Blue Creek No. 1 UG Mine Coal

EE was using a utility knife when the knife slipped and cut EE's left middle finger and index finger. EE was not wearing cut resistant gloves as required by company policy. EE stated that EE had just removed EE's gloves to get a better grip. EE received 5 stitches in EE's left middle finger.

EE was assisting Co-Worker who was attempting to remove a cat pad pin, EE was holding a cut off roof bolt that had been modified for this task while Co-Worker was striking it with a 10lb sledgehammer. The hammer, after striking the head of the roof bolt, ricocheted up striking EE in the left eye. This resulted in a laceration above the left eye.

Injured employee was attempting to remove the wheels from #326 shuttle car when one of the tires that was leaned against the car fell over landing on the injured employees leg.

Injured employee was drilling last hole of the cut up when a rock fell out striking their left hand resulting in lacerations to their left index finger requiring 7 stitches, middle finger requiring 4 stitches, and ring finger requiring 2 stitches.

Injured employee was drilling a test hole when the steel became lodged in the top, while attempting to free the steel it fell out of the hole striking the left ring finger resulting in a laceration that required 10 stitches.

While mining the #3 entry the shuttle car couldn't make the turn to get out of the cut, EE walked to the back of the shuttle car to see if EE could help. The shuttle car pulled forward to reposition to try and make the turn. EE walked in between the shuttle car and the outby rib the operator didn't see EE and pinned EE between the shuttle car and the coal rib.

Employee was in the process of putting tools in a toolbox they grabbed a pair of chain hoist off the track ride and begin to walk toward the tool box when they stepped on some draw rock and twisted their left ankle.

The employee was using a 4 pound sledge hammer to drive a boom pin out of a continuous miner, the miner removed glasses after they fogged up and begin to drive the pin out when a piece of metal came off the pin and embedded into right eye.

Employee was working on putting a conveyor chain together on the company's 310 shuttle car EE was swinging a sledge hammer installing a connector link and felt a sharp pain in EE's stomach EE was diagnosed with a ventral hernia.

As the Fireboss was walking the 3 Mains belt they came upon a roof fall on the Belt line at crosscut 9 they called out to the dispatcher and then reported to MSHA at approximately 3:45 am.

EE pulled a bottom roller out from under the belt and stood it up and placed it on EE's shoulder and tried to stand up and carry the bottom roller. When EE stood up EE felt a sharp pain in EE's lower back and left arm and leg.

The owl shift was in the process of making a belt move when they heard the top working on the left side of the section around 5:00 am the roof fall occurred it fell for approximately 90 ft from number 1 over to the edge of the number 3 entry no one was injured and there was no equipment involved.

EE was preparing to set some timbers in the belt entry when EE noticed some loose top where the timbers was to be set. EE pulled the loose top and was shoveling it up when a piece of coal rolled out from the rib striking EE in the back and left elbow and left foot. EE finished the shift and worked for 4 days before being placed off work for 2 weeks.

After placing a jumper in the 4 mains belt EE started releasing the 3 ton chain hoist to let the pressure off the belt clamps. EE had worked the hoist 2 cranks when the belt broke outby the belt clamps allowing the clamps to fly forward knocking 2 top rollers loose on the belt pinning lower left leg between the belt structure.

Employee stated was removing a DPM Filter from the top storage rack. Employee attempted to pick Filter up, left thumb caught on a broken sign causing the Filter to turn over on thumb creating a laceration to left thumb.

On January 9th 2020 we were notified that this employee was diagnosed with black lung.

Employee stated EE was in the process of hanging fly pads in the #4 entry, on the Unit 2 Section. In the process of EE hanging a fly pad from the 6" ladder, the ladder tipped over, and EE fell to the ground. This caused EE to land on EE's head, shoulder, and back. Note: EE was standing on the bottom 2nd step of the ladder when this incident occurred.

Employee stated was loading the #400 roof bolting machine on the Unit 1 Section. In the process of loading some roof bolts on the bolter the employee felt a pain in lower back

Employee stated EE was operating the #420 Fletcher CHDDR-13 Roof Bolting machine on the Unit 2 Section. In the process of drilling a hole into the top to install a roof bolt a piece of rock measuring 36" Wide X 22" Long X 8" Thick fell out from between the roof bolts and struck the injured employee on top of EE's right foot.

Employee stated ee was operating a roof bolting machine on the Unit 2 Section. EE was in the process of installing a roof bolt. In the process of installing the bolt, the pie pan plate came through the head of the bolt striking the employee in the nose.

Employee stated EE and another employee backed the #420 Roof Bolting Machine out by the No. 7 face after bolting it on the Unit 2 Section. The employee stated EE was going to walk around the machine to check EE's dust box. In the process of walking around the machine EE slipped and fell landing on EE's shoulder resulting in a shoulder sprain.

Employee stated EE and another employee were attempting to make a belt jumper at 3 Head with a piece of extra belt. In the process of squaring up the belt, the knife came out from the belt, and cut the employee in EE's leg. This employee had to receive 4 stitches.

Employee stated was climbing in the #382 Shuttle Car on the Unit 1 Section. In the process of getting into the shuttle car EE's left knee gave out and EE fell to the ground.

Employee stated that ee was operating the #420 Fletcher Roof Bolt Machine on the Unit 2 Section. In the process of placing the drill steel into the drill pot a piece of rock fell from the mine roof and struck the injured employees hand. This caused one of the employee's fingers to be fractured.

Employee stated that they were installing a new tail piece directly under #2 Mains Head. The crew had accomplished that task and was in the process of pulling the belt back together. In the process of pulling the belt back together a bolt head broke off of a roof bolt striking the employee in the top of the head. The employee received 4 stitches.

Employee stated that Ee was attempting to grease the Joy 14/15 C/M head jack pin on the offside of the C/M but could not reach it from the ground. When Ee attempted to climb up on the C/M the scrubber hood fell smashing hand.

EE stated Continuous Miner lost power so EE walked down to power center. When they arrived section electrician was trouble shooting C/M cathead. Electrician then said they were about to put the power in on C/M. In the process of putting power on the machine a cable splice grounded, causing the cable to blow up beside the injured employees face.

Employee stated ee and another employee were attempting to load a mass jack into a scoop on the Unit 2 section. In the process of lifting the mass jack into the scoop bucket, the jack slipped causing the employee to smash finger between the jack and the scoop bucket. The employee sought treatment and received 10 stitches.

Employee stated ee was assisting some other employees to unload a scoop tire from a mantrip. They were using slate bars to unload the tire. In the process of unloading the tire it flipped over into the scoop bucket they were loading it into. This caused the employee to smash finger in between the slate bar and scoop bucket.

Employee stated that EE was attempting to drag a 110V water pump with a 2 inch plastic discharge line. When the employee was attempting to move the pump and the discharge line across the track, EE stepped on the metal track rail which caused EE to fall, landing on EE's right hip.

The operator has been notified that the employee has met the requirements for filing an occupational Pneumoconiosis claim.

Employee stated EE was preparing to make a belt splice on the 3 Mains belt line. EE had finished hooking up the chain ratchets and was going to step down off of the belt. In the process of stepping off of the belt, the employee stepped on a baseball size rock which caused EE to roll EE's ankle.

Employee stated they were picking up a 48 inch piece of belt structure on the surface. In the process of picking up the piece of structure they lost their footing in the mud, slipped, and felt a pain in the groin area.

Employee stated was attempting to install a roof bolt on the Unit 1 Section. In the process of the employee drilling a hole to install the bolt a piece of rock fell from the mine roof, slid down the T Bar of the bolt machine, and struck the injured employee on the top of back.

Employee stated ee smashed the tip of right index finger when ee attempted to toss a crib block. EE index finger was smashed between a crib block and side of a scoop. Employee received stitches and modified duty starting 11/14/2017.

Employee stated ee hit a rut and caused to be thrown up against the canopy of the shuttle car. This employee didn't report this incident to management until the following day.

Injured employee stated that employee was shoveling #4 mains belt when employee felt a sharp pain in lower back. This is a re-aggravation of a 9/5/2017 incident where the employee was moving miner cable and initially strained lower back. Employee was taken off work on 10/17/2017

Injured employee stated that employee was working on the conveyor chain for #302 shuttle car. Employee stated employee was on knees attempting to pry on the ball valve of the shuttle car so employee could add oil. In the process of prying with a slate bar employee felt a sharp pain in employee's lower back. Employee was placed on restricted duty on 10/17/2017

Employee stated ee was operating the tail motor hauling supplies to the section. The lead motor was pulling the supply cars up an incline, the lead motor stalled and rolled back striking the tail motor. Employee stated ee struck shoulder inside the operators compartment causing pain to shoulder. Note: Employee did not notify management until the 9/8/2017

Injured employee stated they were bolting the #8 face when they attempted to scale a rock. In the process of scaling the rock it broke free and struck employee on the right triceps. Note: Doctor did not prescribe any restrictions or time away from work. management elected to provide a light duty job for 4 days.

Employee was moving cables out of the way to pull draw rock down that was over the power center on the 1 Left section. In the process of moving the cables, a piece of draw rock fell from the mine roof and struck the employee in the head and foot.The rock measured 27" X 18" x 2".

Employee stated was in the process of installing a roof bolt in the #5 left entry on the 1 Left Section. Employee stated as EE was drilling the hole a piece of draw rock fell from the mine roof and struck the employee on the left forearm. The rock measured 30" long X 10" wide X 1" thick and fell from approximately 2".

On 8/3/2017 at approximately 4:00A.M. a roof fall was discovered at 20 brk the No. 7 entry on 1 left 3 belt. No accidents as a result of this occurrence.

Employee stated they went to the end of the track in the #3 entry to load structure into an insert, as they were walking they stepped into a hole and rolled their right ankle.

Employee stated ee was going to load supplies onto the scoop on the 4 Mains section. In the process of walking toward the scoop the employee slipped on a roof strap which was on the ground. This caused the employee to lose balance, fall to the ground, and twist back.

Employee stated with another employee was in the process of dropping miner cable. As they unhooked the cable a rope hanger unwrapped around the cable and hooked the injured employee in the left nostril causing a laceration to nose.

Employee stated EE was dropping bolter cable, the cable had tension which caused the cable to drop fast and caused hand to come in contact with a monkey face causing a laceration to EE's right hand. Employee received stitches.

Day shift production crew was heading to the section. An electrician pulled a motor into the run-around on main line 2. The employee failed to throw the switch and the on coming mantrip took the curve into the motor causing the injured employee to strain their back when the mantrip struck the idle motor.

Day shift production crew was heading to the section. An electrician pulled a motor into the run-around on main line 2. The employee failed to throw the switch and the on coming mantrip took the curve into the motor causing the injured employee to strain neck and back.

On 6/3/2017 an incident occurred when a track switch was not thrown back to the straight. The On coming day shift crew did not notice the switch thrown to the curve and the mantrip took the curve striking a parked motor. The employee suffered a back strain as a result. Note: Employee did not go on light duty until 6-13-2017

Day shift production crew was heading to the section. An electrician pulled a motor into the run-around on main line 2. The employee failed to throw the switch and the on coming mantrip took the curve into the motor causing the injured employee to strain their back when the mantrip stuck the idle motor.

Employee was in the process of moving the C/M machine from the #2 entry to the #1 entry. In the process of moving the employee stepped in a hole twisting right knee. The employee scheduled a doctors appointment for 7/23/2018 and received two cortisone shots.

Employee stated while tramming roof bolter to #3 entry from #4, employee was backing out of the #4 entry, the monster mats hit the corner rib, causing them to slide off of the top of the lids and struck employees right hand. Employee was placed on restricted duty 4/29/2017.

Employee stated another employee came up from behind and wrapped arms around ee and kicked leg around ee and when doing so hit ee's lunch bucket into right knee. Causing sharp pain

Employee stated was rolling up extra curtain and cut thumb on a curtain hook. Note: Employee did not receive stitches but was given light duty

Employee stated was pulling a fly pad off of a board to move up. When employee pulled on the fly pad there were loose rocks laying on top of the board. A rock 13"X9"X1.5" thick fell striking employee on the right shoulder.

The injured was drilling the hole for cable bolt when the drill steel fell back out the hole striking employee's finger.

The injured smashed finger between the belt rail and the top belt roller while removing the roller for a belt splice.

Employee was climbing up on the bumper of the continuous miner to get a rock off the miner. Employees right foot slipped on the bumper and left foot got caught under the turn table. EE felt pain in the left knee. Employee continued to work until 5-4-2017 when EE went off for surgery due to this incident.

The injured was walking to the shuttle car when ee stepped in a rut in the mine floor causing ee to fall and land on ee's right knee.

Employee was operating a roof bolter on the l Left Section. EE had drilled 5ft of a 12ft hole and added a pusher to complete the hole. EE started rotation and the kevlar sleeve became entangled on the drill chuck wrapping EE's arm around the drill steel resulting in a broken Humerus bone in left arm.

The injured was pulling down a fly pad from the fly board to move it up to help ventilate the next crosscut when they felt a pain in their back.

The injured was struck on the arm by a rock that fell from the mine roof between the ATRS and the last row of bolts.

The injured was using a slate bar to pry a shuttle car tire from the wheel's stud bolts. The slate bar slipped causing hand to strike against the frame of the shuttle car which resulted in a fracture to thumb.

The injured picked up the miner cable and waterline to move it out of the way when EE felt pain and tightness in the upper part of back.

On Sunday 8-21-16 at approximately 9:10pm a fireboss examiner discovered a roof fall on the 2 left section. The fall was located in the face and intersection of the #6 entry 1 break inby survey spad number 7382. The fall measured approximately 19' wide X 60' Long X 7.5' thick

The injured was operating the roof bolter. EE stepped back from drill station to get a bolt plate when a small rock fell from the top, glanced off canopy, and struck ee in the side of head.

At approx. 11:30am the superintendent discovered a fall in the return of the #1 entry of 1 left section between crosscut 25 & 26. The fall was approx. 45' outby survey spad 7207. The fall was dangered off and is not planned to clean up. The fall measured approx. 35' long x 18' wide and 5-6' high.

At approx. 5:30am on July 26, 2016 the section boss heard a roof fall in the face of the #7 entry of the 1 left section. The fall was dangered off but later continued over to the #6 entry at survey spad # 7283. Agencies were notified and a clean up plan was submitted. The fall measured 18'W X 75'L and fell 6-8' high

The injured was cleaning gob out from around the continuous miner bit clip with pick hammer. The hammer glaced off of the bit lug, which caused EE to puncture self in the left hand with the pick end of hammer requiring 3 stitches.

The injured was setting a timber when a small piece of rock fell from the mine roof striking the injured on the bridge of the nose.

Employee was hanging a continuous miner cable and felt pain in his lower back.

Employee was cutting belt in order to make a splice in the belt. He was using a utility knife to cut the belt and he was positioned next to the rib and movement was limited. As he pulled the knife and it cut thru the belt he struck his left thigh with the blade causing a laceration to his leg that required 4 stitches to close properly.

Employee was installing a curtain on an 8 foot ladder, when as he was dismounting the ladder, it twisted and employee rode the ladder to the ground. He tried to catch himself with his right hand and suffered an injury to his wrist.

The injured was plastering a stopping when some liquid off of the plaster splashed in between his safety glasses and his face getting into his left eye. The injured was prescribed prescription eye drops.

We received a letter dated 8/10/15 that informed us that employee had developed Occupational Pneumoconiosis.

Employee was cleaning 8 Mile No.4 belt, when he noticed a piece of rock in the off side of the belt structure. He alleged he used a nearby crossover to access the rock and removed it from the structure. As he was returning and dismounting the cross over his foot slipped on the step and he fell backwards and injured his lower back.

Employee was helping install belt structure when his leg gave way and employee started to experience pain. As he was driving home, he experienced pain again when pressing the brake pedal. A subsequent MRI showed no acute findings for the lower back area but noted degenerative changes. Note: This employee has a record of several occasions with experiencing back pain

Employee reached down to pick up a C/M Cable and felt pain in his lower back.

Unintentional roof fall was found in a return off our 2 Left Mining section. The area where the fall occurred was bolted with 5 foot resin rods with draw rock pans as needed. The fall occurred in the 45th c-cut on 1 left panel between the #6 & #7 entries. Fall measure 20 feet wide at widest point and approx. 45 to 50 feet long and 5 foot high at the highest point.

When travelling the bleeder system identified as Old 8 Mile panel bleeder an unintentional roof fall was found in the #11 entry beginning at x-cut 23 and extending to x-cut #25. Fall was found at 2:00 PM and could not be determined when it had occurred as this is a weekly examined area. All access area has been dangered off.

Unintentional roof fall occurred in the 1 Left Panel X-cut 35, 72 feet inby spad #5985. It fell in the cross-cut between #6 and #7 entries. All access areas and adjoining entries have been re supported with cribs, rope bolts, torque tension bolts and fall is not planned to be cleaned up. Fall did not disrupt ventilation and entries beside the fall area can be travelled.

The injured was tramming the scoop in the #2 entry when he struck an indentation in the mine floor causing him to strike his shoulder in the operators compartment of the scoop.

Employee was working with concrete and some concrete mix got into pant legs and boots causing some burns to employee's legs and feet.

The injured was unloading roof bolt plates from the scoop bucket causing him to strain his lower back.

Employee was loading a tracking node into a track vehicle and strained his lower back. We received written notification on 12/30/15 that the employee has begun missing work because of this injury.

Employee was bolting and attempted to install a roof mat between the roof and a t-3 strap. He lost his balance and fell off of the man-up provided on the bolt machine He dislocated his shoulder and fractured his humorous (upper arm). His shoulder was placed back into socket and his arm was placed in a sling. He was placed on light duty and allowed to return to work the next day.

An unintentional roof fall was discovered in the #1 entry intersection at 24 break on the 8 mile section. The fall measured approximately 20' Long x 20' Wide x 2.5 -3' Thick.

Miner was operating the bolt machine and picked up a bundle of plates to step onto the man-up. He nearly missed the step but caught himself to regain balance. He hyper-extended his wrist. Later in the day he went to the ER, x-ray was negative. He continued to have soreness and was referred to an orthopedic on 1/6/15 where an x-ray revealed a fracture. He was placed on light duty.

The injured got the tip of his pinky finger caught between the concrete stopping blocks he was loading into a scoop bucket and the top edge of the scoop bucket. The injured was wearing metacarpal gloves at time of the incident.

Employee left office to attend the safety meeting. He forgot his notes and turned back striking his elbow on the handrail. A knot appeared on his elbow. He was diagnosed with Olecranon Bursitis. On 1-16-15, he had the knot removed.

After getting off of a ride the employee began to walking when he encountered rockdust and was having trouble seeing the bottom, he slipped on a piece of rock and twisted his right knee.

A Workers Compensation letter provided to the operator on 6-18-14 list a work loss date for 6-12-14 for the employee. A non-medical award determination was issued for Occupational Pneumoconiosis.

As employee was getting on the roof bolter a rock from the top fell on his back and continued down striking his right leg above his ankle. (fractured right fibula)

Employee was loading Kennedy overcast panels and a panel slid off the flat car striking his foot. He continued to work but the pain persisted after stepping in a hole at home while splitting fire wood. He went to the doctor on 5-11-14 and was diagnosed with a fractured fibula. He began missing work on 5-12-14 and notified the mine site on 5-23-14.

Contractor was plastering a stopping and the plaster splashed behind his safety glasses and into his eye. He flushed his eye and continued working but later his eye became irritated. He later went to the hospital. The hospital flushed his eye and prescribed Maxitrol Antibiotic drops.

Employee stated that while loading a communication and tracking node, he strained his back. Employee went to the dr on 2-28-14 and was placed off of work until 3-3-14. He did not report the incident to the mine site and turned in vacation days for his days lost. The mine site was notified by workers compensation on 3-10-14 that an accident had occurred.

Employee rolled ankle while removing curtain. Employee stepped on gob.

Employee was removing a piece of draw rock from the top of the miner to work on the lights. While moving a rock, the employees fingers were struck by a piece of rock lying on the miner. His ring finger was cut requiring stitches and was fractured requiring a surgical pin to repair.

The employee was making a cable splice and started to crimp the burndy clamp. He was using a splice knocker to crimp the sleeves. When he struck the knocker, a piece of metal chipped off and went into his hand. The employee thought that he removed the metal but went to the doctor on 1-13-14 and underwent surgery to have it removed.

The mine site was notified on 5-27-14 that the employee was diagnosed with Occupational Pneumoconiosis on 11-13-13. The employee is filing for a non-medical ruling.

The injured was lowering the man-up on a roof bolter and smashed his finger on the frame of the machine.

An unplanned roof fall was discovered at 23 break along 2 Right #1 belt in the #7 entry. The fall measured 25 feet long x 25 feet wide x 5 feet thick. The fall did not disrupt ventilation or travel.

The injured was cutting open a rock dust bag when the razor knife slipped causing him to cut his forearm.

Employee was lifting a timber and strained his neck. After further review of workers compensation records this incident has been reconsidered. Upon further consideration this incident/injury having been deemed compensable, this incident is being reported as a lost time injury.

Employee was pulling on a piece of line curtain and strained his back. He went to the Dr and was given a pain medication and muscle relaxer. He rtw the next day but was not allowed to work due to the influence/impairment of the medication. Upon further consideration and this incident being deemed compensable, the incident is being reported as a lost time injury.

A block was knocked off of the top of an airlock door and it struck the employees left ring finger. Upon further review of workers compensation records, it was determined that the employees finger was fractured and this injury was compensable.

An unplanned roof fall was discovered in the heading of 2 Right #1 Belt in the #5 entry at the #34 crosscut. The fall measured 15' long x 15' wide x 4' thick. The fall did not disrupt ventilation or travel. The affected area has been dangered off.

The injured was standing in a crosscut when a shuttle car tire kicked up a rock in the roadway striking the injured in the face.

The injured was loading track ties onto the railcar when he was struck on the ankle by a track tie resulting in a fracture.

Please be advised that the operator was notified on 5-7-14 by AIG Workers Comp Claims that the former employee has met the jurisdictional exposure requirements for non-medical acceptance, finding that the 10/15 year presumption that any respiratory impairment is caused by OP.

EE was directing the scoop operator pulling a belt in for the new section at the 3 mains-outby. The chain slipped and EE flagged the crew to check the chain. It was determined that the chain would stay on and the scoop continued to pull. The chain came off and hit EE in the face.

The injured was operating the roof bolter when a rock fell from the top striking his lower back. The injured wasn't diagnosed until 9/12/2012.

An unintentional roof fall occured in the face of the #6 entry on Rattlesnake section at #61 cross-cut on Rattlesnake #2 belt. The fall measured approximately 55 ft long x 20 ft wide x 8 ft high. The surrounding area was bolted with 4 ft resin bolts.

The injured was operating a scoop as it was being trammed off of a lowboy. The blocking kicked out causing the scoop to drop to the mine floor. The injured struck his head against the canopy of the scoop. The injured wasn't diagnosed until 08/30/2012.

The injured stepped between the frame of the roof bolter and the man-up platform while supplying roof bolt plates. This resulted in an muscle strain to his lower back. The injured wasn't diagnosed until Wednesday, July 25th.

The injured was using a chop saw to grind the sharp edges off a piece of key steel he just cut. In doing so, he cut his finger. The injured returned to work on his next scheduled shift.

The injured was pushing a rub rail closed on a continuous miner and smashed his right index finger between the locking pin and the frame of the machine.

Blue Creek #1 Mine discovered an unplanned roof fall at 12:40pm. The fall was located at 11 break, #7 entry intersection on Mains #3 belt outby the working section. No injuries occurred and ventilation nor travel were blocked by this fall. The fall measured approximately 20' long x 20' wide x 10' high.

Employee was hanging highline and pulled something in shoulder.

An unintentional roof fall occurred above anchorage in the #7 entry between #8-#9 cross cuts on Left Mains #3 belt. The fall measured approximately 7' high by 90' long and 20' wide. No injuries occurred and ventilation nor travel were blocked by this fall.

Carrying a bag of glue and hand popped.

Injured was cutting a rope hanger and cut his right thigh.

BC #1 Mine had an unplanned roof fall in the #9 cross cut between entries #5 & #6 on the left mains section.

While making splice hit (L) hand & index finger with hammer.

EE was shoveling belt and rockdusting the feeder cable on the ground was tripped over and the EE twisted his right ankle.

Drilling hole in top and a piece of rock fell out and hit him in face, broke tooth and nose.

EE was loading cribs into sled, a rock fell and hit his left shoulder and fractured his collar bone.

The injured was bolting and as he was drilling his hole, a rock fell from the top, striking his right ring finger.

The injured was helping install a ram jack on a scoop and smashed his right hand between the ram jack and crib blocks.

EE was putting up a 8' bolt in the #5 entry while holding plate his hand was caught between scicorres clamp and plate resulting in a cut to his right hand.

Miner operator was cutting in the No 2 entry when his cable was sliding under the miner. The operator hooked the cable with his right heel of foot to pull the cable out of the way. He felt a sharp pain going down the side of his right leg.

EE was climbing a ladder to get to the top of a overcast, when the ladder began to tip over. He jumped from the ladder landing on his left ankle. He went to the doctor and x-rays revealed no fractures. EE started missing work on September 2 when he was diagnosed with ligament damage.

An unintentionall roof fall occured above anchorage at the Mains Section in the no. 7 face. The fall measured 7' high by 70' long by 20' wide. No injuries occured and No ventilation or travel was blocked by the fall.

Employee was removing a shuttle car tire. As the tire fell of the lugs, it struck the employee in the right ankle resulting in a fracture.

EE had finished his shift and was walking the off side of the belt with another EE to the outside. Between the no1 & no2 enrty is a crossunder for the belt. EE was in front of injured EE and crossed under the belt at the crossunder first and then heard EE scream. He returned to EE and was told by EE that his hood got hung and pulled his left arm into the belt.

Employee was trying to locate a hydraulic leak on equipment by holding cardboard in the area of the leak. When the hydraulics were activated, his hand was in line with the hydraulic leak resulting in a cut finger.
