Employee stated he was using a slate bar to scale the mine roof on the #4 section in the #3 entry when a piece of rock fell and struck him on top of the head (hard hat). This resulted in a C6-7 disc herniation. This incident became reportable on 3/17/2015 when medical evidence supporting the alleged incident was received. The employee began missing work on 2/24/15.

Employee stated that while attempting to install a roof bolt into the mine roof, a piece of rock fell from the top and struck him on the right foot. This resulted in a fracture of the 2nd toe.

Employee stated that he was working along the 3-B belt when a piece of rock fell from the top and struck him on the back of the neck. The rock was approximately 22" wide and 0" to 1" thick. This resulted in an abrasion and one lost work day. This incident became reportable on 5/22/14.

A rock fell from the mine roof near the face of the #5 entry approximately 100 feet inby spad #13280 on the #3 Unit, striking the employee on the head and neck.

This report is being filed under protest and solely to abate citation 9005626. According to MSHA, employee incurred a reportable injury when he was struck on the back by a piece of roof rock. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee was operating the RBM in the 4L Crosscut. As he was placing the glue and bolt into the hole, a piece of rock fell from the top, struck the canopy of the RBM, and then struck him on top of the head. Medical received on 5/21/13 diagnosed the employee with a cervical strain.

Employee was operating the roof bolt machine on the #3 Section in the #1 entry. As he placed the drill steel against the top to drill a hole, a piece of rock fell from the mine roof, struck the drill pot, and then struck him on the left foot. This resulted in a laceration to the top of the left foot that required seven (7) sutures to repair.

Employee was installing roof bolts in the #4 entry on the #1 Section when a piece of rock approximately 31" x 24" and 1/4" to 4" thick fell from the top and struck him on the right shoulder. This resulted in a fracture to the right 5th rib.

EE was hanging the roof bolter cable when he finished he walked along the side of the bolter to check his curtain. Rock fell in front of and over the ARTS of the bolt machine pinning ee to the ground and landing on his right foot.

The employee stated he was wiring up P-40 pump starter in #9 entry a piece of coal with layered rock struck him in the back of neck. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee was installing his third bolt underneath a brow and a piece of rock broke loose falling and sliding underneath the canopy striking him in the left leg and lower back. The rock measured 90" x 27" x 16". The incident resulted in a Fix to the pelvis.

The employee hanging reflectors with hand on bolter canopy struck by 2" x 8" x 1" rock. FA on fingers. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee was bolting in the 4 left cross cut face (#411) machine on the off side when he was struck on the left hand by a piece of roof rock. The rock measured 2'foot x 3" inches. The event resulted in a Fx.

The employee was struck by draw rock when bolting face of 5 x-c left. The incident resulted in lost time injures.

Rock fell and hit face and right arm stitches on right side of face

While bolting top piece of coal rock fell and bounced off rt rib and back.

The employee was preparing old #5 belt take-up to be moved and relocated. He was bending over to pick up crib block when a piece of roof fell striking his neck.

Bolting top in 4 left and rock fell and hit him on the left shoulder.

The employee was struck by a piece of rock falling from between roof bolts while operating continuous miner on 6/19/10 and continued to work. Medical received September 14, 2010 shows fx to c-6. The employee is now missing work.

The employee was installing and placing roof bolt into the drilled hole in roof and rock fell striking arm. The event resulted in a medical reportable injury (cut/sutures).

The employee stated that as he was walking along D-belt, he was struck in the head by a piece of rock measuring 2'x5"x1" inch. The incident resulted in lost time.

The employee helping to install belt in the belt entry and putting belt together was struck by a piece of rock measuring 2' x 1' x 2" on the neck and shoulder. The rock fell from between two bolts.

The employee was preparing to install his first bolt in the new sequence, immediately after the machine was moved and the ATRS reset, when he was struck in the foot by a piece of rock falling from the mine roof.

The employee was starting to drill, installing a bolt, when rock fell and struck him on the left shoulder.

The employee was struck by a piece of rock that measured 4' x 2' x 6". The employee was a greaser and was lubricating on the #3 section when he was struck.

The employee stated he was struck in the head by a small piece of rock measuring 18" x 12" x 1-1/2" on the head while standing operating the CM in the #5 entry.

The employee was in the process of removing a stuck drill steel in the mine roof when a piece of rock fell striking right hand positioned on the drill pot.

The employee was operating a RBM installing bolts (outside hole) when a piece of rock fell striking him on the neck and (R) Shoulder.

The employee was struck on the left shoulder by a piece of roof material while installing a bolt. He was knocked forward into the RBM striking his left side face against the metal tray.

EE stated that they had just finised bolting an area, when draw rock fell from between two roofbolts, landing on the top of his left foot. EE said X-rays showed two small fractures in bones between two small toes on top of foot; however he finished his shift and bolted four more rows.

EE was drilling outside hole in 4 left break, some draw rock had already fallen. When he started drilling the hole, the drill steel broke loose a 6" X 10" X 2" rock that fell, striking Ee on the left hand.

5 section #1 entry. Bolting top; cap coal 1 1/2 x 3' long fell striking employee on head and right elbow and forearm.

EE was walking up belt entry, rock fell and hit left shoulder.

Employee stated that a draw rock fell on joystick (lever) of bolter which is where his right hand was - smashing his right hand onto joystick.

About 30 Bk, rock fell from top and struck employee on head and right forearm.

Bolting top in #3 section, spad #970, he was starting the steel when a rock fell and struck him on the left hand.

#4 Face, #2 Section bolting top. Setting bolt in row. Rock broke and hit canopy striking ee on left shoulder.

EE was putting hangers: up 3'x16"x3". A piece of rock fell between the bolts and struck him in the head.

He was running the miner. He was cleaning up last cut of day. When a piece of draw rock fall out between roof bolts. The rock hits him in the back of the head knocking his chin into his chest. He felt something pop in his left rib cage.

Employee was hanging curtain on #2 section when a piece of rock 2' wide, 4' long, 8" in the middle to 1/2" thick on all edges fell out striking employee.

While bolting top a piece of rock fell on atrs then fell in on victim hitting him on the shoulder.

Victim was putting supplies on bolter when a piece of draw slate fell between the bolts. 2'x3'x3" striking victim in the head and shoulder.
