Employee looked up into a vessel that was being cleaned to check on personnel when he heard a noise that didn't sound right. When he looked up into the vessel he got sulfur dust in his eyes. He did have face shield and safety glasses on. Flushed eyes and went to clinic after shift for irritation. Diagnosed with conjunctivitis. Given an antibiotic and steroid drops.

Ther employee was walking to bolter when shotcrete came off the rib, striking him in the back and left side. He was taken to the ER for treatment & released to full duty. His knee continued to bother him. Further medical evaluation on Oct. 14, 04, a slight tear to his Meniscus was found and repaired, making the incident reportable.
