While mining in the 8th South longwall panel #10, there was a bounce and outburst of coal at the tailgate at shield #136, which resulted in the mining equipment being down for more than 1 hour. One person was injured and received medial treatment (stitches).

The injured contract employee was assisting to extract shield #89 from the extraction face when a rock measuring 21" long by 11" wide by 2" thick fell from in front of the walker shield and struck EE on the left foot.

Employee was operating the RS-20 while standing on shield #61, as EE lowered shield #67, a rock measuring 12"X 12" X 12", fell from between shields 67 & 66. The rock landed on the cable tray and slid down the face to shield 61 where it fell off striking EE in the right ankle, resulting in a hairline fracture to the ankle. EE was released to modified duties.

An employee (working in the 4th North Section) sustained two metatarsal hairline fractures to left foot when a piece of cap rock measuring 18 inches wide, by 4 inches thick, fell from the mine roof and struck left foot.

The EP was roof bolting when a broken fitting was discovered on the outside of the roof bolter. EP stopped bolting and walked around the back side of the machine and walked up the side and found the fitting that was broken. EP turned around and started back when a rock fell from the top where the roof meets the coal and hit EP in the right ankle.
