The employee was standing on left side of #32 Miner. The miner was off. EE was taking the control box off when a piece of draw slate (1.5"x1'x1') fell out of the top and struck the back side of EE's right hand causing a 2.5" cut. The following were not a factor: rules, regs, mining equipment, protective devices on equipment or clothing, skills, proficiency, training or attitude.

The employee was attempting to place a hook on a shield when a small rock struck him in his right hand. The following were not a factor: rules, regulations, skills, proficiency, training, protective clothing or protective devises on equipment. Attitude may have been a factor. Miner did not start to lose time until 11-2-15.

Miner was unrolling and hanging recovery roof screen on the longwall face. during this process a section of rock fell onto the screen and made contact with his hand. the miner suffered a fracture and received stitches to his hand. The following were not a factor: Regs., rules, protective devices or clothing, equipment, training. proficiency may have been a factor.

The miner was struck, on his upper back, by a section of stone that fell from the mine roof or rib. The following were not a factor: Rules, equipment, protective devises or clothing, skills, proficiency. Attitude may have been a factor.

The employee was operating the trail motor on a supply train. As the train was moving down the haulage a small section of stone fell from the roof. The employee reacted and sprained his knee. The incident did not involve: Rules, Regs., regulations, clothing, protective devices, attitude, skills, or proficiency.

The accident did not involve: compliance with rules or regulations, mine equipment or mining systems, job skills, miner proficiency, training or attitude, or protective clothing, devices on equipment. The employee was struck in his upper back, by a section of rock while tightening crib. He received a contusion. Did not start to lose time until July 17, 2014.

The employee was rockdusting an entry with a power duster. He rockdusted the roof and a piece of stone fell out of the roof striking his hand.

Employee was crawling in low roof when a piece of stone struck him from the roof where the mat screen was separated.

Employee was standing next the rear bumper of the roof bolter with his back to the line curtain. A piece of stone struck him from behind the line curtain on his back. Began losing time on 9/30/13.

Employee was assisting another employee to install fire suppression in a 12 foot high area. Stone fell from the corner of the roof and struck the employee on his hard hat.

Employee was in the process of rolling up canvas. As he rolled up the canvas a piece of stone came down and hit him on the neck. Began losing time on 2-21-13.

Employee was walking down the longwall face. He stopped at #30 shield and some stone started to fall from between the shields. He had his hand on the the hand rail and the stone struck his hand. He sustained a fractured RT. thumb

Employee was working the weekend warrior shift and was standing at the headgate on the longwall when a piece of stone fell from between the roof bolts as the shearer cut out.

EE bent over to pick up bucket, when a rock fell from mine roof and struck his lower back.

Employee was roof bolting at the face when a rock fell from the mine roof and pinned him against the drill head. The employee was taken to the emergency room. He was found to have bruising and swelling in his right hip. Began losing time as of 1/31/09. *Employee began losing additional time as of 3/30/09*

Drilling through a piece of stone that was left hanging. The stone fell from the roof and struck him in the head above the left eye. He received 5 stitches. He returned to work the next day.

EE was examing fresh cut coal face, when a piece of rock, approx 4' X 3' X 2", fell from the top. The rock struck the EE's hand, smashing it against the boom of the roof bolter. This caused a laceration on EE's left hand, right shoulder and lower leg.

Bolting outer bolt, completed,raised stab jack, went to swing to bolt inner. Roof coal fell out striking on back leg.

EMPLOYEE WAS BOLTING UP SCREEN IN A REHAB AREA, WHEN A PIECE OF STONE FELL OUT BETWEEN THE MESH, STRIKING HIM ON THE RIGHT EYEBROW.

Employee was rebolting a rehab area when a piece of stone fell between roof bolts (rib side) striking him on the left lower lip.

EE WAS ROOF BOLTING IN THE FACE. WHILE ADDING A SECOND DRILL STEEL TO THE HOLE, A PIEVE OF ROCK FELL FROM THE ROOF AND STRUCK HIS LEFT FOREARM

Employee was walking to his work area, when a stone fell from roof striking him on the head and neck, fracturing 2 two vertibrae.

EMPLOYEE WAS SERVICING THE LONGWALL SHEARER. HE HAD PULLED IN 3 SHIELDS OVER HIS WORK AREA. HE WAS CHECKING OIL IN THE RANGING ARM WHEN A STONE FELL FROM THE TOP BETWEEN THE SHIELD TIP AND THE FACE. THE STONE HIT THE TOP OF THE CUTTING DRUM, BROKE UP IN PIECES, AND A PIECE OF THE STONE HIT EMPLOYEE ON HIS RIGHT FOOT.

EMPLOYEE WAS WALKING FROM MINER TO POWER CENTER.A PIECE OF SLATE FELL OUT BETWEEN THE 2ND AND 3RD ROW OF BOLTS STRIKING HIM ON TOP OF HARD HAT AND JAMMED HIS NECK, SLATE WAS 6' LONG 2' WIDE AN D 2' THICK. NO DAMAGE OR IMPAIRMENT TO THE MINING OPERATION OR EQUIPMENT OCCURRED. AWARENESS OF ROOF CONDITIONS MAY HAVE CONTRIBUTED TOT HE ACCIDENT.

EMPLOYEE WAS ADVANCING SHIELDS ON THE LONGWALL FACE. HE HAD HIS HAND ON TOP OF THE HANDRAIL OF THE PANLINE. A PIECE OF STONE FELL FROM THE TOP BETWEEN THE SHIELDS AND HIT THE TOP OF HIS HAND . THERE WAS NO DAMAGE OR IMPAIRMENT TO THE MINING OPERATION. THE INJURY DID NOT INVOLVE ANY ASPECT OF COMPLAINCE WIHT REGS , JOB SKILLS, PROFICIENCY OR ATTITUDE. EMPLOYEES ARE TRAIN IN POSSI
