EE was on the 5th floor of the mill washing when a rock (4" + size) came down from the 6th floor striking EE on the Left Elbow
As employee was performing maintenance checks on primary surge stacker, the boom of the stacker collapsed and fell 20'. The employee was standing near the head pulley and suffered a broken leg and vertebra in the back.
Employee was picking up spilled stone out of walkway along the base of small HP800 crusher in mill. A stone approximately 6" bounced out of the feeder and down to the walkway and hitting / lacerating the employee's right elbow.
Employee received a lime in the eye and inhalation injury. The employee was working on a bearing change when the mill bin located above him over flowed and spilled material on top of him. Employee eyes were washed out and sent to the ED for evaluation.
