An equipment operator was operating a 40 ton haul truck when EE encountered a pothole in the road jarring back.
Operator was bolting together flange on a baghouse noltec when dust materials broke free, causing dust to come out between the flange and noltec, striking the operator in the face. Dust went around the safety glasses and entered the eyes.
Employee was assisting another employee in pulling a distributor plug. While handing the plug off, the other employee grabbed the plug with both hands. As EE did so EE's thumb was smashed between the bottom T-handle and the distributor. Part of EE's thumb tip was pinched off.
There was no accident. Employee was at the worksite but had not yet started work, EE had a heart attack at 5:31 am prior to beginning work, but EE was on the site.
On our Updraft Drying Fan failed and we had a fire in our Electrical Room (EER1). 45 minutes prior to this we know the current to the Fan went way up, speed went way down, power draw went way down, motor temps skyrocketed and the fan vibration decreased.
An operator was hosing down floors for housekeeping, he noticed a burning sensation to both feet but continued to work. After several hours, the burning sensation progressed to a point of painfulness at which time he removed his boots and realized he had burns to both feet. Holes in his work boots (both) allowed Sodium Hydroxide exposure to his feet.
Wire Harness shorted out in a 390 Cat excavator. This triggered the fire suppression system to activate. The employee exited the cab of the excavator as the suppression system went off. The employee may have inhaled some of the suppressant and was sent to the local emergency room for evaluation.
Maintenance team replacing a roller. Injured employee was holding a backing plate in position when another mechanic lost control of the roller pin, dropping it on the injured employees fingers. Middle finger on left hand lost nail, broken.
Employee was ascending the west stair tower close to the control room level when his knee popped and gave out on the employee. He was carrying a small battery powered portaband. The employee caught himself and was able to get to the control room level where he was seated in a chair and our first aid protocol was initiated.
Employee was holding a metal collar in his left hand while cleaning it with a hand angle grinder. The wire wheel on the grinder grabbed the collar spinning it into his left palm causing a laceration.
Employee was working in the primary building on primary screen #2 taking off spray bars. While stepping down off the ladder he twisted his knee and it was "a little sore." He was offered rest and an ice pack or medical care and he declined stating it was "no big deal." The employee notified management on 9/29/15 that he had a mildly torn meniscus and may require surgery.
Operator was climbing an extension ladder to secure it when it slipped; the operator remained on the ladder while it slid downward, injuring his right wrist. The employee reported it as a near miss and refused medical treatment, but yesterday afternoon we received a call for Workers Comp information so he is now claiming it is a work related injury.
Employee was removing a vick from a 12" slurry feed line when hot steam/water sprayed out causing burns to the employee's left shoulder, left inner elbow, left hand, left ear.

Employee was driving articulating 40ton haul truck on haul road with a fully loaded box. Turned a corner and the box rolled over. Cab stayed upright.
While flushing the Rev 3.1 with water, he got a cramp in his back, he kneeled down and while kneeling on the metal grating he rolled his right knee over the grating and his knee became very sore.
The employee was asking a co-worker a question about a tool and when he turned around to walk away he tripped over a step on the floor and hit his knee.
Employee was feeding the plant with the loader. The feed was hard to work with -lots of stick, wet, roots - didn't want to fall. A lot of banging around and twisting and turning which caused the employee lower back pain; he can feel it popping when he moves around.
Employee was working on the stack sizer oversize pump discharge side when he suddenly stopped working and he was instantly in extreme pain. The employee was allowed to stop working. He said he just needed to rest and the pain in his knee would go away. After 20 minutes, his supervisor and a fellow crew member helped injured employee into the supervisors truck to go to the hospital.
Straightening a copper tube and it snapped like a toothpick - took off the top skin on my finger.
EE was changing cutting edge on the 966 loader. He was holding the bolt head with one hand and operating the air gun with the other. The nut grabbed onto the threads which spun the bolt and split his finger open.
Worker was fixing rafters on screen plant building when ladder slipped off of ceiling & fell to ground. Worker caught himself on rafter & when he let go he fell approximately 12' landing on the ladder before landing on the ground.
An employee was working in the mobile equipment shop and had mobile equipment running while the garage door was closed. This is against company policy/procedure. The employee suffered minor carbon monoxide poisoning. The employee never lost consciousness. The employee has since missed three days of work and has not yet returned. The employee has 35 years of mining experience.
Replacing belts on primary conveyor, miss stepped, fell and injured his left arm.
While pulling a small boat from the pond, slipped on rocks and pulled left elbow.

Employee was inspecting a propane tank on our man lift. When reinstalling the propane tank, he gave a sudden push with both arms extended outward and strained his neck. He has a sore neck from this incident.
Employee was lifting a section of pipe and felt a pain in his back. He said that he was fine and it didn't hurt again until a couple of days later.
Employee was stepping down off of water truck, slipped off the step and sprained his left ankle.
