Buckingham Mine Coal

The EE was putting a new feeder chain in the section feeder. EE was striking a link of the chain with a hammer to separate it from the chain. When EE struck the link it chipped a piece off and the piece of metal struck EE in the right jaw.

A roof fall occurred at spad #40+40 in #7, #7-8 and #8 entry's in the #2 North East Main intake. The area was previously bolted with 6 ft torque tension bolts and roof screen.

The EE was using a cutting torch while working on a S/C conveyor chain. The torch slipped out of EE's hand. As EE went to pick it up EE's knee contacted the hose and spun the torch head causing it to go across EE's right thumb.

A roof fall at spad #39+00 in the intersection.The area was bolted with 6ft Torque tension and screen.It is contained in that intersection.

Roof fall located on our non producing MMU 002. At spad #16+40. Approximately 20 to 25ft in length by 18ft wide. This fall was in between the pillars.

The EE was cutting a notch into a section of belt so that a splice could be identified that needed replaced on a idle shift. The EE stated that as they were cutting the notch, the knife slipped and cut their left index finger.

EE was cutting a rock box off of an idle tail piece. As EE was adjusting position in the rock box the flame of the torch went across right hand. Leaving EE with a burn on right hand.

An examiner found a roof fall in the #5 entry. The fall was called into the MSHA hotline at 8:45 pm.

A roof fall was found in the #4 entry neutral of Unit 002 (Idle). The fall was reported to MSHA on 6-20-19

EE was carrying a piece of roof screen beside the roof bolter. EE lost footing and twisted knee.

The EE was moving a full piece of 60lb rail to cut a section off of it. When EE slid it to the side it fell down smashing the end of EE's left middle finger.

EE was walking past a battery for a scoop. As EE slid a leg past the battery it contacted the ear on the battery, causing a small splinter of metal to go into EE's leg.

The EE was trouble shooting a hydraulic hose on the foot of the ATRS. EE had swung the off side bolter head out so that ee could get in between it and the ATRS. Another EE was coming up the side of the RB hanging cable and started the machine and swung the head in out of way to hang cable. Which in turn pinched the first EE in between the ATRS and the bolter head.

The EE was helping put the conveyor chain of a C/M together. EE was hitting a master link pin with a small sledge. When EE hit the pin a small splinter of metal hit EE in the right pinky finger.

An Examiner found a Roof fall in the #5 belt entry. The fall was called into the MSHA Hotline at 2:35 A.M.

An employee was walking a belt line and EE slipped mid step, EE felt pain in EE's back.

An Examiner was traveling the 1st North Submain 4 belt Intake, examiner was looking for a broken phone line and found a roof fall in the #7 and #8 entry. The Roof Fall was immediately called into the 1-800 MSHA Hotline.

An employee was transferring from a cricket to a mantrip. While the employee was exiting the cricket it rolled pinching the employees leg.

An EE was retreiving rail, as EE was loading the rail onto the rail car a stick of rail rolled over and pinched the bottom of EE's right middle finger causing a laceration.

The EE was bending roof bolts. EE felt a pain in Scrotum and lower abdomen.

An employee was tramming a scoop loaded with supplies to load a roof bolter. As the employee trammed around the corner of the crosscut, EE grabbed the scoop canopy to pull self up and reposition self. The employee scrapped EE's finger between the scoop canopy and the coal rib, lacerating EE's finger. The employee received 7 sutures.

Am employee was releasing a chain binder on a flinger duster with a roof bolter wrench. When the binder came loose the wrench hit the employee in the forehead causing a laceration. The employee was taken to the Emergency Department and received 9 stitches, then was released to come back to work with no restrictions.

The weekly Examiner was traveling the 1ST North Submain #5 belt #13 cross cut and found a roof fall in the #1 and #2 Entry. This area is in the old works return.

An employee was jerking on ventilation curtain that was tangled in miner cable. The employee stated ee went to take a step after retrieving the curtain and felt pain in back that caused ee to drop to ee knees.

Examiner found a fall in the 65 cross cut at 1st North #4 belt 5-6. No damage to equipment and no persons affected. Management called the MSHA hotline at 4:05p.m.

An Examiner was traveling to MMU 001 section and found a fall in the #3 entry spad #12+60 on 5 belt. MSHA Hotline was called at 1:15 A.M. to report the fall. The fall damaged some belt structure and the outside structure of a VFD drive.

On 11/15/2017 an EE was separating sections of 4" waterline with a crib block. The crib block slipped off of the joint of water line and EE struck finger between the crib block and a fire valve. The EE fractured the tip of finger.

A buggy op was tramming S/C and leaned in to travel around a corner, EE accidentally hit the e-stop bar and stopped the car. it bounced the EE and EE struck EE's arm on the cab. EE was seen at the ER and released with a bruised arm. The EE went to a follow up appointment in the A.M. and EE's arm had swelled over night, the DR. made an incision to reduce swelling.

An EE was using the joystick on a roof bolter to install a roof bolt, the employee felt left wrist pop. The employee received stitches in a procedure to fix the ligament in wrist.

On 8/02/2017 a Roof Fall was found in the Main Return at the 1st North #2 Belt at 5x-cut. The fall was found in the #1 and #2 entries. The fall was called into the MSHA hot-line.

An EE parked EE's scoop and exited to wait for the shuttle car to tram by the EE was standing between the rib and the scoop bucket. The scoop bucket was sticking out past the corner of the rib, when the shuttle car was tramming through the entry and slid into the scoop bucket shoving it against the EE and pinning EE between the scoop and the rib.

A scoop operator, was dragging a structure tray behind the scoop tramming inby. The injured ee was walking behind the scoop and actually walked into the tray, the tray caught on a piece of rail and flipped the tray in the air causing the ee to go airborne. The ee hit the mine floor and the tray came down on top of EE's lower torso.

An Examiner was firebossing the mine and found a Roof fall at 9:20p.m. in the #5 entry at spad # 13+30. The fall was in a fourway intersection headed to MMU 001 at 6 Head (2nd East Submain) 21 x-cut.

A Preshift Examiner, tested the secondary phone system in the Intake and it would not work. The Examiner, walked the phone line and found a roof fall in the #8 entry.

A Foreman called outside to verify the Intake phone was operating. The phone would not work so the employee walked the intake to find the problem. The employee found a roof fall at x-cut 26 in the #8 entry on the North East Mains #2belt. The fall was 4' to 6' High x 17'4 ' wide x 30' long.

A Foreman was walking the 1st North belt and found a roof fall in the number 3 entry.

An Employee stated he was picking up a shuttle car anchor and felt pain in his back.

The Foreman was walking an Intake air course and found a roof fall in the #7 entry.

The Employee stated he was loading parts to take underground. The Employee mashed his left middle finger picking up a hydraulic pump motor. The Employee received 3 stitches.

The Employee stated that he was replacing an area light on a continuous miner. The Employee started to cut the conduit that entered the area light, the Employee was cutting towards himself. The knife slipped and lacerated his left palm. The Employee received 6 stitches in his left palm.

The roof started working at spad #22+20 on MMU 006. shortly after the roof started working it fell in entry numb 8. The fall was approximately 18 feet wide x 100 feet long x 12 feet high. The fall was at 11:34 a.m. and was called in at 11:47 a.m.

The Employee stated he went to help the scoop operator dust the faces. The Employee stated he was gathering the dust hoses for the wet duster and didn't realize he placed his foot under the scoop bucket. The scoop operator lowered the scoop bucket and did not know the other Employee had his foot under the edge of the scoop bucket. The scoop bucket lowered onto the helpers foot.

A Fire boss was walking the belt and found a fall in the#5 entry on the other side of the belt. Spad # was 30+50 off of unit 005. The fall was approximately 30 x 20 x 7. the fall was called in to the MSHA hotline at 6:13 A.M.

The section Foreman went out by to check supplies and found a roof fall in the neutral (#2 entry). The fall was 30'x 20' x 7' the fall was reported to the hotline at 10:32 A.M.

The Employee stated he bent over to move trailing cable for a continuous miner and felt a sharp pain in his back.

A weekly Examiner was walking a return in the 2nd North sub main and found a roof fall in # 2 entry at spad 2+70. It was a four way intersection, the fall was 30 feet wide 25 feet long and 9-10 feet high.

The Employee stated he was replacing the suction hose on a roof bolter. The Employee was cutting the hose to the correct length and accidentally cut his right pinky finger lacerating it. The Employee received 7 stitches.

The employee was Fire bossing the mine and found a roof fall in the track entry on the 1st East submain @ spad 12+90. The fall was dangered off, the fall was 24 feet long x 30 feet wide.

The Employee stated that he was installing a roof bolt and a piece of coal fell out of the roof and hit him in the upper lip causing a laceration.

Employee was struck by or walked into something (he is not sure which) causing a laceration on the right side of his head which required 2 staples. He was released to go back to regular duty.

Employee was cutting a piece of belt with a utility knife. The knife slipped and the employee cut his right thigh with the knife. (stitches required)

The Employee was operating an off standard center drive shuttle car, after being loaded by the continuous miner, the employee swung around to the inby seat with out setting his parking brake. When the Employee was swinging around to the inby seat, the shuttle car started rolling and caught his right leg against the rib. The result was a fractured tibia.

workers in the area found the fall. The fall started in the belt entry #5 entry and fell approximately 12 feet past #6 entry. The fall is approximately 18 feet wide and 70 feet long it fell on approximately 22 feet of belt structure. The fall was in a isolated area of the mine where adverse roof conditions are encountered. The fall area is at spad number 59+60.

The Employee stated he was traveling through a door underground and the door closed on his left arm.

The Employee stated he was unplugging the receptacle for the scoop battery and it arced in his right hand.

Employee was walking the air courses and found a roof fall at 21+50 in the #2 entry in the return. The roof fall was in a four way intersection. the Fall was 17 feet by 15 feet and measured 4 to 6 feet high.

Employee was cutting belt when the blade broke and his hand glazed over top of the blade resulting in a deep cut to his right little finger. Four stitches were required, Employee returned to work on his next schedule shift.

Employee was pulling new belt onto structure with 3/8 inch cable, as the cable was sliding thru his left hand a burr lacerated his left little finger.

EE went into the intake to look at the tracking and communication equipment and found a roof fall. EE called out to another ee called in the roof fall to the hot line. The roof fall was in the intake in #7 entry at the 1st North West submain at spad # 1+70. The fall was 15 feet wide 26 feet long and 7 feet high. The fall was called in at 4:15 p.m.

The Employee stated that he was scooping an entry and caught a roof strap, the strap came around and hit the employee in the ear cutting his right ear.

The Employee stated that he loaded a shuttle car, the shuttle car operator went to leave and over steered bumping him on his left side.

We had a roof fall 7' to 8' high, 18' wide and 22' long in a 4 way intersection on c-panel in # 1 entry @ spad # 4+20. This is the belt entry for the c1-panel.

Underground at mine #6 Buckingham Coal. Installing cutter boom pins. Struck the pin with a hammer and a piece of pin chipped off and lodged in the left cheek.

Employee stated that he aligned his screen on the roof, his steel was started on a strand of the screen. When he rotated the drill steel to drill his hole it wrapped the screen around the tip of his finger pinching the tip of his left index finger off.

Employee stated he was walking by the lamp house and tripped he tried to catch himself and his knee gave out.

The Employee stated that he stepped in a hole firebossing the roadways on unit 006. When he stepped in the hole he felt his back twist.

employee thought he had the safety pin in the boom of the fletcher bolter. he started loosening the fitting to the feed jack and the boom dropped on his right arm.

The Employee stated that he stepped out of the scoop and his leg bent backwards twisting his right knee.

Employee stated that he he was tripped by a trailing cable and twisted his knee.

Employee was lifting a rock off of a shuttle car. The rock slipped and pinched the employee's left hand lacerating the left pinky the employee received 9 stitches in his left pinky.

The employee stated that he placed his drill steel in the chuck of the drill pot on a Fletcher bolter, when he went to use his up pressure the steel popped out of the chuck and hit him between his left thumb and left index finger. The pressure from the hit caused an open wound that resulted in three stitches.

Employee was exiting the operators deck when his feet hit the tram pedal and lever, sliding the operators canopy of the roof bolter into rib. Employee's fingers were between canopy and rib resulting in right hand index finger to be cut off.

The employee was going to move two shuttle car tires from a intersection with a scoop. While tramming the scoop to the intersection there was a metal belt rail in the mud that the scoop operator did not see. The tire of the scoop kicked the belt rail up and cut the employee under his left arm.

Roof fall was in the old works in #5 and 5-6. Spad # 57+50 The fall was 18'x 60'long in #5 entry and wrapped around 5-6. The fall was reported to MSHA at 12:05P.M.

A loose piece of rock fell from the roof striking employees right thumb, causing a laceration and a fracture.

The employee stated he was striping a ground wire and the knife slipped and cut his left thumb. The employee received 4 stitches.

The employee picked up a coupler pin that had a sharp edge on it and it lacerated his right middle finger. the employee received eight stitches.

Employee was unhooking a flat car, he pinched his finger between the pin and the tongue of the trailer when he pulled the pin, he received 4 stitches in his left index finger.

Employee was building an intake stopping part of a block fell and hit him in the right side of his face the block cut his face and he received 8 stitches.

Employee was drilling a hole with a piece of drill steel, the drill steel broke and a piece of the drill steel come back and hit him in the left shoulder causing a laceration.

Employee was trouble shooting a conveyor motor, he tried to start the conveyor motor and the lead tree blew up causing an arc.

EE was cutting a piece of cable open that had copper foil in the cable, a piece of copper cut EE's left hand and he recieved four stitches.

Employee was stacking steel beams, one of the I beams rolled over on his left index finger.

Employee was replacing a roller and his feet were in mud. When the employee turned he allegedly twisted back area.

Piece of cap rock and head coal hit employee in lower back.

Employee was installing roof bolts when a small rock fell from roof and struck employees right middle finger against drill pod. Employee was wearing metacarpel gloves, however, the end of the finger was lacerated and 5 sutures were required.

Employee slipped and fell, receiving slight fracture to his fibula.

Continuous miner operator was tramming the miner from place to place when the miner left track started to spin and kicked the conveyor boom against the miner operator. The operator received 3 broken ribs

Employee was cutting a cable with a razor knife. The knife slipped and cut his left hand. Employee received 5 stiches.

Employee was picking up cable and his foot fell between a wooden bridge and the coal rib and he twisted his knee. The employee continued work until July 7, 2009, and received surgery on July 8 2009.

A small piece of coal fell from the mine roof and bounced off the roof bolting machine and struck employee in the nose.

There was a roof abnormality in the cross cut 2 to 1 that may have contributed to the failure. There was additional support installed in this area before the failure.

The employee was advancing line curtain in the face area with the use of a step ladder, when a rock fell from the roof, striking the employee and knocking him from the ladder. The employee received a strained lower back.

The operator was tramming the shuttle car and hit a hole in the haul road and hit his head on the shuttle car canopy. The operator sustained a bulged disc in his neck area.

Injury was drilling the roof when I needed to change to the 2nd still. the first fell out from the roof striking my finger on the chuck of the roof bolter. This injury did not involve any aspect of compliances with rules or regulations, job skills or attitude.

EE and another EE were lifting a shuttle car tire off of a scoop, when EE felt a "pop" in his lower back and it caused severe pain. This was not a result of horseplay and did not involve any aspect of compliance with rules or regulations, job skills or attitude.

EE was using a utility knife to cut open a 5 gallen bucket, when the knife slipped and cut his forearm. This accident did not involve any aspect of compliance with rules or regulations, job skills or attitude.

A rock 6" x 12" x 2" thick fell from roof striking the front of my hard hat. Hat was knocked from my head, second rock fell striking my head 4" x 8" x 1" thick causing a laceration. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.

Continuous miner hit uncharted abandoned gas well at spad 7+80, crosscut 5-4.

Ee was on a ladder on uneven ground hanging bolter cable when a leg on the ladder bent sending him to ground and twisting his ankle. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.

We had completed our shift and gotten into a 4 man ride to exit the mine. The rest of the crew loaded onto the 12 man trip to exit. The operator of the 12 man ride accidently sent the mantrip into the rear end of our ride. His foot slipped of the brake and onto the accelerator.

Inundation was at 1st east panel, entry #5, # 4th break.

Working in canopy area of new underground mine, I was on a ladder working to take down a miner cable. A cable hook became unhooked from the canopy unexpectedly and the weight of the cable knocked me from the ladder. I fell backward onto my back & head.

I WAS WELDING ON STEEL STRUCTURE WHEN IT FELL. MY WELD DID NOT HOLD. I DID NOT DO IT PROPERLY. I AM A CERTIFIED WELDER. THIS INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE.
