EE struck by falling object. EE was inspecting bucket elevator in plant when light fixture fell striking a glancing blow to EE's hard hat. EE had been treated with first aid until 8/26/22 when physical therapy was prescribed.

Mostrando los 11EE received 2nd degree chemical burns from high PH material that released from the main conditioning tower during plug clearing. The EE was standing ~10 feet away, material fell from the tower, exited the vessel, hit the deck, and splashed onto EE. The material impacted upper chest, neck, cheek, and forehead. Eyes and mouth were protected by a N95 mask and safety glasses.
Employee was installing shell liners when one of the old liners in the next compartment fell hitting another liner in which a piece broke off, entering employee's right leg, thigh area.
While finishing up working at the finish mill, the roll up door came down on contract employee, hitting them on the hard hat, left shoulder and left upper back. Co-worker saw the door on top of employee and came over to pull the chain that controlled the door movement. Re-latched chain securely after incident and faulty door chain reported to appropriate Ashgrove personnel.
Employee was with a crew removing demo material. EE was outside the hole removing buckets that the crew inside was filling up. As EE reached inside to remove a bucket the crew inside shouted at EE to watch out because a chunk of refractory material fell from side wall above door. Due proximity of the falling material, EE was unable to avoid being struck in the left hand.
Employee was holding a bucket in place on the Limestone bucket elevator when a rock fell from one of the buckets above and hit him in the arm.
The employee was walking, when he looked up and had small amount (4 ounces) of kiln dust drop from the ceiling inside the kiln and go under his safety glasses, contacting his eyes.

EMPLOYEE WAS COMING OUT OF A GRAIN BED TRUCK WHEN ONE OF THE CROSS BOWS CAME LOOSE STRIKING EMPLOYEES LEFT LEG.
