Employee was hanging vent tube and coal rolled out from behind mesh striking them on the leg. They did not start to miss work until 1/30/2023.
The shear cut through a fracture zone where sandstone is falling out 3-4', shield 15 to 27. The face sloughed out enough that a tall pile of material was between the conveyor and the face. There was minimal material in the conveyor. A rock fell out of the roof in the face and hit the pile of material. It fell at an angle that allowed the rock to come over the pan striking employee.
A rock fell against a longwall shield and pushed it forward and employees leg got caught between the toe of shield and pan line.
EE was setting shield sprag, when the initial cave behind longwall happened. The air pressure knocked EE off EE's feet and EE landed on EE's back on shield toe.
He was standing on shield No. 67, operating the side shield control lever when a rock, measuring 6"x8"x5" fell between shields No. 67 & 78 striking him on the back of his right hand. The force of the rock striking his hand forced his hand into the leg cover, subsequently causing a small laceration to the palm of his hand.
EE was tightening the top clusters during belt move. He was on the offwalk side and the rib blew out and in the process of getting out of the way he twisted his knee.

When the tub was released from the roof, a piece of loose coal struck the injured employee on the top of the right foot, resulting in a fracture of his right foot. Employee was wearing steel toed metatarsal guard boots.
While backing the miner out of the face, cap rock fell in. The cap rock broke off at the last row of roof bolts landing on the miner. A portion of the slab measuring 3' x 4' x 6' broke off when it struck the boom. As the slab broke it pivoted back landing on his foot. He was standing at the 2nd to the last row of roof bolts facing the shuttle car when the incident occurred.
EE was helping install a ventilation tube in the face of #2 entry when a small piece (2") of coal popped off the corner of the second cut and struck him on the right side of his face below his safety glassed. This caused a 1/2" laceration to his right cheek area.
EE was operating a continuous miner when section of roof fell. The coal section broke into smaller pieces when it came in contact with the shuttle car he was loading. This caused a piece of coal approx. 3'x 4'x 6' to strike the injured EE from the side near his hip and upper leg.

Roof bolter operators were positioning a 4 1/2' roll of welded wire mesh screen in preparation for the bolting cycle. The mesh was positioned parallel to and under the last row of permanent roof support, when a roof fall occurred. The roof rock sheared off at the last row of permanent roof support striking injured EE.
EMPLOYEE WAS OPERATING A JOY CONTINUOUS MINER USING RADIO REMOTE CONTROL. THE SECTION WAS ON A 40' CUT CYCLE AND THE ROOF WAS BOLTED WITH 5' RESIN GROUTED ROOF BOLTS IN CONJUNCTION WITH WIELDE D WIRE MESH. A COAL BROW ON THE LEFT RIB FELL, STRIKING EMPLOYEE IN THE BACK AND PUSHED HIM TO THE FLOOR. HE RECEIVED SOFT TISSUE DAMAGE TO HIS BACK AND STRAINED MUSCLE LIGAMENTS.
