured 30' long x 19' wide x 6' to 10' thick.
An unintentional fall of roof occurred in the #6 heading, at or near spad #5518 on the 2nd southwest mains. The fall measured 25' long x 18' wide x 6' to 8' thick.
During maintenance on the hoist rope a kink was found. The Hoist car was immediately taken out of service. Reference # 03-160809
Employee states that while operating the center car, he hit a hole in the road and it jarred him. He felt a pain in his lower back.
(Report received from Coal River on 12/16/10) - Employee stated that while hanging curtain in the #1 entry on C-section, a piece of draw rock 15" X 12" X 1" thick fell and struck him on the right shoulder.
Employee states that while removing old rope hangers off his lil mac, he cut his (L) hand with a utility knife.
Employee states that he was building a stopping when a cinder block fell and mashed his (R) ring finger.
An unintentional fall of roof occurred in the #8 heading, at or near spad #9739 on the 2nd south mains in the intersection. The fall measured 30' long x 50' wide x 6' to 8' thick.
Employee States that he was picking up a tram motor for a little mac man trip, he felt a pain in his lower back.
An unintentional roof fall was found 4,300 from the 003 section in the #6 entry intersection on the southwest mains. The fall was cribbed off. the size of the fall is 40' long x 20' wide x 6' to 8' thick. It is located at or near spad # 9634
An unintentional roof fall was found in the #3 entry netural of the 2nd south west mains, 4000' from the 003 working section. Fall was breakered off with cribs. Size of fall, 20'X30'WX6-8' thick. Spad #9710.
An unintentional roof fall had occurred in the intake (primary escapeway) on the 4th southwest mains, #7 entry, 3600' from the 001 section, spad # 5458 . The area was dangered off, and breaker timbers were installed. The stopping line was moved over into the #6 intake entry and the lifeline was re-routed to allow for a secure escapeway.
An unintentional roof fall was found in the #4 belt entry on the 1st South East Mains # 2 belt line 8,100' from the 004 section. the fall was breakered off with timbers and Heitzman jacks. Fall size was 20' L x 50'W x 6' to 8' thick.
While working recovery in old "B" section, they were recovering 8" pipe. The pipe got caught on something, broke and then struck employee on the left pinky finger.
Employee states that as he was loading a P-80 pump on the front of the bonnie car, as it was being laid down, his (R) middle and ring fingers got caught between the battery box and pump.
Employee States that while he was bending over to pick-up the miner cable to move it out of the roadway, the rib rolled off and struck him on the (R) leg and knocked him to the ground.
EE states that as he was tramming a rail ride through the air lock doors, one of them started closing on him. The rail ride caught one of the doors causing a cinder block to be displaced and fall. The block struck his hand causing an laceration.
Employee States that he was changing the foot switch on the little Mac man-trip, he received a laceration to the top of his (L) hand.
Unintentional roof fall was found in the 1st south east main return air course. 12,000' from the B&C sections. Heizman Jacks were already set in that area. The fall occurred behind the jacks. Travel way was not blocked. Fall measured 20'l X 5' W X 3-4' thick.
An unintentional roof fall was found in the 003 section return air course of the 2nd south west mains. Travelway is not blocked due to multiple entries. The fall was breakered off with timbers. Size of fall 20' x 20' x 6' thick.
Employee states that while bolting top, the drill steel got stuck in the top. As he was attempting to remove the stuck steel, the wrench contacted his hand.
Weathered sandy shale roof separated above bolt anchorage.
An un-intentional roof fall was discovered in the primary escape way of the 2nd southwest mains. Travel way was blocked but due to multiple entries the airway was not. Stopping line was set over to establish new travel way. Fall was breakered off with timbers. Fall measured 20' L X 18' W X 6' thick. Fall consisted of laminated shale.
EE states that while he was hanging mine phone at the old "C" section power center, as he was attempting to climb up on the power center his left knee hyper extended. EE finished his shift. Accident was not reported to mgmt. until 12/17/09. EE did not notify mgmt. of medical findings until 12/31/09.
An unintentional roof fall was discovered in the return course in the #1 entry of the 3rd SW mains of the 002 section. Traveling is not blocked due to multiple entries. The fall was breakered off with cribs & timers. Size of fall was approx 70'l x 18'w x 6'-7' thick.
An unintentional roof fall was discovered I the intake air course outby 4200' of the 001 section in the #7 entry. Travel way is not blocked due to multiple entries. The fall was breakred off with cribs. Size of fall appr. 50'long x 19' wide x 4'7' thick, the fall consisted of moist tack sandy shale.
Employee states that he exited his electrician's ride to go to the dump end of the # 12 shuttle car. As he was coming around the end of the car, he stepped into a hole and jarred his lower back.
Employee states he was bolting the #30 wall entry, a bolt was not fully up. He attempted to bend the bolt over by using the drill pot on the roof bolter. As he was bending the bolt, his right index finger was caught between the moving parts on the canopy.
Employee states that as he was unloading supplies from the bucket of a scoop he slipped and fell in the bucket striking his (L) lower back on the ram of the bucket.
EE states while installing a head shaft on the #2 shuttle car, the head shaft inadvertently came down and caused his thumb to be caught between the frame of the shuttle car and the head shaft.
Employee states that while bolting top in the #3 entry some small draw rock fell in front of A.T.R.S. He withdrew from the entry and slipped/tripped on a rock beside the roofbolter causing him to fall. As he fell he landed on a rock causing his battery to be pushed into his ribs.
EE states he was trying to cut a piece of curtain out of the conveyor chain on his shuttle car when the utility knife he was using caused a laceration to his right hand.
An unintentional roof fall was discovered in the return air course on the 3rd south west mains of 002 section return. Travelway is not blocked due to multiple entries. The fall was breakered off with cribs & Heintzman jacks. Size of fall 80' L x 19' W x 6'8' Thick.
EE states that he was pulling shuttle car cable. As he was pulling it backwards, his feet slipped out from under him, causing his buttox to strike the ground.
EE was cleaning neutral entry beside B-1 blet, rib rolled off and struck EE. Rib struck EE in the neck and shoulder while in the deck of the scoop. EE was referred to a specialist, appointment 5/27/09.
Employee states that he had bolts made up and staged along the rib. The bolts slid off the rib and the corner of the pizza pan struck his left forearm causing a 1" laceration.
Employee was scooping roadway in wall # 10. When scooping outby corner, the corner broke off and came into scoop deck and smashed/cut employee's left thumb.
Employee had put up outside bolt & was swinging in to put wide bolt up. Stab jack struck bolt lying on ground and spun it around and the pizza pan struck his nose, cutting it.
Loading high line sled in scoop bucket, pushed ram jack out and pinched hand between sled bucket.
Injured was reaching to get a bottom roller, while on move crew, was pulling back and a top roller fell, striking him in the right side of back. He finished the shift, took the next day off, returned to work on Friday then went to the dr after work on Friday.
Dropping miner cable that was hung across entry and pulled back muscle.
Employee had hung a backup curtain in #2 entry and was walking back to feeder when a rock fell from roof, striking him in right forearm causing a laceration.
EE was plastering stoppings when he reached above his head and plaster splashed in his eye.
Wet weathered shale seperated above roof bolt anchorage allowing roof to fall.
Employee was helping recover track rails when one of the track rails slipped in the scoop bucket and caused the other end of the rail to hit him in the right ankle.
Employee was helping to load high voltage cable into a sled. His foot slipped on a rock, causing his foot to slide under the sled. The sled was being pulled by a 35C Fairchild scoop.
EE was walking through mud and twisted ankle.
As EE pulled the curtain down in the #7 entry, a loose piece of coal rib came down with the curtain and struck him on the right ear.
EE was operating a continuous miner on 603 section. He was backing the miner out of a right hand lift when a rib rolled off and struck him in the back, knocking him to the ground. The rock was measured to be 8' x 3' x 19".
Foreman found roof fall while making weekly exams. Fall consisted of weathered stack sand stone shale mix. Roof control plan has been changed to use longer roof bolts when these conditions exist.
EE was squaring the end of the belt for a splice. He was trimming off a piece of rubber, when his knife slipped, cutting his right leg above the knee. *Note: We were 1st notified of this incident by mail on 8/5/08 form Workers Comp. Then I received a report from the mine on 8/6/08.
Employee was holding bulk dust bag and was cutting with a knife a cut his right wrist.
EE was operating a roof bolter on C-section or 003 section. While drilling a hole in the roof, a piece of rock approx 6 X 10 X 1" thick fell from the roof and struck EE in the nose. The rock had sharp edges and cut the tip of his nose off. Given 1st aide and transported to ER.
Employee was making up bolts 2 rosws back from unsupported top when a piece of draw rock measuring approx. 12" wide by 24" long and 3" thick fell from between bolts and hit him in the left shoulder leaving a cut about 3 inches long.
An unintentional roof fall was discovered in front of the #5 seal, At #4 seal, this fall is 60' over from spad #1780, time of fall is unknown, but fell sometime between 4/22/08 and 4/29/08. THe size of fall is 6"thick x 20Long x 20 Wide. the roof in this area is 3'slate with laminated sandstone above slate.
The utility person on the section went to the miner to relieve the miner operator for a lunch break, when the right outby rib rolled out, which caught the utility person in the back of the legs. This caused him to fall to the mine floor. Rib roll measured 3' X 7' X up to 7" thick.
EE was in W28 face installing roof bolt, after tightening bolt, let pot down on his right knee.
While bolting in #6 entry, a piece of draw rock fell out and struck him on the left ankle. The piece of rock was about 24" X 36" X 1"-4" thick. The rock fell ut between roof bolts. EE returned to work for 4 days and stated he was advised not to work, due to possible fx.
Busting rock along walkway with sledge hammer, piece of rock hit him in the left eye.
Making up bolt went to throw ahead of bolter. Bolt caught on a bolt hanging over side of bolter pushing pan into arm.
Tossing a crib block under scoop to scotch up to change tire, smashing right pinky finger between crib block and frame of scoop.
Opening a set of airlock doors when manbus was going thru them. Victim tried to get in the manbus while manbus was in motion resulting in being pinched between the door and manbus.
An employee was conducting a daily inspection of the slope hoist & found the hoist was inoperative. A co-worker happened by at about the same time. This was on a Sunday with nobody working underground. Testing showed a field exciter to be open. The motor was down & had to be taken off & sent to a shop for repairs. The motor was installed on 12-3-07 & hoist was working properly.
Installing roof bolt, after t bar was set, a piece of rock fell from between t bar and last row of bolts. Rock size was 48" x 16" x 6". He was hit from front and knocked backward. He was struck on shoulder, chest and right leg. He returned back to work the next day. 1/21/08 he called and stated his dr advised him not to work.
EE was operating the Mac 8 personal carrier when he ran over something, causing the steering wheel to spin out of his hand, hitting him on the left hand/wrist.
Employee was moving miner cable. He picked up cable, felt pain in lower back and down left leg.
On 8/23/07 @ 3:40PM EE alleges that an injury occurred Monday 8/20/07 but did not report it to his supervisor. He states that he smashed his hand with a crib block but didn't think he was hurt until after work. He said that his hand started to swell and he went to Logan Regional ER about 6 o'clock & was seen about 8 or 9 o'clock. I contacted his work site & there were no witness.
An unintentional roof fall was discovered in the return aircourse of 001 section, at approximately 10:15 am on 6-19-07. The aircourse was last traveled on 6-12-07. The fall was approximately 50' long x 19' wide x 7'-8' thick. The fall consisted of weathered laminated sandy shale.
A roof fall was discovered on top of the overcast. The fall was approximately 4' thick x 12' wide x 18' long. The fall damaged the overcast and was allowing return air from 003 section to mix with intake air going to 001 section. The fall was 1 x-cut over from spad #382. Both sections were idled immediately upon discovering this accident.
Employee was walking from left miner to right miner & tripped & fell over some loose coal. He hurt his right elbow & twisted right ankle, pinky finger & left wrist. He returned to work without seeking medical attention. He called on 26th of March & said he couldn't come to work because the doctor said he had a sprained ankle.
EE was eating a sandwich while sitting on a 3 wheeler and he told another EE he had already taken 1 nitro glycerin pill and was going to take another because he felt short of breath. First-aid was then administered, aferward EE was placed in EMS care onto the ER.
Employee was riding in brake car and his leg slipped out striking ground or object. EE came back to work on light duty for last 10 days, but EE called on 1/25/07 and said he was having complications with his foot Dr instructed him not to come back to work until further notice.
Employee was helping hang bolter cable the reel on the bolter was full of cables slack and became fouled the bolts was traming at this sametime as he were hanging the cable the cable got pulled down and struck ee in back op neck and knock-him to the ground.
Changing chain on feeder. Pinched finger tip between chain and cog. Right hand middle finger.
Hoist lost hydraulic due to pressure valve not working due to contaminates in oil. Valve was cleaned and system scheduled to be flushed out and oil changed.
Brakes on hoist drum would not release (main pump was changed along with 2 relief valves). All checks were made after change-outs and hoist is now fully operational.
Employee had an apparent heart attack at #34 crosscut at the # 3 track switch. Victim was riding (passenger). The mantrip operator stopped to throw the switch, looked back and noticed him slumped over in mantrip.
An unintentional roof fall occurred at the #3 set of seals in a crosscut between #15 and #16 seal to the left of spad #2007. The fall does not hinder the examination of the seals or block the regular travelway. The fall measured approximately 2' to 4' thick x 19' wide x 25' long.
Tramming miner hit hole miner hit him and mashed against rib.
An unintentional roof fall occurred on the #3 conveyor beltline at 9:30 AM. The fall is one x-cut inby spad #2342. The fall is approximately 8'-10' thick x 30' long x 35' wide.
An unintentional roof fall was discovered in the return air course about 3,000' outby the MMU 001 section. There were 2 return entries in this area. Cribs were built inby & outby the fall. The fall measured 18'-19' wide x 20'-25' long x 2'-6' thick. This area was bolted with 6' tension rebar bolts.
An unintentional roof fall was discovered on 9-10-06 at approx. 9:00 PM while pre-shift exam was being conducted. The fall occurred sometime during the weekend, time unknown. It measured approx. 20' wide x 20' long x 8' thick in the #2 entry, last open x-cut. The fall appears to be a result of weak overlying strata that has deteriorated.
Employee bolting 6 right rock came loose from rib and hit him on the leg.
An unintentional roof fall was discovered behind the #11 scal that was being constructed. The fall occurred over the weekend while no one was present. Discovered on 6/19/06 by foreman at spad #5050. The fall was estimated to be 10' thick X 20'wide X 40' long. Slick and slide rock were present in the fall. MSHA inspector investigated the fall while examining the sealed area.
The PC board on the DCS 500 drive went down on slope car drive. We ordered the part from Frontier-Kemper (who installed it), and it arrived at 2:45 AM on 4/27/06. Repairs were finished at 3:30 AM and a spare was bought also to prevent this problem again.
Recovering rail, dropped a rail on right hand.
While setting timbers said he felt something pull in his groin on right side affecting right leg.
FALL WAS FOUND IN RETURN WHILE CHECKING SEALS, 2 BREAK OUTBY SPAD 1835. FALL CONSISTED OF WEATHERED SLATE & SANDSTONE. AREA HAD BEEN BOLTED WITH 6'DOUBLE LOCK BOLTS. FALL WAS 30'LONG X 18'WIDE X 6-7'HIGH. CRIBBED AROUND FALL SO NO ONE COULD ENTER THIS AREA.
CAUGHT LIFT INDEX FINGER BETWEEN ROOF BOLT AND PLATE AND TOP.

RIB ROLLED OF AND HIT RIGHT ARM AND LEG AND PUSHED HIM INTO MRS CUTTING LEFT SIDE OF HEAD.
EE WAS LIFTING SPEED REDUCER INTO PLACE WITH CHAIN RATCHET WHEN LIFT EYE CAME OUT OF REDUCER AND CAUGHT EE'S LEFT HAND IN CHAIN.
SET BOLT IN POT. WHEN PUTTING UP BOLT, GOT GLOVE CAUGHT ON ROTATING BOLT.
Stepping off Mac 8 and twisted ankle in mud.
Fell of of in-line tail piece while crawling across. Bruised left side & back between waist & shoulder & middle of back.
While making splice on miner cable knife slipped and cut right leg above knee.

While pillaring in #1 entry block #55. 1415 joy continuous miner got stuck. while getting crib block and chain to pull miner out top fell in on miner and blocking two mrs's.

WHILE DOING WEEKLY AIRWAY A FALL WAS FOUND AT #4 SET SEALS IN FRONT OF SEAL #5. THE FALL CONSISTED OF LAMINATED SANDSTONE AND WAS BOLTED WITH 4' GLUE BOLTS. THE FALL HAS BEEN CRIBBED OFF FROM ALL SIDES. FALL WAS 5' HIGH, 18' - 20' WIDE & 50' LONG.
STARTING ROOF BOLTER FROM OUTSIDE DECK SAID INCH TRAM LEVER WAS STUCK IN WHEN BOLTER STARTED. BOLTER STARTED TRAMMING PUSHING HIM INTO RIB.
BELT CHUTE GOBBED OUT WHILE PRYING SLATE LOOSE WITH SLATE BAR, ROCK BROKE LOOSE & PINNED RING FINGER ON LEFT HAND AGAINS ROCK BOX
A PIECE OF DRAW ROCK FELL FROM TOP HITTING ROOF BOLTER OPERATOR IN BACK
WHILE TRAVELING RETURN AIR COURSE OFF OLD 002 FOUND A FALL 20 FT BY 20 FT X 6FT THICK IN AN INTERSECTION 1 BREAK INBY SPAD 2795. FALL CONSISTS OF WEATHERED SHALE GOING SLATE. FALL HAS BEEN TIMBERED OFF.

EE WAS STANDING ON SLICK ROCK. HIS FOOT SLIPPED AND WENT UNDER SCOOP BUCKET.
EE WAS LOADING BLOCK & TRIPPED OVER BUSTED BLOCK AND FELL LANDING ON RIGHT SHOULDER.
EE WAS STAND BESIDE OF ROOF BOLTER TALKING WITH THE OPERATOR WHEN A PIECE OF SHALE ROCK APP. 18 X 18 INCH AND 6 TO 7 INCHES THICK FELL BETWEEN RIB AND OUTSIDE ROOF BOLT STRIKING EE ON LEFT SHOULDER AND LEG.
EE WAS ROOF BOLTING IN XR XCUT WHEN A PIECE OF LOOSE ROCK FELL ACROSS T BAR ON ROOF BOLTER APP. 2FT WIDE 1 FT LONG AND 2 IN. THICK. ROCK BROKE THEN HIT THE PERSONAL CANOPY OVER THE OPERATOR TURNING UNDER CANOPY STRIKING OPERATOR KNOCKING HIM DOWN.
EE WAS PLASTERING STOPPING AND GOT SOME OF THE PLASTER IN HIS RIGHT EYE.
EMPLOYEE WAS BENDING OVER BESIDE A ROOF BOLTER, BENDING ROOF BOLTS, WHEN A PIECE OF ROCK FELL OUT BETWEEN ROOF BOLTS STRIKING THE BACK OF HIS HEAD ON THE LEFT SIDE.
OPERATOR WAS PULLING DRILL STEEL OUT OF ROOF WHEN HE LET THE DRILLPOT DOWN ON HIS RIGHT FOOT.
ROOF BOLT OPERATOR WAS BOLTING IN 5R XCUT. OPERATOR WAS ON HIS KNEES. WHEN HE ROSE TO CHANGE THE DRILL STEEL, HE TWISTED HIS KNEE AND FELT SHARP PAIN.
EE WAS LOADING SUPPLIES ON ROOFBOLTER, WHEN HE TWISTED BACK AND FELT SHARP PAIN, CONTINUED TO WORK UNTIL 3-2-04, WENT TO DR 3-5-04 BECAUSE PAIN HAD CONTINUED SINCE 2-4-04.

WHILE MAKING HIS WEEKLY EXAM, THE FIRE BOSS FOUND A ROOF FALL IN THE IN-ACTIVE 2ND RIGHT PANEL OFF OF THE #2 SECTION 1 BREAK INBY SPAD #1585. IT WAS APPROX 40 FT LONG X 19 FT WIDE X 5 FT THICK . IT DID NOT IMPEADE VENT OR TRAVEL.
A ROOF FALL OCCURRED ON THE TRACK ENTRY 1,225 FEET OUTBY #2 SECTION, 1 BREAK OUTBY SPAD #1571. IT WAS APPROX 20 FT LONG X 20 FT WIDE X 5 FT THICK.
A FALL WAS DISCOVERED IN AN UNBOLTED INTERSECTION LAST OPEN BETWEEN #7 & #8 ENTRY ON #3 SECTION ~100' INBY SPAD #1946. FALL ~19' WIDE X 32' LONG& RANGED FROM 0" TO 7 - 8 FT THICK.
EMPLOYEE WAS TRAMMING BOLTER FROM #4 TO #5 ENTRY WHEN THE BOLTER SLID INTO RIB & CAUGHT INJURED EMPLOYEE BETWEEN RIB & BOLTER. CONDITIONS, WET & MUD & UNEVEN BOTTOM.
EMPLOYEE WAS WORKING ON THE BATTERIES OF A 488 SCOOP, THE BATTERY BELL GROUNDED AND SPARKED AND HURT HIS RIGHT HAND.
EE WAS ON THE #11 SHUTTLE CAR, DUMPING HIS LOAD OF COAL ON THE FEEDER, A ROCK ROLLED OFF THE RIB & MASHED RING & LITTLE FINGER ON HIS LEFT HAND. 4 STITCHES.
EMPLOYEE WAS BENDING A ROOF BOLT TO INSERT IN THE ROOF, THE BOLT SPRANG BACK, HURTING HIS RIGHT THUMB AND WRIST. * 1ST LOST WORK DAY 7/20/01.
AT APPROX. 3:00 A.M. A ROOF FALL OCCURRED IN THE #2 ENTRY, LAST OPEN X-CUT. THE FALL MEASURED APPROX. 5 FT THICK X 19 FT WIDE X 40 FT WIDE.
EE WAS FLIPING OVER A ROCK INT HE WALKWAY OF THE TRACK ON #2 SECTION, HE SLIPPED ON A SMALL LOOSE ROCK AND FELL ACROSS THE TRACK, LANDING ON HIS RIGHT SHOULDER AND LOWER BACK.
EE WAS HELPING TO BUILD A STOPPING ON THE NO. 1 SECTION. WHILE REACHING FOR A CINDER BLOCK, HE RAKED HIS LOWER ARM OVER THE METAL BAND THAT HAD HELD THE BLOCK IN PLACE CUTTING HIS ARM. 8 STITC HES.
EE HAD PUT OIL IN HIS SCOOP. WHILE WALKING BACK TO GET INTO THE OPERATOR DECK SOME SMALL PIECES OF ROCK FELL FROM BETWEEN THE BOLTS AND HIT HIM ON THE TOP OF HIS SHOULDERS AND UPPER BACK.
EMPLOYEE WAS LIFTING A RUBRAIL FOR A CONT. MINER AND FELT PAIN IN HIS LOWER BACK.
AT APPROX 8:45 PM A ROOF FALL OCCURRED IN THE #4ENTRY ON #2 SECTION. THE FALL OCCURRED FROM THE FACE (UNBOLTED 20 FT) TO OUTBY APPROX 40 FT TOWARDS THE INTERSECTION, PULLING AT LEAST 3 ROWS OF BOLTS. NO INJURIES OR EQUIPMENT WAS INVOLVED.
EE WAS HELPING THE MINER OPERATOR CHANGE MINER BITS, A SMALL SLIVER OF METAL CAME OFF OF THE MINOR OPERATORS HAMMER AND IMBEDDED IN EE'S RIGHT CHECK OF HIS FACE.
A ROOF FALL OCCURRED ON THE NO. 2 SECTION AT APPROX 7:30 PM BETWEEN THE NO 5 AND #6 ENTRY. THE FALL WAS APPROX 80 FT LONG X 19 FT WIDE AND RANGED FROM 2 FT TO 6 FT THICK IN THE MIDDLE.
