Employee stated EE and EE's bolt partner just finished putting up the first row of bolts in the #4 Left entry on the Unit 1 Section. In the process of sitting in the operators compartment of the roof bolting machine a rock which measured 30 x 18 x 2.5 thick fell from the mine roof and struck the employee in the right hand. This caused a laceration and EE received 6 stitches.

In the process of drilling a hole into the top to install a roof bolt a piece of rock measuring 36" Wide X 22" Long X 8" Thick fell out from between the roof bolts and struck the injured employee on top of EE's right foot.

Employee stated was attempting to install a roof bolt on the Unit 1 Section. In the process of the employee drilling a hole to install the bolt a piece of rock fell from the mine roof, slid down the T Bar of the bolt machine, and struck the injured employee on the top of back.
Injured employee stated they were bolting the #8 face when they attempted to scale a rock. In the process of scaling the rock it broke free and struck employee on the right triceps. Note: Doctor did not prescribe any restrictions or time away from work. management elected to provide a light duty job for 4 days.
Employee was moving cables out of the way to pull draw rock down that was over the power center on the 1 Left section. In the process of moving the cables, a piece of draw rock fell from the mine roof and struck the employee in the head and foot.The rock measured 27" X 18" x 2".
Employee stated was in the process of installing a roof bolt in the #5 left entry on the 1 Left Section. Employee stated as EE was drilling the hole a piece of draw rock fell from the mine roof and struck the employee on the left forearm. The rock measured 30" long X 10" wide X 1" thick and fell from approximately 2".
EE was drilling a hole in preparation to install a roof bolt and a piece of mine roof approx. 12" wide X 3.5" thick X 27" long fell and struck ee in the ribs.
