Employee was looking at sump pump with another employee. He fell backwards through an open hole, falling about 12 ft. into the sump, hitting the back of his head on the way down. He was transported by company ambulance to NNRH ER where he was pronounced dead on arrival.
Worker was fatally injured while driving a CAT 769C Water Truck along a standard mine haul road, returning to the water stand to re-fill the water tank. The water truck drifted left, onto the berm, travelling approximately 160' before "cartwheeling" and ejecting the worker. The worker does not appear to have been wearing the supplied seat belt.
The employee had backed under a feed chute to get a load. He dismounted the truck to activate the feed belt. He mounted the truck and moved it to spread the load evenly as it filled. He dismounted the truck to shut off the feeder and the truck rolled forward, pinning him between the truck and a cement pillar.
THE EMPLOYEE WAS ENGULFED BY HOT CALCINE FROM THE NORTH ROASTER, THEN FELL APPROXIMATELY 30 FEET TO CONCRETE FLOOR. HE WAS TRANSPORTED TO THE E.R. WHERE HE WAS PRONOUNCED DEAD.
THE MINE RESCUE TEAM WAS PARTICIPATING IN AN UNDERGROUND MINE RESCUE EXERCISE IN AN ABANDONED MINE. AS THE TEAM WAS WALKING UP A STEEP DECLINE ON THEIR RETURN TO THE SURFACE, TWO TEAM MEMBERS COLLAPSED. THIS VICTIM WAS PRONOUNCED DEAD AT THE SCENE. THE SECOND TEAM MEMBER WAS TRANSPORTED TO THE HOSPITAL WHERE HE DIED ON 10/23/02.

THE ORE BU CKET HAD BEEN CALLED FOR SO THAT THE CREW COULD RIDE BACK OUT TO THE COLLAR FOR THE BLAST TIME AND SHIFT END. AS THE ORE BUCKET ENTERED THE BUCKET WELL OF THE GALLOWAY, THE EMPLOYEE LEANED OV
THE EMPLOYEE WAS OPERATING A WATER TRUCK ON THE 1525 LEVEL. HE BACKED INTO THE 3475 ACCESS TO AN OPEN STOPE AND WENT OVER THE EDGE. HE WAS EJECTED FROM THE VEHICLE DURING THE FALL AND FELL APP ROXIMATELY 75' ONTO THE ROCKS BELOW. EMERGENCY RESPONSE TEAM MEMBERS RESPONDED IMMEDIATELY.
EMPLOYEE WAS INSTALLING GRATING ON TOP OF THE CIL TANK THAT WAS UNDER CONSTRUCTION. WHILE HANDLING A SECTION OF GRATING, EMPLOYEE FELL APPROX. 55' TO THE FLOOR OF THE TANK.
THE VICTIM AND A CO WORKER WERE CLEANING THE BALSTED AREA AT THE TOP OF A 10-FOOT DIAMETER ORE PASS. THE COWORKER WAS WEARING FALL PROTECTION WHILE WORKING AT THE EDGE OF THE ORE PASS. THE VIC TIM WAS WORKING NEAR THE ORE PASS WITHOUT FALL PROTECTION. THE COWORKER LOST HIS FOOTING AND FELL. THE VICTIM ATTEMPTED TO CATCH HIM AND FELL INTO THE ORE PASS, A DISTANCE OF 150 FEET.
THE ACCIDENT OCCURRED ON THE 1625 LEVEL-3375 ACCESS. A LOADED 30-TON HAUL TRUCK BACKED INTO A SPACE OCCUPIED BY A 22 TON HAUL TRUCK COPNTACTING THE OPERATOR'S COMPARTMENT AND RESULTING IN CRUS HING INJURIES TO THE OPERATOR. ER CAR WAS PROVIDED AT THE SCENE AND THE EE WAS TRANSPORTED BY AMBULANCE TO HOSPITAL. HE WAS LATER PRONOUNCED DEAD IN THE EMERGENCY ROOM.
DARKNESS, POOR VISIBILITY, HIGH BERM, MECHANIC VEHICLE RUN INTO AND RUN OVER BY HAUL TRUCK.
INJURED WAS HOLDING BACK-UP WRENCH WHILE 2 OTHER HANDS TORQUED STUCK DRILL RODS. BACK-UP WRENCH SLIPPED WHEN RODS WERE FULLY TORQUED, ALLOWING OTHER WRENCH TO FLIP OVER RODS AND STRIKE HIM IN THE HEAD.
EE APPROACHED BERM IN REVERSE TO UNLOAD HAUL TRUCK ON 5450 BAZZA DUMP. THE TRUCK WENT UP AND THROUGH THE BERM. THE TRUCK THEN ROLLED TO THE BOTTOM OF THE DUMP AND UPON IMPACT, BURST INTO FLAME S. EE REAMINED IN THE CAB OF THE TRUCK. THE FIREWAS FOUGHT BY AN EMERGENCY RESPONSE TEAM FOR TWO HOURS.
EE HAD BACKED UP TO THE BERM TO DUMP WHEN HE WAS NOTIFIED HE HAD A WHEEL MOTOR FIRE. THE ENGINE COMPARTMENT THEN CAUGHT ON FIRE AND HE WAS BURNED AS HE DECENDED THE LADDER.
EMPLOYEE RESPONDED TO BUILDING FIRE AT THE MILL REFINERY FACILITY, WITH EMERGENCY RESPONSE TEAM, IN THE COURSE OF FIGHTING THE FIRE, EMPLOYEE WAS FOUND DOWN ON THE FLOOR IN FULL FIRE GEAR AND SCBA. EMPLOYEE WAS TRANSPORTED BY AMBULANCE TO ELKO GENERAL HOSPITAL WHER HE WAS PRONOUNCED DEAD.
EQUIPMENT TRAVELING DOWN HILL SPRAYING WATER. IT GAINED SPEED VEERED OFF COURSE TRAVELED ACROSS 2 DOZER CUTS 18 INCHES DEEP. REAR TIRES HIT LAST CUT DRIVER EJECTED FROM EQUIPMENT OPERATOR LAN DED ON GROUND CRUSHED BY L REAR TIRE
EMPLOYEE WAS ASSISTING IN HOOKING A CHAIN UP BETWEEN A PICKUP AN DA LIFTALL FORKLIFT. THE FORKLIFT ROLLED BACKWARDS AND STRUCK THE EMPLOYEE.
