EE was bolting top on #15 belt, break #35 when a piece of draw rock fell, striking him on the back of the head. He was taken outside to an awaiting ambulance but refused transport. EMT place a liquid bandage on the wound, he later transported himself to the ER and received 2 stitches to close the wound.

EE was bolting in a roof fall area on 1 section when a piece of draw rock appx. 6"x3"x1/2" fell striking his arm. The rock caused a laceration on his arm. He received stitches to close the wound.

EE was exiting the A.L. Lee Lo-Trac underground forklift 2 breaks outby #4C Head drive in the #3 entry. He placed his hand on top of the canopy to assist himself out of the deck and a piece of draw fell and struck him on his left thumb, causing a laceration. He was taken outside and transported to the ER where he received stitches to close the wound.

Member was scaling roof in the intake when a rock fell knocking him to the mine floor. The rock measured 44" X 35" X 6".

Walking from Glory Hole area along track. Employee 6'6" tall, bumped rock with head, rock loose, fell, struck employee on the left forearm and wrist area. Employee taken to ER for treatment. Stitches used to close wound.

Employee stayed back on mid-night shift to help set air lock doors out-by D-4 hd. A piece of draw rock fell onto his shoulder & back area. Taken to E.R. x-ray negative, RTW 10-15-08. Worked until 10-22-08, saw another Dr., taken off work for 2 wks.

Employee walking down #3 entry, had left hand resting on his SCSR, a piece of draw-rock fell from between bolts onto hand. He completed shift, went to doctor next day, x-ray positive; broken bone on top of hand.

Employee injured 3-28-08; rock fell while installing bolts, struck right side and knee area, went to E.R. x-ray negative, saw specialist 4-2-08 schedule MRI for 7-7-08, after MRI surgery to be scheduled, had surgery and began to miss work.

EE was shoveling on B-1 belt break #12 when a piece of draw rock approximatley 15"x18"x1" fell hitting him on the left shoulder.

EE was working on power center at B-9 Belt. He was bent over with doors open, piece of rock fell onto his left upper back causing injury. He went to the Williamson Memorial E.R. for treatment.

004 Section, #2 left entry, Member put bolt against roof, rock (1" x 6" x 2" thick) slid down steel, struck left thumb. He completed shift, drove self to E.R. X-ray positive, released and returned to work. 10-26 had follow-up appointment, and taken off work until next visit on 11-06-2007.

Preparing to move RB up for next row of bolts. Rock fell from between rib and outside bolt, striking right side and falling down onto right ankle, causing injury. Taken to ER for treatment. X-ray of right ankle positive.

Installing bolts #5 heading. Draw rock fell when starting steel, struck right wrist, causing injury. Taken to ER for treatment, stitches used to close wound.

Employee putting up bolt in 3L on 001 section. Piece of draw rock fell onto left arm causing laceration. Taken to E.R. for stitches to close wound.

Employee was installing half header over bolts out-by in old area, a piece of draw rock fell striking his left ear. He was taken to e.r. where stitches were used to close the wound.

Operating miner on pillar section, was positioned across intersection at MRS, rock fell onto left foot causing fracture to big toe.

EE WAS ROOF BOLTING ON #2 SECTION, IN THE FIRST ENTRY. EE HAD PUT UP THE FIRST BOLT AND WAS SWINGING BOOM OVER TO PUT UP NEXT BOLT WHEN DRAW ROCK FELL, STRIKING EE'S RIGHT KNEE.

EE CHANGING BIT ON STEEL, HEARD POP, TURNED LEFT TO MOVE AWAY. DRAW-ROCK FELL FROM BOLT TO RIB ONTO INSIDE RIGHT LEG. EE WAS PERMANENTLY TRANSFERRED TO OUTSIDE JOB.

EE WAS IN THE #2 ENTRY WAITING TO DUST THE PLACE. A PIECE OF DRAW ROCK FELL OUT, STRIKING HIM ON THE BACK OF THE HEAD, NECK AND SHOULDER AREA. HE WAS TAKEN TO THE E.R. AT WILLIAMSON MEMORIAL FOR TREATMENT. X-RAYS WERE NEGATIVE AND STITCHES WERE USED TO CLOSE THE WOUND.

As ee was setting the roof bolter up, a piece of drawrock fell striking his right shoulder causing injury.

WHILE INSTALLING A ROOF BOLT A PIECE OF DRAW ROCK MEASURING 1"X18'X2" FELL STRIKING THE EE ON HIS RIGHT FOOT. AN APPOINTMENT WITH AN ORTHOPEDIC SURGEON WAS SCHEDULED FOR 3-20-02 AT WHICH TIME. EE WAS REMOVED FROM WORK FOR AN UNDETERMINED AMOUNT OF TIME ME.

EMPLOYEE HAD WALKED UP TO THE CONTINUOUS MINER TO DISCUSS A PROBLEM WITHT HE OPERATOR WHEN A PIECE OF DRAWROCK FELL FROM BETWEEN BOLTS AND STRUCK HIM ON THE LEFT WRIST. HE SUSTAINED A LACERAT ION REQUIRING STITCHES.

EE WAS DOING MAINTENANCE WORK ON A CONTINUOUS MINER ON THE 001-0 SECTION WHEN A PIECE OF SANDSTONE FELL FROM THE MINE ROOF. ROCK STRUCK EE PINNING HIM IN A CROUCHED POSITION MOMENTARILY UNTIL FELLOW WORKERS COULD REMOVE THE ROCK. EE SUSTAINED A FRACTURE TO BOTH THE FIBULA AND TIBIA OF THE RIGHT LOWER LEG.

EE WAS STANDING BESIDE THE CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLTS STRIKING HIM ON THE HEAD AND NECK. EE WAS TRANSPORTED TO WILLIAMSON MEMORIAL HOSPITAL WHERE HE WAS DIAGNOSED WITH MUSCLE STRAIN. AN APPT. WAS MADE FOR EE AT SCOTT'S ORTHOPEDICS W/DR. HEIG. HE WAS AGAIN DIAGNOSED WITH MUSCLE STRAIN IN THE NECK AND BACK AND PLACED ON RESTRICTED DUTY.
