While using a hammer and chisel to knock out bolts after being torched, IE received a sliver of metal in the web of EE's hand between the thumb & index finger. Cause of this injury was awareness of this employee. Compliance with company policies & PPE may be a factor along with miner proficiency. Regulations/mining equip/systems, not a factor.

The employee was taking the draw bar out of an off track lowboy. The motor pulling in bumped the lowboy and caused it to roll over EE's foot before EE could get out of the way. Filing under protest to comply with a citation from MSHA. An employee was tramming a motor through the South Mains Dip. An unattended motor that was up the hill drifted down and caused the collision. 1 employee was entrapped in the #2 Portal elevator from 8:30am to 10:15am. A capacitor went bad causing the electrical system to shut down. The capacitor was replaced which corrected the problem and the elevator was put back in to service.

Stone fell in the walkway and impeded travel at 42xc of the South 60 Return just inby the #1 Left Side Seal. It did not fall above the anchor point. Stone was in the walkway for 15' and the area was supported with floor to roof support the next shift. Weathering of the roof was determined to be the cause.

The employee was operating a scoop car when it ran over a bolt that was buried in mud. When the tire ran it over, the bolt flipped out of the mud and struck employee in the lip. 9 employees were entrapped in the #2 Portal elevator from 5:56pm to 6:34pm. The elevator contacted the low over travel switch. It was adjusted in to the proper landing zone and the fault was corrected.

The employee was moving a de-energized cable when a piece of cable struck EE in the lower back. The employee claims pain in lower back due to this contact. The ee was dismounting a dozer. While maintaining 3 points of contact, ee strained shoulder when used arm to put weight on hand rail. The employee was standing on left side of #32 Miner. The miner was off. EE was taking the control box off when a piece of draw slate (1.5"x1'x1') fell out of the top and struck the back side of EE's right hand causing a 2.5" cut. The employee was carrying a bundle of 5 roof bolts to supply the bolting machine. When he lifted the bolts to place them in the tray, he felt a pain in his left shoulder. The employee was using both hands to put his trolley pole on the wire. When the pole sprung upward, he caught his right hand between the pole and a beam. The employee was attempting to climb the ladder in to a 777 truck at the start of the shift. He fell backwards while trying to get on the first step and injured his back. On 10/9/15 a roof fall was discovered in an air leg that is in common with the Main West Belt between 29.75xc-30xc. Weathering of the roof was determined to be the cause of the accident.
