EE WAS TRAMMING A TRACK MOTOR AND A PIECE OF DRAW ROCK FELL AND BROKE THE LIFE LINE. A HOOK FROM A ROPE HANGER ATTACHED TO THE LIFE LINE STRUCK HIM ON THE THUMB, RESULTING IN A CUT.

While traveling to the section, EE was operating a mantrip and a piece of draw rock from between bolts and struck him on the right side and lower back.

EE was in the No. 3 entry of the 003-004 MMU bolting top. He was positioning his drill steel against the mine roof and a piece of draw rock fell and struck him on the left hand causing a cut on the left ring finger.

EE was cleaning out the dust box on the roof bolter and a piece of draw rock fell and struck him on the left arm, resulting in a cut.

EE was operating a continuous miner on the 004 mmu section and was turning the right crosscut in the #7 entry. He was positioned on the left side of the entry, approximately 12 ft. inby the miner and a piece of draw rock fell from between bolts and struck him on the head, back, and legs. The rock was approx. 7"thick x 4' wide x 9' long.

EE was operating a roof bolter. He was drilling a hole in the mine roof and had his left hand on the drill pot. A piece of draw rock fell, stiking the left hand, and breaking the middle finger.

EE is saying a piece of rock fell on his left shoulder while he was plastering a stopping. Didn't report to foreman. Didn't fill out accident report. Took 8 hours before he went to hospital - not even a scratch. (I'm protesting this).

EE was installing roof bolt in the top, when a piece of draw rock fell from between the TRS& Canopy, hitting him on the back and right knee.

EE was installing the second bolt in the bolting sequence when a piece of draw rock fell from between the rib and the first bolt, striking him on the lower back and the legs.

Setting on tool box rock fell out between bolts and hit him in back between shoulder baldes.

EE was operating a continuous miner in the #2 entry of the 002-0mmu when a piece of draw rock fell, striking a shuttle car, then flipping off on him, injuring his back. EE was treated and released at Pikeville Medical Center.

EE was talking with the continuous miner operator in the #2 entry of the 002-0 mmu when a piece of draw rock fell, striking a shuttle car then flipping off onto him, injuring his left arm, right foot and head. EE was treated and released at Pikeville Medical Center.

EE was operating a shuttle car in the #3 entry of the 002-0 mmu. He was hauling a load of coal to the feeder when a piece of draw rock, approx. 24" wide, 41" long & 3" thick fell from between the bolts and struck him on the head, shoulder and chest. He was transported to the medical center and admitted for observation. Accident occurred in the first crosscut outby the face.

Employee was operating a continuous miner in the #1 entry of the 002-0 mmu. He was sitting on his knees when a piece of draw rock approx. 3' long, 1' wide and 1" thick fell and struck him on the left knee. He continued to work at his regular duties and did not seek medical attention until 4/19/05, at which time he was recommended for restricted duty until next dr's office visit.

EE was operating a continuous miner in the #8 heading of the 002-0 section. He was backing the miner up to change sides of the cut when a piece of rock fell on the miner. The rock broke, and a piece of rock (approx 5 1/2' long, 2 1/2' wide, & 3" thick) flipped off miner and struck EE in the head & back. - Broken back.

EE was walking to the surface after servicing the #2 head drive when a piece of draw rock fell, striking him on the head, causing neck and back pain.

EE was walking around a continuous miner that was being serviced by section electrician when a piece of draw rock apprx. 12" wide, 12" long and 1" thick fell, striking him on the right hand and causing laceration to right thumb. accident occured int he #3 entry of the 003 mmn.

EE WAS HELPING ON CONTINUOUS MINER CABLE WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON TOP OF HARD HAT AND THEN SLID DOWN HIS BACK AND LEFT LEG. HE WAS TAKEN TO HOSPITAL ER. DIAGNOIS OF SPRAI NED NECK AND RECOMMENDED 3 DAYS OF LIGHT DUTY WORK. RETURNED TO REGULAR DUTY 6/23/03.

WHILE RIDING INSIDE A SMALL PIECE OF DRAW ROCK FELL STRIKING EMPLOYEES LEFT HAND CAUSING LACERATION.EMPLOYEE RECEIVED 6 STITCHES.

INJURED WAS FLOATING MINER OPERATOR OUT FOR DINNER HE BACKED THE MINER UP TO EVALUATE THE ROOF WHEN HE DID THIS HE WAS LOOKING TOWARD THE FACE OF A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS ( I6" THICK &24' SQUARE) BEHIND HIM STRIKING HIS LEFT SIDE & LANDING ON HIS LEFT FOOT

INJURED WAS PREPARING TO ENTER MANTRIP WHEN A PIECE OF DRAW ROCK FELL CAUSING A SCRAPE & BRUISE TO LOWER BACK

A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS STRIKING INJURED IN THE HEAD.

THE INJURED EMPLOYEE WAS STANDING BEHIND THE MINER OPERATOR (OPERATING A REMOTE MINER) ALONG THE RIGHT RIB OF THE #3 ENTRY ON THE 001 WORKING SECTION. A PIECE OF SLATE FELL (APPROX. 30" X 24" X 3") BETWEEN THE RIB AND THE 48" RESIN GROUTED ROOF BOLTS AND STRUCK THE EMPLOYEE'S LEFT ARM AND SIDE.

EMPLOYEE WAS LYING ON THE MINE FLOOR ON HIS LEFT SIDE WORKING ON THE STAB JACK ON A FLETCHER ROOF BOLTER A SMALL PIECE OF SLATE (4INX4INX1/2IN)FELL FROM THE ROOF & STRUCK HIS RT EAR

MINER OPERATOR WAS STANDING, HOLDING THE REMOTE CONTROL BOX, AND TRAMMING THE MINER. A PIECE OF SLATE (APPROX. 8"X14"X1"0 FELL AND STRUCK HIM ON TOP OF HIS LEFT HAND.
