1 RIGHT 5 SOUTH LONGWALL FACE. MAN WAS BETWEEN #1 SHIELD AND MINE RIB, SHIELDMAN ON #2 SHIELD RAISE TOES OF #1 SHIELD. MAN SLID CRIB BLOCKS UNDER TOES OF #1 SHIELD TO RAISE TOES OFF THE BOTTOM . WHEN FINISHED HE STEPPED BACK AND RIB ROLLED OUT BETWEEN TWO DUKE JACKS AND HIT HIS LEFT KNEE.
LOCATION, IN A CROSSCUT ALONG THE MAIN LINE AT 14-07. MAN WAS PUTTING UP A PANEL FOR A KENNDY STOPPING. A SMALL THIN PIECE OF SLATE FELL FROM THE ROOF HITTING HIS RIGHT ARM BETWEEN THE CUFF OF HIS PROTECTIVE GLOVE AND HIS SHIRT SLEEVE.
A PIECE OF ROOF FELL OUT BETWEEN STRAPS (5" X 8"X 3 1/2'). IT HIT TOP OF MAN'S HARD HAD KNOCKINGHIM TO THE GROUND.
SETTING BITS AND ROCK FELL AND HIT TOP OF HAND. DID NOT INVOLVE ANY ASPECT OF COMLIANCE WITH RULES AND REGS. DID NOT INVOLVE MINE EQUIPMENT OR MINING SYSTEM. MAY HAVE INVOLVED PROFICIENCY BUT NOT TRAINING OR ATTITUDE. MAN HAD PROTECTIVE GLASSES ON.
