Employee was preparing 8' section of 10" pipe to be welded and installed. Employee unbolted upper portion of pipe to install purge valve. upper section of pipe came free began to slide off rack, employee grabbed for pipe and his left thumb for caught between pipe and angle iron of pipe rack.
A miner had parked his haul truck and climbed down out of the vehicle, he then proceed to swing the wheel chock into place under the tire when he felt a pull to his right arm

Employee was trying to close a 6 inch water line valve when he felt a strain or pull in his right shoulder/bicep muscle. At the time he didn't feel the need to be seen by a physician, he later requested that he be seen for observation.
Employee was loading hard insulation shields into a man-lift for installation when he pinched the middle finger on his right hand. The pinch resulted in a laceration that required sutures to close. The employee was released back to work without restrictions.
EE was pulling a 3 conductor number 6 cable with eight other miners, the pull was 450 feet. Men were spaced about 50 feet apart in cable tray. The pull was on 12/8/2012 and the injury was reported on 12/10/12. The injury was to the left shoulder.
Employee putting away tool bucket into a gang box which was pretty full already but decided to shove it in the box anyway. While doing so his thumb got bent back resulting in a sprained right thumb. Is on limited duty currently, but is working. We have work within his limitation.

A metal shaving entered workers glove (open cuff) and became embedded in index finger of left hand.
Two Ironworkers were setting a rebar column onto a double rebar mat footing. After the column was guided off Ironworker climbed the column to cut extra rebar off. Injured Ironworker started to finish tying column dowels to rebar mat. Ironworker cut rebar loose and accidentally dropped one and it fell hitting injured in the back causing a 1 1/2" laceration to the right rear flank.
A plant operator was taking a material sample when some of the materials got onto his sleeve, he then shook his sleeve and some materials flipped up under his safety glasses, he stated that he rubbed his eye and it became irritated and he washed it out. He requested that he be seen by a physician and was given a prescription as a precaution.
EE was breaking bead on right front tire of 40 ton articulated haul truck, when ram broke loose, striking EE on the left side of the face.
