EE was bolting top on #15 belt, break #35 when a piece of draw rock fell, striking him on the back of the head. He was taken outside to an awaiting ambulance but refused transport. EMT place a liquid bandage on the wound, he later transported himself to the ER and received 2 stitches to close the wound.

WHILE INSTALLING A ROOF BOLT A PIECE OF DRAW ROCK MEASURING 1"X18'X2" FELL STRIKING THE EE ON HIS RIGHT FOOT. AN APPOINTMENT WITH AN ORTHOPEDIC SURGEON WAS SCHEDULED FOR 3-20-02 AT WHICH TIME. EE WAS REMOVED FROM WORK FOR AN UNDETERMINED AMOUNT OF TIME ME.
EMPLOYEE HAD WALKED UP TO THE CONTINUOUS MINER TO DISCUSS A PROBLEM WITHT HE OPERATOR WHEN A PIECE OF DRAWROCK FELL FROM BETWEEN BOLTS AND STRUCK HIM ON THE LEFT WRIST. HE SUSTAINED A LACERAT ION REQUIRING STITCHES.
EE WAS DOING MAINTENANCE WORK ON A CONTINUOUS MINER ON THE 001-0 SECTION WHEN A PIECE OF SANDSTONE FELL FROM THE MINE ROOF. ROCK STRUCK EE PINNING HIM IN A CROUCHED POSITION MOMENTARILY UNTIL FELLOW WORKERS COULD REMOVE THE ROCK. EE SUSTAINED A FRACTURE TO BOTH THE FIBULA AND TIBIA OF THE RIGHT LOWER LEG.
EE WAS STANDING BESIDE THE CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLTS STRIKING HIM ON THE HEAD AND NECK. EE WAS TRANSPORTED TO WILLIAMSON MEMORIAL HOSPITAL WHERE HE WAS DIAGNOSED WITH MUSCLE STRAIN. AN APPT. WAS MADE FOR EE AT SCOTT'S ORTHOPEDICS W/DR. HEIG. HE WAS AGAIN DIAGNOSED WITH MUSCLE STRAIN IN THE NECK AND BACK AND PLACED ON RESTRICTED DUTY.
