EE had closed the gate at the mine entrance and was walking back to his truck, he slipped on the metal grate injuring his shoulder. Due to conflicting reports of injury location a medical review was conducted to determine the validity. On 8-24-11 the report indicated the injury was related to the accident. He started missing work on 8-4-11 and surgery was performed on 8-5-11.
Employee was cutting a piece of plastic pipe when knife slipped and cut his hand requiring stitches.
A roof fall occurred on the 1st NW Submains near the mouth of NE-1 panel, that is a continuation of a previous fall that occurred in 2010, that now is approxiamately 120'L X 19'W X 5+'H, the fall is located near spad #34328. A larger area was dangered off and the NE-1 EP was relocated.
A roof fall occurred in a neutral entry at the C-seals set. The fall was approximately 15'L X 19'W X 5.5' in height blocking access to one seal in the C-seals set. MSHA was notified and investigated the fall. All entrances to the fall area have been breakered off.
An unplanned roof fall occurred in the return entry 2400' from the active face, inby survey spad #34863. The fall was approximately 20'x20'x8'. There were no injuries or equipment involved in the fall.
EE was bolting top on #15 belt, break #35 when a piece of draw rock fell, striking him on the back of the head. He was taken outside to an awaiting ambulance but refused transport. EMT place a liquid bandage on the wound, he later transported himself to the ER and received 2 stitches to close the wound.
On Oct. 5, 2010, due to an occupational illness, as substantiated by medical records and reports, employee became unable to work beause of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time.
An unplanned roof fall occurred approximately 5000' from the active face at survey spad #60381 in the return entry. The approximate size of the fall was 250'x18'x9'. No injuries or equipment was involved in the fall.
EE was bolting in a roof fall area on 1 section when a piece of draw rock appx. 6"x3"x1/2" fell striking his arm. The rock caused a laceration on his arm. He received stitches to close the wound.
A roof fall occurred on the active 001 MMU section that was approximately 80'x20'x10'.
EE was exiting the A.L. Lee Lo-Trac underground forklift 2 breaks outby #4C Head drive in the #3 entry. He placed his hand on top of the canopy to assist himself out of the deck and a piece of draw fell and struck him on his left thumb, causing a laceration. He was taken outside and transported to the ER where he received stitches to close the wound.
An unplanned roof fall occurred 2,200 feet from the active face in #5 entry SS #46108. The fall was appx. 25'x20'x9'. There were no injuries or equipment involved in the fall.
A roof fall appx. 240'x19'x6' occurred in the #7 entry on 17B. No equipment or persons were involved.
On June 17, 2010 due to an occupational illness, as substantiated by medical records and reports, Jackie Thacker became unable to work because of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time.
IEE and two other members were removing a 12" I-beam from the walkway and stopped to rest. When they picked it up again the EE felt a pain in his abdomen and neck. He was fitted with a C-colla r and was transported outside and then to the ER where he was diagnosed with a bi-lateral hernia.
Member was walking along #14 belt and bent down to cross under a water line, slipped and fell on an old piece of conveyor belt, causing a laceration to his knee. He went to Williamson Memorial Hospital ER where he received stitches and a full release to return to work.
Member was cutting the metal band from a new bundle of roof bolts and one of the metal bands snapped and struck his arm, causing a laceration. Member transported himself to hospital where he received stitches to close the wound and a full release to return to work with no restrictions.
Employee was at the roof bolter when he felt pain in shoulder. Employee continued to work until 2-16-2011 when he had surgery on his shoulder and started missing work.
While hanging a miner cable with a rope hanger, the hook of the rope hanger caught his thumb on his left hand. He received 8 stitches and was released to return to work.
While shoveling slope belt employee lost his footing and twisted his left knee. He continued to work until 07/27/2010, when he had surgery to repair a torn meniscus and a tear behind the knee.
Employee was shoveling on the slope belt and felt a pop in his shoulder. He continued to work while receiving physical therapy but did not show any signs of improvement and was scheduled for surgery on 12/01/2010. He continued to work until 11-30-2010.
Member had trammed the roofbolter into #3 left to begin bolting sequence, and as he prepared to get out of the operator's deck a rock , fell onto the ATRS of the bolter, causing the bolter to tilt up in the rear. Member placed his hand on top of the canopy to assist himself out of the deck, and when the bolter tilted up his hand was caught between the canopy and the roof.
Member was scaling roof in the intake when a rock fell knocking him to the mine floor. The rock measured 44" X 35" X 6".
Victim was tramming a twin boom roof bolter out of the #1 face. The roof bolter struck the rib pinning his right middle finger between the frame of the machine and the canopy post resulting in loss of the end of his middle finger to the second joint. Due to the mine down for issues with seals he has not missed any scheduled work.
While getting tools out of a tool box, tool box lid fell and caught the last 3 finger on left hand. Was transferred to surface and taken to emergency room and received 7 stitches.
Roof fall occurred on 001-0 MMU in #4 entry 3 crosscuts inby survey spad # 34505 at 1:50 PM. The fall was approximately 20' wide x 50' long and 6 feet in height. No ventilation was affected by the fall. This fall was investigated by MSHA and OMSL.
Walking from Glory Hole area along track. Employee 6'6" tall, bumped rock with head, rock loose, fell, struck employee on the left forearm and wrist area. Employee taken to ER for treatment. Stitches used to close wound.
Discovered roof fall at 8:50 A.M. in intake, #7 entry, 001 section, 12,000 ft from face. Fall was 20' X 20' X 6' high. No equipment was involved, vent not affected. Area will not be rehabed, both MSHA & OMSL were notified and investigated.
Taking 3rd cut fom #4 entry, squatted down, roof soap stone, rock fell from between rib and bolt striking employee's right shoulder area. Taken to ER for treatment, x-ray negative. Saw orthopedic 3-31-09, diagnosed with contusion to right rotator cuff and right collar bone.
Employee left work due to pain in low back, saw Chiropractor. RTW 3-4-09, work until 3-11-09, return to Chiropractor; taken off work until 3-13-09. 3-13-09 saw Chiropractor, taken off work again, RTW 3-17-09.
Employee was removing rail from old # 18 panel. One rail hooked, had scoop operator turn scoop bucket in order to hook other rail. Bucket hit rib, caused material to fall onto employee's right leg. Taken to PMC ER for treatment X-Ray positive. Was no indication of bad rib.
Employee on the off-side of bolt machine. Operator moved bolter, got on employee's left foot. Treated at the scene, taken to E.R., x-ray indicated hairline break on top left foot.
Helping put wreck motor onto track. Had jack under motor, putting other jack in place with header on top. Jack turned, pinched finger. Taken to ER for treatment, stitches used to close wound.
Employee installing guard under belt, while belt running. Got left arm between belt and roller.
Tightening belt tensioner, channel lock slipped, hand hit sharp metal on guard, injured fingers. Taken to ER for treatment, stitch used to close wound. RTW 11/22/08.
Changing rollers 12-A belt, right hand between roller & bracket. Injured right ring finger. Taken to ER for treatment, stitches used. RTW 11-7-08.
Fall discovered 10:30 am #7 entry intake outby spad #60648. No passage of persons affected. No vent affected, no equip involved. Fall was 18' x 10' x 5' high. Area breakered off. MSHA and OMSL notified and investigated. Moving stopping & life line around fall & danger off.
Fall discovered in #1 entry, return, 20' outby spad 44529, fall 19' X 60' X 8' high. Vent not affected, passage of people not affected, equipment not affected. Area breakered off, will not be rehabilitated, both MSHA & OMSL notified.
Discovered fall in return, #1 entry of 010 MMU 20 ft inby spad 44529. Fall was 19' X 60' X 8' high. No equip involved, no passage of persons affected, no vent. disturbed, entrances were breakered off, will not be rehab, MSHA and OMSL were notified.
Employee stayed back on mid-night shift to help set air lock doors out-by D-4 hd. A piece of draw rock fell onto his shoulder & back area. Taken to E.R. x-ray negative, RTW 10-15-08. Worked until 10-22-08, saw another Dr., taken off work for 2 wks.
Employee walking down #3 entry, had left hand resting on his SCSR, a piece of draw-rock fell from between bolts onto hand. He completed shift, went to doctor next day, x-ray positive; broken bone on top of hand.
EE was attempting to insert a roof bolt into the top, in order to do so he had to bend the bolt to get it started. When he attempted to bend the bolt he felt a pain in his wrist.
Employee pulling wagon w/tools and parts (200+ lb). Cross over threshold of mine building, pulled hard on wagon, fracture #5 left rib.
EE was starting to drill the inside hole to install a roof bolt and the drill steel bent and struck him on the ritght shoulder.
At D-3 Belt, #2 break; squaring belt to make splice. The knife slipped, causing injury to right leg above knee cap. Taken to E.R. for treatment, stitches used to close wound. RTW next shift.
Employee at #5 entry, dropping RB cable, snagged sharp edge of pizza pan, cut through glove, contacted left index finger cause laceration. He was taken to the E.R. where they used stitches to close the wound.
Employee removing cover from mantrip. Cover slipped, slid across right hand cutting small finger on right hand. He was taken to the E.R. where stitches were used to close the wound.
