Miner was lifting a box, turned, felt pain in left knee, laid box down, stretched resuming work. Next AM, knee was stiff saw PA for some over the counter meds, knee brace, reported to work as self-treat/no restrictions. While on R&R, saw doc who placed on restrictions. The PA misunderstood initial location of injury while on duty underground; we are reporting accordingly.
Employee felt irritation in EE's right eye, EE reported to medic the medic found nothing. The irritation continued employee was taken to the doctor small particle removed antibiotics given.
Driller's helper was lowering a core tube into the drill rod string when employee's left index finger was pinched between the latch head and the drill rod resulting in an avulsion to employee's fingertip.
A miner was loading ground support bolts, EE leaned down to load supplies and felt a sharp pain in lower back. Self-treated until leaving site for R&R on 06/29; 06/30 & 07/01/17 saw a chiropractor then physician on 07/03/17, whom placed EE on physical therapy and Unable to return to work from 07/03/17 - 07/11/17. EE's normal schedule is 2:1; with return for work on 07/06/17.
While attempting to remove an oversize rock, with two other workers, from the #2 conveyor belt the miners left hand was struck by the rock when it began to move between the belt and rail; resulting in the miner receiving sutures for a laceration.
Employee was shoveling snow for two days before noticing swelling, tenderness and pain in left wrist and forearm. Employee "self-treated" until requesting to be seen by a physician on 01/13/17, who placed employee on 4-6 weeks restricted duty, left hand only, to minimize use and allow recovery; scheduled for a follow-up around 02/28/17.
Driller was installing a wedge to block in the drill, buildup on the installed wire mesh ground support flaked off behind safety glasses causing eye irritation. Employee was sent to the medic where a small foreign object was removed with a q-tip. No damage was done to the eye, but eye drops were prescribed as a preventative measure.
Employee was lifting Poly material decking, ring finger on left hand pinched between grating and metal support.
A mill employee was mixing copper sulfate within the designated hopper. When EE opened the lids and began placing the copper sulfate into the hopper, free hand was resting on the edge of the hopper when the lid fell down striking and pinching the left hand pinky finger between the lid and hopper. Operator was wearing all required PPE. ***Resulting in three sutures.
Sunday 12-6-2015 employee had notified his foreman that he bumped his knee on something and it was a little swollen.
Employee was applying Bentonite to the mud mixer; safety glasses and 1/2 face North respirator PPE on at time; employee got foreign object in Left eye
Miner was rigging out Jumbo drill. After de-energizing the drill, he disconnected the strain relief cable from the J-hook and the 3/8 in. relief cable sprung back and hit the miner's safety glasses causing a Corneal abrasion to right eye.
While installing a pipe the gasket was not set right. When EE went to set the gasket the pipe shifted and pinched his finger between the 2 flanges
Miner reported to GF and medic on 2/1/2015, of slight pain in his lower back; he refused treatment. Miner was treated by the on-site medic on 2/2/2015; refused treatment. Miner said the pain continued to get worse and was taken to town to be treated on 2/5/2015. Miner is not sure what caused the pain to start but thinks it may have been from moving a pump.
Employee was performing maintenance work on the rod holder unit of core drill. A bolt under torque used to hold back cylinder sheared causing cylinder to release and pinch employees hand.
A miner was taking a water pump off the boom of the jumbo. The cable used to suspend the pump had a sliver. When the miner was handling the cable to remove the pump, the sliver of cable inadvertently went through his glove which resulted in a puncture to his right thumb. This incident was not reported until (08/18/14) 10 days later; resulted in an infection requiring Rx medication.
A section of 10" HDPE arctic pipe was being offloaded from a roller assembly used to guide pipe sections for welding. While the pipe was coming off the rollers a laborer grabbed the picking eye at the end of the pipe string and caught his hand between the pipe flange and the roller pinching his hand and breaking his right middle finger.
Employee was adding a rod to the drill string and pinched his finger between the rod and feed frame. he states he was wearing his gloves.
Employee and helper were lifting sample reject bags from a pallet in an effort to locate specific sample information for special project. Employee injured upper portion of his back (area between his shoulder blades) while performing a two-person lift of a heavy (100 lb) sample bag.
Mill Operators had finished cleaning the spray bar for the SAG Mill. While reinstalling the spray bar end slipped out of the far back holding bracket which caused the spray bar to slip down in the back end and the front end with the plate to go up. The Operator got his hand caught between the plate and the lip of the trommel cover.
Installing a huggy clamp to couple 2 pieces of SDR9 8" DRISCO pipe one end of the pipe released, striking the miner on the left cheek, temple and eye. Initial eval; concussion, released for 7 days of modified duty. Returned to work, 2 days later sent back for re-eval, admitted for 2 days, observation only. Released, diagnosed w/ post-concussion NO Work until follow up in 7 days.
The miner was loading 9'X 5' welded wire on to a tractor. While lifting the wire it became entangled in the other wire left on the stack , the miner turned with the wire causing a injury to the back.
Crew was pulling rods to change bit, the tube was stuck in the barrel so the driller got above the barrel to pull it out of the head and went to hand it down to the driller assistant, when handing the barrel to his partner, the barrel slipped from the driller's grip, dropping about four feet and landing the assistants right foot.
Mucker #4 pulled out of shop and wasn't able to turn. A MEM mechanic went out to check the machine out, he then saw the "Articulation Locking Pin" was still pinning the front of the machine to the rear of the machine. The mechanic pulled the pin out of the lock and when installing it the pin pinched his finger between the frame and the hole for storage.
Warehouse tech was looking out of the man door window at back of warehouse with his left hand resting on the overhead door track. When tech saw the warehouse semi was driving towards the door to bring it in for the night he pushed the button to raise the door. Warehouse tech's fingers were caught. Tech was wearing gloves.
Employee was taking down damaged vent bag, while doing this the employee got dust and debris in both eyes. It's company policy to wear safety glasses at all times while underground and the employee wasn't wearing safety glasses at the time of the injury.
The stope intersected a diamond drill hole that was making about 80gpm. EE was attempting to install a packer into the hole when he cut his finger on the wire that had been trimmed to install the packer.
A leak had developed on CIP tank #2. While trying to apply a patch the operator was sprayed in the face and on the body with slurry.
Miner was repairing a Warman pump and to do so he had to remove the front of the guard. While repairing the pump the back of the guard swung down smashing his hand resulting in the Miner needing sutures.
He was carrying door jams and went to set them on the ground, they slipped off his shoulder, and he tried to catch them and the edge of the metal sliced his right thumb open. EE then went straight up to upper camp and had the onsite medic prep his thumb. The medic called Delta Junction's medical center and told them EE was on his way.
The operator was draining the strip vessel. When the operator opened the drain valve the hose on the other end came out of the sump spraying the operator with solution. The solution landed on the operators forehead and ran down into the operators left eye.
Employee, while installing a florescent light fixture, cut his left index finger. This laceration required seven (7) stitches from a medical doctor. Employee was NOT wearing cut gloves, or other suitable hand protection, while performing this task. Employee was cut by a sharp edge on the ballast cover portion of the fixture.
While completing a function test on a pneumatic hand grinder employee began to walk around side of table and inadvertently stepped on the air hose. This pulled the grinder out of his left hand and the cutting wheel struck back of right hand while wheel was still rotating.
While removing a victalic coupling from a knife valve, which was secured by a come along, the knife valve dropped slightly and swung over pinching the employee's little finger between the corner of the valve and the nearby piping.
While carrying a "Finhoe" to be placed into his tractor, employee stated it slipped out of his left hand and landed on top of his right hand. Distance tool fell was approx. 3 ft. Tool weight approx. 4 lbs. Result of impact was a hairline fracture to the 5th metacarpal.
Employee was changing out a motor on the CIP storage tank, he was having to look up to guide the motor out of the top of the tank. After the job was completed employee felt something in his eye. Upon further assistance from medic it was determined that there was a small piece of plastic embedded in left eye.

While attempting to change out a speedy sleeve on a 9 Yrd. loader sleeve slipped out of his hands and while attempting to grab the sleeve recieved a laceration to the left thumb.
Employee was turning the feed frame from flat hole to 90. Top sprauge was put in pocket backwards. When Feed frames got turned sprauge fell out and struck employees hand.
While employee was loading back holes, lost his balance. As he grabbed for the wire panel to catch himself, a small piece of the wire went through his gloves resulting in a small laceration in his webbing between his index and middle finger of his left hand.
Employee was lifting and removing empty 55 gallon drum barrels, approx weight of barrels were 6 to 7 pounds. While employee lifted the barrel approx 1 ft. off the ground and twisted around to lay the barrel back down felt a pop sensation in his lower back.
