Employee was assisting another employee in pulling a distributor plug. While handing the plug off, the other employee grabbed the plug with both hands. As EE did so EE's thumb was smashed between the bottom T-handle and the distributor. Part of EE's thumb tip was pinched off.

An operator was hosing down floors for housekeeping, he noticed a burning sensation to both feet but continued to work. After several hours, the burning sensation progressed to a point of painfulness at which time he removed his boots and realized he had burns to both feet. Holes in his work boots (both) allowed Sodium Hydroxide exposure to his feet.

Maintenance team replacing a roller. Injured employee was holding a backing plate in position when another mechanic lost control of the roller pin, dropping it on the injured employees fingers. Middle finger on left hand lost nail, broken.

Employee was removing a vick from a 12" slurry feed line when hot steam/water sprayed out causing burns to the employee's left shoulder, left inner elbow, left hand, left ear.

Employee was removing a vick from a 12" slurry tank line. While removing the vick the employee was burned by hot water/steam. The employee sustained burns to both forearms.

Employee was pulling out the ramp on a trailer and twisted his back causing lower back pain.

Employee was working on the stack sizer oversize pump discharge side when he suddenly stopped working and he was instantly in extreme pain. The employee was allowed to stop working. He said he just needed to rest and the pain in his knee would go away. After 20 minutes, his supervisor and a fellow crew member helped injured employee into the supervisors truck to go to the hospital.

Straightening a copper tube and it snapped like a toothpick - took off the top skin on my finger.

Pipe swung out and hit him in the head. The pipe caused a cut in his head.

Employee was inspecting a propane tank on our man lift. When reinstalling the propane tank, he gave a sudden push with both arms extended outward and strained his neck. He has a sore neck from this incident.

Employee was lifting a section of pipe and felt a pain in his back. He said that he was fine and it didn't hurt again until a couple of days later.
