The Employee stated that he was installing a roof bolt and a piece of coal fell out of the roof and hit him in the upper lip causing a laceration.
The employee stated that he had just placed a roof bolt in the head of the roof bolter and was pulling his arm back when a piece of shale 2'x3'x 2" fell hitting the Employee in the right forearm and hand, fracturing his 5th small metacarpal hand.

Employee was installing roof bolts when a small rock fell from roof and struck employees right middle finger against drill pod. Employee was wearing metacarpel gloves, however, the end of the finger was lacerated and 5 sutures were required.
The continuous miner operator was positioning the mining machine to remove rock from the top of the miner, when he thinks he must have tripped falling down onto the remote control box causing the tail boom to swing and the rock to slide off on him. The miner operator was not exactly clear on what occurred.
The employee was advancing line curtain in the face area with the use of a step ladder, when a rock fell from the roof, striking the employee and knocking him from the ladder. The employee received a strained lower back.
A rock 6" x 12" x 2" thick fell from roof striking the front of my hard hat. Hat was knocked from my head, second rock fell striking my head 4" x 8" x 1" thick causing a laceration. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.
Employee was drilling a hole in the roof to install belt hanger, he was getting ready to use pusher to drill. He had looked toward the controls when a rock fell and struck his left hand. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.
I was unloading crib blocking from the Eimco scoop bucket by hand with a co-worker. A piece of head coal & slate 4" x 2' x 3' fell and struck my hard hat and upper body. A small piece of this sharp slate hit my head after my hard hat was knocked off. This injury did not include any aspect of compliance with rules or regulations, job skills or attitude.
I was installing a roof bolt when a piece of rock 6" x 12" by 2" thick fell and struck my right forearm. This injury did not involve any aspect of complaince with rules or regulations, job skills or attitude.
WE WERE IN AN AREA THAT HAD BEEN BOLTED DISCUSSING A BOLTING PLAN. WE WERE STANDING IN FRONT OF BOLTING MACHINE WITH 4 OTHER EMPLOYEES. A PIECE OF ROCK FELL FROM BETWEEN THE BOLTS 36" X 36" X 1" THICK & STRUCK ME ON THE HARD HAT, FACE & EAR. THIS INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS, ATTITUDE OR PROTECTIVE ITEMS.
EMPLOYEE WAS GETTING READY TO MOVE CABLE BY THE ROOF BOLTER. A SMALL ROCK FELL BETWEEN BOLTS ANDSTRUCK HIS HARDHAT, SHOULDER AND KNEE. THE ROCK WAS SHARP AND CUT HIS LEFT EAR AND KNEE.
EMPLOYEE WAS OPERATING SHUTTLE CAR HAULING COAL FROM MINER TO FEEDER WHILE HE WAS WAITING TO BE LOADED A PIECE OF COAL FELL STRIKING EMPLOYEE IN FOREARM. THERE WAS NO IMPAIRMENT IN MINING OPER ATIONS OR EQUIPMENT. EMPLOYEE WAS ADVISED TO KEEP UNDER PROTECTIVE CANOPY AT ALL TIMES WHEN POSSIBLE.
