EE was tightening a mickey bolt with the jackleg when so small pieces of rock fell from the back. The employee stepped back to let the rock fall in front of EE. After the rock hit the ground the employee looked up at the back and was struck in the chin by another small piece of rock.

The employee was bolting up the work heading when ee looked up towards the back a small rock that had worked loose from the wire hit the employee in the upper lip causing a laceration.
While blasting the hung raise from the 5370 level, a rock came down the raise striking the employee in the right hand on the index finger.
At 6:52 A.M. on the morning of the 10th of July, 2014 a rock burst occurred on the 6350 level of the 56 ramp. A SMALL ROCK BURST WHICH WAS NOT BIG ENOUGHT TO REGISTER ON RICHTER SCALE BUT MEASURED 5MM OCCURRED & SOME ROCK CAME OUT OF THE BACK & HIT THE MINER WHILE HE WAS MUCKING. THE ROCK BRUISED HIS SHOULDERS & CAUSED LACERATIONS TO HIS LEFT ARM AND KNEE, WHICH REQUIRED SUTURES.
EE WAS SACKING VENT BAG AND TURNED TO WALK TO ANOTHER PART OF THE STOPE WHEN A ROCK FELL OFF THEBACK AND LACERATED HIS RIGHT FOREARM REQUIRING STITCHES.
THE MINER & HIS SUPERVISOR WERE BEING INTRUCTED ON HOW TO DRILL A ROUND BY A GEOLOGIST WHEN A LARGE SLAB OF CEMENTED SANFILL FELL FROM THE BACK STRIKING THE MINER.
A SMALL ROCK BURST BLEW FLYROCK OUT STRIKING & LACERATING HIS RIGHT ARM & RIGHT CHEEK.
WHILE BACK DRAGGING WITH A 2YRD WAGNER, ROCK FELL FROM BACK. ROCK HIT HIM ON LEFT ARM. IT BRUISED HIS ARM. ALSO A 1/2" LACERATION THAT REQUIRED A STITCH.
CLIMBING LADDER IN VENT RAISE HE WAS WORKING IN A SMALL ROCK FELL ON HIS LEFT SHOULDER. X RAYS SHOWED A HAIRLINE FRACTURE OF THE SCAPULA.
EE WAS MUCKING HIS ROUND OUT. A PIECE OF CEMENTED BACKFILL FELL FROM THE BACK. IT STRUCK HIM ON THE LEFT SHOULDER. HE RECEIVED A FRACTURED SHOULDER SCAPULA.
A SIX MILLIMETER ROCKBURST OCCURRED WHILE EE WASHANGING FENCING FOR GROUND SUPPORT. A ROCK HIT HIS RIGHT ELBOW. THIS CAUSED A LACERATION REQUIRING STITCHES.
SPILING HAD BEEN DRIVEN ABOVE BROW. EMPLOYEE WAS WORKING ON STEEL SET AND WAS UNDER THE SPILING. A SMALL SLAB HAD WORKED LOOSE IN THE BACK AND FELL ON THE SPILING AND BROKE W/PIECE OF IT HITTI NG EMPLOYEE ON THE BACK. THIS WAS AT THE END OF HIS ROTATION. HE THOUGHT HE WAS OK. AT THE STARTOF NEW ROTATION, HIS ARM WAS BOTHERING HIM SO HEWAS SENT TO DR. GOT LIGHT DUTY RELEASE 6 DAYS.
HE WAS OPERATING AN LDH LOADER WHEN A SMALLROCKFELL FROM IN BETWEEN THE FENCING GROUND SUPPORT CUTTING HIS LEFT FOREARM.
EE STATED THAT AS HE WAS INSTALLING GROUND SUPPORT, MATERIAL FELL OUT OF THE BACK, KNOCKED HIM DOWN AND FRACTURED HIS LEFT LEG.
STRUCK BY FALLING ROCK WHILE IN BEANHOLE RAISE.
EE WAS BARRING DOWN LOOSE ROCK USING A DWY BAG ROCK BOLT. A ROCK OTHER THAN THE ONE THE EE WAS BARRING STRUCK HIM ON THE LEFT ARM.
