While the IP was loading snow covered drill steel onto their Light Vehicle, the steel slipped from their right hand, forcing the left hand into a plastic bucket, injuring their middle finger. (Fracture of the bone under the fingernail)

Limited access between the wall, rod holder, drill string required the helper to use an awkward position while attempting to break the drill bit free. When the bit released, EE's foot slipped from the pipe wrench and EE's shin struck the rod holder, causing a laceration requiring stiches.

The IP is employed as a mechanic and was leaning over vehicle trying to loosen seized bolt when had onset of lower abdominal pain.

The IP reports that they were walking on the stair after MSHA class for lunch when they felt their Right knee "catch" and began to be painful with weight bearing and lateral movement. Progressively gotten worse. The IP was notified they needed surgery 11/14/25, did not return to work on 11/27/2025. The IP had surgery 12/12/25.

While stepping down from the driller's platform facing forward, the injured person (driller) placed weight on EE's right foot as EE moved from the step onto the grating. At that moment, EE's right knee buckled, causing EE to lose balance and fall to the ground

IP was setting up trip cable for remote mucker, while pulling bungee cord tight, cord slipped and hook pierced into left palm between thumb and index finger. 2 sutures applied to close open flap of skin.

Diamond drill helper was pulling a tube on a down hole. The tube slipped back down into the ball clamp, helper moved left hand lower to reestablish grip. Their left hand pinky was past end of lifter case when tube entered back into ball clamp, pinching the side of LH pinky, they then pulled their hand free when they felt contact.

On 08/18/2025 @ 3:00 PM Geology teams entered XV 1457-320, where they heard ground support fail (1 split bolt). Geo Tech teams inspected the same day and ordered the heading shut down after the heading was bolted. On 08/19 mining crew completed ground support. No further access was granted until 09/08 when Geo Tech team entered to inspect, the FOG was discovered, MSHA was notified.

Employee was searching for a propane leak, slipped on a rock and twisted knee.

The Injured Person was taking tooling apart and when it broke free, they impacted the back of both hands. The impact caused a laceration on both hands requiring two stitches per hand.

Mobile mechanic had been torquing a fastener on a loader bucket tooth when the hand tooling slipped, resulting in the mechanic twisting the left knee. "The accident occurred on 8/29/25, however, the EE was cleared to go back to work the same day. The EE chose to return to place of residence and got an off-site diagnosis, which was provided to the mine operator on 9/23/25."

After filling a water tote secured in the back of a flatbed LV at the water stand , IP went to dismount from LV bed and felt a crack in their ankle which caused them to slip and fall.

Diamond driller was removing a bit from the barrel using a pipe wrench. When the bit broke free, EE's right hand went past the pipe wrench and hit the rib.

IP was completing paperwork for light vehicle service and shut left pinky in door of light vehicle.

Graduate Engineer was tasked with getting a guide tube. While opening a new bundle of guide tubes with a pair of Klein tools the band snapped back, causing a laceration between the 4th and 5th fingers of the right hand. The engineer was not wearing gloves at the time of the incident

Worker was moving a knife valve in a the IT basket when the end of the chain EE was using fell through a grate. The IP attempted to readjust the knife gate by lifting and tilting it to unstick the chain when the worker felt a pop in their left shoulder.

Transfer case caught on the rear bed guard and then fell onto IP's finger causing a laceration to the left ring finger.

Nipper tripped while carrying a small section of bull hose, put a hand out against the rib and injured their right wrist.

The IP arrived at the onsite clinic reporting apparent groin discomfort with an apparent hernia, they left site for further diagnostics.

The dozer Operator had dismounted the dozer using a three point contact body position. When their left foot contacted the ground, the dozer Operator felt and heard and felt a "pop" in their ankle with immediate pain.

The IP stepped up 18 inches to a semi-trailer, while stepping the IP heard a pop in left knee, then fell back landing on the hitch area of the semi tractor, ERT members stabilized and on site then transported to the ER to continue treatment. Employee was released from the ER on the 11th, then on the 15th, was released to restricted duty by a orthopedic surgeon.

IP was washing off cv010 belt after getting grating off of cv10 IP smashed finger on grating that was dropped.

IP (Helper) pinched LH middle finger while wearing impact gloves. Evaluated immediately after incident and given full RTW prior to following shift. On 03/12/2025 - IP had second evaluation while on days off - Noted "no fracture identified" - received second full RTW. On 03/29/2025 - IP wanted additional evaluation - evaluation indicated fracture in LH middle finger.

IP was using a Parmelee wrench to break latch head by applying downward pressure when EE's hand slipped. The wrench spun around the tube. When the IP's hand slipped it caused EE's head to lower and the wrench struck EE in the lip, causing a small vertical split in EE's lip.

IP works as a diamond drill helper, IP was scheduled to work night shift 1/30 (starting at 5:45 PM), IP reports waking up at 11:00 AM on 1/30 with pain and burning in forearm and had a numb hand. The IP reported to the mine site clinic at about 3:00 PM, rested 1/31 on site and went to a Dr. appt where IP was given work restrictions and rescheduled Feb 7 to reassess.

At the end of shift a worker had been disposing a section of drill rod into the steel waste bin. While attempting to throw over a T45 drill rod into the steel waste bin, the worker felt a strain in their right shoulder. The injured person (IP) reported that their left footing had slipped while throwing the rod into the waste bin.

Lifted a pipe multiple times underestimating its weight, until pain occurred, then alerted shifter, then taken to the onsite clinic.

During the process of adding rod to a drill string an offsider pulled a rod off the rack striking self in the face causing 2 small lacerations above the right eyebrow which were sutured.

Slipped on ice/snow at the end of shift. On the surface near the admin building.

9:15am Construction Shifter entered the 1820 to do a work area inspection for an upcoming paste pour and discovered the fall of ground - the worker put up sign to keep people out and immediately proceeded to the surface to report it.

Haul truck operator was getting into the cab of their truck to answer the radio. The operator stated that with their left hand, they had grabbed the handle on the inside of the door and with their right hand they reached up to use the grab bar and struck their right index finger on the bar.

Helper was dropping the station barricade when the helper tripped over a hose. EE fell forward landing on hand and knees. Took EE to site medic, the medic recommended x-rays. After a CT scan the following day it was discovered EE fractured the knee cap.

EE was adding rods on an up hole. A piece of gravel came down and contacted them between the lip and nostril. There was no broken skin, pain or anything to otherwise indicate an injury. They wiped the dirt off lip and went about the day. Approx. 4 hours later they ate a banana and a tooth in the front right (about where the gravel contacted them) came out while chewing.

A haul truck operator of a Sandvik TH551i (UT 198) bumped their elbow while being loaded with oversize at the 477 Muck Bay.

The IP was removing rock at the face with a hand tool, to drill a lifter when loose rock fell from the face striking them between the shoulder blades. They walked to the LV, went to the onsite clinic and was sent to town for further diagnostics.

The IP was transferring blank emulsion from a transfer tanker U/G, while adjusting the transfer hose, the cam lock fittings on the hose disconnected under pressure, striking IP in the lower lip

Operator slipped on a mud covered step causing a laceration to EE's right hand.

Mechanic strained back while performing tire change on loader.

After completing a pump rebuild in the gravity area. The IP tried to lay over an old impeller by hand and felt a pop in EE's bicep.

Finger was cut while replacing the 4th of 4 drive belts from an electric motor to a large pump.

IP was cutting anchor (deadman) for drill rig with cutting wheel. While spooling down preparing for next cut, the wheel contacted their knuckle resulting in small laceration to knuckle. Shifter notified immediately, walking boss took IP to medic. Medic treated and used 2 stitches. Gloves, face shield and guard were in use at time of incident.

Dust/debris fell into eye behind glasses while installing cable bolts. IP thought IP had removed debris, but reported to clinic at 5:45 PM, IP went to a specialist the next day for further evaluation, and removal of debris.

A fall of ground occurred in the rib of the NZ3 ramp between the 808 fuel bay and the 617 air doors. The fall of ground was below the spring line and measured approximately 40 feet long by 15 feet high by 6-8 feet deep.

Pinched index finger while positioning liner handler, causing a small laceration.

Operator was driving down the road in a surface loader, when the operator continued to warm the hydraulic oil by raising and lowering the boom and curling the bucket. While doing this the cold hydraulics responded slowly and the bucket kept dumping which drove the teeth of the bucket into the road causing the loader to come to a complete stop. Operator was wearing a seatbelt.

Operator was cleaning heat exchanger plates in the CIP circuit, while setting plates on rack, some of them got caught and fell. While attempting to catch the falling plates the operator's right middle finger was sliced open.

The operator of JB241 was scaling the face in the development heading at 1412-600 and while scaling the drill steel contacted a misfire which has initiated.

IP was lifting a 50lb air pump and hanging it on the rib when it fell and struck the IP on the top of the right foot. IP was seen at the clinic and sent offsite for further evaluation.

Pump Fitter received a laceration to the left hand and ring finger while removing impeller from pump.

The injured employee was placing a panel of mesh on the rib. The mesh began falling towards the employee and employee placed right arm up to stop the wire. The weight of the mesh pulled employee's arm back causing injury to the upper arm.

The IP stated that when EE started to get down from the back of a pickup, EE's left knee was at an angle and EE had felt a pop which caused EE to fall to the ground.

Helper was working in a clean dry station maneuvering rods from the string when they pinched a finger between the rod and feed frame of the drill. EE was taken to the medic for review and referred to hospital for x-ray which confirmed a right ring finger distal phalanx fracture. EE has been released to full duties as tolerated and placed on a return to work plan.

Nipper received a laceration to the left eyebrow from carrying wire mesh, resulting in stitches.

IP was putting the pump suction hose into the water, they twisted and threw the 2" suction hose and felt a pop in their mid to lower back. IP fell down after they threw the hose and then stood back up and walked to the rib to sit down.

IP was taking a a pump off of the back of their truck. The pump caught EE's shirt pocket causing EE to lose balance, dropping the pump and falling on top of it. Their safety glasses struck the pump causing a laceration to the corner of EE's left eye. The site PA placed 6 stitches to close the laceration.

R hand dominate Laborer for RAM. Was climbing down ladders in Mill "press" yesterday. EE slipped and fell approx. 8 ft. EE grabbed the ladder to stop EE's fall resulting in a jolt to R shoulder.

Worker was removing an air hose when it came off the fitting. Workers hand struck a metal shroud cutting EE's finger.

Mechanic fell into the engine compartment, striking L shoulder. EE was on restricted duty for a previous injury to the same arm.

Millwright was trying to move a pump case into place with EE's hands, they slipped and caught their left hand between the pump case and volute pinching EE's left index, middle and ring finger. Fracture to the tips of middle and ring finger.

Employee was taking apart a jumbo nip, when the stuck nip released causing the employee to strike the rib with right pointer finger knuckle. The employee was cleared to return to work. During their scheduled time off the employee scheduled an appointment for the 29th of June, the day before they were return to work. The doctor has placed restrictions on their return to work.

IP was driving a haul truck and began developing lower back pain. Upon arriving at the surface shop EE's back began to hurt and EE was unable to pick up the wheel chocks.

Mucker was doing a final back drag of the heading when they a small amount of fly ruck and dust. Backed the mucker out of intersection. Walked in and found an exploded booster. Secured scene.

IP was repairing wireline cable, when IP pulled on wireline cable, the cable slipped through their hand, causing the cut end of the cable to slice through their nitrile exam gloves lacerating their left palm just below the thumb. IP was given 7 stitches by the site medic

IP had cut a sheet of mesh down to finish bolting to grade line. After cutting the sheet they moved the wire into place, while moving it one of the cut ends of the wire hit them in the ankle. The piece of wire penetrated the IP's boot and socks and punctured into the skin near their ankle.

Employee was retrieving a hose off the rib, lost their footing, twisting in the process, injuring their knee. Note: Employee was released back to work after being seen offsite. After their next rotation the employee saw an orthopedic specialist and needed surgery. Employees first missed day after surgery was 07/27.

During annual firearms training the injured employee had shot several other guns when they used the .338 rifle. After the first shot they adjusted their stance placing the scope close to their face, upon shooting the kick back caused the scope to contact the bridge of their nose causing a laceration. It was repaired with 7 sutures.

While on break, employee was carrying a small cardboard box and was walking outdoors from wing 700 towards wing 600, when they slipped and fell on the ice covered parking lot. During the fall the employee injured their right shoulder and wrist and their left elbow.

Worker rolled down hill and was unable to return to the top resulting in hypothermia.

IP slipped and fell striking lip on the equipment that the IP was working on. IP received 11 stitches.

Employee stepped on rock while exiting IT, rolling EE's ankle

Injured party was removing drill steel from the jumbo. When pulling back on it, pinched right hand/pinky finger between the steel and bushing on the jumbo, causing a laceration.

Injured party was stocking a heading with ground support. EE had picked up 6 rock bolts and tripped while walking. EE's thumb went into the slot in the rock bolt and made contact with the rib causing EE to fall and cause injury to their hand.

First Aid- 4 stitches - Returned to full duty. While attaching a multi tool to a scaling bar, miners hand had slipped and made contact with the blade of the multi tool, causing approx 1" laceration to the thumb of EE's left hand.

Warehouse technician complained of sore wrists from the repetition of the work being performed, including computer data entry.

IP SUSTAINED AN INJURY TO LEFT BICEP WHILE REMOVING 5 GALLON OIL CAN FROM LV BED

Driller had gone into area to start bolting. Ground had started to deteriorate and EE deemed it unsafe and rigged out.

The worker was using damaged broom that broke during the sweeping and broken metal handle caused cut on the workers hand/finger. After receiving 10 stitches worker was returned to regular duties.

IP reported to the supervisor that they had soreness in the lower back while working at 1695, they stated there was nothing that directly caused the pain.

IP was on a loose berm of muck getting into a man basket with weight on both feet, right foot slid down the berm putting the weight on the left foot, injuring it.

Injured party was working at 1998 stope access and stepped up on a windrow. Employee lost footing and rolled left ankle.

Haul truck operator was traveling down the 1260 ramp, hit a large rock causing the truck to bounce. The operator's knee was caught under the shifter, striking the shifter, injuring the operator;s knee.

Employee felt pain in their shoulder which continued to get worse over a few months. This led to pain in their upper back which had to be evaluated by a specialist. From the evaluation, the doctor identified the condition was due to the repetitive work of charging holes. The employee was placed off of work with a treatment plan. First lost day after evaluation was 10/20/2022.

IP stepped down a short ladder, mis-stepped, and twisted EE's ankle

IP was working to help gear in a jumbo, as EE was approaching the jumbo a water hose struck the IP in the head, on the hard hat, the impact caused facial injuries. It is believed that the hose had hung up on the jumbo, stretched, then released, striking the IP.

Injured party was climbing down the access ladder of haul truck and felt a pop in lower back and went to the onsite PA. IP was released to go home on regularly scheduled time off. Two days prior to returning to shift, IP visited an orthopedic doctor and was not released to come back to work on 10/27/22.

Helper was in the process of adding an NQ rod and felt a sharp pain in upper back along the edge of right shoulder. The pain went away until they started loading core trays into the LV.

Helper was pulling tubes from the drill rod and placing them on the rod stand. They noticed that their back was becoming agitated. After the 6th "run", they started to have a sharp pain in upper back. Shortly after, they said that it progressed to "pins and needle felling" in their right arm down to their hand. The helper notified the driller who then notified the walking boss.

IP was trying to pass between the passenger side jack of the Normet powder truck and the right rib. IP stepped along the rib at an angle causing EE to roll EE's ankle as EE stepped around the jack. Reported to the Physicians Assistant and treatment of sprained ankle.

The miner was removing old pipe from the heading. When the miner dropped the pipe to the ground EE had knocked some dust that was collected at the top of the pipe that went into EE's right eye. Employee had debris removed from EE's eye and was given prescription eye drops. No lost work days

On night shift of September 16th, a NSMS production loader operator was remote mucking the final firing of the BD 1184 100 P1 stope. While retreating with a full bucket, the stope brow failed back to the intersection, partially burying the loader. No personnel were in the vicinity of stope at the time. 800-1000 tons of material fell causing damage to front of mucker.

Employee was communicating with co-worker who was running the porta power to break bead off tire. The employee told the co-worker to apply pressure and it began pushing the bead off the rim. The tool began to fall and the employee grabbed the tool to catch it. The jack then suddenly blew off the rim and pinched the employee's hand between the stud and the jack.

Helper was in the process of setting up drill site. EE was getting ready to hang the 480 cable when EE stepped back from the rib. At this time, EE felt 2 "pops" in the right knee, set the cable down and sat down. The driller notified the mine shifter and Swick walking boss. Statements and pictures were taken. The medic checked out the helper's knee.

IP was washing a vehicle with a 1 inch water hose when IP strained a muscle in the lower back

The IP was repairing SSE 83 battery cables. As a piece of heat shrink was being cut to length, the tips of the cable cutter cut the left index finger.

Loader operator opened the loader door from the outside, as the door swung open they tried to catch it and prevent it from slamming. The operators finger became caught between the door handle guard and an accumulator cylinder.

Drill crew stopped to dispose of waste scrap metal at the bin located near the water treatement plant, while exiting the truck from the passenger seat, the driller stepped out onto a large rock. Rolling ankle and losing balance, caused IP to fall to the ground. Drill crew contacted supervisor on radio and IP was taken to the site medic for evaluation.

While mixing Xanthate operator smashed finger between hammer handle and bottom of barrell of Xanthate.

The IP was holding a piece of metal stock with one hand while attempting to use the other hand to operate a hand held cutting tool. The cutting tool kicked back during the cutting of metal and contacted the IP finger causing injury requiring sutures.

IP felt neck pain while performing duties related to removing pin from loader. IP indicated this has been an ongoing problem. IP has own physician treating the condition at home for the recurring issue. Original discomfort occurred on May 11th but was not diagnosed until May 26th.

After mechanic was finished diagnosing issue on drill rig, EE started walking back to the truck. As EE approached the truck, EE felt pain in the lower back. The only activity taking place at this time was walking.

While exiting ug loader, IP felt discomfort in abdomen. IP examined offsite where it was detemined to be a failure of mesh from previously repaired hernia. No heavy lifting or exertion was involved when IP felt discomfort.

Helper was in the task of trying to remove the core barrel out of the head. Barrel was coming out of the head when the balance of the barrel shifted causing it to angle toward the driller and control panel. The helper attempted to divert the barrel and in the process the barrel hit the control panel and made contact with the helpers right wrist and hand.

Service crew operator slipped on a rock and fell causing injury to their left knee.

Miner was walking up the stairs in the break room area to fill their water glass. The miner slipped and dropped the glass, breaking the glass as it struck the metal handrail. When the miner reached for the handrail to keep themselves from falling, the broken glass cut their left palm and ring finger.

Employee was changing out G.E.T and was informed to use a steel hammer to remove then a copper hammer to reinstall. EE struck G.E.T with the steel hammer and the third one EE hit had broken a shard of hardened steel and it lodged into EE's finger. EE went to the on-site medic and had it removed, then got three stitches.

Helper was pulling wireline through stuffing box and had to change the rubbers. Helper pulled glove off to retrieve an allen wrench and did not put glove back on. Proceeded to pull the wireline with the gloveless (left hand) and suffered a very minor laceration from a barb in wireline.

THE CORE HOLE WAS TD'D. DRILLER AND HELPER WERE PULLING CORE BARRELL OUT. CORE BARRELL GOT HUNG UP AND HELPER ASKED DRILLER TO BRING THE CHUCK DOWN TO HELP EASE THE AWKWARDNESS. AS THE DRILLER FED THE CHUCK DOWN THE CARRIAGE PLATE CAME INTO CONTACT WITH HELPER'S FOOT THAT WAS HANGING OVER THE DECKING CAUSING MINOR FRACTURE AND LACERATION TO BIG AND INDEX TOE ON LEFT FOOT.

Injured miner was spotting an equipment moving material when EE slipped and fell down.

While stepping down off loader, right foot slipped off of step resulting in shin striking lower step causing laceration of right shin that required sutures.

Drill Helper was emptying a 10ft NQ inner tube. EE struck the inner tube with a dead blow in an attempt to free core and dislodged a small piece of core from the lifter case. The rock fell along the tube and struck the drill helper on the upper lip leaving a small laceration.

Injured person stretched torso prying rock from discharge.

IP tripped while unrolling hose, hit elbow on wall, hurting a shoulder.

IP was exiting light vehicle and slipped on ice injuring left knee.

Miner was struck in the face by a shovel full of rocks while hand cleaning area

After finishing a run, green offsider was attempting to install a loading chamber. After getting the treads, offsider felt a snap in their wrist and a sharp discomfort that went to EE's elbow. The injured area immediately started to swell. Offsider stated that their pinky went numb. Driller shut down the operation, and contacted walking boss and shifter.

Helpers hand got pulled into the sheave wheel while holding wireline. Driller didn't see helper holding wireline as driller was focused on the drum to the right. Helper was out of peripheral vision of driller. Helper received sutures. Helper seen by orthopedic surgeon to repair tendons on left hand middle finger and pinky finger.

Fall of ground in the 917-100 level that was in the process of the bolting cycle. Bolting had not yet been completed when the back failed, partially covering the front of the equipment bolting the back and ribs. No injuries to miners working in the area or from this event. Contributing factors under investigation.

During installation of 4" pipe, a worker was injured when pipe slipped off the basket and pinched the 3rd finger on left hand resulting in finger having to be medically amputated.

IP grouting cable bolts when hose came loose from tube spraying grout into IP'S face. IP did not report incident to operator until Aug 3, 2021.

After connecting the water hose in the L1CL sump employee's Rt forearm contacted a bolt on the rib causing a laceration that required sutures.

IE was working on a small access platform (2 steps) near the Courier Analyzer and turned to step off onto the walkway and misjudged the height of the step and fell onto the walkway.

IP, while performing quality control duties, was looking up, following an air-line, and tripped over a horizontal scaffold pole and fell to the floor. IP attempted to break the fall with IP's left arm. Sustained a closed non-displaced fracture of the left radius.

Welder struck in face with line boring hydraulic drive motor couplings.

Employee rested hand on frame after lifting hood. Hood came down on fingers.

Plant operator washing filter press fell from top deck.

Miner gets little finger caught between the grout pot and the work basket.

Jumbo operator slipped while walking in drift. While EE placed arm out to brace self, EE cut hand on a rock which required stitches.

The Miner was moving 8" HDPE pipe over the top of 4" HDPE pipe. The larger pipe slipped falling on miner's left hand middle finger. Conditions were cold and windy, temperature approximately -15 with snowy ground conditions.

Employee slipped on ice and sprained shoulder.

1659 drive intersected a known fault that had been previously supported. Drift was blocked by fallen material.

Handling core tube splits, caused laceration to left hand.

Employee was operating an overhead crane while assisting with the installation of a loader cutting edge. The strap broke, the cutting edge fell, and bounced off EE's left steel toe boot. This took place inside the surface shop on a concrete floor. The employee was seen in the emergency room the following day and diagnosed with a broken left big toe.

Pump fitter cut hand while assembling a mono-pump requiring stitches.

Miner experienced soreness in back. Injury may have been influenced when EE removed a section of hose wrap during a maintenance repair of a hydraulic hose.

On 5-7-2020 associate was mixing soup using an immersion blender, the associate reached for seasoning to add to the soup, and lost control of the mixer, the mixer was failing out of the soup kettle when the associate tried to catch it with EE's hand which in the process came into contact with the spinning blade of the mixer causing multiple lacerations.

Driller was in task of trying to re-align Kelly rod, in process when raining head the rod released from chuck on to helpers right hand between box & pin of the two joining rods, fractures to two fingers (middle/ring) from occurred injury. Employee was released to light duty work same day of hospital visit.

A miner slipped on ice and fell on surface road at beginning of shift. *UPON DEFINITIVE DIAGNOSIS, A SMALL ROTATOR CUFF TEAR REQUIRED SURGERY, AND RETURNED TO REGULARLY SCHEDULED ROTATION WITH RESTRICTIONS ON 3/29/2020.

Miner sustained a chipped tooth while moving a wire mesh panel.

IE was moving a pallet of materials with pallet jack. IE was pushing loaded pallet jack with materials into place and strained left lower leg.

Plow trucks hydraulic tank came dislodged and truck ran it over jostling operator.

3 miners were running cable in the 926 Fun Zone Down Ramp; requiring a new cable spool on a stand they attached Vic clamps on each side of the cable spool to keep the spool centered. 2 miners pushed the spool to allow room to install the VIC clamp which caught the tip of 1 miner's ring finger which was caught between the spool and the Vic clamp removing the tip of the finger.

Employee leaned out of the operator's window of UG haul truck to speak to another employee, when the window unexpectedly retracted striking the employee in the mouth. The employee's left upper central incisor was cracked requiring repair.

The employee was clipping the detonators onto the det cord at the face of 1249. EE looked up to grab the det cord and a small rock fell from above striking EE in the mouth damaging EE's front upper teeth.

EE was climbing down off the scissor deck when EE lost balance and landed on top of the jack stabilizer housing. For potential severity concerns of the injury to EE's groin, EE was immediately sent to the Hospital ER for evaluation and was placed on Restricted Work Duties.

IW hurt shoulder removing windshield from jumbo. Saw PA in Fairbanks 8/27, MRI 8/28 no definitive diagnosis at time. IW flew home 8/28. Orthopedic specialist seen 9/18 due to appt. availability, Physical Therapy x 3wks started 9/23, with definitive paperwork for no work allowed for IW.

The highwall above the 1525 portal collapsed, damaging the portal entrance support & mine air heater intake structure. Evacuation was initiated, all personnel removed & accounted for, UG mine tag-in board was cleared. On surface area of the 1525 portal was barricaded & all area personnel removed. All power, propane & water lines associated were isolated & tagged out.

The operator of SSE 105 / Gehl forklift finished filling the DEF (Diesel Exhaust Fluid) tank, reached up with left hand to close the door covering the DEF tank and battery box, not realizing right thumb was hanging over the enclosure the door smashed the tip of thumb causing a laceration which required stitches.

Two people were moving a pump, it began to slip and fall and while reaching to grab it from falling the employee felt a twinge in the lower back area. Tried to self-treat it and after two days requested to be seen by a physician whom placed EE on restricted duties.

An estimated 30T FOG occurred within the X Vein 625 level, at the intersection of the X Vein ore drive and access to the North Zone orebody; a three way intersection. Believed to have occurred at the 0600 firing time; a long-hole stope firing on the X Vein, from the 695 down to the 625, a little over 500 ft away. The 625 is chained off with a No Entry sign, barricade and berm.

A miner was working in L1-876 hanging trough and was unable to reach the wire from the ground and chose to use a 5 gallon bucket to stand on. It became unstable causing miner to fall injuring right knee. Self-treated for 3 days, was sent to Fairbanks for an evaluation, was released to duties as assigned, while on R&R saw personal physician placing miner on No Work as of 07Aug19.

Team member foot slipped off step resulting in a loss of balance. Team member put out left hand to break fall and sprained shoulder.

Discovery of a FOG in intersection at LD ramp intersection w/ the 1556 level access. During shift a FOG was discovered as a wedge failure dropping approximately 40ST of material from the backs at the intersection to the ground. Several sheared/snapped split set bolts were found in material and backs. The area was closed w/ barricades, berms, & warning signage to prevent access.

Operator clearing lime transfer chute, wearing face-shield, safety glasses. Upon completion operator closed inspection hatch, liberating lime dust particles into left eye causing corneal abrasion, treated w/ Rx antibiotic & lubricating eye drops 4x daily.

Powder man finished check scaling the face and proceeded to make up caps, walked around to the truck bed to place caps in the basket, their back was to the face when a rock fell off and struck them in the mid-back; resulting in a compression fracture of the spine (vertebrae) and the employee was placed on Modified/Restricted duties initiated on 10/15/18.

A miner was switching equipment when ee heard a small air leak, during investigation for the air leak ee heard something falling to the ground, upon arrival to the 812 location ee discovered a large portion of the right hand rib, spring line and back had fell to ground.

Employee was operating haul truck #2 going down east deep ramp. While traveling down the ramp the truck hit a bump in the roadway. Employee felt pain in lower back. Reported to the onsite clinic and self-managed onsite until Monday (06/04/18) when the employee was seen by physician in Fairbanks whom placed employee on restricted/modified duty.

Driller was lowering core tube retraction tool (also known as the overshot tool) down into the cased hole. As the driller was returning the overshot tool to its resting location, EE inadvertently tripped hydraulic lever to the "up" position, rather than the "down" position, which pulled the driller's arm upwards.

Miner was lifting a box, turned, felt pain in left knee, laid box down, stretched resuming work. Next AM, knee was stiff saw PA for some over the counter meds, knee brace, reported to work as self-treat/no restrictions. While on R&R, saw doc who placed on restrictions. The PA misunderstood initial location of injury while on duty underground; we are reporting accordingly.

Employee was cutting asparagus with wet hands and no cut glove and cut EE's right index finger

EE had wet down the muck-pile, while rolling up water hose EE slipped on some residual hard-cap paste landing on EE's right arm. At the end of shift EE requested to be seen by the Pogo physician's assistant where it was determined to send EE to Fairbanks for an x-ray; which was definitive for a fracture. EE is onsite currently w/restrictions and a splint/cast.

Employee felt irritation in EE's right eye, EE reported to medic the medic found nothing. The irritation continued employee was taken to the doctor small particle removed antibiotics given.

The 1958 East stope was shot end of shift, the night shift miner began workplace exam to wet down muck piles post blast, noticed a cracked shotcrete within the 1958 E/W intersection; immediately retreating to equipment parked in the muck bay approx. 300 feet away from the referenced intersection. The miner contacted the supervisor, soon after the fall of ground occurred.

Employee was working to repair electrical grounding issues, bent over checking grounding connections. As ee straightened up, ee felt a cramp or tightness in back, attempted to walk it off; it didn't clear up and continued to get worse, ee stopped work and reported it to the supervisor. On 11/06/17 was seen by a Dr. whom placed ee on restrictions for lumbar strain.

A mechanic complained of irritation in right index finger. EE was taken to site medic a metal sliver was removed from finger and first aid dressing was applied. The mechanic noticed swelling in finger 3 days later, EE was taken to a doctor. The doctor diagnosed an infection and prescribed an antibiotic.

The employee was working at the face of the 857 East loading a round. When EE turned to retrieve supplies, EE's foot was caught in some mud and EE's knee "twisted" and popped causing the employee fell to the ground. EE was taken to the local ER, treated for an apparent patellar dislocation, given a splint, released w/out restrictions and is to follow up with personal physician.

Driller's helper was lowering a core tube into the drill rod string when employee's left index finger was pinched between the latch head and the drill rod resulting in an avulsion to employee's fingertip.

A miner was loading ground support bolts, EE leaned down to load supplies and felt a sharp pain in lower back. Self-treated until leaving site for R&R on 06/29; 06/30 & 07/01/17 saw a chiropractor then physician on 07/03/17, whom placed EE on physical therapy and Unable to return to work from 07/03/17 - 07/11/17. EE's normal schedule is 2:1; with return for work on 07/06/17.

6-19-17: Fairbanks AST Dispatch notified by Pogo that 2 contractors were attacked by bear 5 miles offsite, reported 1 fatal & 1 w/ minor injuries, confirmed w/AST bear was killed. 3 state official's onsite to investigate. Names were withheld pending positive SME identification. Living victim was taken to Fairbanks Memorial Hospital. Investigation continues.

While attempting to remove an oversize rock, with two other workers, from the #2 conveyor belt the miners left hand was struck by the rock when it began to move between the belt and rail; resulting in the miner receiving sutures for a laceration.

On 03/04/17, mechanic was walking in weld bay to retrieve a piece of pipe when employee ran into the boring bar that was set up on a bucket, striking self in the mouth breaking a tooth. Initially seen by onsite PA, on 03/06/17 employee saw a dentist for definitive diagnosis/treatment plan; given a temp. cap, full work release, instruction to follow up with personal dentist.

Employee was shoveling snow for two days before noticing swelling, tenderness and pain in left wrist and forearm. Employee "self-treated" until requesting to be seen by a physician on 01/13/17, who placed employee on 4-6 weeks restricted duty, left hand only, to minimize use and allow recovery; scheduled for a follow-up around 02/28/17.

Driller was installing a wedge to block in the drill, buildup on the installed wire mesh ground support flaked off behind safety glasses causing eye irritation. Employee was sent to the medic where a small foreign object was removed with a q-tip. No damage was done to the eye, but eye drops were prescribed as a preventative measure.

The employee was fabricating a drill steel carrier onto a Skyjack fork lift. They had been using a die grinder to oblong some holes when they felt a prickly sensation in glove on right thumb. They found a metal sliver in the pad of their thumb that they removed. The next night felt discomfort in right thumb and was taken to a medic.

Employee was lifting Poly material decking, ring finger on left hand pinched between grating and metal support.

EE was working on new basket installation on KBT20, was welding, cutting and grinding using all PPE. EE did not notice irritation to L eye until later in evening. EE was not sure if eye was scratched or irritated. EE sent to town clinic on 7/19. 7/26 received a definitive diagnosis from Dr. that the debris was imbedded and removed easily w/instrument.

A mill employee was mixing copper sulfate within the designated hopper. When EE opened the lids and began placing the copper sulfate into the hopper, free hand was resting on the edge of the hopper when the lid fell down striking and pinching the left hand pinky finger between the lid and hopper. Operator was wearing all required PPE. ***Resulting in three sutures.

Employee states "after I had measured the depth of drill hole for casing set-up, I attempted to retract the tape." "It appeared to be stuck, yanked on the tape, and it cut through my glove." Employee sustained minor laceration (6 stitches) to webbing area between the ring and little fingers of Right hand.

While foam grouting in the 1300 EXP, a wedge in the back failed, pulling ground support out with it and also impeding passage. Grouters were approx. 6 feet away from failure in the deck of a getman scissor lift. Grouters were pumping chemicals into a grout hole when the wedge came out. It is believed that the grout being injected displaced the rock and caused it to slip

Sunday 12-6-2015 employee had notified his foreman that he bumped his knee on something and it was a little swollen.

Mechanic V rolled RT ankle on a 3/4" hose. Self-treated `til flew to WY 10/06 for 1st appt, x-ray, MRI on 10/07. Snow delayed flights, reschedule 10/08; neg x-ray, MRI rescheduled 10/09. Doc called into surgery cancelled MRI, rescheduled MRI 10/12 definitively diagnosed 3rd degree sprain/strain put in cast, NO Work starting 10/13; follow up appt 10/19 to decide surgery or not.

Employee was applying Bentonite to the mud mixer; safety glasses and 1/2 face North respirator PPE on at time; employee got foreign object in Left eye

Miner was rigging out Jumbo drill. After de-energizing the drill, he disconnected the strain relief cable from the J-hook and the 3/8 in. relief cable sprung back and hit the miner's safety glasses causing a Corneal abrasion to right eye.

While installing a pipe the gasket was not set right. When EE went to set the gasket the pipe shifted and pinched his finger between the 2 flanges
