Skyline Mine #3 Coal

Miner was stepping down a ladder and received a Jones Fracture to EE's left foot as EE transferred EE's weight.

The employee was standing on the roof bolter. While moving the hung miner cable by hand, they lost balance falling backwards hitting the drill mast, resulting in a 1-inch laceration on the right side of their neck.

On 08/30/2025 at 14:15 pm a roof fall was discovered in the intersection of crosscut 17 #2 entry in 12 right tailgate. It was determined at that time the roof fall was above the anchor point for the primary roof support. The Msha Hotline was notified of the roof fall at 14:22 pm 08/30/2025

Working in a 2-man crew, driller opened rod clamp while chuck was not engaged with 300 ft of drill rod in the horizontal borehole. Water flow from the borehole pushed the free rods backwards into the helper behind the drill rig, striking their right side and causing bruising.

On 6/27/2025 EE was hit by a softball size piece of coal coming off shearer drum. CT scan was negative, and EE returned to work. On 7/5/2025 EE was diagnosed with pleurisy. Workers Comp investigated alleged diagnoses and could not determine if it was related or not. They accepted the claim making the incident reportable on 7/22/2025.

While Climbing off the machine, Employee Placed EE's right hand on the engine cover Resulting in EE's fingers contacting the cooling fan.

While pulling old conveyor belt off and pulling new one on the a frame pulled loose landing on employee's foot, this caused two metatarsal fractures.

Employees were loading a butterfly valve onto an underground truck. The valve shifted and accidentally rolled onto employee's hand that was on the bed of the truck. Employee pulled EE's hand out from underneath the valve causing injury to two fingers.

Surface Miner was walking to the forklift. EE stepped over something buried in the snow. When EE did, EE stepped on a belt bottom roller buried in the snow. EE fell back and to the side landing on a roller. This caused broken ribs on the left side.

While troubleshooting fan shroud on equipment, Employees thumb was struck by fan.

Once underground employee got out of mantrip to check equipment. When getting back in EE shut their thumb in the door.

While traveling to check on pumps in 7Rt Bleeder, the roof fall was found.

During the non-producing shift, 12RTG belt went off on remote. Miners walked the belt and found the roof fall on the belt.

Surface Miner was trying to close a damaged roll up door. The door was tweaked so EE was trying to raise one side in order for door to be level. The door dropped onto a crib EE had placed under the door. EE's thumb was caught and was fractured. No surgery was required.

Employee was cleaning and working on beltline and smashed a finger while handling a belt roller.

Seal #6 in the Mine 2 West Mains failed due to water impounding behind seal

Employee suffered a stroke while walking in the beltline. They later suffered a severe stroke at the hospital. Mine investigation determined it was a personal medical incident.

The top in front of the end tailgate shield and in the immediate tailgate entry of the 11RT longwall section fell and restricted the airflow below the required minimum quantities. MSHA's hotline was called and a modification was approved to mine with low air flow.

At approximately 4:00 pm January 29, our longwall Tailgate had closed off enough that we needed to go on our Longwall Tailgate Blockage Plan. Field Office Manager was contacted. A field inspector came to the mine and issued a K-Order. We properly trained our LW miners and set up to mine under the TG Blockage and K-Order. The TG Blockage was due to an unforseen displacement.

EE could see a cable could be hit by the loader bucket. EE stepped alongside the bucket to quickly move cable. The bucket slid to EE's side and struck EE's ankle. EE received a fractured ankle.

Employee fell from the bed of a supply truck while pulling a roll of lay flat waterline off a pallet. Employee braced the fall with right hand and this resulted in a hurt shoulder. Employee was taken to hospital and X-Ray was negative. Employee did not miss any work. It was later found on an MRI that there was damage. Employee had surgery on 12/13/2022 making it a Lost Time injury.

An unplanned tailgate blockage at the tailgate drive area of the longwall face that impedes passage from the face and into the tailgate entry. K-Order 9157666 was issued and Skyline's tailgate blockage plan was put in place. Skyline's CFM delivered to the face, and the FPM on the face stayed in compliance. No additional action was required to continue mining.

Employee held onto the end of a cap piece while jaws on can setter closed. The tip of right hand thumb was pinched.

Employee was throwing a crib behind a steel set leg and pinched right hand pinkie finger between the crib and the steel set leg. Employee was wearing cut resistant gloves. This incident resulted in a laceration to tip of finger and loss of nail.

While traveling into 7RT Bleeder, roof fall was found. The top at crosscut 18 had weakened and fell. The measurements are 20 feet wide, 20 feet long, and 10 feet high at the middle of the fall. Area is not wet. Area was bolted with 7 foot rebar resin grouted bolts. Ventilation was not disturbed.

EE was hand unloading face conveyor flyghts that were brought in to section in back of dodge truck. As EE was putting flyghts in jaws of the can setter, their index finger was pinched. This caused a fracture to the tip of the finger.

Water Supervisor found this roof fall at 6:30 am while investigating a water issue. This intersection was mined 5+ years ago. This intersection has little to no water in the roof strata. Roof condition in adjoining entries look good. This roof fall is not in a haulageway, travelway, or escapeway. It does not impede weekly examination, and does not affect ventilation.

EE was hanging bolter cable in the last open crosscut. Outby approximately 250 feet the 4 inch discharge water pipe came unhooked from the roof and fell. It dominoed inby striking EE on the shoulder. EE was taken to the hospital and a small hairline fracture was found. EE also has a deep tissue bruise. EE was given a full release to RTW on next scheduled shift.

Directly in front of shields 1, 2, and 3, the immediate roof fell. The roof fall area was approximately 20' X 20' X 10' high. This fall was from the shield tips to the already loose top that had sat down on the Longwall crusher and stage loader. An MSHA plan was approved for rehab on 5/31/2022

On 5/21/2022 EE slipped and twisted knee. EE was taken to the hospital and X-Ray was negative. EE returned to work and has continued to come to work. An MRI was 6/9/2022 and the results show a strained ACL. EE had ACL surgery 10 years ago and this incident has made it that EE needs Physical Therapy.

Injured was polishing a 22" long, 2" in diameter pin on the shop lathe. EE was using a 4 foot long piece of emery cloth. When the emery cloth was worn, EE choked up on one end of the emery cloth to have better grid on the emery cloth. As EE was working the emery cloth back and forth, EE got EE's thumb too close to the bar and the rotating bar pulled EE in and broke EE's left arm.

In a 48 hour timeframe we had a 350hp pump failure at XC87 an 200hp sump failure at XC5 in SM1. These failures allowed water to flow to the SM1 seals. Once the water roofed out in XC6 in SM1, normal ventilation was affected. A 103(k) order was written. As of 4/13/2022 water level has lowered to allow normal ventilation in SM1.

Employees were removing a Kennedy Stopping with a mandoor. Injured employee was holding the mandoor with right hand as the clips on the cross bars were being removed. When the clips on the panel next to the door were removed, the door fell to the side and cut the palm of the EE hand. EE was wearing cut resistant gloves and still received a cut that required stitches.

10RT Longwall encountered low air velocity at the Tailgate. A 103(K) order was issued and a low velocity plan was submitted and approved.

Pumper going in to North Mains water handling location found roof had fallen. It was in the roadway along the old east mains. This area has not had active mining for many years. The location was supported with primary support only.

The shear cut through a fracture zone where sandstone is falling out 3-4', shield 15 to 27. The face sloughed out enough that a tall pile of material was between the conveyor and the face. There was minimal material in the conveyor. A rock fell out of the roof in the face and hit the pile of material. It fell at an angle that allowed the rock to come over the pan striking employee.

Intake CFM and face velocities required in Skyline's base plan still cannot be acquired. A new lowered intake CFM and face velocity plan was approved from MSHA to continue mining.

Enough roof and rib fell in the longwall tailgate that egress off the face was not possible and needed intake air and face velocities could not be acquired. Field office was notified and a low intake CFM and face velocity plan was approved from MSHA to continue mining.

Bolter operator was pushing the end of a mat up to wire it in place so a bolt could be installed. Operator was pushing up with their head while wiring. The mat slipped off operator's hardhat and fell striking the operator. This caused a 1 1/2 inch laceration that required 3 stitches.

Surface miner was on foot under BC20 reclaim belt looking to see if the de-icer sprays were working. A small piece of coal fell from between the spill plate and the chain link fence and struck EE on the right ear causing a laceration. Miner was wearing safety glasses and hard hat.

Enough roof and ribs fell in the longwall tailgate that needed Intake CFM and face velocities could not be reached. A low intake and face velocity plan was approved from MSHA to continue mining.

A severe floor heave occurred 100' outby the LW face in the intake. In addition to the floor heave, the pillar rib burst. There were three miners at the end of the can line. One miner received fractures to their left leg. Another miner received stitches to the left side of their face. The third miner was check out and was fine.

Miner was waiting for equipment at the end of the intake canline. The floor heaved and the rib blew out. Miner was hit by something and received a cut to head, just above the ear.

Roof in the longwall tailgate fell enough that egress off the tailgate face was restricted. Field office was notified and tailgate blockage was implemented.

Top weakened and fell on the 9RT Longwall belt. Fall was 25 foot long X 19 foot wide, and 7 to 11 foot high.

While unloading a pump out of the back of a pickup truck, the pump slid to the side. The tip of the miner's middle right hand finger was pinched. This resulted in 8 stitches.

EE was using a handheld cordless bandsaw to cut rib bolt into 1 foot lengths. EE was holding the bolt with the left hand and was using the saw with the right hand. When the blade contacted the bolt, it rolled up the bolt and cut EE's hand. EE was wearing cut resistant gloves. Using the gloves greatly reduced the laceration.

Roof fall was found in primary escapeway in 7RT Bleeder. Fall was not above the anchor zone. Bolts broke at 3 foot level. It did not affect ventilation. Size was 17 feet X 17 feet X 3 foot high

Employee was in the process of installing the kickstand on the sloughing plate of the shear. Another employee had started to pull a staple to repair a leaking fitting. The employees were reminded to get the plate blocked. Work on the fitting stopped. Sloughing plate come down pinning the employee. The pinned employee received 2 staples in the back of EE's head.

While installing a rib bolt, the wrench broke and hit operator over the right eyebrow causing a laceration requiring 8 stitches.

EE was installing 7' roof bolt. Bolt was pushed into hole most of the way. The bolt bent so EE adjusted drill mast to straighten bolt. When EE tried to push it in again, the wrench broke and hit EE in the cheek. This caused fractures to EE's cheek.

Employee got out of the passenger side of a parked mantrip. The mantrip had parked on a piece of wood with the rear tire, leaving the wood leveraged up 4". The employee did not see the wood in walkway and got foot caught under it. This caused the employee to fall with foot still stuck under the wood piece. This resulted in a broken ankle.

EE was replacing a scoop push plate ram. Two miners were assisting employee. As the connection pin was being hit through the block and into the ram eye, the injured EE stuck a finger in the block hole to see if it was lined up. At that point the eyelet dropped a couple inches and the tip of the finger was pinched. This resulted in 11 stitches and the tip of finger fractured.

EE bent over to pick up one end of a belt roll hanging bar. EE barely had it off the ground when they heard a pop in shoulder and pain down right arm. It is determined by Specialist that Bicep Tendon Tore off. First day of missing work is 01/11/2021.

A fire at the tail of BC8 surface belt was found by surface employees. This fire was in the tunnel exiting the crusher building. Fire was out by 5:00 am. A distance of 135 feet of top belt and 135 feet of bottom belt was burned. All communication to and from the mine was lost. Was able to contact MSHA at 4:35 am by cell phone by leaving the mine property.

Two employees were pushing a 150 hp pump to the back of the bed of a dodge truck to unload it. The pump was top heavy and it tipped over and pinched the pinkie finger of the employee.

Person in the area said the top started dribbling and then the intersection fell in. Area that fell was 8 feet high, 45 feet long in the beltline and 15 feet into the crosscut.

EE was trying to split a small wedge with an axe. EE held it flat against a set can and struck the wedge. It did not break so they lifted it up with left hand and with the axe in right hand. When they struck the wedge against the can a thumb was under the blade of the axe. EE received 3 stitches.

Roof fall was found during belt preshift. It had only fell 3' high. Examiner reported it to shift coordinator. When coordinator arrived with timbers and workers, it had fell up to 8' high, making it reportable. Coordinator had it reported. Fall did not affect mining, ventilation or escapeway(s).

Employee was standing by foreman. Employee fell over forward and died. Miners tried CPR and Defibrillator but could not revive them. We believe the autopsy will reveal a heart attack. This fatal is not mine related.

EE tossed a bottom roller into the bed of a dodge truck. The roller hit already loaded rollers and bounced back at EE. The tip of EE's left hand ring finger was pinched between two rollers, resulting in a crack in the tip of EE's finger.

Miner was squatting at the 8-bay looking at the screen. The rib blew out pushing miner against the 8-bay covering miner with rib rash. It took approx. 45 minutes to get the miner uncovered & out of the area. The distance between the rib & 8-bay prior to the incident was approx. 3 feet. The rib was supported with mesh & 4 foot bolts.

What was described as a rib roll by the onshift foreman happened at the 8-bay. Rib roll occurred at 1:50 am while crew was working on face chain plug. While face chain was plugged, the graveyard shift cleaned up rib roll and started setting rock props. The dayshift prep crew fininhed setting rock props.

EE was shoveling at the tail of a surface belt. When throwing a shovel full of material onto the belt, slipped backwards and hit elbow on the cement floor. XRay to elbow was negative. EE returned to work. Elbow has healed but still has pain in shoulder. EE had an MRI done on 8/21/2020 and has began missing work 8/26/2020 due to pain in shoulder.

Employee had just pushed pan and was going to pull up shields tight to the face. The coal and rock face fell into the already full pan. The rock hit the top of the spill tray and some came over onto the shield toes. A piece 2' X 1' X 1' hit miner on the side of left knee causing three fractures. Two below the knee and one above the knee.

On 5/26/2020 a roof fall was found in the #1 entry just inby crosscut 26 in the mine 4 submains. This fall was not above the anchor zone, and did not restrict airflow. However, it impedes the travel of the weekly examiner.

Employee was in the beltline helping remove a 10' length of monorail. EE's end was placed on the ground and EE stepped back to go to the other end and stepped on a small piece of coal. This resulted in a sprained ankle and employee missing 4 days of work.

Miner was breaking up a 3'X3'X2' rock on the shield toe with a Jack Hammer. The tip of the Jack Hammer slipped and the Jack Hammer went forward causing the miner to pinch finger between the Jack Hammer handle and the rock.

Crew pulled belt out of LW belt bank. They were in process of pinning the belt back together. EE got under elevated belt on knees to help line up splice. Once they were done, EE scooted out from under belt and when they went to stand up, they strained their back. EE did not return to work on the next scheduled shift.

A roof fall occurred in the face of #1 entry in the mine 5 mains. Fall was 12' long of unsupported face, and 15' long of supported face. The fall was full entry wide and above the anchor zone of the supported top that fell.

Weekly examiners found a roof fall in the return entry in the south west reserves sub-mains. Fall was 3 foot high, 40 foot long, and 18 foot wide. This is not an escapeway. There was no change in ventilation. Fall did not go above anchor zone.

Employee was walking past the feeder breaker coming out of the beltline. Employee states employee stumbled and fell onto back and tweaked it. Employee was taken to the hospital for X-Ray. X-Ray was negative and doctor offered RTW full Release. Employee asked for two days off to heal, thus making it a Lost Time Incident.

Miner was trying to hang buggy cable anchor with cable still in it, by self. EE was using a high voltage hook to hook it to the mesh while EE clevised it to the belt hanger plate. EE lost control of it and the high voltage hook caught EE's finger and caused a laceration. EE was wearing gloves.

EE stepped backwards and rolled ankle. It was level ground and EE did not step onto anything. X-Ray showed no injury and EE returned to work. EE had an MRI on their own and saw a doctor on 3/9/2020. Doctor says EE needs surgery. This incident is now reportable and EE will have surgery in two weeks.

Belt man was walking to new location to shovel belt. EE was carrying a shovel in EE's right hand. As EE started down a grade EE got closer to the belt to hold on to dust pipe to sturdy self. The floor was wet closer to the belt and EE slipped and rolled EE's ankle. X-ray showed EE fractured EE's Fibula.

Rib blew out and struck miner in the head causing small laceration.

EE was helping another miner carry 4" X 20' aluminum pipe. They both had pipe on shoulder. EE in back fell. IE was in the front and felt pain in right shoulder. IE did not want to seek medical attention until January 14, 2020. IE went on own to the doctors and then wanted workers compensation. Workers compensation investigated and approved claim on February 15, 2020.

While using a razor knife, miner cut index finger. Miner was not wearing the required cut resistant gloves.

A three way intersection in the primary escapeway of the 4 Right Longwall section fell above the anchor zone.

While breaking up rock with sledge hammer at the Longwall Tailgate drive, a rock fell off the gob plate and landed on employees left foot causing a fracture on top of foot.

While a mechanic was trouble shooting a Utility Vehicle, they accidently pulled forward and the trailer wheel smashed miners calf.

Injured person fell backwards onto Longwall panline and hurt their lower back.

While removing a radiator cap from equipment, employee received burns on forearm.

Roof fall above the anchor zone in the intersection of Mine 5 LOB seam XC-204 in the #2 entry.

A roof fall was found by a pumper. This fall was not in an escapeway or travelway. It was in a return crosscut. There was no change in ventilation.

A coal burst come from the top of the face area hitting a longwall propman in EE's airstream helmet. This resulted in a cut to the right side of EE's head and likely slight concussion.

While starting up a stationary ladder, employee slipped and fell backwards causing hand to be fractured.

While removing a lump of coal off the longwall Bretby, the lump of coal fell onto employee's foot. EE's foot was on the drawbar knuckle. The downward pressure of the lump caused upward pressure from the knuckle and fractured the employee's big toe.

Employee was walking down the longwall face carrying a rolled up hose. Their right foot slipped forward and they fell backwards injuring the left ankle as they landed on it. EE did not seek medical attention until March 12, 2019. At this time an X-Ray showed a fractured Tibia.

A rib burst happened on the face and hit shearer operator. The piece of coal that hit them caused their safety glasses to cut their face.

Roof fall in the old 9 East intake was found by preshifter.

Injured person was walking up the longwall face. They tripped and fell forward and cut the palm of their hand on a piece of rock or something sharp.

Employee was using a pointed bar to operate a track jack. Employee stepped on the point with right foot to push down. The point went through EE's boot and broke the skin.

After cutting through the headgate, the shearer began heading to tailgate. When the headgate drum was at shield 7, the injured miner stepped from shield 3 to shield 4. The upper part of the face wall came in, filled up the pan and knocked injured against shield leg ram.

Injured was hit by rib slough and fractured the collar bone.

Loader operator dropped bucket on Employee's boot while Employee was loading a vent tube in bucket.

Employee pulled groin lifting a box. Employee was seen at the ER and was released to RTW. After a follow up visit with a specialist, it was determined on 08/31/2018 that surgery was needed. Employee has not missed work.

While working on shuttle car conveyor, employee was struck on the hand by a hammer

Employee pinched right ring finger when the employee placed a fire hydrant on front end loader.

While stepping off loader, employee twisted ankle.

While removing a 90 pound brake from shuttle car, employee's hand was hit and received lacerations.

Maintenance was being performed on A 9" long bolt flew through the air and hit the face.

Roof fall on the longwall stageloader. This fall was 12' X 18' at the widest point. It separated at 7 1/2 foot level and laid down on the stageloader. It did not impede passage or block escapeway. MSHA was called and a plan was submitted and approved.

While shoveling the employee pulled a muscle in lower back. This occurred on 12/12/2017. Employee did not report this incident until 12/19/2017. EE seen a doctor on 12/21/2017 and then at a follow up visit on 01/02/2018 it was determined employee needed days off for physical therapy.

Employee was stepping in to the back of the shield to do repairs. foot slipped off shield and bend backwards. Employee refused to seek medical attention until 1/4/2018. At that time it was discovered employee had a fractured foot.

Employee was assisting on the roof bolter. While the right side operator was putting in a rib mesh, injured tried to grab a bundle of plates that had slid under supply tray. At the same time the operator lowered the mast and pinched the employees hand.

Employee was pulling manifold line from pumping and felt pain in ee stomach area.

Employee was working on a Longwall shield. Employee pulled a staple out of a valve fitting and was struck by the valve in the forehead resulting in a 1 inch laceration.

While unhooking a 3" soft water discharge line, the drisco pipe flipped up and hit employee in the teeth.

Employee lifted a 3" drisco pipe with shoulder. Employee felt a pop and was taken to hospital. ER Dr. gave employee a return to work release. Employee has not missed work due to this incident. Employee seen a Dr. on their own on 2/17/2017 and had surgery on 5/11/2017. It is now determined that the incident on 12/5/2016 is reportable.

During an escapeway drill, a crew found a location where the roof rock had fell up to 4 foot high in the secondary escapeway.

While putting a metal cover back on roof bolter, employee smashed the tip of EE's index finger

During the weekly examination, the examiner found a roof fall that was above the bolt anchor zone. This area was not in an escapeway, travelway, or roadway. And, it did not impede travel or affect the ventilation.

As employee was walking past lube truck, his left ear was caught on a piece of damaged rib mesh. This resulted in a laceration to his ear lobe.

While traveling from return to intake, employee stepped off cement pad onto a water pipe and twisted his ankle.

While pulling down a ventilation curtain, employee felt his knee pop as he fell to the ground.

Injured person traveled through overcast mandoor to the intake side. While shutting door, he jammed his ring finger on the face of the door, much like a basketball player jams a finger.

Roof fall was found during weekly examination.

Longwall downshift crew was moving power. While moving the power center out of crosscut 14, several rock props were inadvertently knocked out. Before crew could reset rock props, the roof took weight and fell. Area that fell was approximately 20 feet wide, 20 feet long, and maximum of 4 foot high. This area during advancement had been bolted with 7 foot rebar, resin grouted bolts.

Coal and rock roof approximately 33 feet long by 10 feet wide and up to 7 feet high fell on and around the long-wall stage-loader.

Crew was pulling disco pipe into beltline with loader. Connection anchoring pipe to loader broke and struck injured.

The mine was rock dusting when the employee went down to pre-shift. He was looking at the Roof and Ribs for hazards. Due to the rock dust being in the air, visibility was poor. Employee lost his footing and slipped and fell and strained his knee.

Miner went from managing a pump to aid dewatering at the face transferring slop from 5 gallon bucket into a tote. When dumping bucket, miner felt a "pop" and experienced pain in the back. Miner was brought out of heading and was taken to the hospital.

While collaring a bolt hole with a jackleg drill, a slab of rock approx. 12" x 12" x 4" fell from the top of the rib and struck the miner on the back of his left hand, cutting through his glove and causing him to sustain a laceration on the back of his hand resulting in six stitches.

Roof fall occured in the 2 North Mains at crosscut 4 in the return air-course.

Operator was cutting a crosscut. As mining advanced, operator stepped away from rib mesh to look at cutting. At this point a rib fell and knocked operator to the ground. Operator received two broken ribs, cuts to the face, and a pulled shoulder.

While operating Hilti Fastener Gun, employee placed his hand over barrel and gun fired.

EE was installing a primary support when a rock hit the ground and rolled into his left foot and ankle.

Employee was installing the primary roof support with a jack leg drill. The air hose came loose striking him in the right hand.

Injured was assisting bolter operator. While waiting for operator, injured rested his arm on rack. When operator retracted boom the injured's finger was in pinch-point

Injured inadvertently activated jaw on roof bolter and caught his thumb in jaws

Employee was attaching a foot/ski to a steel leg at the portal when he was struck by debris causing him to fall forward suffering a superficial laceration above his eye. He also suffered a contusion on the back.

Walking up longwall face to tailgate to do onshift examination. When stepping past shield 131 employee tripped and fell. Laceration to right hand palm occurred

Employee was putting away tools after changing bits on the continuous miner, when a rib fell and rolled onto employee and caused fractures to both ankles.

While cutting conveyor belting with razor knife, the knife slipped and employee was cut on the left leg by the knee.

Injured was paying out wirerope cable from tugger winch through sheave wheel. His hand was on cable and he inadvertantly payed cable in and hand was caught in sheave wheel.

A waterline leak was being investigated in our old mine 2 west mains. When the leak was found it was due to a roof fall at crosscut 81 in the old beltine.

Injured employee was loading a 30" X 10' vent tube into the back of a Jeffrey Ramcar. After lifting one end of tube onto tailgate, injured employee lifted other end and pushed tube. During the time he was pushing tube, he wrenched his back.

Section mechanic positioned himself on the right side of the roof bolter to begin repairs on a bantam rockduster. The right side bolter operator finished installing two rib mesh bolts and swung drill mast in and back in order to move roof bolter. Left side bolter operator found mechanic caught between bolter frame and right side drill mast that was swung back in place.

We started to rehab a crosscut in our bleeder system in order to re-route walkway. After beginning re-hab we found that the center of crosscut had fell above roof bolt anchor.

Three men were pounding splice on 4 left belt. 1 man was moving plunger while other two were pounding. He thought they were done and reached for plunger and was struck by hammer.

In process of loading shield on to shield hauler. Sling was already hooked into shield toes. Injured person was holding on to sling ring with left hand and then grabbed winch hook to hook to sling. Hook twisted under pressure and crushed fingers.

12" Drisco pipe was being pulled by utility vehicle. Employee was standing next to parked Dodge truck. As pipe cleared Dodge truck it quickly slid over and struck EE on lower left leg.

Employee was using a long handled sledge hammer to pound a pin in on shearer. He was in a lose quarters so he choked up on handle. When he swung he missed pin and his middle finger was cut on pin. (right hand)

While pulling up shield, high pressure hose fitting broke. Hose and emmulsion hit employee in Air-stream helmet.

Tailgate drive cover came off. Cover came down coveyor to shearer. Cover jammed into shearer gear box and sprocket. Gear box bolts sheared sending bolts flying. One bolt hit back of employee's left hand against shield leg ram.

While roof bolting a shot area for overcast, a rock fell and struck EE on the shoulder.

EE was lifting bolter cable while another employee was hanging cable to roof mesh with sealing bar. Bar slipped off hook and struck ee in the mouth chipping teeth.

Outby crew and longwall crew were setting roof and rib protection along stageloader and headgate. Injured person was between crib and meshed rib while topping crib. There was a bounce and employee was pushed into the crib.**Initial ER visit showed no injury. MRI taken 11/27/06 showed injury. Results received 11/30/06.**

While clearing a setting on shield with left hand, EE placed right hand on inside of bretbe trough. At this moment, a large piece of coal coming down LW conveyor, caught right hand and caused injury.

Intersection roof fall at x-cut 78 in m2wms's. This fall was an abandoned main line belt. this area of belt was made common air with intake.

A ROOF FALL OCCURRED IN THE 6-LEFT MAIN HAULAGE WAY. FALL MEASURED 20' X 70' X 23'. AREA WAS BOLTED WITH 6', 16' & 24' RESIN BOLTS. DETERIORATION TO THE STRATA FROM WATER, AIR AND POSSIBLE MOV EMENT CONTRIBUTED TO THE FALL. ALL PERSONS CHECKED IN THE MINE WERE ACCOUNTED FOR. WORK IS LIMITED TO THE CLEAN-UP AND SUPPORT OF THE FALL AREA.

EMPLOYEE WAS DRIVING AN EIMCO M3 SHIELD HAULER UP THE FAULT WHEN THE RIGHT REAR TIRE BEHIND THE DRIVER'S COMPARTMENT BLEW, CAUSING THE MACHINE TO DROP TO THE GROUND AND JARRING THE EMPLOYEE'S LOWER BACK. OPERATOR'S SEAT WAS AN AIR RIDE SEAT, WHICH APPEARED TO BE WORKING. EMPLOYEE WAS WEARING HIS SEAT BELT. EMPLOYEE SUSTAINED INJURY TO LOWER BACK. HE HAS PRE-EXISTING BACK CONDITION.

TWO EMPLOYEES WERE BOLTING TOGETHER. #1 WAS OPERATING THE GOPHER DRILL & #2 WAS ASSISTING. #1 HAD COMPLETED DRILLING THE HOLE & WAS INSERTING A BOLT. AT THIS TIME #2 WAS DOWNHILL. AS #1 PUSHED THE BOLT INTO THE HOLE HE YELLED TO #2 THAT HE WAS LOOSING CONTROL. #2 TRIED TO GRAB THE DRILL TO ASSIST. THE DRILL KICKED TOWARD #2 STRIKING HIS FACE, CAUSING A LACERATION & BROKEN TEETH.

A ROOF FALL APPROX. 3'HIGH X 6'WIDE & 10'LONG WAS DISCOVERED BY THE WEEKLY EXAMINER WHILE DOING HIS WEEKLY EXAMINATION. THE FALL WAS LOCATED IN THE #2 ENTRY BETWEEN X/C 4 & 5. THIS IS THE SECONDARY ESCAPEWAY FROM THE MINE. ALL INBY MINING WAS STOPPED & THE AREA REHABILITATED BEFORE MINING RESUMED. NO EFFECTS ON VENTILATION WERE RECORDED ON THE FAN CHART OR NOTICED BY PRESHIFT

ROOF FALL DISCOVERED BY THE WEEKLY EXAMINER. MEASURES 45' X 18' X 12'. ROOF FAILURE ABOVE THE ANCHOR ZONE OF THE BOLTS DUE TO LAMINATION SEPARATION, WATER IMPREGNATION OF THE ROOF STRATA. AREA DANGERED OFF AND BREAKER ROWS SET TO PREVENT FURTHER FALL AND PERSONNEL ENTRY. NO EFFECT ON VENTILATION OR ESCAPEWAY TRAVEL.

ROOF FALL WAS ENCOUNTERED BY THE WEEKLY EXAMINER THE 1-LEFT TAILGATE #2 ENTRY X/C 40 ABOVE THE ANCHOR ZONE. FALL MEASURES 20'X 30'X 9' HIGH. SEPARATION OF ROOF STRATA DUE TO EXPOSURE OF AIR AN D WATER OVER TIME ARE CAUSE OF FALL. ALL ACCESS TO THE FALL AREA WERE DANGERED OFF TO PREVENT PERSONS FROM ENTERING THE AREA. NO CHANGE IN VENTILATION OR BLOCKAGE OF ESCAPEWAYS RESULTED FROM

ROOF FALL WAS DISCOVERED ON TOP OF THE STORED MUCK WHICH WAS STORED IN ENTRIES X/C 11 THROUGHT X/C 13 IN THE #1 ENTRY OF SOUTH MAINS. THIS ENTRY HAS BEEN USED FOR THE STORAGE OF MUCK AND NOT C HANGE IN VENTILATION WAS EXPERIENCED. IT IS NOT KNOW HOW LONG THIS CAVE HAS EXISTED DUE TO THE MUCK WHICH WAS SOTRED HERE. ALL ACCESS TO THIS AREA HAS BEEN DANGERED OFF AND ADDITIONAL SUPPORT

CLIMBING INTO A GEHL, HE BUMPED HIS RT. ELBOW ON THE CAGE. THE ELBOW SWELLED & HE WENT TO THE DR. HE WAS RELEASED TO RETURN TO WORK.

2 OTHER EE'S ALONG WITH INJURED WERE INSTALLING AN IMPACT BED FRAME AT BELT. 2 EE'S WERE ON OFF-SIDE OF BELT, & INJURED WAS ON WALKWAY SIDE PUSHING IMPACT BED FRAME IN. WHEN THE FRAME FELL INT O PLACE, IT PINCHED HIS THUMB BETWEEN THE FRAME & THE RAIL CAUSING A LACERATION TO RT. THUMB.

SHEARER WAS AT SHIELD 22-25 GOING TO THE TAILGATE. FACE WAS RUNNING IN AUTOMATIC MODE. INJURED WALKED PAST THE LAST PULLED SHIELDS AND CONTINUED TO THE HEADGATE. WHEN STEPPING FROM SHIELD 11 TO 10, SHIELD 10 WAS ACTIVATED. INJURED RIGHT FOOT TWISTED OFF THE TAILGATE TOE AND BETWEEN ITAND THE DRAW BAR, PINCHING FOOT BETWEEN THE SHIELD AND KNUCKLE, CAUSING LACERATION AND FRACTURE

INJURED WAS BOLTING NO. 3 FACE DRILLING 16-FOOT HOLES. HE BEGAN TO REMOVE DRILL STEELS FROM COMPLETED HOLE WHEN ONLY THE BOTTOM STEEL CAME OUT. LEAVING THE MIDDLE AND TOP STEELS STUCK IN DRILL HOLE. HE PLACED HIS HAND ON TIP OF THE WRENCH WHICH WAS STILL UPRIGHT IN DRILL BOX. WHILE HAND WAS ON TOP OF THE WRENCH THE TWO STUCK DRILL STEELS BROKE FALLING AND STRIKING HIS FINGER-FRACTU

PC-038 HAD A BROKEN PIN ON THE 480 VOLT DUST RECEPTACLE COVER. AS EE TRIED TO REMOVE THE BOLTS THAT HELD THE GEAR MOUNT IN PLACE HE DISCOVERED THAT THE NUTS ON THE INSIDE OF THE POWER CENTER WERE TURNING. EE HAD NOT DE-ENERGIZED THE POWERCENTER BEFORE TRYING TO REMOVE THE ACCESS PANEL.WHIEL MOVING THE ACCESS PANEL THE 480 VOLT PRIMARY SIDE CAME IN CONTACT WITH GROUND.

EE WAS TRYING TO REMOVE A BOLT FROM AN AIRCOMPRESSOR. DIFFICULT LOCATION AND TIGHT QUARTERS RESULTED IN AN OVEREXERTION AND STRAIN SITUATION. WHILE OVEREXTENDING AND PULLING TOWARDS THE EE BOD Y HE FELT SOMETHING IN HIS CHEST POP. LATER ON THE WAY TO THE HOME HE DECIDED TO STOP AT THE HOSPITAL AND HAVE AN X-RAY. IT WAS DETERMINED HE HAD BROKEN A RIB.

HORIZONTAL MOVEMENT IN THE ROOF STRATA ALONG A CUTTER SEAM IN THE ROOF SHEARED OFF ROOF BOLTS 4-6' UP IN THE TOP AND A FALL ALONG THE CUTTER SEAM.

PUT WEIGHT ON 46" PIPE WRENCH TO SECURE DRILL RODS. WRENCH SLIPPED AND WRENCH SWUNG DOWN ONTO ADJACENT STACK OF DRILL RODS. FINGERS ON RIGHT HAND HIT STACK OF DRILL RODS. FRACTURED MIDDLE FING ER BETWEEN TOP KNUCKLE AND TIP.

EE WAS MOVING A BOTTOM SEAM ROLLER ALONG THE BELT LINE. AREA IS CONSTRICTED IN DEGREE OF AVAILABLE WORKING SPACE. WHILE TRYING TO MOVE THE ROLLER EE'S FINGER WAS CAUGHT BETWEEN A TIMBER AND TH E ROLLER WHEN THE ROLLER CONTACTED THE GROUND. LEFT HAND MIDDLE FINGER WAS CUT AND REQUIRED 11 STITCHES.

ROOF FAILURE OCCURRED IN THE 4-LEFT, ENTRY #1 X/C #7. AN AREA 20' WIDE X 20' LONG X 15' HIGH WAS DISCOVERED BY THE WEEKLY EXAMINER. AIR, WATER & STRATA DETERATION CONTRIBUTED TO THE FAILURE OF AREA. REHABILITATION OF AREA IS UNDERWAY. THIS AREA IS INSPECTED BY THE WEEKLY EXAMINER. NO DISTURBANCE IN AIR FLOW HAS BEEN OBSERVED. AIR MOVEMENT IS DETECTED INBY & OUTBY THE FALL AREA.

EE WAS IN THE BOOM TRUCK BACKING UP WITH THE MOTOR TO BE UNLOADED WHILE SOMEONE WAS GUIDING HIM. THE BOOM TRUCK OPERATOR STOPPED WHEN THE CAB OF THE BOOM TRUCK CAME TO THE HANGING DRISCO PIPE. SPOTTER TOLD OPERATOR TO CONTINUE BACKING UP & TO BRING THE PIPE OVER THE CAB TO BRING THE PUMP CLOSER TO THE STOPPING. OPERATOR WAS LEANING WITH HIS HAND ON THE CAB TOP & HAND WAS CAUGHT.

995 D BOX TRIPPED TO PC. ANOTHER EMPLOYEE RESET; WHILE ANOTHER EMPLOYEE SET BREAKERS ON D BOX, PC TRIPPED AGAIN. EMPLOYEE WENT TO SET D BOX AND ASKED INJURED TO WATCH PC. NIP AT PC BLEW CAUSIN G FIRST & SECOND DEGREE FLASH BURNS TO INJURED'SHANDS & FACE.

ROOF FALL IN THE 7 LEFT RETURN MEASURING APPROX 10' HIGH 30' LONG BY 18' WIDE WAS FOUND ON THE WEEKLY EXAMINATION. FALL WAS ATTRIBUTED TO WATER DETERIATION OF THE PERMANENT SUPPORTS AND AIR SL ACK IN ROOF STRATA. STRATA WAS MADE UP OF SHALE ROCK AND SANDSTONE. SUPPORT ANCHOR ZONE WAS IN THE SANDSTONE AREA OF THE ROOF.

AREA 50'X 20' X 12' HIGH FELL IN THE BACK OF 7 LEFT PANEL. AREA WAS DANGERED OFF AND ADDITIONAL SUPPORT WAS INSTALLED IN ADJACENT ENTRIES TO PREVENT FURTHER CAVING.

PULLING DRISCO PIPE TOGETHER WITH COME-ALONG. WENT TO REPOSITION AND SLIPPED ON FRONT FENDER TO FLOOR (USING MANTRIP TO STAND ON). DID NOT SEEK MEDICAL ATTENTION UNTIL FEBRUARY 24, 2002.

AT APPROX. 1:45AM A REPORTABLE ROOFFALL WAS ENCOUNTERED @ X/C 10 6-LEFT MAIN HAULAGE. THE FALL MEASURED APPROX. 16'X 16'X 7'. AIR SLACK & MOISTURE WAS DETERMINED TO BE THE MAIN REASON FOR THE FALL.

EMPLOYEE AND 2 FOREMEN WERE TRYING TO UNLOAD PUMP FROM THE BACK OF A DODGE TRUCK WHERE IT WAS NEEDED TO BE CHANGED OUT. WHILE TRYING TO SLIDE THE PUMP TO THE BACK OF THE TAILGATE AND POSITION IT UPON A STACK OF 12" PIPE THE EMPLOYEE TOLD THE OTHERS HE NEEDED TO GET A BETTER FOOT PLACEMENT. WHILE TRYING TO DO SO HE SLIPPED UPON THE PIPE RIGHT FOOT LODGED AND TWISTING RIGHT KNEE.

EMPLOYEE WAS HELPING INSTALL LENGHTS OF 10" BY 20' STEEL PIPE. HE GOT HIS RIGHT RING FINGER CAUGHT BETWEEN TWO LENGTHS. THE FINGER REQUIRED SUTURES TO REPAIR.

ROOF FALL IN 2 ENTRIES FROM ACCUMULATION OF WATER UP TO ROOF IN AREA THAT WAS FLOODED DUE TOLARGE AMOUNT OF WATER WEST MAINS AREA. SLIP ZONEWAS PRESENT WHERE ROOF FALL OCCURRED.

INSTRUCTED TO HELP PULL PIPE THRU DOORS. EE STARTED TO WALK UP ENTRY TO FIND OUT WHAT NEEDED TO BE DONE. BEFORE HE WAS ABLE TO COMMUNICATE WITH THEM THE SIGNAL WAS GIVEN TO GO AHEAD AND PULL P IPE THRU THE OORS. AT WHICH TIME HE STARTED TOWARDS CROSSCUT TO GET OUT OF WAY AND PIPE CAUGHT HIM ONT HE LEG.

IN OUT BY RETURN AT HEAD OF OLD 7 LEFT PANEL THE REUTRN ON WEST SUB MAINS WHICH WAS AN ALTERNATIVE ESCAPE WAY FOR THE LONGWALL BETWEEN XC 7 AND 8 A ROOF FALL OCCURRED APPROX 4 1/2' HIGH 18 FT WIDE BY 40 FT LONG. ALTERNATIVE ESCAPE WAY FOR LONG WALL HAS BEEN RE-BOLTED AND THE CAVED AREA IS GOING TO BE RE-SUPPORTED.

EE WAS DRIVING UV, PULLED OVER TO LOAD PARTS ON IT. WHEN HE WAS GETTING OUT HE HAD TO TWIST HIS BODY. WHEN HE DID HE FELT A POP IN HIS RIGHT KNEE. WHEN HE GOT ALL THE WAY OUT HE WAS IN PAIN A ND COULD NOT STRAIGHTEN HIS KNEE.

THE INJURY OCCURRED WHEN HE STARTED PRYING DOWN A LOOSE RIB WITH A PRY BAR. THE WHOLE RIB LINE APPROXIMATELY 25' LONG FELL OVER AND A PART OF THE RIB HIT HIM ON THE RIGHT FOOT CAUSING TWO SMAL L FRACTURES TO THE RIGHT FOOT.

EE WAS DUSTING BELT, HE FINISHED HIS SEG AND WALKED TO OTHER EE. ON WAY BACK HE TRIPPED ON UNEVEN FLOOR. THEY WERE WORKING IN HEAVY DUST AND AIR WAS NOT MOVING. UNEVEN FLOOR LEFT FROM MINER.

INITIAL CAVE FROM LONGWALL STARTUP CAUSED AIR BLAST THAT DAMAGED SEVERAL STOPPINGS IN RETURN AREA OF LONGWALL. VENTILATION WAS CHANGED DUE TO THE AIR BLAST. 77,000 CFM WAS STILL MAINTAINED ALO NG FACE AREA. STOPPINGS WERE REPAIRED, VENTILATION RESTORED, MINING RESUMED. NO ACCIDENTS OF INJURIES.

EE STEPPED UP ONMTO THE LUNCH TABLE AREA TO GET A NOZZLE FOR A FOAM PACK. AS HE STEPPED OFF THE LUNCH TABLE AREA, HE PUT HIS LEFT FOOT DOWN UPON A CABLE THAT OTHER EES WERE PULLING THRU THE KI TCHEN AREA TO A POWER CENTER TO BE NIPPED IN. AS HE STEPPED OWN FROM THE TABLE AREA, HE PLACED HIS FOOT UPON A CABLE WHICH IN TURN ROLLED HIS FOOT OUT, TWISTING HIS ANKLE, FALLING FORWARD TO

EMPLOYEE TRYING TO ALIGN U-JOINT IN DRIVE LINE ON SHUTTLE CAR USING SCREWDRIVER TO MOVE OR TWISTDRIVE LINE SLIPPED AND CUT HIS LEFT RING FINGER.

2 PUMPMEN WERE INSTALLING A PUMP AT X/C 27 SSM M1 RETURN THEY KNOCKED A HOLE IN STOPPING BETWEEN HIGH PRESSURE AREA AND LOW PRESSURE AREA. WHILE PUTTING PUMP THROUGH HOLE ONE OF THE PERSON'S R ECEIVING PUMP LOW PRESSURE SIDE BUPMPED STOPPINGDISLODGING DEBRIS WHICH WAS BLOWN UP INTO HIS FACE UNDER SAFETY GLASSES INTO EYE.

ACCUMULATION OF CO WAS FOUND A SEAL #26 1 LEFT TAILGATE BY THIS MINE EXAMINERS WHEN DOING WEEKLY. EXAMINES.

EE KNELT DOWN TO PLACE A WEDGE IN A GAP BETWEEN THE BOTTOM OF A 3C CAN & FLOOR.(A CAN IS A ROOF SUPPORT DEVICE THAT IS USED TO SUPPORT TAILGATE ENTRY, MEASURES 8' HIGH X 36" WIDE & IS FILLED W ITH ASH OR OTHER MATERIAL AND SEALED ON ENDS). UPON INSPECTION TO LEVEL CAN EE COULD SEE A LUMP OF COAL UNDERNEATH. EE SWEPT HAND UNDER CAN & FELT A SCRATCH & HAD A LACERATION REQUIRING STITC

ROOF FALL @ XCUT #6 INTERSECTION 1 TAILGATE ENTRY #1 ( FALL DISCOVERED DURING WEEKLY EXAM). FALL MEASURED 9' HIGH X 16' WIDE X 20' LONG. 72" RESIN & 10' CABLE BOLTS WERE USED IN AREA. 10' CABL E BOLTS BROKE WITH ANCHOR POPINT STILL INTACT. SEPARATION OF ROOF STRATA DUE TO EXPOSURE OF AIR & WATER OVER TIME, I FEEL IS RESPONSIBLE FOR FALL AT THIS LOCATION.

EE WAS REPALCING RUBBER SKIRTING ON OUTBY END OF TRANSFER. HE WAS CUSTOM FITTING CONTOUR OF BOTTOM OF BELT WITH RAZOR KNIFE. HE WAS CUTTING TO THE SIDE OF BODY AS EE WAS CUTTING CONTOUR KNIFE HAD ANGLE TOWARDS HIS LEFT KNEE, AND BLADE SLIPPED OUT OF BELTING. BLADE HAD ANGLE IN BELT WHILE CUTTING.

WASHDOWN HOSE ON ROOF BOLTER 008 WAS SPRAYING AND WHIPPING AROUND ON THE GROUND WHEN THE WATER WAS TURNED ON TO THE BOLTER. HE TRIED TO GRAB THE HOSE TO CONTAIN ITS MOVEMENT AND TURN IT OFF. W HEN HE SO TRIED, THE HOSE IMPACTED HIS CHEST AND WATER WAS HOT UP UNDER HIS SAFETY GLASSES AND INTO HIS RIGHT EYE.

PACKING BELT ROLLERS WITH OTHER MEN PULLED MUSCLE IN LOWER BACK, DIDN'T REPORT THE ACCIDENT TO OUR COMPANY UNTIL AFTER SLEEPING WHEN HE WOKE UP HE SAID HIS BACK HURT.

EMPLOYEE WAS ATTEMPTING TO MEASURE DISTANCE FROM TAIL PULLEY EDGE TO BEARING SUPPORT. BELT WAS RUNNING. EE STUCK STEEL TAPE MEASURE THROUGH GUARDING, WAS HOLDING TAPE MEASURE CASING WITH LEFT HAND AND FEEDING TAPE WITH RIGHT HAND. END OF TAPE TOUCHED EDGE OF TAIL PULEY AND TAPE WAS IMMEDIATELY SUCKED BETWEEN PULLEY AND BELTING. TAPE WAS PULLED THROUGH RIGHT HAND CUTTING ALL 4 FINGE

EMPLOYEE WAS TRYING TO OPEN AIR DOOR. DOOR WAS WEDGED AND AIR PRESSURE WAS CAUSING DIFICULTY. EMPLOYEE TRIED THE 3RD TIME WITH MORE BODY FORCE CAUSING DAMAGE OF INJURY TO RIGHT KNEE.

WHILE PACKING BELT ROLLERS WITH OTHER MEN HE PULLED MUSCLE IN LOWER BACK. REPORTED NEXT DAY - HEWOKE UP AND HE SAID HIS BACK HURT.
