Employee was manually sliding a piece metal off the forks of a forklift when the metal rotated and slid off striking the employees left foot above the steel toe resulting in a hairline fracture of the metatarsal.

Employee was wearing a full silver hot suit, pants, jacket and hood. While removing a hopper of hot kiln inlet feed from the discharge area, The pant of the hot suit raised above the employees boot due to the seated potion resulting in hot material in their boot resulting in a burn to the left foot arch area. The employee also got dust in eye when they hastily removed hood.

EE was bending down to pick up a shovel when EE stood up EE felt a pop in lower back, EE was diagnosed with a back strain and issued light duty

Employee was walking when EE rolled EE's ankle. Employee was evaluated by an ortho doctor and given physical therapy

While working next to the kiln area employee obtained a heat rash on 6/21/24. Heat rash had calmed down except for some spots on the left leg. employee reported this on the 6/27/24 and got medical attention were they prescribed a steroid cream.

Employee stated had pain in right foot from compensating for a previously reported injury to EE's left foot. The company is awaiting an Independent Medical Exam to determine liability of the claim.

Employee was supporting maintenance in swapping a bucket on a bucket elevator, as employee propped the bucket up felt discomfort in the left calf. Employee was taken to local clinic and received modified duty.

While trying to perform an inspection on the skid steer drive belt employee's hand made contact with the belt causing a few lacerations to ring, middle and pointer fingers.

As miner was descending a fixed ladder, miner lost footing. Miner slid down the ladder about 3-4 Ft. and landed on their feet. during the events landing miner twisted their back.

EE strained right foot big toe joint when they stepped on a rock walking across the limestone yard, the employee was issued Physical therapy on 1/5/2023 making the incident reportable.

Employee was cutting wear plate when it fell off the cutting table and struck the employee in the right foot causing a potential break, a prescription was issued by the doctor

Employee was cleaning the seal on the finish mill door when the 3/8" hose coupling failed, allowing the hose to strike the employee in the face resulting in a bruised eye, the employee was prescribed eye drop on 9/6/2022 making the injury reportable.

The EE had right pinky hit by a hammer swung by a coworker when they tried to close a door. The EE sustained a crushing injury requiring surgery to close the wound.

Employee was lowering a ladder into a manhole when a piece of metal from the ladder lacerated EE's right hand requiring 6 stitches.

The employee drove a warehouse forklift across a railroad track crossing when the counterweight hit a rail causing the employee to feel pain in lower back.

The employee was operating a CAT 980K FEL through the limestone field. When EE drove across a pot hole, the operator's seat bottomed out causing the operator to experience sharp pain in their back.

A miner was unable to open the door of the elevator EE was in and was stuck inside for 35 minutes.

An employee carrying a bucket downstairs and felt a pop in EE's left knee.

The employee was lifting a tire from the ground to put it on the conveyor belt when they felt a sharp pain in lower back.

Employee was climbing up a ladder to the track mobile when EE felt a pop in the left shoulder. It was not until an independent medical review on 8/26/21, and submitted on 9/7/21 determined the injury to be work-related and not caused by underlying health conditions.

An electrician bent down to pick up a motor. When the electrician stood back up felt pain in lower back.

Maintenance employee got foreign body object in left eye.

Employee received a laceration on right hand while tightening a bolt on a piece of electrical equipment.

Bearing heater wiring caused short to metal table employee was using to install a bearing. A spark was noted at first use & Ash Grove was notified to repair unit. Unit was repaired by Ash Grove. Job was completed. At cleanup, employee touched table with gloved hand & received shock. Employee was taken to E.R. by EMTs.

Driver was walking back from the main plant office at Ash Grove in Seattle and tripped over cement and the train tracks. Driver fell to the ground. Driver used right hand to stop fall. Fractured right wrist from making contact with the ground.

While finishing up working at the finish mill, the roll up door came down on contract employee, hitting them on the hard hat, left shoulder and left upper back. Co-worker saw the door on top of employee and came over to pull the chain that controlled the door movement. Re-latched chain securely after incident and faulty door chain reported to appropriate Ashgrove personnel.

An unknown incident occurred inside the kiln during preheating. A rapid expansion of air that caused a couple explosion panels to pop off. No injuries or any significant property damage as a result.

The employee was moving a piece of flat stock and dropped it on their foot. The employee reported it immediately to the supervisor.

Employee was stepping on to a forklift when employee felt a "pop" in left foot.

Employee got foreign debris in eye while using a garden hoe

The miner was opening a door to a trailer when the door struck ee in the face causing a laceration to lip.

The employee thinks may have hurt back Friday (2-1-19) while doing some cleanup. Employee thinks the injury might have occurred when the employee lifted a 25 lb. chute liner. At the time, the employee did not notice any pains or discomforts in back and decided not to report it until the following Wednesday (2/6/19).

Employee had been working at the Raw Mill for the previous 5 days doing many different jobs that required repetitive movements and fine motors skills. By day 5 EE complained of pain in EE's hands.

Employee was struck in the arm by falling debris while working on a tire elevator. The employee was examined at the local emergency department. No fractures were noted by the attending physician. The employee was released to restricted duty and advised to follow-up with primary care physician.

The employee was riding bicycle in to work when EE fell somewhere between the entrance of the plant and the front office. The employee does not remember much prior to the fall or 20 minutes after the fall. EE was taken to the local ER for medical evaluation.

An employee was pulling on a wrench trying to break loose a fitting when the employee felt pain in left wrist. The employee decided to change positions and try pushing the wrench rather than pulling it. Employee started to feel a sharp pain in wrist when employee started to bear down on the wrench as it was loosening.

The employee had a possible minor allergic reaction to a beaded compound.

While installing a door on a crusher when the wind blew some crushed rock dust into employee's left eye. EE states that EE was wearing EE's safety glasses.

Employee was performing an overhead patch repair on the kiln eyebrow. EE was tamping plastic using a hand held pneumatic pounder. The air exhaust from the tamper blew material behind safety glasses into right eye.

Employee was attempting to pull a gate closed. While attempting to force the gate closed the employee felt a muscle tightening in EE's back.

Employee was descending a ladder with a piece of material inside raw mill. One of the ladder legs shifted causing employee to lose footing and fall down. Employee was taken to hospital and diagnosed with rib fracture and fracture of transverse process vertebra.

The employee was attempting to remove a broken Carbis cage, but it was stuck. EE kicked the cage to free it, resulting in an injury to right foot. Initially, the pain was not severe, and EE thought it would subside. On 04/03, EE reported to their supervisor that they would miss work due to the injury. EE later saw a doctor and found that they had fractured their 5th metatarsal.

Employee was moving a piece of refractory on a conveyor when it rolled and pinched employee's hand between the refractory and the frame work of the conveyor. When employee's hand was pinched, it caused a laceration that resulted in 7 stitches on employee's ring finger of employee's left hand.

Employee was working in the Raw Mill trying to loosen an Allen head bolt when ee felt pain in ee right arm near the inside of ee elbow.

Employee was shoveling material at the stacker when EE struck a handrail with the shovel. When this happened, the employee felt tugging in EE's back that radiated up EE's right side.

Employee was removing a vibrator from a railcar when EE felt pain in EE's right shoulder and neck.

Employee was holding a bucket in place on the Limestone bucket elevator when a rock fell from one of the buckets above and hit him in the arm.

Employee was clearing a collector box when he got cement powder in his eyes. Employee was wearing PPE The fine dust became airborne and got underneath his safety glasses.

Employee was coming down the stairs from the break room when he slipped and strained his left knee.

Employee was exiting the track mobile when he slipped from the step and strained his right hip.

While removing liners in finish mill, employee was using sledge hammer to loosen a liner. When employee struck liner a small chip of metal broke off and punctured right calf/shin area. Employee received two stitches at medical care facility.

Employee was using a pry bar to close the 'Dump' door underneath a rail car when he felt pain in his lower abdomen.

Employee was replacing a vent cover disc when the employee strained his left forearm. The employee was assisted by another employee lifting the 40" diameter 60lbs disc in place when the employees arm gave out.

Employee complained of numbness and soreness in both wrists due to over-use spanning 10 years of employment.

During routine cleaning of the preheater tower riser duct, employee discharged a high pressure CO2 system. An inspection door opened from the discharge ejecting hot dust. The hot dust landed on the back of the employees neck causing second degree burns. Employee was treated at a local clinic and returned to work the same day with restriction to keep wound clean and dry.

EE was grinding pipe when metal shavings blew under face shield and safety glasses into eye. Eye flush was done on site, but later taken to urgent care for removal of shavings. Initially the Dr. note was unclear on First Aid or Medical Treatment. Confirmation was given on 11/04/2014 that the procedure was Medical Treatment.

Employee was struck on the right side of face with an air hose while attempting to dislodge material blockage from a chute. The air hose was attached to an air lance via a quick disconnect assembly with a whip check properly in place. The quick connect came apart allowing the air hose to whip around and strike the employee on the right side of the face.

The employee was working in a skid steer and finished the task he was assigned. When he was done, he stepped out of the skid steer and landed wrong on his left foot. He felt a sharp pain in the tendon above his left heel. The working area was clean and paved with no debris or abnormal working conditions.

Trainee was looking for his cell phone and said he was going to the office to see if he left it there. He instead climbed back on top of the trailer without fall protection and hit his head on the overhead shoot causing him to fall off the trailer toward the ground. He landed on the drawbar between the trailers.

Employee was reaching over bulk bags to tie strings and felt a sharp pain in his rt shoulder and reported the incident to the shift supervisor. An ice pack was applied and the employee returned to work the same shift. The employee went to the doctor on May 29 and was put on modified duty starting May 30. The doctor prescribed OTC pain medication along with icing injured shoulder.

Employee was helping to re-brick the calciner. His hard hat fell off when he bent over to retrieve a tool. Before he could replace it a small piece of plywood fell from the scaffold and struck him on the top of the head.

Employee was rebricking inside the kiln and using a 4" portable grinder to smooth the metal surface on 12/10/2013. Safety glasses were worn at all time while in the kiln and safety glasses and a face shield were worn while grinding. Employee reported on 12/11 his felt material on his eye. Treatment was sought the following day.

Noise exposure on and off site

Employee was helping to re-brick the kiln. While moving the bricking rig, a brick fell from a stack striking the finger and causing a laceration.

Employee was operating a 980 front end loader on a Barge to cleanup the deck of residual limestone. The loader bucket caught the edge of the barge and jarring the loader operator.

Employee was using a razor knife to trim a plastic swab stick to fit a piece of tubing to be cleaned. The knife slipped and lacerated the thumb and index finger.

Employee was taking a return dust sample from the return dust screw area. He took the sample from the dust relief port, not the sampling port. His 2nd and 3rd fingers on his right hand were amputated.

While getting ready to fire Cardox tube. I hit a beam with tube, knocking down dust onto himself and his hardhat. After hooking up wires, he looked up, and dust that was on his hard hat fell behind his face shield and his safety glasses, getting into his eyes and face. He felt burning and rinsed out eyes using eye wash station

While using the spud bar to try and remove build-up from the calciner wall I hit a hard chunk of material which jammed my shoulder. At that time I felt a sharp pain in my shoulder.8/27/12 EE reported that he still had pain, 8/28 am. 8/28/12 EE was seen by doctor and was prescribed medication for the pain.

Employee was walking from main office and slipped on smooth concrete. Ground was wet from rain. Ground is slightly graded in this spot. Landed on his back and reported pain.

While removing brick/ refractory from cooler tube in preparation of rebuilding the cooler tube, EE sustained an injury to his right middle finger. He was using a pneumatic chisel to break brick. A lifter with base was above the brick to be removed. As EE was chiseling said brick the lifter/base came loose and injured the finger.

On 6/25/12 EE was going down a ladder on the inside of a compartment to the lowest level module. He was going to step onto the floor, but there was some build up of material and he felt his ankle twist. EE did not report any pain at this time and continued work. Reported discomfort on 7/16/12. Taken to The Work Clinic for Xray. Found to have a sprained ankle.

While trying to free a hook assembly from its retaining strap on the loader hoist rigging, a shackle was twisted and caught in place. The shackle broke loose, creating a pinch point between the hook assembly and the portable hopper. Employee's finger was caught in that pinch point.

Employee and crew were hoisting stage IV tipping valve into position when they recognized the tipper was 180 degrees from the correct position. They attempted to cross the chain falls hooks while lifting to to spin it to the correct position. Tipper spun faster than anticipated. Employee was in the line of fire and struck in the knee.

Employee was removing a nut from a liner bolt using a ridged threader. He positioned a wrench on the nut and was holding the wrench over the frame of the machine. He inadvertently stepped on the guarded foot activator activating the machine and pinching the finger between the wrench handle a machine frame.

EE was using a large pry bar to free a truck tire jammed in the feed chute of the TDF system. The bar slipped, causing him to strike his hand against the chute frame.

EE was stepping out onto the small sweeper. His left foot slipped off of the step causing his shin to strike against the step.

Removing a grate from a sump pump pit. As the EE lifted the grate, he felt a pop in his back. He believed this occurred on 6/29/10, but did not seek treatment until 7/27/10.

Employee was in the preheater elevator ascending to upper floor. At floor level, elevator stopped suddenly. The employee requested on 5/6/10 to have a medical check for an injury he attributing to this event.

Employee struck against a cable for the kiln gear shroud.

Following a prolonged shut down at plant operations, a quantity of coal began to smolder inside the coal storage silo. On startup of the coal mills, hot coal caused a fire on the belt conveyor. The fire was quickly extinguished within 30 minutes, but coal continued to smolder inside the pant leg of the silo.

Employee was attending a training session in a classroom environment. He reached for a bagel at the refreshment table and stated he injured his back.

Injured EE was eating lunch in the lunch room engaging another employee in light hearted kidding. The other EE tapped the head as he passed by. He claimed that this contact caused back and neck pain.

Maintenance personnel, responding to a dust emission report at coal mill #2 dust collector, found pressure relief panels (2 of 4) were open. Believing that an over pressure event occurred, MSHA was notified @ 2:25 AM. At 5:00 AM, MSHA was again notified of a fire that had begun @ 5:00 AM. This fire was quickly extinguished prior to 30 minutes.

Using an airlance (45') to clean motor and gearboy on the column, dust got into eyes. Employee was wearing safety glasses with side shields.

Re-adjusting under pressure rebar dowel. Rebar released causing injury.

While installing a chain in a hopper, the chain slipped. Employee reached out and grabbed the chain. He reported that he over reached and strained lower back. Sought treatment on 5/8/09.

Tying #9 wire - Pliers slipped causing the forehead laceration above the right eye above the safety glasses.

Employee was cleaning area around a conveyor using a shovel. Employee claims he struck his wrist against the frame on the conveyor. He stated the incident occurred on 2/13/09 and sought treatment on 3/23/09. Restrictions given on 3/26/09.

Lost flame on burner in kiln about 11 hrs into preheat. Operator notified supervisor that he could not verify that he had flame & then deselected the gas panel shutting down gas flow. Shortly after two concussions occurred. The resulted in no damage to kiln, no injuries to personnel.

Contractor employee was using an excavator to remove cement from inside a storage dome. An unexpected movement of cement engulfed to excavator and operator. Contractor: LVI Environmental - ID P574. No Ash Grove employees involved.

EE stepped off stairway after descending stair onto uneven floor and the left ankle rolled.

Employee was reporting to work at 5:50pm 9/11/08. His scheduled shift was to begin at 6:00pm. Entering the main office building, he started ascending the stairs. The tip of one of his sandal type shoes caught the top of a stair riser causing the sole to fold under. This caused the foot/toes to directly contact the stair tread. The foot slipped off the stair & abraided toe.

The EE was operating a manlift to access through the window to weld new roofing panels in place. The manlift was positioned under the roof & used to elevate the worker's head & shoulders between the open roof support beams. The employee's right hand rested upon the safety cage bar over the controls as he ascended and the middle finger pinched between the cage bar & a support beam.

Reported injury on 4/14/08. EE stated that while installing a plate overhead on the slant wall of the calciner, he strained his shoulder.

The employee was removing a refractory brick from cooler tube #4. He slipped as he was carrying the brick and extended his left hand against the tube wall to regain stability - the brick struck his left thumb.

Employee was descending stairs at the reclaimer and his knee gave out. He did not slip, trip, or experience any other such event that lead to his injury.

A fire occurred in #2 dust collector during the mill shut down/cool down phase. No plant personnel were exposed and there were no injuries associated with the event. Lost all of 208 dust collector bags, damage to the dust collector was minimal and the unit was returned to operation later the same day.

Ee sprained left wrist reportedly from cleaning the sample bowl from the shatter box. He reported that his initially felt discomfort on 6/29/06. The discomfort continued to increase and on 7/30/06 he sought treatment

Two employees were installing a new guard on portable hopper/conveyor tail pulley. The new guard hung up on its supports and the injured lifted up on the guard frame to free it. The guard became free & unexpectedly moved pinching the finger between the frame of the hopper/conveyor and the side of the guard.

EMPLOYEE WAS CLEANING METAL FROM LATHE AFTER COMPLETING WORK PROJECT. MACHINE WAS NOT IN OPERATION, HE GRABBED A PIECE OF "RIBBON" TO REMOVE IT FROM THE CUTTING TOOL CHUCK & CUT HIS HAND. HE WAS NOT WEARING GLOVES THAT WERE PROVIDED, THEY WERE IN HIS BACK POCKET OF HIS COVERALLS.

Employee was struck by a bar during efforts to free a piece of ramp section from the forks of a lift truck. The bar was used to anchor the section in place while the forklift backed up. The bar was pulled toward the employee during this process striking him.

Employee was using an acetylene torch to cut an access part in to conveyor airslide. Sparks ignited belt no employees inured

Attempting to lift the cover plate off an air slide chute. The chute slipped and while attempting to hold onto it strained left wrist.

Employee was changing a conveyor impact saddle. Afer the saddle was removed from the conveyor frame, it tipped over onto his feet.

EMPLOYEE OPERATING FORKLIFT OVER WET ROADWAY. EE DROVE THROUGH STANDING WATER & LEFT FRONT TIRE FELL INTO AN OPEN MANHOLE. UNKNOWN AS TO WHAT HAPPENED TO COVER. BELIEVED TO HAVE BEEN PUSHED UP BY WATER & REMOVED UNKNOWINGLY BY A LOADER DURING CLEAN-UP.

EE NOTIFIED COMPANY OF MEDICAL TREATMENT RECIEVED 3/27/05 FOR AN INCIDENT THAT OCCURRED ON 02/28/05. EE WAS STRUCK BY A TIRE THAT WAS PULLED THROUGH THE TIRE SLIDE ON THE CULCINER LEVEL

EE was welding near freshly applied spray expansion foam and ignited a small area of the foam. Employee blew onto the material and extinguished it and claims to have inhaled some smoke. Employee ended his shift at 6 am without claiming injury. He reported the injury at 3 pm, 9 hrs after his shift ended.

Employee on 12/29 received treatment for an injury he reported on 11-10-04. At the time he was assisting in the replacement of conveyer belt skid plates. While pulling a skid plate out of position for replacement he felt a pull in his groin area.

EMPLOYEE WAS GRINDING ON A PIECE OF CHANNEL IRON. MATERIAL BOUNCED OFF OF HIS SAFETY GLASSES INTO EYE. EMPLOYEE WAS WEARING A FACE SHIELD AND SAFETY GLASSES.

EMPLOYEE BEGAN LOSING TIME ON 3/27/05 FOR SURGERY OF HIS LEFT KNEE, UNKNOWN AT THE TIME THAT THE INJURY WAS WORK RELATED. RECENT MEDICAL DETERMINATION THAT THE INJURY WAS WORK RELATED. EMPLOYEE STATED THAT HE SLIPPED WHILE HOOKING A CRANE TO A PORTABLE HOPPER ON 12/5/04.

EMPLOYEE WAS PLACING A VIBRATOR ON A RAIL CAR HOPPER CHUTE TO UNLOAD SLAG. REPORTED THAT HE STRAINED BACK WHILE PLACING VIBRATOR.

EMPLOYEE WAS USING AN "IRON WORKER" TO CUT A LENGTH OF 3X3 ANGLE IRON. WHEN THE PIECE WAS CUT, IT SLID BACK OUT OF THE V-HOLDER STRIKING THE FINGER BETWEEN THE PIECE AND THE TABLE.

EE STATED THAT HE DID NOT KNOW WHY OR WHAT CAUSED HIS BACK TO BE SORE.

USING A JACKHAMMER TO REMOVE REFRACTORY FROM INSIDE THE KILN AND STRAINED BACK.

TWO EMPLOYEES WERE REMOVING THE THIRD OF 12 KILN FRONT END CASTINGS WHEN THE PART SHIFTED, STRIKING THE THUMB OF ONE OF THE EMPLOYEES.

EE MOVING A WHEELBARROW STRAINED LOWER BACK. INCIDENT RESULTED IN A MEDICAL TREATMENT ON NOVEMBER 3. NOTIFIED EMPLOYER OF RESTRICTIONS ON NOVEMBER 10, 2003.

EE WAS SETTING A FAN HOUSING IN THE BASKET OF THE MANLIFT. THE HOUSING SHIFTED CATCHING THE LEFT HAND BETWEEN THE LIP OF THE PAN HOUSING AND HANDRAIL OF THE MANBASKET.

WALKING DOWN A SMALL RAMP. EE STEPPED OFF THE RAMP @ 6" TO FLOOR & TWISTED KNEE. TREATMENT BY PHYSICIAN BEGAN ON 5/29.

EMPLOYEE STEPPING OVER A PIPE AND TRIPPED ON PIPE.

DURING THE CHARGING OF THE FINISH MILL WITH GRINDING BALLS, THE SAFETY LATCH ONT HE CRANE HOOK WAS DAMAGED ALLOYING THE BARREL CRADLE TO JUMP THE HOOK. A PART OF THE CRADLE STRUCK THE EE IN TH E FACE.

EE WAS SEATED IN A CHAIR WHILE STANDING UP FROMT HE SEATED POSITION EE FELT PAIN IN HIS KNEE. THERE WAS NO CAUSE FOR THE EVENT AS HE DID NOT SLIP, TRIP, TWIST, ECT. FLOOR WAS CLEAN-UP.

EMPLOYEE WAS USING AN AIRLANCE TO REMOVE BUILDUP FROM A CONVEYOR TAILPULLEY. HE INSERTED THE AIR LANCE THROUGH THE PULLEY GUARD. IT WAS CAUGHT IN PULLEY PULLING HAND AGAINST THE GUARD.

EE WAS CLEANING UP A PRODUCT SPILL. HE STRUCK HIS HARD HAT AGAINST AN I-BEAM WHILE CROSSING OVER THE SPILL TO GAIN ACCESS TO THE OPPOSITE SIDE.

EE REPORTED ON 3/5/02, THAT THE INJURY CLAIMED ON 10-6-02 REQUIRED MEDICAL TREATMENT. EE CLAIM THAT THE INJURY RESULTED WHEN HE WAS USING A PRESSURE WASHER TO CLEAN A LOADER HE PULLED ON HOSE AND TWISTED THE UPPER BODY AT THE SAME TIME.

EE STATED THAT HE WAS ADJUSTING HIS WELDING HOOD WHEN SOMETHING ENTERED HIS EYE. STATED THAT A PIECE OF DIRT OR SOMETHING WAS ON THE HOOD AND IT FELL OFF INTO HIS EYE WHILE PLACING HOOD ON HIS HEAD.

DURING THE ROUTINE CLEANING OF THE CALCINER EE WAS USING A SPUD BAR TO CLEAN AN INSPECTION PORTHOLE. DURING THE PROCESS OF CLEARING MATERIAL FROM THE PORT EE STRAINED BACK.

EE WAS RETRIEVING HIS LUNCH FROM A CABINET AND PULLED A GLASS PLATE OFF THE SHELF. WHILE ATTEMPTING TO CATCH THE PLATE IT BROKE AND LACERATED SMALL FINGER ON LEFT HAND.

EE STATED THAT ON 2/4/02 HE WAS ADJUSTING A CONVEYOR BELT ON THE STACKER. HE CONTINUED DOING HIS REGULAR JOB AND ON 3/6 NOTIFIED HIS SUPERVISOR THAT HE HAD A HERNIA ON 3-15-02. EE MISSED REGUL AR SCHEDULED SHIFT TO RECEIVER FROM SURGERY.

EE REPORTED DRY SKIN BELIEVED TO BE CAUSED BY OR RELATED TO CEMENT. NO EXACT CAUSE COULD BE ATTRIBUTED.

EMPLOYEE WAS CLEANING WORK AREA, LIFTED A PIECE OF SCRAP METAL AND STRAINED HIS BACK.

EE WAS CUTTING STEEL WITH A TORCH WHEN HOT SLAG FELL INTO EAR CANAL.

EE REPORTED THAT OVER THE PAST WEEK SHOULDER DEVELOPED A PAIN. COULD NOT STATE DATE OR TIME OR ASPECIFIC INCIDENT THAT CAUSED CONDITION. ILLNESSPOSSIBLY RELATED TO USING A JACKHAMMER.

EMPLOYEE WAS USING A VACCUUM HOSE TO CLEAN THE INSIDE OF A DUST COLLECTOR. A PARTICLE STIRRED BY THE WIND THROUGH THE OPEN DOOR ENTERED EYE POSSIBLY AROUND SAFETY GLASSES.

EE REPORTED ON 7-23 THAT HE HAD A HERNIA AND WAS TO RECEIVE MEDICAL TREATMENT THE NEXT DAY. EE COULD NOT STATE WHEN OR HOW THE ILLNESS OCCURRED BUT BELIEVE IT DEVELOPED OVER TIME. COULD NOT CI T ANY CONTRIBUTING SPECIFIC EVENT.

EMPLOYEE WAS INSTALLING THE SOUTH HATCH DOOR ON FINISH MILL #1. AS THE DOOR WAS BEING PULLED UP, THE DOOR HANDLE SEPERATED FROM THE DOOR AND SHOT UP AND HIT THE EMPLOYEE ON THE CHEEKBONE. INVE STIGATION SHOWED THAT THE WELDS ATTACHING THE HANDLES TO THE DOOR FAILED. ALL DOOR HANDLES ARE BEING INSPECTED AND REPLACED.

ATTEMPTING TO DRIVE A BOLT ON GUNGEN FOR WEYH SECREW #2 WITH 5LB HAMMER HAMMER GRAZED A FLIGHT &REDIRECTED HAMMER. THE HAMMER STRUCK LEFT THUMB

EMPLOYEE WAS DESCENDING STAIRS AND STEPPED ON A ROCK BENEATH THE FIRST STEP. SHE STATED THAT IT HAD OCCURRED ON 9/18 BUT REPORTED THE INCIDENT 9/25/00.

AFTER COMPLETING CLEAN UP OF THE BARGE, EMPLOYEEWAS ASSISTING IN REPLACING A DOOR ON THE BARGE. THE DOOR WAS STOOD ON END, LEANING AGAINST THE SIDE WALL TO ATTACH A HOIST CHAIN TO A LOADER BUC KET. THE DOOR FELL AND STRUCK EMPLOYEE'S FOOT.

EE WAS OPENING DOOR FOR THE COAL MILL #1 FEED CHUTE. WHEN THE LATCH WAS OPENED THE DOOR 1' X 1' FELL OPEN PINCHING FINGER.

EE STATES THAT HE DEVELOPED A BLISTER ON THE HEEL OF HIS LEFT FOOT. THE BLISTER HE BELIEVES WAS CAUSED BY HIS WORK BOOTS ON 7-17. RECEIVED TREATMENT FOR AN INFECTION.

EE WAS REMOVING TABLE LINERS FROM THE RAW MILL. AS HE STRUCK THE WEDGE TO LOOSEN THE LINER A PIECE OF LINER BROKE AWAY. THE PIECE STRUCK THE EE ON THE JAW LACERATING THE LIP. EE WAS WEARING A FACE SHIELD.
