EE was using Metabo Grinder in an inverted position and cutting against the direction of the blades rotation. the blade kick backed, causing laceration to the left index finger.
EE looking in Gang Tool Box and the lid unexpectedly closed due to broken shocks and struck EE in the face impacting safety glasses and nose.
EE slipped on a face shield while exiting building.
Injured employee was using a grinder to remove bolts in a unit. The grinder handle was preventing EE's reach from bolt and EE decided to remove the handle. Without a strong securement handle, the grinder kicked back and made contact with the employee's left middle finger, cutting through a glove. This resulted in a laceration requiring 3 stitches.

Operating light vehicle and struck a bollard resulting in a broken right radius.
Injured employee alleges knee pain when stepping on to grating that was on the ground in a laydown yard. The grating was placed on the ground to prevent slip hazard in muddy conditions.
Operator used air pressure to clean a line and opened valve before bleeding pressure. Air pressure burst a catch trap and oxalic acid with debris hit operator's face knocking safety glasses off.
EE was stepping over a small containment wall. They stepped over with left foot and stepped on a grading that was to large for the hole it was covering. It caused the grading to flip and the EE fell into the hole and ripped their pants and cut their right upper knee on the grading.

WSP employee entered a porta-potty supplied by the Mine site that was not secured or staked. After entering the porta-potty it tipped over causing the employee to be trapped and sprain ankle.
Docking lift gate level pulled in right index finger of EE into roller. Amputation of right index finger at 2nd knuckle.
Employee was walking in 6-8" of mud on a slope; EE slipped in the mud and EE's knee hit some concrete.
Broken rib resulting from slip and fall onto horizontal scaffolding bar. Thought it was just back pain and reported late.
Hearing shift. EE has loud hobbies and prior hearing problems but audiologist did not confirm not work related. EE works in various areas of the plant.

Employee was walking downstairs and rolled their ankle. Stairs and boots were checked and all were in good order.

Employee was filling up haul trucks and had to walk over a separations berm. As they were walking over the berm they slipped and caught themself before hitting the ground and stained their shoulder
Employee was pulling on a hose that was stuck and the hose broke free. When the hose broke free the employee fell backwards and hit the back of their head on a handrail causing a laceration.
Employee entered a sump that had what they believed to be only water to install a pump. The sump had residual caustic in the water. The employee did not put on their chemical resistant boots and water topped their boots causing chemical burns to their feet.
Removing bolts from a differential using a Hytorc and wrench. The wrench slipped off the bolt and struck the worker in the safety glasses. The safety glasses broke and caused a laceration to the right eyebrow. Worker received 10 sutures.
Employee was walking through area and slipped on material that had been spilt on floor. Employee fell and hit head.
Employee slipped on wet floor while stepping out of a sump falling and injuring back.
IW was cleaning preheater tubes using a hydroblaster flex lance. As they pulled the flex lance out of the tube, they mistakenly held pressure on the bypass (foot pedal), causing the lance to continue spraying. The whip hit them in the bicep, resulting in an injury.
While traveling downhill operator lost control of UTV on gravel causing the UTV to roll over.
Employee received stitches while removing a pump.
Maintenance worker walking through the mill pad with contractor. Contractor's pants caught on valve opening the valve and getting caustic in eyes
Employee was climbing down a scissor lift without gloves, when EE caught middle finger on a metal loops attached to the railing of the scissor lift.
Contributing conditions are unknown at the time. Employee could not pinpoint when accident took place and reported late.
Operator was lifting a heavy object and injured EE's right shoulder
Employee stepped on uneven ground causing EE to fall to the ground and sprain EE's ankle.
Employee was moving pallets and a pallet caught and twisted EE's hand resulting in wrist injury
Descending stairs inside building while using handrail to go on break and missed step causing employee to fall. Stairs and handrails are in good condition and stair treads are grating therefore the coefficient of traction was not a contributing factor.
Employee was stepping from a concrete pad to a gravel surface, slipped and fell striking their knee on the ground.
EE walked behind the truck for setup. During setup, co-worker extended the rear hydraulic bumper striking EE and pinching EE between the bumper and concrete wall. EE did not feel as if had received any major injuries but pain increased when bearing weight on right leg. EE was then transported to a medical facility for further evaluation.
Crew members were walking to the lunch tent due to a weather stand-down due to high winds. While walking behind another person, the wind blew that person's hard hat off which struck the injured employee in the mouth resulting in a chipped tooth.
Employee was walking to the Change Room when boot got caught in the grading causing them to fall and twist an ankle.
Employee was performing cleanup work when EE tripped on an air hose and twisted EE's ankle.

Employee was doing pre start inspection at the crusher on C10-CV01 tail pulley and guards. Employee stepped back and turned to their right when their ankle rolled and fell to the ground. Employee was assisted by co worker and lead person.
Employee was holding the threader against the conduit to start the threading process. When the employee let off the trigger to readjust how they were holding the threader, the threader kept running spinning around the conduit. Employee attempted to unplug threader when the threader hit them in the leg causing fall to the floor landing on shoulder.
Employee received a significant laceration to thumb while using a grinder.
Employee was conducting maintenance work. When EE moved hand away from the part, EE received a small cut to finger. Employee reported minor incident to the supervisor. On July 9, 2021, employee informed supervisor that the finger had become infected and employee was sent to occupational medical provider for treatment.
Employee was walking on elevated floor grading and did not see a portion was removed for maintenance. Employee fell approximately 2 feet to lower level before they caught themselves. When falling, their right foot struck a storage vessel.
Employee was cleaning up debris on uneven ground when EE's foot slipped and EE twisted their ankle.
Driller was lifting a drilling rod/steel into position when the top of the rod/steel contacted a brace on the drill mast which caused the bottom of the rod to swing outward striking the miner's finger causing a dislocation injury.
While removing rigging from a main I beam located above the CHP steam header, a 3T beam clamp weighing ~25 lbs. fell 10-15 feet and ricocheted off a cable tray into an employee's left forearm. This resulted in bruising and restricted mobility of employee's fingers.
Operator was heading out to do samples when stepping into some mud inside the containment of sump 3. This caused them to slip and twist right ankle and fall to the floor. This incident happened by pump 32 inside the sump 3 containment.
Employee was pulling rubber tubing off a beaker. The beaker broke and sharp piece of glass cut the left forearm requiring stitches.
Ground personnel walking and surveying road project from Seps pad to Mill pad. Employee stepped on rock which rolled out from under boot, causing employee to fall backwards into a sitting position and then on to back. Knee is reported as slight strain, no swelling, slight limp to walk.
Employee was pressure washing when pressure washer cut through a boot and hit big toe.
Employee was in a confined space with an electrical heating element still energized. Employee touched element receiving second degree burns.
Operator was using the forklift at the new packaging area. When EE stepped off the forklift, EE rolled left foot and ankle on an uneven surface.
Miner was walking backwards down truck ramp when they fell. Miner received a laceration to the back of the head.
PER MSHA DEFINITION, FIRST AID INJURY ONLY UNTIL 07/01/2020. Employee was attempting to avoid falling 30lb fire extinguisher so jerked leg backwards. However, foot stayed planted and knee was hyperextended.
Employee was clearing debris from conveyor when they experienced pain in right shoulder.
Employee was conducting clean-up activities at Paste when EE slipped and fell while carrying conveyor parts.
Employee was exiting utility vehicle when first step teetered causing EE to strain EE's left knee.
Employee was using a hand shovel to clean up concentrate when EE strained lower back.
Prep Lab Technician was grinding blast holes and inadvertently placed a prepared ring and puck bowl on the upper ledge of the ventilation hood which was attached to the work bench. The ring and bowl fell from the shelf and impacted employee's left thumb.
Employee smashed right middle finger in door while lifting door to reinstall on vehicle.
Supervisor checked on progress of a welding project. After speaking with welders for about ten minutes and assuming the metal was cooled, the supervisor placed hand on the metal and received second degree burns to left fingers and palms.
Employee used hammer to free impeller motor from a vertical pump. Hitting the impeller, a piece of metal broke off & entered employee's hand. Employee treated onsite by contract medical. On 2.25.19, employee notified employer that wound not healed correctly. Employee referred to offsite medical facility where a metal fragment was found embedded under the skin.
Employee exited the office trailer going down the stairs onto the concrete pad. Due to icy conditions, employee lost footing and fell. Employee suffered a scraped knee.
Employee was removing tail stock from the lathe which required strenuous pushing and pulling. Employee felt a twinge of pain in groin area but did not report until 09.22.18. On 9.22 employee declined need for medical care. On 10.22.2018 employee advised that they were still experiencing pain and was referred for professional medical care.
On 9/15/18 employee reported blurred vision and eye irritation. Employee was packaging Mill Concentrate when some dust got into EE's eye.
On 09/07/2018 employee reported that they stepped on pillar to avoid getting wet while testing eyewash station. Employee slipped off the pillar and hit their chin on the eyewash bowl resulting in a laceration to the chin which required 4 sutures.
Employee was cleaning liquid floc from floor by using hose to push the liquid flock into the floor sump when ee slipped and fell on left shoulder. The floc is both wet and slick.
Employee #1 standing on platform pushing material through hopper for packaging. Employee #1 encountered a rock and tossed it over the edge of the hopper. Employee #2 was rounding the corner near the hopper and was struck in the head by the rock.
Komatsu grader parked on ready line in a ditch with door opened completely to the left. Employee grabbed door handle with left hand and braced right hand at the door frame. Employee had to use force to shut the door because of the angle. Door impacted right index and right middle fingers.
Employee was on top of the pla removed cloth & walked backwards tripped on a head metal brackets & fell backwards into the gap left from removed plates. Employee fell aprox 6 feet & restrained by EE's fall restraint system from contacting floor. Medical treatment not initially sought. On 4.20.18, employee requested & rec'd medical
At the P7 packaging area, the employee was coming down the ladder (facing away from the ladder) as they descended they twisted around facing the ladder missing the bottom step causing them to trip and stumble twisting an ankle.
On 2/14/2018 employee reported that on 2/10/2018 employee was operating a skidsteer while packaging mill concentrate and a gust of wind blew the concentrate in left eye. Employee was wearing a half-mask respirator and safety glasses. Employee then switched to a full-face mask and completed shift. Upon return to work, employee eye was red and irritated.
Employee was working in the paste tailing basement using a skid steer loader to clean up. He reached over to pick some trash when he felt a sharp pain in the back of his right leg.
Employee was starting a HCL burner when he looked into the flame eye and he received burns to his face.
Employee was removing trash from a floor sump grating when she stepped out of the area and slipped and fell causing her to bruise her knee.

Employee was attempting to remove a 2" nipple that was frozen into a valve with two pipe wrenches. He was using great force and felt pain in his abdomen. Was not a reportable incident at the time but now is scheduled for surgery to repair umbilical hernia.
Employee was walking between bags of product when he tripped on a pallet. He latter complained of back pain and received physical therapy treatment for a strain.
Employee was dumping a press and scraping materials off of the plates to load into product bins, when he got to the last three plates, he went to move one of the plates and splashed product in his eyes and mouth. The employee was wearing ppe at the time

Employee was assisting in repositioning of trailer mounted welding machine in order to attach to truck hitch so that welding machine could be moved to Pronto Constructors yard.

Employee was troubleshooting pressure switch problems at a PLC cabinet, when he turned to walk away and his right knee gave way.
Employee was removing lockout locks from a column cell when he accidentally got acid in his boots, he didn't notice it at first until later he realized that his foot was burning and he was treated for burns on his foot
Employee was exposed to CL2 vapors while conducting an inspection of the area, he received medical treatment for lung irritation.
Employee was unplugging a drain hose when he was sprayed with materials which got under his safety glasses and in his eye.

Employee was using a Metabo Grinder. The Metabo bounced back after coming in contact with the 1 1/2" pipe being cut. Metabo blade contacted employees middle left finger.
Employee reported that he had hurt his left shoulder, he alleged that it might have occurred when he was putting together office furniture and disposing of tires into a dumpster
Employee was securing safety lanyard to life line with left hand when left shoulder popped dislocating shoulder.

While moving a 6" pipe in pipe rack #4, employee had his right index finger caught between the pipe and a steel beam.
Employee was trying to close a 6 inch water line valve when he felt a strain or pull in his right shoulder/bicep muscle. At the time he didn't feel the need to be seen by a physician, he later requested that he be seen for observation.
Parking lot was iced over from the previous nights snow storm and employees were waiting to get the ok to go to work. GF went to truck to get a sign in sheet to take names of those who were there. Slipped on ice and dislocated left knee.
Employee was loading hard insulation shields into a man-lift for installation when he pinched the middle finger on his right hand. The pinch resulted in a laceration that required sutures to close. The employee was released back to work without restrictions.
EE was pulling a 3 conductor number 6 cable with eight other miners, the pull was 450 feet. Men were spaced about 50 feet apart in cable tray. The pull was on 12/8/2012 and the injury was reported on 12/10/12. The injury was to the left shoulder.

Cold conditions caused the outside layer of cable to break while the employee was stripping the cable. Possible handling of the cable.
A plant maintenance inspector was inspecting the wear liners in a ball mill when his foot sunk into a soft spot of mud and he was unable to get it unstuck without assistance. No injury or exposure occurred to the individual however this was called in out of an abundance of caution.

Avulsion - Left Middle Finger - Pulling a welding lead to the 3rd floor, lead got caught, reached hand under decking to release, finger was cut by sharp edge
Employee using a tool called (Klever Kutter) to remove electrical tape that was used to hold together 3 wires being pulled through a conduit. Employee was cutting the tape towards his left hand and the cutter slipped and caught his glove, cut the glove and consequently cut the top of his hand between the left forefinger and thumb area.
IW was striping wire using a recessed blade cutting device (Klever Kutters) and sustained a 2" laceration to the left index finger when the cutting device slipped and contacted finger.
Employee was cleaning up at end of shift, moved gangbox out of way, felt a 'pop' in right arm and chose not to seek medical treatment. Worker returned to work 8/14/12 and requested medical treatment and was diagnosed with right bicep rupture.
While putting a wall piece into place, pressurized mud blew out, knocking the employee into a tank on the east floor.

Worker was setting concrete forms, he went to take a set and tripped over some electrical conduit. He fell forward landing on his right hand, the hand rolled under it's self. He was taken to Medcor to be evaluated, it did not appear broken, and went back to full duty.He was taken to Medcor for checkups and on 7/2/12 he was taken to see a doctor and x-ray show it to be broken.
EE was on morning break and was retrieving her lunch box off of a wooden work table to RTW. EE is very short, so she stepped onto the bottom storage area of the work table so she could reach her lunch box, when doing so a wood splinter from the plywood top of the table entered her upper right leg.
Worker strained lower back while setting up 40' extension ladder.
Employee stepped off of Cooling Tower foundation approximately 31", stepped down without proper access/egress and twisted his right ankle on a 2 X 4 piece of lumber that was under some plastic.
While walking out of a building, he missed the first step and dropped onto the left knee with his weight, causing a strain.
Non-compliance of company & mine site safety policies in regards to equip. & PPE. EE while wearing safety glasses, did not don face shield while using an elec. hand grinder that he had remove the handle from. While prepping a weld, 1/8" abrasive grinding wheel got caught between pipe openings, kicked back into EE's face, causing 1' cut to his face just outside of his left eye.
Employee cut the left thumb with a pocket knife while cutting a zip tie.
The employee had been welding and grinding when it was noted that her right eye was red.
While searching through boxes in Connex trying to locate bolts in order to complete daily task, EE states that his footing slipped on a rope in floor. As he caught his balance he reached out and placed his hand through a pane of glass in Connex window.
Employee was descending the stairway and tripped while turning onto the permanent landing, causing a strain to the left knee.
Cutting a 2x4 with Skill saw by hand, not on saw horse. Grabbed 2x4 in different position, pinched the blade. Saw kicked back and lacerated hand.

Employee was walking across wooden scaffold planks. His foot came into contact with a piece of 9 gauge wire, causing him to lose his footing. Employee caught himself on the scaffold by hooking his arm around the guardrail. Employee received chest wall contusion, sprain and bruising of shoulder and upper arm. Received prescription medicine.
Employee treated on his own for unknown illness, later reported that he had Bronchitis, then later reported that he had Chemical Pneumonitis from breathing unknown chemicals
Worker was cutting 2X4 lumber with a circular saw. The lumber was lying on a saw horse unsecured. Worker made the 1st cut without difficulty. The 2nd cut started and the saw got stuck in the lumber causing it to jerk back cutting his left ring and little finger. Lacerations to both fingers needed 7 stitches each to close the wounds.
Laceration to forehead (above left eye) - was helping coworker ream holes in header with an air gun, coworker lost grip on gun, gun spun around and hit employee in forehead.
Contusion/Cellulitis - Right Middle Finger - Walking to work area, carrying tools, tripped over torch hose, fell down, 8 lb sledge hammer fell on hand.

"Filed under Protest" No reportable injury or illness occurred. First Aid was administered after EE complained of light headed, nauseated.
Injured employee and another employee moved a filing cabinet to gain more access to a printer that was jammed. Injured employee tripped over the moved filing cabinet and fell on her shoulder dislocating her shoulder. Onsite medical team treated her immediately and an ambulance picked her up and took her to an emergency room.

EE tried to step up to concrete slab 2.5' high in his effort his body weight impacted his right knee and caused sprain on his right knee. He was taken to the Medcor facility on site where he was treated as First Aid.
Employee was walking to his foremans truck at the end of his shift when a gust of wind blew his hardhat off his head. While trying to recover his hardhat he tripped and landed on his right hand with his arm extended. His right elbow was dislocated due to the incident and he was taken to the hospital to have his elbow relocated and was given a restricted duty release.
Employee was using an airline to unplug a line, he uncoupled the line and Splashed materials into his face and eyes.
A metal shaving entered workers glove (open cuff) and became embedded in index finger of left hand.
Tightening bolts with a spud wrench that had a sleeve bar attached. Wrench slipped off the nut. Finger was pinched between sleeve bar and girder beam.
Two Ironworkers were setting a rebar column onto a double rebar mat footing. After the column was guided off Ironworker climbed the column to cut extra rebar off. Injured Ironworker started to finish tying column dowels to rebar mat. Ironworker cut rebar loose and accidentally dropped one and it fell hitting injured in the back causing a 1 1/2" laceration to the right rear flank.
A plant operator was taking a material sample when some of the materials got onto his sleeve, he then shook his sleeve and some materials flipped up under his safety glasses, he stated that he rubbed his eye and it became irritated and he washed it out. He requested that he be seen by a physician and was given a prescription as a precaution.
EE was breaking bead on right front tire of 40 ton articulated haul truck, when ram broke loose, striking EE on the left side of the face.
Employee was using a pneumatic chisel to install seal ropes on a filter cloth plate. He did not note any specific event but stated that his wrist began to hurt later. He was seen by a physician.
Employee was taking a ph sample in the Redox tank, when he opened the lid to get the sample and was exposed to chlorine vapors. He was given oxygen on site and then transported to a physician for observation
A trash dumpster fire occured between 1:25 - 1:35 p.m. that burned/smoldered for longer than 30 minutes. The fire was controled using the mine water trucks and equipment, the fire department was called in as it continued to smolder. Damage to the dumpster was minimal. The fire was reported to the district and the 800 number
Miner demonstrated a possible recordable shift in his left ear above his initial baseline.
On 5/20/09 employee complained of tearing and discomfort in left eye, upon observation a foreign particle was observed in his eye and he was transported to a physician for medical treatment. The employee stated that he was not aware of an incident that could have caused it but thought that it might have occurred when cutting and threading pipe the day before.
Employee was pressure washing the plates on a filter press and attempted to twist the nozzle on the end of the sprayer to adjust the spray pattern. While twisting the nozzle, his little finger slipped off of the end of the spray gun into the high pressure spray. This resulted in a small laceration to the employees right little finger
After removing his gloves for a better grip, employee was trimming the liner, cutting away from himself with his right hand while pulling the newly cut liner the opposite way with his left hand. This action resulted in a laceration at the base of the employee's index finger on his left hand. Laceration required 7 stitches.
EMPLOYEE STATED THE DROVE HIS PERSONAL VEHICLE TO WORK AND PARKED IN THE EE PARKING AREA. THE EE ALLEGED THAT AFTER HE EXITED HIS PERSONAL VEHICLE, HE WALKED AROUND TO THE PASSENGER SIDE TO TAKE HIS LUNCH COOLER, WHEN HE NOTED SOME ANTS ON THE GROUND. IN TRYING TO AVOID THE ANTS THE EE ALLEGED THAT HE SLIPPED & PULLED HIS LEFT CALF MUSCLE.
A FACILITY OPERATOR ALLEGED THAT HE STRAINED HIS LOWER BACK WHILE WEIGHING AND RESTACKING WOODEN PALLETS. THE EE STATED THAT HE WAS PLACING THE LAST OF ELEVEN PALLETS ONTO A STACK WHEN HE FELT A PAIN TO HIS LOWER LEFT BACK.
A MILL OPERATOR WAS USING A LADDER TO ACCESS A HOSE FITTING ON A LEACH TANK WHEN THE LADDER SLIPPED AND THE ELPLOYEE FELL APPROXIMATELY 2 FEET. THE EMPLOYEE USED HIS HANDS TO BREAK HIS FALL BU T THE RESULTING IMPACT CAUSED SPRAINS TO BOTH OF THE EMPLOYEE'S WRISTS.
A MAINTENANCE MECHANIC REPORTED ON 8-30-02 THAT WHILE ON SWING SHIFT HE HAD BEEN FREEING A POSITIVE DISPLACEMENT PUMP BY USING A 48" PIPE WRENCH, THE EE STATED THAT HE HAD NOT NOTICED ANY BACK PAIN DURING SHIFT, HOWEVER THE FOLLOWING MORNING HE COMPLAINED OF LOWER BACK PAIN. THE EE WAS SEEN BY A PHYUSICIAN AND RELEASED, HE LATTER RECEIVED MEDICAL TREATMENT ON 10-2-02 BUT CONTINUED
A WATER TRUCK OPERATOR WAS BACKING A CAT 773 WATER TRUCK UP A RAMP BUT AT 90 DEGREE TO THE RAMP WHEN HIS RT REAR WHEEL OVER RAN THE EXISTING BERM CAUSING THE TRUCK TO OVERTURN. THE OPERATOR RE CEIVED BRUISES TO HIS LEGS AND BACK AND A LACERATION TO HIS RT. LOWER LEG. THE OPERATOR WAS RELEASED TO RESTRICTED DUTY BUT RECOVERED. THE OPERATOR BEGAN MISSING WORK ON 5-20-02.
