Miner was scaling with a scaling bar and a rock fell onto the scaling bar and the weight of the rock resulted in downward momentum of the bar and the employee pinched their right hand between the scaling bar and the mine floor resulting in a fractured pinky finger and metacarpal.

While performing start of shift hoist checks, the weight brake would not release once it was set.

Miner was sealing a new airlock door and the door came into contact with left ring finger causing a tuft fracture to the fingertip.

While performing the daily safety checks on the hoist, the hoist operator pushed the emergency stop button to test the weight brake and the hoist tripped on an E-stop fault. Electricians assisted the operator in resetting the hoist. Hoist was back online at 5:53 pm.

Miner was attempting to get a socket seated onto a corroded nut on a vic coupling by hitting the socket with a pipe wrench. EE missed the socket and struck EE's left thumb with the pipe wrench.

At 4:15am the hoistmen was attempting to clutch in when the weight brake set and EE could get it to release. Maintenance was called and found that a safety was tripped causing the hoist to dump air pressure setting the weight brake. maintenance reset the safety bringing the hoist back into operation.

Employee was operating an open cab haul truck and while turning a strand of wire from the wire mesh ground support on the rib caused a puncture wound to the back of the employee's left shoulder requiring two sutures.

Silver Shaft hoist operator heard noise coming from the drum while the hoist was in operation. The hoist operator called maintenance to come inspect the hoist and they found that a saddle and both nuts on a Crosby clamp had come loose. The clamp that came loose was one of 4 extra clamps that were added to hold the tail of the rope on the inside of the drum.

Employee was trimming the front wallplate with a battery powered saw and while making a cut, the saw slipped causing a laceration to employee's thigh. Employee received 8 sutures.

Silver Shaft hoist was down due to east pinion brake trip fault that would not reset.

The hoist tripped due to the gap on the right brake stand-2 upper caliper was detected on the hoist control system to be outside of tolerance at 2mm. The gap was checked and adjusted to 0mm at which time the hoist was reset and back operational. The Silver Shaft Hoist (primary) and 2 Shaft hoist (secondary) were operational throughout.

Lost incoming power to the mine site for a short period of time due to a weather event which caused the hoists to trip. The hoists and compressors needed to be reset and started back up. Hoists were back operational by 7:04 PM.

Employee was transporting a tote of diesel fuel and got a flat tire on the 1-yard forklift they was using to transport the fuel. Employee was pulling into a cutout to get it out of the roadway and as they were pulling in they hit a bump and reached their arm out toward the rib and strained left shoulder. Employee ended up having to get surgery for a rotator cuff injury.

EE was moving a jumbo drill steel and pinched left ring finger between the steel and the metal box on the loader fender.

Miner was mucking into a muck bay with a 2-Yard LHD. They had a full bucket and when attempting to dump it they got on uneven ground and as they raised the bucket the loader tipped over onto the right side. After it tipped, oil and fuel began to leak onto hot components of the exhaust and possibly the turbo which caused the fluids to ignite, resulting in the loader catching fire.

At 12:37 AM the hoist tripped due to a communication fault.

EE was diagnosed with Carpel Tunnel Syndrome after 40 plus years of mining.

After 2 weather related power bumps, the hoist tripped and was not able to be reset due to "Brake Pump #2" and "Safety Circuit Fault" codes.

When the hoistmen tried to reset the Service Hoist it would not reset and gave a YA1 fault. The hoistmen called electricians and while diagnosing they found a bad fuse. Electricians replaced fuse and got the hoist operational.

EE was opening a gate and got fingers caught between the pole for the gate and the fencing.

While the shaft crew were working in the shaft we lost power to the hoist. The electrical dept was able to isolate the issue and get power to the hoist again.

Employee was moving a 45kva transformer unit when the unit caught of the decking and fell over pinching EE's right ring finger.

Employee was finishing mucking scraping the face when something struck the employee in the right side of the forehead.

The hoist tripped out at 4:10 PM. The electricians on duty found a loose relay in the PLC is what caused the hoist to trip.

EE was installing a 18" split set with a Jackleg. As the EE was pulling the jackleg out of the bolt, the jackleg pivoted and pinched EE's hand between the jackleg and the wingnut.

The air pressure to the Silver Shaft hoist got too low to operate the hoist due to two compressors shutting down.

At 2:20 the hoistmen tried to start 2 shaft hoist but it would not start. Electricians were called to diagnose problem and found the pinion break pressure was at 70 psi which needed to be 1500 psi to start the hoist. It was determine that the oil filter was partially plugged causing the issues.

A flashover on the brush holder boxes occurred in the hoist motor causing the hoist to be down.

EE was manually moving a barrel of oil onto the containment when it started to tip. EE tried to stop the barrel from tipping and got hand caught between the barrel and a steel I-beam.

Driller and helper were adjusting the height of the control panel while the driller was getting ready to insert the pin the panel was released and smashed EE's thumb causing a crushed fingernail that required 4 stitches.

EE was re-installing the guard onto the tail pulley of BC-5 when the guard slipped from their hands and pinched 2 fingers on right hand.

Hoistman had a fault on the Service Hoist which would not allow the hoist to operate. Electricians found a faulty brake gap sensor and replaced it.

EE smashed right thumb between lock bar and cage door on the top deck of the service hoist. EE was attempting to open the cage doors while it was stopped at the 5900 level when the incident occurred.

It was determined that the YA1 converter breaker was defective and needed to be replaced. After consulting with ABB's drive specialists and changing out the breaker, the service hoist was tested multiple times without any erroneous YA1 faults and was deemed back in good operation.

At 10:30 pm there was a report that 2-shaft top station was hazier than normal but gas readings with a Ventis pro 5 gas detector were normal. A supervisor bump tested an MX6 gas monitor & accompanied employees back to 2-shaft portal to investigate. When the 2nd air door was opened, there was smoke so they retreated & initiated the mine emergency response plan.

EE pushed the 4' split set bolt into the drilled hole in the back by hand. As EE was positioning self to drive the bolt the rest of the way in with the jackleg, the bolt fell out of the hole and struck EE on the right forearm.

The compressor plant went down causing the mine to lose compressed air. The brakes on the hoist would not release because it takes compressed air to release them.

EE was removing an electrical jumper cable from the rib and felt a "pop" in EE's stomach. EE was diagnosed with a small hernia. EE continued mining at full capacity. EE elected to have a procedure to fix the small hernia on 7/3/2023. EE returned to work after procedure on EE's next scheduled shift of 7/8/2023.

A breaker for the battery backup for the #2 hoist tripped during a severe weather event during the night. This caused the batteries to wear down because they could not charge due to the tripped breaker. Once the battery level got low enough, it tripped another breaker to the Grinnell System. This caused the hoist to shut down.

A lightning strike caused one of our main Avista (Power Company) transformer to break down. We lost all power to the site and could not operate the Silver Shaft Hoist.

EE was struck in the face by a rock that was dislodged from the face. The miner was installing a bolt into the fingers of the bolter when the bolt hit the face, a rock slide down the bolt and struck the employee.

EE was operating a haul truck on the mainline when they felt something go into their eye. EE was taken to the eye clinic and checked out. EE was given a full release at that time. On 11/21/2022 they contacted the eye clinic with discomfort in the injured eye. EE was given an antibiotic to treat the eye making it a reportable injury.

While bolting a piece of rock fell through the chain link fencing and struck the employee in the arm.

Electricians were performing annual battery replacement for the KA1 control cabinet. While replacing the batteries for the hoist controller PLC UPS the breaker feeding the hoist controller PLC was tripped. When power was restored B11.28 I/O module was in error and was causing the hoist trip under several erroneous faults.

The hoist would not reset following beginning of shift tests.

The employee reported feeling something enter EE's left eye while riding down the shaft on the conveyance. The employee reported that EE was wearing safety glasses, but the material entered between the frame of the glasses and EE's face.

EE was standing on the 5900 station awaiting the conveyance for a ride out of the mine at the end of shift. The conveyance went by the station and blew some dust into the EE's eyes. EE was wearing safety glasses but somehow some debris got into EE's left eye and had to be removed by a doctor.

The employee was inside the onsite job trailer retrieving tools when EE tripped and fell forward. Trying to catch EE's fall, EE caught EE's hand on an old plastic bucket that shattered with EE's weight and force, resulting in a laceration of the right hand requiring several stitches. EE was released to full duty the same day.

HOIST WAS DOWN DUE TO COMPUTER PROGRAM PROBLEM.

EE was attempting to uncouple a victaulic coupling from a 4" bull hose that was connected to a compressor. The hose was under pressure and when the EE loosened the victaulic coupling the hose broke free and struck EE in the leg.

EE was tightening a mickey bolt with the jackleg when so small pieces of rock fell from the back. The employee stepped back to let the rock fall in front of EE. After the rock hit the ground the employee looked up at the back and was struck in the chin by another small piece of rock.

EE Was removing the seals on a Wilson-Snyder pump with a 1 ton overhead hoist when the hoist fell from the hook and struck the employee on the hand.

The ESS cooling motor fan failed causing the hoist to lose power. This was due to heat buildup.

Due to a valley wide power outage, the hoist was inoperable.

At approximately 8:05 the electricians hit the E-Stop Button on the KA1 PLC as part of their weekly testing. The hoistman tried to reset the E-Stop but the hoist would not reset and a error code was displayed on the ABB hoist system screen. Electricians diagnosed the problem and cleared the error. The hoist was operational at 9:35am.

Employee was painting the face to drill when a rock fell from the face striking EE's hand.

After the Silver Shaft Hoist was idle for a lengthy time the weight brake would not lift fast enough or high enough to release. A consultant was called in to help diagnose the problem and identified that the brake set regulator pressure was not high enough to engage the brake boost solenoid. The regulator pressure was turned up correcting the problem.

While doing EE's hoist tests on the silver shaft hoist weight brakes the hoistman could not get the weight brake to come back up. Mechanics diagnosed that an air regulator for the brakes was not working correctly. The mechanics changed the regulator correcting the issue.

The silver shaft hoist would not clutch in or out on the north side. Mechanics diagnosed that the weight on the hoist rope from pulling muck for a full shift caused the clutch to stick. The Mechanics chaired the cage on top station after emptying the skip to get any residual weight off of the clutch so it would clutch in and out.

The south brake controller on the Silver Shaft Hoist was tripped and would not reset. Electricians diagnosed a failed UPS unit and replaced the unit.

Silver Shaft Hoist pinion brake would not release.

EE stepped down off of a fixed ladder to a level landing. When EE put EE's weight onto EE's left leg, EE's ankle rolled.

An employee was waiting to get truck loaded, after taking a drink from water jug the employee was putting the water jug back in its holder on the truck and an S hook that was attached to a rope on the water jug, swung and hit the employee on right front tooth causing it to break.

Employee was working as a helper in a ramp when they became overheated.

EE injured back when EE attempted to lift a 6" x 8' piece of thin wall HDPE pipe.

An employee was loading a mini transformer into the back of a vehicle, when employee got it about half way up the employee's shoulder made a popping sound. The employee did not seek medical treatment at that time. The employee's shoulder did not get better so employee was evaluated on 06/17/2021 by a physician at which time employee was put on restricted duty.

EE was using a knife to cut some plastic off of an oil containment tote when the knife struck something and kicked out of the plastic. The downward force of the knife struck EE's leg causing a laceration.

The Silver Shaft hoist was down due to a bad Brake Wear Limit Switch and worn out limit switch roller on the south main brake. Both were changed out.

EE received a laceration to left knee when a rock they were attempting to dislodge rolled down the rib, hit a small ledge and struck them in the knee.

While installing ground support, the employee got finger caught between the machine and the leg.

EE gets middle and ring fingers pinched between traverse slide and rotation slide cylinder nut of drill after resting hand on the side of the traverse slide not realizing EE had put two fingers in the opening for the rotation slide cylinder.

The hoist went down due to a power bump, it caused a YA1 hoist drive to trip. The YA1 was reset and the hoist started to work again.

Due to a large wind storm, the mine lost power and this caused both hoists to become unusable.

Employee was unloading 4'split set bolts in a laydown. As EE was stacking them, one of the bundles slipped and when EE tried to catch it, EE's left hand ring finger got caught under the bundle of bolts and smashed the end of it.

At 9:41 am the silver shaft hoist went down due to electrical issues. The crew was put on stand down while the electricians diagnosed the problem. The electrician determined that it was a programming issue with the limit sensors and corrected the problem. The hoist was back up and fully operational by 11:50 am

Employee Strained back while riding passenger in the lube truck. The driver hit a bump and the employee came off the seat and then landed back down on the seat. The employee was wearing a seatbelt. The employee did not seek medical care until 12/28/2020 when employee was place on restricted duty.

An employee was removing a large nut from the torque arm on the gear box on conveyor, when pulling on the wrench the employees left middle finger popped. The employee did not seek a medical evaluation at that time. On 12/10/2020 the employees finger was swollen and was not improving so EE was taken to a medical facility for evaluation at which time EE was put on restricted duty.

We lost the control input to the main breaker on the drive for the hoist. The breaker would not close when required.

A transformer kicked out causing us to lose power to the Silver Shaft Hoist.

Employee was inspecting a cylinder head when a piece of metal got into eye.

An employee was helping load a trench round in 15-E, EE was bent over loading a hole in front of the vent bag and the EE next to EE was filling a hole full of stemming material, while doing so some of the dust from the dry material blew in to EE's face getting some into EE's right eye.

While pulling muck from 5970 pocket the Silver Shaft hoist tripped with an error code 59 (dump switch south). The hoist man contacted the electrical department where it was determined that a relay had went out.

EE was using an impact wrench with a socket on it to tighten a nut on a yoke. The socket came off while in use and struck the EE in the mouth.

The mechanics just finished their daily checks on the hoist when the hoist tripped out. The hoistman tried to reset the hoist with no success. The hoistman then tried to restart the hoist with the same result. The maintenance department discovered that the governor lever in the lilly's had fallen out of place. With the lever out of place the hoist will trip out & not reset.

EE was cutting off a bolt in the rib with a Sawzall, when a rock fell from higher up on the rib and struck the EE in the mouth.

Lost power to both hoists due to a power outage caused by heavy snow. Power coming into the mine site was lost.

Due to a power bump caused by a snow storm, a trip coil in the MG set contactor for the #2 hoist burned out. A replacement was found and the hoist was put back into service.

Employee felt a sharp pain in the right side of their neck and right shoulder while pushing a 2200# Tote. The employee did not seek medical attention until December 10th after reporting discomfort while trying to sleep. The examining physician suspects a strained rotator cuff and has restricted work to a 20lb weight limit and no over head lifting.

At approximately 11:33 am after performing the weekly emergency brake test on the north side of the #2 Hoist, the hoist would not reset. Troubleshooting and repair could not be completed within 30 minutes so the mine was evacuated.

An electrician was attempting to clean up the exciter comm of the #2 shaft hoist. The electrician made the determination that it could not be cleaned properly and that it needed a new armature. The decision was made to shut down the hoist, thus all underground personnel were evacuated. They will be installing the new armature on 2/5/2019.

A fall of ground was noticed in the 54 ramp off of the 4900 level. The ground fell out past anchorage. the area was barricaded.

At approximately 10:22 am the Silver Shaft Hoist began experiencing problems with the release of the brake weights during the weekly hoist and conveyance inspections. The brake weights would not build sufficient air pressure to release once set. Repairs could not be completed within 30 minutes so the mine was evacuated at 10:45 am.

#2 Hoist began to hoist crew from 4900 level of the mine and the hoist would not build amps. The cage slowly lowered and stopped approximately 25' below the station after multiple attempts by the hoistman to raise the cage. 3 individuals were on the cage at the time and were not able to exit until the hoist was repaired.

Employee was working in the stope and felt something in eye. The employee was unsure when the incident occurred but had 3 small metal fragments in left eye that had to be removed.

While pulling ore from 5970 the north skip was accidentally double loaded. This caused the hoist to trip out due to the weight. All work was stopped and personnel were moved to secondary escapeways. Hoist was returned to service at approximately 15:30. 18 persons were UG at the time of the incident

EE was lifting a wooden board while cleaning up the laydown. When EE lifted the board, EE twisted to side to put the board back down. As EE did this, felt a tear in groin area.

Employee stated they were experiencing ongoing neck pain while bolting. This is the first time the employee has associated it with bolting but has experienced similar symptoms times prior to the date reported. It was determined on 2/27/2017 that this was a reportable incident.

EE was stepping onto a metal ladder when their foot slid on the rung from the left to the right side. Employee turned in a report of the incident, but it did not start to hurt until a few days later. The EE worked for several weeks thinking the pain would go away. When it did not EE was taken to the doctor where EE was diagnosed with a strained knee.

EE was removing the last bolt from the sheave liner and had one hand pulling on the ratchet and the other holding a bar so the sheave wouldn't turn. EE went for more throw on the ratchet and started to turn the bolt to loosen and right pinky finger got caught by the bar.

EE was breaking shipping bands on a pallet of rock bolts and was cut on the left wrist by a broken band.

Rockburst occurred in the 15-Stope 6400 West. Burst went with the round when it was blasted at 17:15. Damage to the ground and ground support was found by the next shift (shift foreman)when they entered the area to investigate after the seismic event was picked up by the local monitoring system. Incident originally reported as per 57.3461

The injured employee hit forehead and nose on the radio mount of the Kubota when the vehicle slid into the rib.

EE was attempting to empty a barrel of grinding balls in preparation for charging the ball mill. When the barrel got hung up EE gave the barrel a push to try to free it up. EE felt pain in lower back, was taken to a medical provider for evaluation and was placed on restricted duty.

EE was pumping resin into a cable bolt hole when the wand for the pump blew apart. This caused the resin to shoot into the employees face. The force of the resin knocked the goggles off and got material in eyes.

EE was using a pry bar to try to adjust liner plates on a bin feeder in the crushing plant. The tip of the bar slipped down to where a moving pan feeder contacted it and caused the bar to lever forward crushing EE's left hand between the bar and the bin feeder.

A section of the cemented backfill in the back fell out during the course of the day. It measured 25' in length. The cause is still under investigation.

EE was bolting up a hole in the wire in the back at the intersection, EE said they drilled by either a bolt or through a slip. When EE pulled the machine back out of the hole it hung up. When EE got it to let go the leg was released all the way and the machine came down all at once.

While a contractor was digging a ditch along an access road they contacted a underground conduit, causing a power bump that knocked out power to the #2 hoist. Three miners were stuck in the shaft for 6 hours.

Employee was cutting a couple of inches off slicker pants, as employee cut with the blade coming towards body, knife slipped and cut left thigh.

At approximately 9:15 AM a miner was cleaning up at face and bucket hit a miss hole which was set off. Hole was approximately 18 inches from the floor. No injuries were sustained. The 1.5 yard JCI loader had a portion of the bucket lip damaged.

Employee was helping install a sand wall, when EE moved the bottom cap to square the wall up EE felt a pop in EE's left forearm. The employee was taken to the local medical facility for evaluation.

A fire started in the G pump power switch in the substation on 5300 Level. The main breaker tripped thus putting out the fire. The mine was cleared of gases and returned to normal mining activity.

At 07:35 am our #2 Shaft hoist experienced an electrical issue which caused the hoist to require repair. The hoist was down until 12:30 to diagnose and repair a failed electrical component. All personnel were hoisted from the mine at 08:10 am and MSHA was notified (Boise Office). At the time of the incident there were 48 Lucky Friday employees and 31 Contractors UG.

The Silver Shaft hoist faulted out during the pre check due to the pinion brake not releasing.

An employee was spotting a co-worker who was positioning a pallet with a forklift when a VF bearing assembly with shaft rolled off of a pallet located next to the pallet EE was positioning and smashed the employees left foot.

An employee was putting a 4" pipe into a warman pump tank when EE's feet slipped causing EE to fall into the tank at which time EE's left hand was smashed between the pipe and the tank.

Employee had arms extended while EE was helping the miners hang chain link fencing to get ready to bolt. When the employee brought their arm back to their side EE felt a pain in their right elbow bicep area. The employee was taken in to the medical clinic for evaluation.

EE and partner were using a jackleg drill for installation of rock bolts. While pulling hard on the jackleg to extract the drill steel from the hole the steel suddenly pulled free causing EE to fall backwards and right thumb got caught between the jackleg and a concrete slab causing crushing injuries to the thumb.

Employee suffered lightheadedness and chest pains while working in the ramp. Employee removed self from the environment and felt better. Employee went to doctor for eval and after several visits to a specialist it was determined to be work related due to heat stress. We received report from doc on 5/23/2016 stating it was work related.

When EE was stepping down from a LHD right foot got hung up in the step causing ee to step down awkwardly onto left leg. EE felt something pop in the calf of left leg.

Employee was attempting to remove a mickey bolt from the back with a 4' drill steel, when the mickey broke loose, the 4' drill steel came back and struck the employee in the forehead.

EE injured right knee while exiting a RTV, Injury was not reported until 3/29/2016 and deemed work related by physician on 8/11/2016. EE had surgery to repair.

The employee sprained wrist while attempting to loosen a wing nut on a 4" bull hose with a hammer. The employee was initially treated and released to full duty, as of 5/19/2016 the injured required a cast and restricted duty to assist with healing of the tendon.

While tightening a bolt at head level, the wrench slipped striking the employee in the tooth resulting in a broken front tooth.

The employee was bolting up the work heading when ee looked up towards the back a small rock that had worked loose from the wire hit the employee in the upper lip causing a laceration.

Employee had just left car and was walking across the parking lot when ee slipped and fell causing ee to strain left knee.

EE was leaning over a rail by the 5900 ore pocket and was using a long hook to pull rock bolts off grizzly. While attempting to pull on a rock bolt the hook and bolt hung up causing EE to feel pain in his groin area. Later while operating a haul truck EE turned to see behind the truck and he felt pain in his back. EE was taken to a medical provider and was put on restricted duty.

At approximately 5:00pm the Silver Shaft Hoistman was performing his checks when the hoist faulted and displayed a North Brake not Released Fault. No Miners were on either conveyance. Electricians found the North Brake Release Limit Switch was out of adjustment. Electricians corrected the problem and pre-shift checks were completed and Hoist was released to Production.

Employee cut his left hand while handling a 15 gallon grease barrel. He was assisting another employee put in the barrel on the lube truck when his hand contacted one of the opening tabs on the lid. The cut required sutures to close.

The employee strained his right wrist while removing an oil cap for the 630. The employee originally returned to work with no restrictions but eventually required wrist surgery in March of 2016.

At approximately 20:38 the mine experienced a complete power outage due to a severe storm causing both Silver Shaft Hoist and #2 Hoist to become in-operable. At this time all the underground employees were instructed to cease all mining activities and were asked to gather at the 5900 and 4900 stations.

Employee was bolting his heading when a rockburst occurred lifting him off the ground causing his neck and shoulders to be strained.

Employee cut his left forearm on a metal mat while reaching for a split set plate. The metal mat was part of the ground support he was installing.

The employee was bolting the lower hole on the right rib with a Jackleg drill, when the employee started to pull the machine out of the hole he felt a pain in his lower back. The employee received medical treatment on 7/24/2015.

EE was operating a haul truck when the e brake set, causing the EE to be jarred in the operators compartment.

The #4 shaft hoist was out of service due to an electrical issue. There were men in the shaft when the hoist malfunctioned. The men were safely hoisted out of the shaft using the emergency escape hoist. The #4 hoist electrical issues was diagnosed, repaired, and returned to service in approximately 85 minutes.

After completing pre-shift checks Hoistman was setting up to pull muck when the Hoist tripped out. Electricians arrived and Hoistman was directed to reset both UHM and HC. Hoist was test ran and released to Production at approximately 6:04pm.

The employee received a minor laceration on his right thumb while using tin snips to cut thin sheet metal.

The employees right hand was pinched between to form panels while attempting to remove the shackle.

The employee received two minor lacerations to the back of his left hand while attempting to cut off and plastic handle on a water jug.

At 5:10 pm a rockburst occurred in 55 ramp at the 6250 sublevel causing the power to the stope to trip out. The rockburst created a bag in the wire in the substation and a crack in the muck bay pillar. The miners in the area were removed for precautionary measures.

Micro Seismic activity was registered in the shaft above the 7500 station. Miners were voluntarily withdrawn for over 24 hours, this allowed time for a remediation plan to be implemented.

At approximately 7:11 pm the Silver Shaft Hoistman was in the process of skipping muck when the Hoist tripped out and could not be reset. The Electrical Department was notified and it was diagnosed that there was a faulty HC inverter.

EE strained his back will drilling a hole in the face with a jackleg. He was attempting to pull the drill steel out of a lifter hole when he felt a pain in his lower back.

An employee operating the Crushing Plant, shut down the plant to do a normal clean-up cycle. As the employee descended the stairs leading to the BC-5 and BC-6 belts he felt his right knee twist and pop.

A Rockburst occurred near 15 stope. No damage was found. The miners in 15 stope were withdrawn due to precautionary measures.

The employee pinched his right middle finger between two drill steel while helping the operator connect the two sectional drill steel.

While blasting the hung raise from the 5370 level, a rock came down the raise striking the employee in the right hand on the index finger.

The PLC on the Silver Shaft Hoist lockup up. Gave the hoist man a warning on the computer screen and the hoist would not run.

EE was operating a forklift. He stopped, got off forklift and bent down to put wheel chocks in place. When he bent over he felt something pop in his back. He reported that he had pain in his lower back and was taken to doctor for evaluation. He was diagnosed with lower back strain and was placed on restricted duty.

EE was working on muck pile to scale and wet down. He stepped backwards and stumbled and fell on his back. He reported pain in his lower back and was taken to doctor for evaluation. EE was diagnosed with lower back strain and placed on restricted duty.

The employee was tying rebar at the 6500 level north bin when after approx. 15 minutes of work he complained of being hot. The employee was told to take a rest period and get some fluids. While at rest and after drinking water the employee became faint and lost consciousness for one minute. The employee was taken to the clinic and received a full duty release.

Seismic activity was registered by the mine monitoring system post blast, the ground movement was in the vicinity of the #4 shaft 7580 station. There were no miners in the area at the time. After blast gasses had cleared Management inspected the area to find some cracks in the shotcrete and other signs of ground movement.

The mine lost power due to severe weather and a large accumulation of snow. The compressors and hoist had to be restarted once power was restored. The outage affected all the surrounding cities.

At 7:38 am the hoist would not reset on the remote estop, the crew reinstalled the jumper & still could not reset the hoist. A faulted estop contact block was found during troubleshooting, a loose wire and a pinched wire on the PA1 control desk. There were no miners in the shaft during this incident. The hoist was restarted, tested, and returned to full service at 9:58 am.

The Mine suffered a Power Bump from local Power Company; when this happens the Conveyance that is near the bottom of the Shaft must be brought back to the Surface to recalibrate PLC.

The employee strained his lower back while lifting bucket tail chains off of a rack.

a malfunction in the pinion brake control system caused the brake to stay engaged. Thus the hoist would not reset, rendering the secondary hoist unusable.

EE suffered two hernia' s in his groin while hanging pipe(air and water lines) in the slot to the stope.

The employee received Shotcrete burns to his Genitalia and upper thigh. It is believed that the Shotcrete on his shirt spread via perspiration to the affected area.

On 9-15-14 E was pushing a 12' dywidag up into a hole drilled in the back when a piece of material fell behind his safety glasses and got into his right eye. E flushed the eye repeatedly. When E returned to work on 9-16-14 he was taken to the eye doctor where a piece of embedded metal was removed from his eye.

Two employees were entering the man riding bucket when one employee kicked muck/shotcrete off his boot, the injured was splashed by wet shotcrete that entered his left eye behind the side shield. The injured began to wash the eye immediately and was later taken to the Dr. He received medical attention in the form of a prescription and had a few days of restricted duty.

A rockburst occurred near 14 stope. No damage was reported however we withdrew the miners for precautionary reasons.

E was placing jackleg drill onto bolt deck after completion of bolting. While laying drill down into holding cradle E's right hand got pinched between drill and holding cradle causing lacerations to his right index finger that required sutures.

EE was helping the miner hang wire in the stope. The dwyidags they were hanging the wire from were rusty and when they tried to tighten the nut on the bolt, rust flaked off and became airborne. The particles swirl around in the ventilation and got around the employees safety glasses.

At 6:52 A.M. on the morning of the 10th of July, 2014 a rock burst occurred on the 6350 level of the 56 ramp. An employee was working at the 5550 level in 34 slot installing wire fencing when he received a laceration on his left knee when he was inadvertently knocked to the ground while attempting to vacate his work area during the burst.

On 07/07/2014 E was operating a 1 yd loader to stack muck in a muck bay. While the bucket was elevated at the muck pile a rock rolled behind the front wheel opposite the operator. When E began backing up with the bucket elevated the wheel rolled up on the rock causing the loader to tip over on its side.

On 07/01/2014 E was pulling on a wrench with his right hand to loosen a nut. The wrench slipped and fell on his left hand pinching his left pointer finger against a bolt. On the night of 07/08/2014 the finger started swelling and turning red. E reported the incident on the morning of 07/09/2014 and was taken to medical care giver and received antibiotics to treat infection.

EE was mucking with a 2 yard LHD along the left rib and felt a pain in his lower back. EE stated he was jarred in the operators compartment and that's when he felt the pain.

Brakes would not release on the #2 Hoist.

The #4 shaft hoist controller malfunctioned causing the hoist to trip out. The electrical department was notified of the problem when hoist failed to reset. The electrician's removed and replaced the hoist controller thus fixing the problem. The hoist became operable at 7:55 AM, was tested, and returned to duty at 8:10 AM.

There appeared to be a software issue with the Profibus for the HC/PLC. The system was repowered/rebooted and was up and running by 10:45 pm.

The Silver Shaft hoist tripped off on over-speed while skipping muck from the 5970 pocket.

E was welding and grinding at #1 Zn conditioner in Mill. A fan was being used to add extra ventilation. When E stopped grinding and lifted his welding hood there was loose debris on the hood that fell away from the hood and the fan blew the debris into his face with a foreign object becoming embedded in his left eye.

An employee was removing Bucket Bits from a loader, as he was removing the nut from the plow bolt the bolt pushed out of its recess causing the bolt to spin. The employee put his hand on the head of the plow bolt and tried to remove the nut, at that time the bolt spun again causing the sharp edge of the bolt head to cut through his leather glove lacerating his finger.

Hoist tripped out with South Conveyance approaching the Dump and the North Conveyance approaching the 5970 Loading Pocket. No riders were on either Conveyance.

The graveyard supervisor was making his rounds checking on the stopes when he found a portion of the back had fallen out in 6250-15 West stope. No one was working in the stope at the time of the failure.

Maintenance personnel discovered a compressed air valve turned off to the clutch pump. This valve was inadvertently turned off earlier in the evening while performing hoist maintenance. hoist was down from 12:15 am to 3:40 am.

E was lifting bundles of split bolt plates by hand transferring them from a loader bucket to a shipping container. while lifting bundles he felt a "pop" and pain in his right wrist. E had right wrist put in a splint and is on restricted duty.

A 24vdc to 120vac converter went out. Had to change out faulty invertor.

The employee was walking up the muck pile when the pile began to slough out from underneath the employee's feet. The employee instinctively put his hand against the rib to stabilize himself from falling cutting his hand on the rock from the rib.

EE was burned on his right bicep. EE had been bolting with a 4' split set driver and leaned it against his bicep while it was extremely hot from driving in split sets.

Our ESG seismic system picked up a rock burst at 3:01 pm on the east end of 15 stope at day shift blasting time. At 5:00 pm it was confirmed by the night shift crew that a rock burst did occur with the day shifts blast and did damage to the east side of 15 stope, dislodging 5ft of the back for a distance of about 16ft at the end of the stope.

Employee was struck by a rock that fell out of the back while he was bolting the back.

Cementation's #4 Shaft Hoist experienced a power interruption determined to be a converter board. There were seven (7) men on the shaft bottom at the time of the power interruption. They were evacuated from the shaft bottom by means of the Egress Hoist. Approximately two (2) hours later the Hoist was repaired, ran, safety checked, and returned to service.

A rockburst hit the 08 stope at 8:32 am with no damage occurring. a small series of rockbursts continued for the next 45 minutes. around 9:20 am the decision was made to withdraw the miners from the stope for precautionary reasons. The stope settled down and afternoon shift went back into the stope to work.

EE received cement burns to both feet while walking through water after the shotcrete crew cleaned off the cement trucks.

Monitor Screen locked up. Hoist would still operate. ABB dialed in remotely and determined that spare PC installed on Oct 20th had too many programs running. ABB Tech Support suggested we switch our built in spare computer and the PA1 Computer. Swapped computers around. ABB did remote testing & released the Hoist at 3pm.

Main Computer PC in PA1 Cabinet failed. No personnel were at risk. Changed computer to Maintenance computer.

Transformer Fan Warning lamp failed

Crews were transported underground without event. The Hoistman began the required brake tests & identified the Pinion Brake. A Brake test rating of 1500 could not be attained by a small margin. Crews were idled while the corrections were made. Maint came up the secondary & buffed the contacts. Normal hoisting was resumed and crews were released for work.
