Employee states he went to the Brody bathhouse after his shift to clean-up. He was attempting to turn the water supply to the sink on when the incident occurred. He placed his left knee on the sink and reached up to turn the valve. The sink fell off of the wall and broke, resulting in a laceration to his left shin. Sutures were used to close the wound.

Employee stated he was shoveling at the 3-B belt tail and when he bent over he felt pain in his lower back. He was diagnosed with a lower back strain and worked under restricted duty for one (1) day.

Employee stated he was lifting a piece of 3/16" sheet metal measuring 24.5" x 8' into place on an overcast when he lost his grip on the sheet metal and it slipped striking him on the lower right arm. This resulted in a laceration, and sutures to close the wound.

Employee stated he was in the process of picking up a 60" bottom roller that was lying on the mine floor when he felt pain in his groin. On 3/19/15 the employee was diagnosed with a hernia.

Employee stated that he was shoveling along the 4-I Belt and as he lifted a shovel of material to place it on the belt he felt a sharp pain in his groin. On 03/13/2015 the employee was diagnosed with a hernia.

Employee stated he was using a slate bar to scale the mine roof on the #4 section in the #3 entry when a piece of rock fell and struck him on top of the head (hard hat). This resulted in a C6-7 disc herniation. This incident became reportable on 3/17/2015 when medical evidence supporting the alleged incident was received. The employee began missing work on 2/24/15.

Employee stated he was pushing down on the water clamp that connects the waterline to the section manifold when he felt pain in his groin area. On 2/26/2015 he was diagnosed with a small left inguinal hernia.

Employee stated that while attempting to install a roof bolt into the mine roof, a piece of rock fell from the top and struck him on the right foot. This resulted in a fracture of the 2nd toe.

Employee stated that he was operating the shuttle car on the #4 section when he hit a bump in the roadway and felt pain in his right side. He was diagnosed with a muscle strain and placed on restricted duty until 1/20/2015.

Employee stated that he was in the process of installing the rub rail on the continuous miner when his right index finger was caught between the rub rail and the frame of the continuous miner. This resulted in a crushing injury, which required sutures to repair.

Employee stated that he was supplying his bolt tray and as he turned around he struck his head against the mine roof. This resulted in a strained neck and restricted work days.

Employee stated that he was operating the 14-man battery powered mantrip on the #3 section. The mantrip went through a crosscut and ran into the beltline. This resulted in bruising and strains to the left leg.

Employee stated that he was operating the shuttle car on the #4 section. He was loaded and returning to the feeder to dump when he hit a bump in the roadway which caused him to be jarred into the canopy. This resulted in a strained neck and restricted work days.

Employee worked for Brody, Mining, LLC until 2/22/2014. After ending his employment he was diagnosed with occupational pneumoconiosis.

Employee stated that he bent over the side of the scoop bucket to pick up two (2) bundles of roof bolt plates when he felt a sharp pain in the area around his navel. This resulted in a hernia.

Employee stated that he was walking outby the section on the roadway when he stepped on a slick rock and twisted his right knee. This resulted in a knee strain and restricted work days.

Employee stated that he completed performing maintenance work and was stepping off the feeder when his foot slipped, causing him to fall to the ground. This was a same level fall that resulted in a back strain.

Employee stated that he was examining the 2-C beltline when he walked into low top and struck his hardhat/head against a belt hanger that was sticking down from the top. This resulted in a strained neck and restricted work days. The incident did not become reportable until 7/16/14 when the employee was placed on restricted duty.

Employee passed away from unknown cause. The investigation is not complete. The time the employee passed away was an approximate time.

Employee was employed by Brody Mining, LLC until 10/28/2012. On 7/10/2014 the operator was notified that the employee had been diagnosed with occupational pneumoconiosis.

Employee stated that he was removing a cable bolt from the tray of the roof bolt machine and as he pulled on the bolt his hand slipped and his right wrist struck the bolt tray. This resulted in a contusion and restricted work days.

Employee stated that he was walking beside the section power center when he tripped over the feeder cable and fell to the ground. This resulted in a sprain/strain of the lower back.

Employee was diagnosed with occupational pneumoconiosis.

On 05/27/2014 information diagnosing the employee with occupational pneumoconiosis was received by Brody Mining, LLC.

Employee stated that he attempted to trim a tree limb using his pocket knife, when the knife slipped, and cut his left pointer finger. This resulted in a laceration that required 4 sutures to repair the wound. This incident resulted in no lost time.

On 5/12/14 an event occurred at this mine which resulted in the entrapment of two (2) miners. Upon subsequent recovery it was determined that both miners had perished. An investigation is on-going. The causes are unknown.

Employee stated that he was making repairs to a diesel mantrip. After lifting the rear end into place he felt a burning sensation in his abdomen and groin. Medical received on 6/19/14 diagnosed the employee with a hernia.

Employee stated that he dismounted the RBM to man the drill station and as he stepped forward he tripped on a rock. As he fell, he extended his left arm and his hand struck the drill pot. Medical received 05/09/2014 states a fracture to the base of the fifth finger.

Employee stated that he was working along the 3-B belt when a piece of rock fell from the top and struck him on the back of the neck. The rock was approximately 22" wide and 0" to 1" thick. This resulted in an abrasion and one lost work day. This incident became reportable on 5/22/14.

Employee stated that on an unknown date and time he was operating a diesel mantrip when he hit a bump and bounced into the air. This caused him to pinch his left testicle between his leg and the seat of the mantrip. A medical diagnosis has not been received at this time. This incident became reportable on 5/22/14.

Employee stated that he was installing a rib bolt and as the bolt entered the hole it bent and then the rotation was engaged, which caused the bolt to strike/twist his hand and wrist. This resulted in a contusion and restricted work duties.

Employee was walking into the face of the #7 entry. A roof strap was hanging down from the top and as he walked into the entry he struck his right cheek against the strap resulting in a laceration. The incident became reportable on 6/27/14 due to complication and subsequent lost days.

Employee was diagnosed with occupational pneumoconiosis and awarded 10% PPD.

Employee stated that he was using a slate bar to straighten the front basket of a mantrip. As he pulled on the slate bar, he felt pain in his right shoulder causing him to lose his grip on the slate bar and fall to the ground. This resulted in a back and right shoulder strain.

Employee stated he was stacking structure on a flat car, when back and shoulders started hurting. This resulted in a shoulder and back strain.

An unintentional ignition of gas occurred in the #6 entry of the #2 section while the 006-0 MMU was in the process of extracting coal. There were no injuries to miners or damage to equipment during this event.

Employee stated that he was walking in the number 7 entry when he stepped on a piece of coal and twisted his left knee. This resulted in a knee sprain.

Employee was replacing cat pads on the continuous miner. As he positioned the cats for installation, his finger was caught between the joint of the pads. This resulted in the amputation of the distal portion of the left little finger.

Employee stated that he was walking into the warehouse and as he passed through the door he felt pain in his left knee.

Employee stated that he was loading crib blocks on the service scoop and as he turned to placed one of the blocks onto the scoop he tripped over a rock and fell to the mine floor injuring his back and right shoulder.

Employee stated that he lifted his personal tool box and then his back began hurting. This resulted in a back strain.

An inundation of gas occurred on the #2 Section while retreat mining. There were no injuries to miners or damage to equipment during this event.

Employee stated that he was performing belt maintenance on the belt head and as he stepped and turned he twisted his right knee. Employee continued to work until 2/24/14 when he underwent surgery to repair a torn meniscus.

Employee stated that he was walking across the #2 section when he stepped in a hole and twisted his knee. This resulted in a left knee sprain.

A rock fell from the mine roof near the face of the #5 entry approximately 100 feet inby spad #13280 on the #3 Unit, striking the employee on the head and neck.

Employee stated that he was hauling supplies with the diesel scoop when a plaster bucket fell off of the scoop bucket, hit the roadway, and splashed up into his right eye.

Employee stated that he was using a belt knife to cut a piece of belt rubber and as he pulled the knife slipped and cut him on the top of the left hand. This resulted in a laceration that required sutures.

Employee was installing the cutter head pin on the continuous miner. As he struck the pin with a sledge hammer a piece of the pin broke off and imbedded into his right leg just above the knee. The foreign body was not removed and the wound was closed with sutures.

Employee stated that he was hitting the core breaker pin when a piece of metal went behind his safety glasses and into his right eye. Medical received on 12/19/2013 confirmed a medical reportable incident.

Employee was making a splice in the shuttle car cable and as he was striking the knocker he accidentally struck his left thumb with the hammer. This resulted in a laceration that did not require sutures.

Employee moved the RBM into the 3 left crosscut and dismounted the operator's deck. As he was exiting the deck of the RBM, the continuous miner trammed into the flypads directly behind the roof bolter, striking the employee and knocking him to the mine floor. This resulted in multiple sprains/strains of the upper body.

Employee stated that he was pulling back on a roof bolt to line it up with the drill pot. As he applied pressure on the bolt he felt something pull in his right shoulder. This resulted in a right shoulder strain.

Employee stated that he was pulling a water hose through a crosscut to wash off the duster batteries. As he walked through the crosscut he tripped over a rock dust hose and fell to the mine floor injuring his shoulder. Medical received on 08/08/2013 diagnosed the employee with a left shoulder sprain.

Employee stated that he was holding timbers that were being cut by two other employees by sitting on top of the timbers. As he dismounted the stack of timbers, he got his foot caught in the bindings that hold the timbers together and fell to the ground. Medical received on 08/15/2013 diagnosed the employee with multiple sprains/strains.

Employee stated that he was pulling a bag of garbage out of the trash can in the bathhouse and strained his left shoulder. Medical received on 08/28/2013 diagnosed the employee with a left shoulder strain.

Employee was walking down a grade on the section and slipped and fell backwards. This resulted in a strain to his left leg.

Employee stated that he was picking up a piece of belt structure on the mine yard and hurt his back. This resulted in a back strain and medical received on 08/15/2013 confirmed that the employee was receiving medical treatment.

Employee was picking up rocks and putting them on the belt line. As he twisted with the rock he felt pain in his upper left back. This resulted in a back strain.

This report is being filed under protest to abate citation 9002341. According to MSHA, an employee SSN 8129 has incurred a reportable illness of occupational pneumoconiosis disease. Brody denies that it breached any duty to report and reserves the right to contest or amend the report. Employee previously awarded 10% & 20%, no additional awards from 06/24/2013.

This report is being filed under protest and solely to abate citation 9002340. According to MSHA, an employee (SSN 3433) has incurred a reportable illness of occupational pneumoconiosis disease. Employee was loading blocks into the scoop bucket. As he was walking toward the scoop carrying a block he tripped, causing him to fall and strike his left knee against the ground. This resulted in a laceration to the left knee that required sutures to repair.

This report is being filed under protest and solely to abate citation 9005626. According to MSHA, employee incurred a reportable injury when he was struck on the back by a piece of roof rock. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

This report is being filed under protest and solely to abate citation 9002339. According to MSHA, an employee (SSN 4733) has incurred a reportable illness of occupational pneumoconiosis disease. Brody denies that it breached and duty to report an occupational illness and reserves the right to contest the issuance of the citation and to amend and/or retract this report.

Employee was operating the RBM in the 4L Crosscut. As he was placing the glue and bolt into the hole, a piece of rock fell from the top, struck the canopy of the RBM, and then struck him on top of the head. Medical received on 5/21/13 diagnosed the employee with a cervical strain.

Employee was carrying jacks from a flat car to the belt line. As he stepped down off of the flat car he twisted his right ankle. This resulted in a fracture of the distal fibula.

This report is being filed under protest and solely to abate citation 9005625. According to MSHA, employee incurred a reportable injury when he lifted a spool of cable and felt pain in his back. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee was operating the roof bolt machine on the #3 Section in the #1 entry. As he placed the drill steel against the top to drill a hole, a piece of rock fell from the mine roof, struck the drill pot, and then struck him on the left foot. This resulted in a laceration to the top of the left foot that required seven (7) sutures to repair.

This report is being filed under protest and solely to abate citation 9005624. According to MSHA, employee incurred a reportable injury when he was carrying a bag of rock dust at 3A drive and had pain in his back. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee was mounting the walk-thru roof bolt machine and as he stepped up onto the back of the bolter he struck his head on the thread end of a torque tension bolt that was sticking down from the top. This caused him to strain his neck.

This report is being filed under protest and solely to abate citation 9005623. According to MSHA, employee incurred a reportable injury when he complained of pain in his hips and back due to carrying the scsr. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee was using a jack to raise the rear end of a diesel mantrip. As he pushed down on the jack bar he felt pain in his groin. Employee continued to work until 4/10/13 when he was diagnosed with a hernia, had surgery to repair the hernia, and began missing work.

Employee was bolting on the #3 Section in the #6 Entry 130' inby spad # 12262. The bolt crew had stacked plates and pans on the base of the t-bar where the t-bar meets the frame of the RBM. When the boom of the machine swung, the plates and pans fell off and struck the employee on the side of the rt. knee. Employee was diagnosed with a strain and began missing work on 3/8/13.

According to MSHA, employee (SSN 5489) incurred a reportable injury when the tongue of a flat car came loose and struck him on the left hand. An unintentional roof fall occurred in the #1 Intake at 32 Break. (1 break inby spad no. 289)Fall was approximately 18' x 20'.

According to MSHA, employee incurred a reportable injury when he was operating the RBM and struck on the rt. forearm by a rock. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee was installing roof bolts in the #4 entry on the #1 Section when a piece of rock approximately 31" x 24" and 1/4" to 4" thick fell from the top and struck him on the right shoulder. This resulted in a fracture to the right 5th rib.

Employee stated that he was down on his knees with a come-a-long ratchet wrapped around a pipe when he used one hand to lift the pipe and the other to pull the ratchet from under the pipe. This resulted in a lumbar spine strain.

According to MSHA, employee incurred a reportable injury when he struck himself in the mouth with a slate bar resulting in a broken tooth. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee stated that while installing I-beams along the #4B Belt he was struck on the head/back by a beam when the employee holding the beam lost his balance and dropped the beam. Employee began missing work, complained of back pain, saw a doctor on 01/31/13, and medical received on 2/7/13 stated "mild depression of the T5 & T8 vertebral bodies, with no acute fractures seen".

EE was hanging the roof bolter cable when he finished he walked along the side of the bolter to check his curtain. Rock fell in front of and over the ARTS of the bolt machine pinning ee to the ground and landing on his right foot.

Employee stated that while cleaning the No. 4A Belt Head, he stepped on a turn-buckle and twisted his right ankle. Employee began missing work on 1/17/13 due to a sprain.

The employee stated he caught his right index finger between a ratchet and miner frame causing an open wound that needed sutures. He was tightening ripper motor mount bolts.

The employee stated he stepped off the walk-through RBM and miss judged the distance causing him to twist his ankle. He is missing work.

The employee explained he reached over and picked up a roof bolt and pie pan out of the roadway and pain shot down his hip and left leg. The initial WC BI-1 shows prior surgery prior injuries on his lumbar spine and not acute injury.

The employee stated he jumped off the Phillips s/c landing on the mine floor after making repairs. He complains of pain in lower back down to left knee. He is missing work .

The employee claimed pain to his back hooking a duster to a tractor 11/15/2012. He continued to work until November 26th. He has been seeing a medical Doctor for other complications of the same body parts (chronic in nature) history of lumbar disk.

The employee injured a permanent tooth by being struck in the mouth by a chain. He pulled on the to free it from supplies causing the chain end to flip striking his mouth.

The employee strained his lower back picking up a 5 gal container of water to remove from a pressure line he was working on.

The employee was loading roof bolts onto the storage tray for the Fletcher RR II and ruptured his groin. The event has resulted in a hernia lost time injury.

The employee contacted a sharp object near the exhaust system on the #766 diesel vehicle during welding. The incident resulted in a laceration requiring sutures.

The employee stated he caught his right index finger between the cable and the bolt plate on pizza pan for rib support. The incident resulted in a medical reportable cut/sutured injury.

Based on information obtained 11/30/2012. The employee claims he tripped and fell carrying a timber and felt burning pain in his neck. The medical and impressions describe no acute processes. Also possible for strains cervical and thoracic.

Information from insurance (via phone) the employee reported slipping/falling off s/c. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee stated he was moving rock off miner and pinched his finger (left middle) between rock and top. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee stated he was pushing and twisting on roof bolt for installation and injured a mid section vertebrate. He is missing work as a direct result of this event.

The employee standing and hanging curtain near the rib line was struck on his right side above hip by rib material falling. The event resulted in two days away from work.

The employee stated he strained his back lifting on rock at the 3A belt head while cleaning the area. The incident has resulted in lost time.

The employee reported on August 24th at 4:15 AM he experienced left knee pain and it hurt when turning it. He continued to finish his shift and worked up to 14th day of September. Medical received at the mine on September 28, 2012 states the problem is chronic in nature and not from an acute process.

The employee stated he stepped into a hole twisting his back when walking to the old 3-section checking a pump. The event resulted in a lost time injury.

The employee stated he was wiring up P-40 pump starter in #9 entry a piece of coal with layered rock struck him in the back of neck. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee was dismounting the diesel Wallace D-U-V-14 man ride and stepping in a dip twisting his right ankle. The event has resulted in a lost time injury case.

The employee opening an airlock door felt burning and pulling in his lower back. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV.

The employee hanging miner cable by himself stated he felt a burning sensation from the middle back up to including his neck. "As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV". 8/7/2012 this claim was denied (no medical evidence).

Based on information received 11/30/2012. The employee claimed he fell off a ladder at the belt drive rock box and bruised his right arm pit. Medical received 12/03/2012 suggest no acute injuries. The employee has medical history on body parts.

The employee tripped and fell striking his left knee against the section power center. The event resulted in a lost time injury to his left knee.

based on med. Fx 5th metacarpal walking stumbling. there was no incident filed. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee was installing his third bolt underneath a brow and a piece of rock broke loose falling and sliding underneath the canopy striking him in the left leg and lower back. The rock measured 90" x 27" x 16". The incident resulted in a Fix to the pelvis.

The employee stated he was walking thru fly pads and lost his balance and slipped and began to fall. He tried to catch himself grabbing to buggy anchor. The s/c started causing his finger to catch between mine top and anchor. cut/sutures. "As requested by msha, the following information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee riding a man-trip to the outside accidentally extended his left arm beyond the seating compartment where is was caught between the man-trip and rib. The event resulted in cut/sutured injury.

The employee was struck on tip on nose by pin wrench (24'). He continued to work and missed the 11th (Family Health Care Assoc). "As requested by MSHA, the following information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee hanging reflectors with hand on bolter canopy struck by 2" x 8" x 1" rock. FA on fingers. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee alleged he strained his back bending a roof bolt on 4 section during the non-producing owl shift. Medical received on 05/14/2012 states spondylosis and lumbar lordosis which suggest presence of an illness rather than an acute process.

This incident was reported to management 4/17/2012 at 12:18PM. The employee stated he was prying lost balance fell into belt & structure. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee stated he was supplying the RBM in the face of no. 2. He was transferring bolts from the scoop bucket onto the bolting machine when his right hand slipped causing the bolts to his left hand to be between the bolts and the scoop frame.

The employee tripped over a loop of miner cable and fell. He continued to work until April 16, 2012. He overextended his right arm as a result of the fall, and is now anticipating surgery at t he Bicep area.

The employee stated he was loading coal and began to retreat back slipped on rock and fell striking shoulder on mine floor. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee scooping ran over broken up bottom and jammed head into canopy. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee stated he cut the tip of his finger (left ring) during splicing. The incident did not require sutures. No medical. "As requested by MSHA, the following information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee reported he was lifting Omega block and wrenched his neck when standing up contacting roof with hat. "As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

The employee was operating drilling rotation using a pusher steel not properly affixed inside the hex drill pot and his hand placement was where he received an injury to his wrist.

The employee was lifting and placing shield over shuttle car reel and claimed lower back injury.

The employee contact the mine roof when walking the 2B belt. He did not miss any work days (payroll/medical insurance). "As requested by MSHA, the following information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV".

Upon an investigation the employee was struck on the right side by a rib rock. The rib rock fell without signs and/or warning during rib bolt installation. The event resulted to a lost time injury.

The employee stated he was struck in the chin with a bolt wrench. He stated he was in the #2 entry face installing a bolt. The wrench broke.

Upon an investigation the employee was observing the belt up-hold roller alignment when he contacted the bottom moving conveyor that caused his right hand/fingers to be caught between the bottom belt and the hold-up roller near the v-scrapper.

The employee was bolting in the 4 left cross cut face (#411) machine on the off side when he was struck on the left hand by a piece of roof rock. The rock measured 2'foot x 3" inches. The event resulted in a Fx.

The employee hyper extended his left wrist operating the RBM standing at the inch controls. The machine dropped suddenly in soft and busted up bottom causing the control lever guard to strike the employee's wrist.

The employee was struck on the left knee by sharp slate falling out from top right rib. The event resulted in sutures to a laceration and also lost time.

The employee reported he had been experiencing pain in his lower back riding mantrip (Diesel rubber tire). Medical received November 12, 2011 stated the image taken for the L/S spine was neg. for acute injury but stated there was lumbar levoscoliosis. He also suffer back injury 2009.

Employee stated he was dropping drill head bring steel out of the hole using rotation. The drill steel was rotating outside the drilled hole allowing it to make contact with the employee's mouth/jaw. The event resulted in lost time injures.

The employee claims he suffered a back strain lifting miner cable hanging it. He is missing work, and the company has not received any medical as of 10/24/11. The incident was charged "No action taken" initially.

The employee stated he was walking 2-east roadway and at BRK 53 he slipped on rock and landed on portable radio with his hip (right side). He is missing work.

The employee complained of head, shoulder, neck complications on 10/05/2011 at the mine. Medical received at the mine on 10/12/2011 showed no acute abnormality, injuries, malalignment, bony abnormality or convincing evidence of same body parts but medical complications from past injuries and repeated trauma of same body parts.

The employee reported he has been experiencing knee pain during his work activities at Brody as an RBMO. His pain has intensified and the employee has visited a medical facility. According to the MRI report received 9/19/2011 there is no change from the last study taken 1/24/2010. He has had prior surgery on the same knee.

The employee stated he bounced inside the operator's compartment and contact the seat with his tail bone causing an injury. The event has resulted in a lost time injury.

The employee was operating RBM installing a header board and catching his index finger between the header bracket and header. The incident resulted in a medical reportable case. Cut/sutured.

The employee was dragging a piece of curtain through the intersection of #5 and tripped falling down. He attempted to catch his fall by extending his left arm forward. The event resulted in a lost time injury a Fix to the arm.

EE was riding in on mantrip, when mantrip ran over rock in haulway,EE stated he hit his head on canopy of mantrip and came down hard on tailbone.

Rock fell and hit face and right arm stitches on right side of face

EE was loading timbers along 30 roadway. EE eas laying a timber out to cut and it slipped and smashed index finger between timber and block left finger.

EE pulled wrong lever on bolter causing mast jack to smash his right foot under the drill pot.

The employee using a chain ratchet hoist to splice feeder chain was struck in back of head by hoist hook when being suddenly released under pressure.

While bolting top piece of coal rock fell and bounced off rt rib and back.

Upon an investigation the employee,CMO,was struck by rib materials falling from the inby corner of the intersection in the face of no. 1 entry causing him to be forced into the s/c. He received head and facial injuries.

Unintentional Roof Fall: 20'x20'x7' Primary roof supports used: 5' resin rods 2 west mains old works cc 41 inby spad 3672.

He was lifting barring to put on 4" roller when he took barring from his cart to his shoulder he fell to ground and said he hurt his lower back 10 to 15 min later right left went numb.

The employee stated his left thumb made contact with a sharp object. He continued to work until April 9th and start missing work due to infection. Also the mine was informed on April 19th the injury required sutures on April 2nd. The injury has resulted in a lost time case.

The employee was lifting and pulling on a water pump mounted on maintenance diesel vehicle. He is diagnosed with rotor cuff damage.

Pulling welder cable and heard something pop in shoulder. Did not receive report until 4-1-11.

The employee stated he was loading a 13 HP de-watering pump into a diesel vehicle and injured his finger between the pump and vehicle frame. The injury resulted in a medical reportable event. He finger required sutures.

The employee caught his right little finger between the scoop frame and a crib block. He was attempting to remove the wooden crib block from the scoop bucket. The event resulted in a lost time injury.

Working on RB Boom and got (L) arm caught in drill pot of machine.

The employee stated he injured his right middle finger allowing to rest on the drill pot while installing a bolt and the finger was caught between the drill pot and bolt. He is missing work.

Twisted knee while bolting top

The employee stated he was walking carrying metal jack along with a co-worker when he slipped and twisted his left knee. The incident resulted in a lost time injury.

The employee stated he reached to activate the joy stick control on the operator side of the machine to push up and spin the bolt and strained his right shoulder near the scapula. he is missing work.

The employee was using a slate bar to pry out a hinge pin for joy continuous miner pan. The bar slipped causing him to fall into and against the machine frame. The event resulted in a lost time injury.

The employee was traveling through a mandoor and twisted his ankle. He did not miss work until 2/9/2011. The event has resulted in a lost time injury.

The employee was removing stack steel that was stuck in the mine roof. One of the pieces of steel fell out, striking his left hand, causing a laceration to his ring finger that required 8-sutures.

During a pre-shift examination prior shift start an unintentional roof fall was discovered along the #3 East Mains neutral entry beginning at CC31 and extending inby too CC33. The fall description was approx. 200 feet in length x 19 feet wide x o to 8 foot in thickness. Type of supports in the affected area was 5' fully grouted rods.

Employee was pulling cables. He backed into a roof strap and cut back.

The employee was advancing the RBM using the inch controls when a rib corner fell out striking him in the middle back area. The event resulted in a lost time injury.

Upon an investigation the D-8R stockpile dozer was shoving coal into No.1 feeder and when backing around stacker the machine went down into small void at feeder No.2. To prevent a re-occurrence spot lights were installed, upon dozer operator's request, for visibility when operating at and near feeders, and changing operating procedures to prohibit backing passing feeders.

The ee was experiencing pain in neck and elbow when handling miner cable 4-section #3 entry CC outby face in intersection on 11/30/10. Medical received 12/8/10 shows there was no acute injuries of neck, shoulder, arm or elbow. However other medical shows other issues exist causing pain to the affected areas.

Twisted left ankle while loading roof bolts supplies.

The employee was operating #803 battery permissible 4-wheeler along East Mains roadway leading to EP's. The front tire contacted a rock in roadway causing the steering wheel to rotate suddenly contacting his right index finger causing a cut / sutured injury.

Was at 3c Head taking roller out with chain ratchet. Belt head came off frame hitting right shin, roller fell and hit knee on right leg.

The employee, Production Supervisor, was examining the faces on the # 4 section and reported he stepped into a hole twisting his right knee. The company has not received any medical documentation on the claim but he is missing work.

The employee was setting and seated in the rear of the diesel powered mantrip going underground to his section. The mantrip bounced causing the employee's head to contact the stationary handrail. The incident resulted in lost time injures.

The employee was loading empty wooden pallets onto a supply car and reported he strained his lower back. He started missing work on October 20, 2010.

Running roof bolter & drill steel struck top of hand & bruised hand & swelled up. Didn't ride ambulance.

The employee was preparing old #5 belt take-up to be moved and relocated. He was bending over to pick up crib block when a piece of roof fell striking his neck.

The employee, #2-section, was cutting on shields for cont. miner, hot slag (Foreign body) went into eye. The hot metal slid off down onto pool of water on top of miner frame and splashed underneath safety glasses into right eye. He had lowered his cutting hood.

EE has alleged he was electrocuted when handling the miner cable with his left hand. He missed work starting on 7/13/10, and RTW on 7/19/10 without medical excuse, and is to RTW 7/23/10 with medical information. The Company has no medical evidence of the specific event or is there any witnesses to confirm the employees descriptions of alleged event.

The employee was walking airways and stepped into a hole causing pain in his lower back, upper leg, and knee. He is missing work.

Walking in old roadway. Stepped in mud and twisted his knee.

The employee was struck by a piece of rock falling from between roof bolts while operating continuous miner on 6/19/10 and continued to work. Medical received September 14, 2010 shows fx to c-6. The employee is now missing work.

Upon an investigation, the employee was operating RBM drill head with drill steel in rib. He attempted to remove the drill/pin steel and got his glove caught, causing a compound Fx to his left ring finger.

The mine received a phone call on 5/27/10 from the employee who stated his neck had pain radiating down through his shoulder. He stated he did not file an accident/illness prior leaving the property. Medical received on 6/3/10 states there is no acute injury but chronic disease.
