EE was tightening a mickey bolt with the jackleg when so small pieces of rock fell from the back. The employee stepped back to let the rock fall in front of EE. After the rock hit the ground the employee looked up at the back and was struck in the chin by another small piece of rock.

EE was cutting off a bolt in the rib with a Sawzall, when a rock fell from higher up on the rib and struck the EE in the mouth.

The employee was bolting up the work heading when ee looked up towards the back a small rock that had worked loose from the wire hit the employee in the upper lip causing a laceration.

While blasting the hung raise from the 5370 level, a rock came down the raise striking the employee in the right hand on the index finger.

At 6:52 A.M. on the morning of the 10th of July, 2014 a rock burst occurred on the 6350 level of the 56 ramp. An employee was working at the 5550 level in 34 slot installing wire fencing when he received a laceration on his left knee when he was inadvertently knocked to the ground while attempting to vacate his work area during the burst.

Employee was struck by a rock that fell out of the back while he was bolting the back.

Worker was rock bolting and looked up as a small rock fell from the under the screen and struck his forehead.

While bolting the back, a rock fell and struck the miner causing a laceration. The laceration required sutures.

A miner was struck by falling rock while rock bolting with a jackleg drill. The miner suffered a laceration and break of the left thumb.

Miner was injured by falling rocks due to a rockburst.

Mucking with a LHD and a rock fell out of the back and hit the miner on the wrist.

Employee was bolting the back when a slab of rock came down and hit the leg of the machine. The machine kicked over and the head of the jack leg drill hit him in the left ribs. This was originally considered a first aid injury, but he had some problems later on and had to see doctor again & ended up with a restricted duty release.

Employee was barring down in stope when a small bunch of rocks came down and knocked her hard hat off and caused a laceration to the back of her head. The laceration required sutures.

Employee was rock bolting low on the rib. A small slab came out of the face and hit his hand & rt. index finger while he was holding on to the jackleg drill causing a fracture of his finger.

While Employee was bolting the back a small rock came down and bounced off the machine and hit EE just above the eye, causing a laceration which required 3 stitches.

While removing last monorail in the raise, a small piece of rock fell, hitting the man in neck area causing a 1" laceration.

EE WAS PAINTING THE FACE WHEN A SMALL ROCK CAME OUT OF THE BACK AND HIT RIGHT FOREARM CAUSING A LACERATION ABOUT 3/8" LONG. THE WOUND TOOK 3 SUTURES

A SMALL ROCK BURST WHICH WAS NOT BIG ENOUGHT TO REGISTER ON RICHTER SCALE BUT MEASURED 5MM OCCURRED & SOME ROCK CAME OUT OF THE BACK & HIT THE MINER WHILE HE WAS MUCKING. THE ROCK BRUISED HIS SHOULDERS & CAUSED LACERATIONS TO HIS LEFT ARM AND KNEE, WHICH REQUIRED SUTURES.

EE WAS SACKING VENT BAG AND TURNED TO WALK TO ANOTHER PART OF THE STOPE WHEN A ROCK FELL OFF THEBACK AND LACERATED HIS RIGHT FOREARM REQUIRING STITCHES.

THE MINER & HIS SUPERVISOR WERE BEING INTRUCTED ON HOW TO DRILL A ROUND BY A GEOLOGIST WHEN A LARGE SLAB OF CEMENTED SANFILL FELL FROM THE BACK STRIKING THE MINER.

A SMALL ROCK BURST BLEW FLYROCK OUT STRIKING & LACERATING HIS RIGHT ARM & RIGHT CHEEK.

WHILE BACK DRAGGING WITH A 2YRD WAGNER, ROCK FELL FROM BACK. ROCK HIT HIM ON LEFT ARM. IT BRUISED HIS ARM. ALSO A 1/2" LACERATION THAT REQUIRED A STITCH.

CLIMBING LADDER IN VENT RAISE HE WAS WORKING IN A SMALL ROCK FELL ON HIS LEFT SHOULDER. X RAYS SHOWED A HAIRLINE FRACTURE OF THE SCAPULA.

EE WAS MUCKING HIS ROUND OUT. A PIECE OF CEMENTED BACKFILL FELL FROM THE BACK. IT STRUCK HIM ON THE LEFT SHOULDER. HE RECEIVED A FRACTURED SHOULDER SCAPULA.

A SIX MILLIMETER ROCKBURST OCCURRED WHILE EE WASHANGING FENCING FOR GROUND SUPPORT. A ROCK HIT HIS RIGHT ELBOW. THIS CAUSED A LACERATION REQUIRING STITCHES.

SPILING HAD BEEN DRIVEN ABOVE BROW. EMPLOYEE WAS WORKING ON STEEL SET AND WAS UNDER THE SPILING. A SMALL SLAB HAD WORKED LOOSE IN THE BACK AND FELL ON THE SPILING AND BROKE W/PIECE OF IT HITTI NG EMPLOYEE ON THE BACK. THIS WAS AT THE END OF HIS ROTATION. HE THOUGHT HE WAS OK. AT THE STARTOF NEW ROTATION, HIS ARM WAS BOTHERING HIM SO HEWAS SENT TO DR. GOT LIGHT DUTY RELEASE 6 DAYS.

HE WAS OPERATING AN LDH LOADER WHEN A SMALLROCKFELL FROM IN BETWEEN THE FENCING GROUND SUPPORT CUTTING HIS LEFT FOREARM.

EE STATED THAT AS HE WAS INSTALLING GROUND SUPPORT, MATERIAL FELL OUT OF THE BACK, KNOCKED HIM DOWN AND FRACTURED HIS LEFT LEG.

STRUCK BY FALLING ROCK WHILE IN BEANHOLE RAISE.

EE WAS BARRING DOWN LOOSE ROCK USING A DWY BAG ROCK BOLT. A ROCK OTHER THAN THE ONE THE EE WAS BARRING STRUCK HIM ON THE LEFT ARM.

EE WAS LOADING HIS ROUND. A ROCK FELL 2X2X1/2 DEFLECTING OFF THE LOWER PART OF THE GROUND THE ROCK STRUCK HIM JUST IN FRONT OF THE RIGHT EAR.

EMPLOYEE HAD CUT GROUND SUPPORT WIRE AWZY FROM RIB HE BENT OVER TO PICK UP THE PIECE OF WIRE & A SMALL ROCK FELL CUTTING HIS ARM

ROCK BURST OCCURRED ENTRAPPIG MINER CAUSING SOFT TISSUE BRUISES. MINER WAS RESCUED AT 11:15 AM.

EMPLOYEE WAS LOADING ROUND FOR BLAST WHEN A ROCK FELL BACK STRIKING BRUISING LEFT ANKLE.

WHILE BARRING A TRANSFER POCKET, A ROCK HIT HIS RIGHT FOOT CRACKING THE SECOND TOE.

WHILE CHANGING STEEL ROCK FELL FROM BACK STRIKING & LACERATING LEFT WIST & 3RD FINGER LEFT HAND.
