Mine #15 Coal

EE travels the air ways ee was in the process of walking to the date board to date up. EE was watching were ee was stepping with head down and when ee looked up ee came into contact with a piece of draw rock that was hanging down. It injured nose and left eye. EE was taken to the hospital were ee was treated and released.

Employee was hanging a miner cable overhead, they slipped and injured right shoulder.

Employee was building a stopping. When installing a cap wedge employee hit thumb with a hammer

EE said EE was going to pick up cat head and it arched and burnt EE's right

Employee was servicing a Joy 14-15 continuous miner. EE was on top of the miner when EE slipped and fell to the ground (approx. 6-7 feet) and landed on EE's face. EE was taken to the hospital by ambulance and was kept one day at the hospital. EE was diagnosed with a concussion and released. EE will be kept off from work for at least 30 days.

While putting the power blades in at the splitter box, the employee fell on right shoulder. Employee did go to the doctor but missed no work at that time. Employee has now been taken off from work due to that accident.

Employee was riding to workplace in rail man trip when EE said it hit a bump injuring EE's lower back. EE completed the shift and went to the doctor the next day. EE was taken off from work for 3 days for a pulled muscle. After the 3 days EE went back to the doctor and was taken off until 04/25/2018 and EE returned to work on that day.

Employee and another worker were lifting a shuttle car tire when employee injured left hip and leg. Employee was taken to the hospital and has not returned to work. Employee will receive an MRI to diagnose.

Employee was pulling a rock out of tail piece. A rock became fouled at the 4a Head Drive # 1 Tailpiece. Employee was pulling the rock out when EE Strained/sprained EE's right lower back. EE took self to the hospital to be checked. EE was taken off from work for 2 days. EE returned to work on 02/14/2018 with no restrictions.

Employee was loading crib blocks when EE's little finger on EE's right hand was mashed between two blocks, as they rolled on EE's finger. EE was taken to the hospital where EE received 4 stitches. EE returned to work with no restrictions.

Employee was repairing a railcar coupling, weighing about 200 to 250 lbs. EE picked up the coupling several times and as EE rotated or twisted EE's back EE strained/sprained EE's lower back. Took self to the hospital the following day. EE missed two days of work and is now back to work on 01/19/2018.

Employee was lifting a lid that had fallen off of the bolt machine, to put it back on. The lid weighed approximately 70 lbs. Employee twisted their back trying to lift the lid alone. Employee was taken to the hospital and has not returned to work.

Employee pulled a rock weighing approximately 200 lbs. from the top of the continuous miner onto ee right foot. EE finished the shift and took self to the hospital where an X-Ray showed a small cracked bone in the foot. EE missed 2 days of work and returned to full duty.

IE was trying to help operator get EE's steels out of EE's hole in the roof at the roof bolt machine. IE spun the steel slightly and IE's right glove became stuck causing it to twist IE's ring and pinky fingers on IE's right hand, resulting in fractures of both fingers. IE took self to the hospital and has missed no work.

Replacing rub rail on continuous miner. Employee bent over to pick up rub rail and pulled lower back. Employee took self to the hospital and has not returned to work.

Employee was working on miner in face when a rock fell on ee back pushing ee down into the 14/15 Joy miner, fracturing both collar bones and ribs on both sides. EE was hospitalized and received surgery.

Employee was removing the rub rail from the continuous miner and it slid off the miner and struck left big toe.

Very small piece of metal embedded in right thumb from hammering on miner chain. Did not seek medical attention until 08/10/2017. Had metal surgically removed at hospital on 08/16/2017. Employee missed no work.

Employee injured right knee as ee attempted to rise to a standing position while sitting down. The employee went for an MRI at a later date and has now had knee surgery for a torn meniscus.

Employee was walking through an open air lock door when the door closed back on employee's left foot. Employee was taken to the hospital to be checked. Foot was only bruised and employee was taken off work for two days. Employee is now back to work.

Employee was putting a scoop tire back on a DBT scoop using a pry bar. The pry bar slipped, trapping their right thumb between the scoop tire and the wheel. This resulted in a laceration to their right thumb. The thumb required 6 stitches to repair the laceration. Employee was treated and released and missed no work.

Employee was setting up a breaker on a 995 volt miner when the cst head blew out burning both hands of the employee. Both hands received second degree burns. Employee was off from work for 20 days and returned to work on 6/14/17.

Employee was using a manual hand pumped grease gun to grease rollers on a conveyor belt line when they injured the right arm and shoulder. They took themselves to the hospital and missed no work until today when they brought in a doctors excuse stating that they will be off from work for 16 days.

Employee was bolting roof when a rock fell from roof on rib side and struck right foot. Employee was taken to hospital and had a contusion on foot.

Employee was putting up bolt and bolt bent. EE then spun the bolt around and the plate struck right forearm lacerating it.

Employee was crossing the belt line with the dust hose in employee's hand when employee fell striking the track rail. Employee bruised employee's ribs

Employee was replacing a panel on a shuttle car when a piece of draw rock approx. 23" X 8' long and 2 1/2 to 5" thick fell between bolts and slid down EE's back before striking EE's left foot. Foot wasn't broken but EE was hospitalized in order to receive antibiotics through an IV. EE was released on 03/26/2017. EE returned to work on 03/27/2017.

Employee was walking and employee slipped on mud falling to the ground. Employee struck employee's right knee and hip on the ground and the track rail underground.

Employee and one other person were lifting a tire from the mine floor. Employee finished shift and reported ee had strained lower back but did not intend to seek medical attention. EE did go to the doctor the next day and has not returned to work.

Employee struck left little finger with a hammer.

Employee was putting a shield back on the continuous miner. The shield, weighing 50 to 75 pounds slipped and mashed the tip of the middle finger on left hand, bruising the finger. Employee continued to work that shift and until 10-03-2016. Went to the doctor on 10/04/2016.

This is to report a miner that has exercised EE's rights as a part 90 Miner.

EE was walking towards power center. EE slipped and fell, left leg twisted behind EE. The weight of the EE's body landed on left leg.

Logic Box on hoisting car Burnt up. This is on our secondary escape way.

EE was operating track mounted mobile equipment. EE was using the sanders to disperse sand on the track to obtain traction. EE caught his left thumb between the sander bar and roof of mine causing a laceration on EE's thumb.

EE was working on continuous miner. A piece of draw rock fell from the top striking the EE in lower back.

EE was making splice in S/C cable. Knife slipped cutting EE on the back of the right thumb. EE received seven stitches.

EE was shoveling on his knees, a piece of rock fell striking him in the lower back.

While bolting top, piece of steel hung in hole. Tried to pull piece of steel out of hole and strained left shoulder.

EE was operating the C/M. EE was struck by a piece of draw rock on the right leg.

EE was fixing a slip roller on conveyor belt. EE slipped and fell hitting his mouth which resulted in the EE breaking a portion of his tooth.

A non-injury accident occurred at this mine at 2:18 P.M. The slope hoist brake car sustained damage to the PLC control system and would no longer function in full capacity. All miners were removed from the entire underground portion of the mine.

Top of hydraulic jack ruptured blowing hydraulic oil on EE's upper body. A portion of the oil went into EE's eye. EE refused an ambulance but later went to the doctor on his own.

EE had put last row of bolts up. The pin crew had backed up to install a cable bolt. EE was trying to remove a broke piece of steel out of drill pot and a piece of rock measuring 24"x18"x3" thick fell striking EE on the back of the neck and left hand. EE received two stitches in his left hand.

There has been an unplanned roof fall located in the No.1 entry 1 crosscut out-by survey station No. 3762. This fall does not impede travel and there has not been any disturbance in the mine ventilation. This fall has already been reported by phone to the MSHA hotline and a roof control inspector has been at the mine to investigate the fall.

While foreman and State Inspector were traveling Intake airway they discovered a roof fall in the #4 southwest mains measuring 20 feet by 20 feet by 8 feet thick. In the #2 entry.

While lifting a piece of ice off Bonnie Car he felt sharp pain in back and down leg.

The main electrical power that supplies the mine had been off. When the electrical power was restored, the elevator would not operate. No one was underground at this time. MSHA District 6 was notified of this condition.

EE was cutting a steel beam into. The beam was lying across the forks of a forklift. When the beam was cut into, one end of the beam slid off the forks and landed on the concrete floor, then bounced and struck EE on top of his right foot.

EE was loading a metal toolbox into a rail runner and when he lifted on the toolbox, he felt a sharp pain in his low back.

EE was walking down the slope beltline and slipped and fell, causing pain to his lower back. He was seen by his family DR. on 9/3/2013 who took him off work until his next Dr. visit.

PRE SHIFT START TIME EMPLOYEE GETTING STUFF OUT OF HIS VEHICLE AND WAS STANDING TO CLOSE TO DITCH BERM AND DITCH GAVE WAY AND EMPLOYEE TUMBLED INTO DITCH. FIRST AIDE ADMINISTERED AT HOSPITAL. EMPLOYEE STATED DR. DID NOT PUT HIM OFF FROM WORK, BUT HE CAN'T COME BACK YET

A non-injury roof fall occurred on 004mmu in #4 entry approximately 106 feet inby spad#4962 during retreat mining the roof fall on the continuous miner they were taking the last lift from the left pillar block

EE WAS TRAMMING A TRACK MOTOR AND A PIECE OF DRAW ROCK FELL AND BROKE THE LIFE LINE. A HOOK FROM A ROPE HANGER ATTACHED TO THE LIFE LINE STRUCK HIM ON THE THUMB, RESULTING IN A CUT.

While walking bleeder entry with MSHA inspector on 12/10/13 Glade Taylors back gave out causing him to fall in some rocks. He felt it was caused by an accident he had earlier in the year,at this time he felt pain in neck and back his first day off was 12/11/13.

While traveling to the section, EE was operating a mantrip and a piece of draw rock from between bolts and struck him on the right side and lower back.

The shaft hoist failed to operate which eliminates one of the two required escapeways.

EE was working on a regulator and while lifting cinder blocks his back started hurting. He said the pain got worse as the day went on.

EE was pulling a miner conveyor chain with a scoop and another chain. The chain attached to the scoop broke and struck EE on the left knee resulting in a cut requiring three stitches.

EE HAD DRILLED A HOLE IN THE MINE ROOF AND THE DRILL STEEL STUCK IN THE ROOF. WHEN HE LOWERED HIS DRILL POT, THE DRILL STEEL FELL FROM THE HOLE AND STRUCK HIM ON THE LEFT HAND.

EE was lowering the head of the continuous miner on some crib blocks that were being used to block the head up. When pressure was applied to the crib blocks, one crib block twisted out and struck EE on the right jaw.

Another miner was cutting a 42" belt with a Razor Knife, When employee walked by and was cut on the calf with the knife.

A NON INJURY ROOF FALL OCCURRED IN THE NO. 9 RETURN ENTRY AT SURVEY STATION NO. 1359. THE FALL IS APPROXIMATELY 18FT. WIDE X 7FT. THICK X 30FT. LONG. THE FALL WAS REPORTED TO MSHA BY TELEPHONE ON 9/20/2011.

EE was holding onto a shuttle car cable as a certified electrician was making a cable splice. As the utility knife came out of the cable jacket, it cut EE between the fingers on his right hand.

EE was standing beside a scoop that was unloading a scoop battery charger. A chain that was attached to the charger became fouled on a hook on the scoop ejector, causing the hook to break and strike EE above the right eye, resulting in a cut.

EE WAS AT THE CONTINUOUS MINER TALKING WITH THE OPERATOR ABOUT A NOISE THE CUTTER HEAD WAS MAKING. THE CONTINUOUS MINER WAS IN THE RIGHT BREAK OF THE #4 ENTRY AND EE AND THE OPERATOR WERE STANDING IN THE HEADING. THE CONTINUOUS MINER CUTTER HEAD WAS RUNNING AND EE WAS STRUCK IN THE LEFT EYE BY A FOREIGN OBJECT.

EE WAS HOLDING A HEADER BOARD BETWEEN THE SCOOP BATTERIES/BUMPER AND THE SHUTTLE CAR BUMPER TO PUSH THE SCOOP OUT OF THE HAULROAD. WHEN THE SHUTTLE CAR PUT PRESSURE ON THE HEADER BOARD, HIS THUMB WAS CAUGHT BETWEEN THE HEADER BOARD AND THE SCOOP BATTERIES.

EE was cleaning out the dust box on the roof bolter and a piece of draw rock fell and struck him on the left arm, resulting in a cut.

EE was adjusting an slip/sequence sensor that was attached to the top belt structure on the conveyor belt. As he was adjusting it, his hand slipped and his arm went under the belt and over the top roller. When he pulled his arm to free it, he cut his arm on the top structure.

A ROOF FALL OCCURRED IN THE #7 ENTRY OF THE 2ND NORTHEAST MAINS. THE FALL IS APPROXIMATELY 20' LONG X 19.5' WIDE X 7' THICK. THE FALL IS LOCATED ONE X-CUT ACROSS FROM SURVEY STATION 484.

EE had walked to the continuous miner to talk to the operator. When he squatted down beside him, the coal rib rolled off and struck him on the back and lower legs. This resulted in a hairline fracture to the right lower leg and three fractures to the left lower leg.

EE was pulling a continuous miner cable from under a piece of rock and the rock slid down and cut his finger.

EE was using a lifting jack to put a supply motor back on the track and strained his back while applying pressure to the jack handle. EE worked regular duties until he sought medical attention on April 8, 2010 and was taken off work.

A roof fall occured on the 002-0 mmu during retreat mining. The continuous miner was taking the left cut in the coal pillar in the #3 entry when the mine roof fell on the continuous mining machine. This was 150' inby survey station # 2480. The fall was approximately 16' wide, 20' long, and height is unknown.

EE was walking between two track motors that were parked 8' to 10' apart and one motor(with operator in deck) suddenly moved forward and pinched the back of EE leg between motors.

EE was helping on the continuous miner in the #6 entry of the 002-0 mmu, when a piece of draw rock fell from between the side bolt and coal rib and struck him on the right side and hip. The rock was approximately 54" long x 30" wide x 2"-4" thick.

EE was loading belt structure and stepped on a roller and sprained right ankle.

A roof fall occurred in the intake air course 1 break outby Survey Station #1439. The fall is from the #3 entry thru the crosscut to the #4 entry. The fall is approximately 20' wide x 80' long and 2' to 8' thick. The fall was approx. 2200' from the active section. This fall was reported to MSHA within 15 minutes.

EE was operating a continuous miner in the #5 right crosscut and a piece of draw rock fell between the roof bolts and rib and struck him on the head.

A roof fall occurred on the 3rd Northeast Mains. The fall is in an area of the mine that is used to travel to the bleeder system. The fall is in the #6 entry one break inby survey station no. 685. The fall is approximately 16' wide x 20' long x 5' thick. The fall was found during a weekly exam and reported to MSHA.

During a weekly exam of the airways on 11-21-08, a roof fall was discovered one break across from survey station # 1823 in the #1 entry. The fall measures approximately 210' long x 19' wide x 4'-10' thick. Fall was reported on 11-21-08 at 1:00pm on the 1-800 number.

Pinched his finger between the pot and head of roof bolter.

EE was plastering a brattice and when he dropped the plaster bucket lid, plaster splashed into his eyes.

EE was walking around the roof bolter and tripped and fell, striking his knee on a rock that was lying on the mine floor.

The brakes were locking up on the hoist in the hoist house.

EE was greasing the tailroller at the section tailpiece. As he was starting to stand up, his foot slipped causing him to twist his right knee. He continued to work regular duties until 6-3-08, when the MD restricted his activities.

EE was rolling a section of track rail over with a slate bar and little finger on his left hand got caught between slate bar and mine roof.

Pinning top and glove got caught in bolter.

A bolt became loose in the magnetic door latch that activates the latch mechanism. As the elevator was being raised from the bottom of the shaft (no one was on the elevator) the latch vibrated loose, causing the elevator to stop 26 feet above the bottom of the shaft. Elevator was out of service for approximately one hour before repairs were completed.

EE was pulling on the continuous miner cable to get it out of the haul road of the #4 entry on the 003-0 mmu and felt a sharp pain in his back.

EE was walking in the #3 entry of the 003 mmu and struck his head on a cable bolt that had been installed in the mine roof, requiring stitches.

EE was operating a roof bolter. He was drilling a hole in the mine roof and had his left hand on the drill pot. A piece of draw rock fell, stiking the left hand, and breaking the middle finger.

EE was shoveling ribs in the #6 track entry 11 breaks outby #3 belt drive and a brow fell and struck him on the right side, injuring his right arm and head. Brow was approximately 3' long, 2' wide, and 6" thick.

A ROOF FALL WAS LOCATED BY MINE FOREMAN ON 11/24/07. HE IMMEDIATELY NOTIFIED THE OUTSIDE PERSONNEL WHO NOTIFIED MSHA. THE FALL OCCURRED THREE BREAKS INBY SURVEY STATION # 1273 IN THE #8 ENTRY. THE FALL IS APPROXIMATELY 70' LONG, 20' WIDE, AND 11' THICK. THIS FALL IS LOCATED APPROXIMATELY 2500' FROM THE ACTIVE 001/002 MMU SECTION.

EE WAS HELPING MAKE A BELT MOVE. HE WAS CARRYING A STRUCTURE RAIL AND SLIPPED AND FELL ON A HAMMER THAT HE WAS CARRING ON HIS SIDE, INJURING HIS HIP.

EE WAS UNLOADING TIMBERS AND DROPPED A TIMBER WHICH LANDED ON TOP OF HIS LEFT HAND WHICH HE HAD LYING ON A SCOOP BUCKET.

EE is saying a piece of rock fell on his left shoulder while he was plastering a stopping. Didn't report to foreman. Didn't fill out accident report. Took 8 hours before he went to hospital - not even a scratch. (I'm protesting this).

EE was helping haul supplies to the # 1 section. As the track motor rounded a curve on a steep grade, the motor man couldn't hold the motor back and it slid down the grade and struck another motor that was sitting at the bottom of the hill. EE was in the opposite end of the motor and received bruises to the back form being jolted against the track motor frame.

EE was helping hang the continuous miner cable across the #3 entry on the 001-0 mmu. When he lifted on the cable, he felt a sharp pain in his back and groin area.

EE was aligning belt structure on the 3A conveyor belt aproximately 18 feet inby survey station # 368. EE stated that as he attempted to move the structure, a timber that was set next to him, fell over on him, striking him between the shoulders and injuring his back.

EE was walking in the #4 entry of the 001-0 mmu when he slipped and fell, resulting in pain in his back.

A ROOF FALL OCCURED IN AN INTERSECTION OF THE #7 ENTRY 4 BREAKS INBY SURVEY STATION # 1215. THE FALL WAS IN THE SECOND TO LAST OPEN LINE OF CROSSCUTS ON THE 004-0 MMU. THE FALL MEASURED APPROXIMATELY 10' WIDE, 20' LONG, AND 8' THICK.

EE was walking in the #6 entry of the 004-0 mmu and struck his head on a cable bolt, that was installed in the mine roof, causing injury to his neck.

EE was shoveling #3 belt on the off side of belt when rib rolled off and hit him in the back.

Employee was operating a roof bolter in the #3 left break on the 001-0/002-0 mmu. He was putting the drill steel in the drill pot on the Fletcher roof bolter when his left glove got caught between the chuck and drill steel. He engaged the rotation at the same time resulting in a fracture in the little on the left hand.

EE was installing the second bolt in the bolting sequence when a piece of draw rock fell from between the rib and the first bolt, striking him on the lower back and the legs.

EE was changing a belt roller on the #2 belt and got his right index finger caught between the top roller structure and belt rail structure, causing a laceration that required suctures.

Busting block hurt left shoulder. Busting block with hammer.

EE was setting a 7", 8x8 timber on the beltline. As he was standing the timber up, he lost control of it; timber fell, mashing his left hand between the timber and the belt structure.

Employee was loading an 8x8 timber into the dipper of a scoop. Another employee was pushing on the timber and EE had his left hand on the end of the timber, resulting in his hand being mashed between the timber & and the ram of the scoop.

Employee was picking up a slate bar that had a burr (small sliver of metal) on it, resulting in laceration & burr being embedded in left thumb.

Setting on tool box rock fell out between bolts and hit him in back between shoulder baldes.

Rock fell off miner boom and landed on employee's foot.

Loading tanks on flat car, dropped tank on right hand, mashed middle finger.

EE was energizing the continuous miner breaker at the section power center in the #6 entry of the 001-0/002-0 MMU. When the miner breaker was energized, the cathead blew up, causing flash burns to his hands and face.

The No. 1 Slope hoist wire had incurred damage from the inside as determined by a nondestructive test, resulting in the hoist being down for more that 30 minutes while the rope was being replaced. MSHA and OMLS were notified.

EE was walking in the #6 entry of the 001-0/002-0 MMU outby the section power center and stepped in a hole and twisted his left knee. EE continued to work at his regular duties until 5/2/06 when MD placed him on restricted duty.

After dinner, injured employee was riding the #1 sinking bucket back to the shaft bottom to begin mucking. Sinking bucket #2 was on the shaft bottom and not completely clear. The #1 bucket brushed against the #2 bucket as it came down causing the injured employee to lose his balance and hit his left arm against the inside of #1 sinking bucket.

Injured employee was removing spilled material from the shaft doors when the sinking bucket was lowered striking him in the lower right leg.
