While installing a 6' roof bolt the bolt became stuck on the side of the drill pot. As the I.E. was freeing the bolt up they felt a pop in their left shoulder creating a sharp pain. I.E. was taken off work until an MRI could be conducted. First shift missed was 12-7-2020.

I.E. was lifting standing support jacks to move to another area. Upon lifting the jack from the mine floor I.E. felt a pain in their lower abdomen. Employee continued to work until doctor took them off for hernia surgery. First missed shift was 12-6-2020.

I.E. was picking up trash in the unit 5 intake. While picking up trash to put in the mantrip I.E. twisted their body and felt a pain in their left knee. Employee missed the following shift due to their injury.

I.E. was cleaning up pile of coal chunks next to a belt line. When the I.E. went to throw a chunk a coal onto the belt when they felt a pull and sting in their stomach. I.E. went to the doctor and had hernia surgery the next day.

I.E. was loading 6' roof bolts on the roofbolter. While loading the bolts I.E. felt a sharp pain on the inside of left elbow. I.E. continued to work and received lifting restrictions from a doctor on 10-13-2020. I.E. is continuing to work. Reporting as a restricted duty case.

I.E. was hanging high voltage cable. While lifting cable felt a pop in their right shoulder. I.E. has continued to work. Doctor put them on restricted duty on 11-4-2020 and they are continuing to work.

I.E. was lifting high voltage cable during a power move. While lifting the cable the I.E. felt a "pop" in their right shoulder along with pain. I.E. has continued to work and is currently working. Reporting due to I.E. receiving an injection shot for pain and starting therapy on the shoulder the week of 10-5-2020.

EE was attempting to change batteries on coal hauler in the battery barn. While moving the battery jumper cable it became caught on the edge of one of the batteries. EE attempted to move the cable by whipping and pulling the cable. The cable suddenly came loose causing the EE to stumble twisting their knee. EE continued to work until doctor took them off 10-1-2020.

EE was attempting to pick up a rock out of the walkway while cleaning a belt drive. While picking up the rock to throw it on the belt, EE felt a pull in their lower back. Rock measured 18"L x 10"W x 8"Th. Employee continued to work until doctors restrictions took them off work on 06/08/2020.

While lifting a pack of 6' glue to put on the roof bolter, employee felt a pull in their lower back. Employee continued to work until doctor restrictions took them off work on 01-29-2020.

Employee was putting the spray block cover back onto the continuous miner. Employee felt slight pain in lower back when lifting. Employee has continued to work with restrictions from doctor given on 01/09/2020. Restricted Duty.

While loading belt structure I.E. and other employee attempted to stack a top roller frame, when the I.E. caught their left index finger between the two frames. Finger required stitches and no work was missed.

While loading belt structure, employee flipped the last piece of structure to stack on another section of structure. During this employee smashed the distal portion of their left middle finger between the handles on the frames. Injury required surgery and employee did not miss any work.

I.E. and another employee were moving miner cable over to the rib. While moving the cable I.E. heard and felt a pop in their left shoulder. Employee continued to work until surgery on 02-06-2020.

I.E. was hanging miner cable from the top. In the process of doing this felt a sharp pain in groin. Employee continued to work until doctor restrictions on 12/17/2019.

Employee was cutting the metal bands off a bundle of screen wire. Went to go reach for another band and cut their middle right finger on one of the already cut bands. Employee received stitches on right middle finger. Employee did not miss any work.

Employee was loading screen wire onto the roof bolter. When employee got the screen wire above EE's head, employee felt a pop in their right shoulder. Employee continued to work until surgery on 01/09/2020.

Employee was hanging high voltage cable for a power move and felt strain in EE's lower stomach area. Employee continued to work until surgery on 06/29/2019. First actual day away from work is 07/01/2019.

Employee was roof bolting and got something in EE's left eye. Went to doctor and was prescribed eye drops.

While spray foaming a stopping felt pop in right shoulder. Employee continued to work until 4-18-2019.

Employee was lifting a roller onto the belt structure and felt something pop on left side. EE has a cracked rib.

While shoveling on the 4th North beltline employee felt and heard a sudden pop in left wrist with swelling. Employee has continued to work but had surgery to repair on 06/20/2019. Employee is still continuing to work with no lost workdays.

I.E. was changing out dust bags on the roof bolter. There was an excessive amount of dust in the bag. As the I.E. grabbed the bag and pulled it out of the dust box, the weight caused them to twist their back. Reporting as restricted duty case.

While hanging miner cable across entry, picked up cable and felt something pop in right shoulder. Employee continued to work until surgery on April 25th, 2019.

Employee was hanging miner cable and felt a pain in EE's stomach. EE continued working until EE had hernia surgery on January 25, 2018.

Employee was lifting the miner cable to hang it and felt a pain in left shoulder.

Employee strained EE's left knee while installing resin in a roof bolt hole. EE continued working until EE had surgery on 2-22-2018.

Employee and 2 other repairmen were cribbing up the continuous miner for maintenance. When the machine was lowered onto the cribbing, a crib tie came out and struck the employee on the right side of employee's face.

I.E. was taking covers off the top of the continuous miner. While replacing a hydraulic hose another miner moved a cover and it fell on the I.E.'s left hand. This caused a laceration and fracture of the left middle finger.

EE was loading materials and caught left middle finger between a wooden pallet and the bed of a trailer. EE continued to work until EE had surgery on finger on 10-10-17.

Employee was helping carry a wet, muddy timber and they felt a sharp pain in their lower back. They worked restricted duty until they were fully released on 06/22/2017.

While working on a continuous miner, loose debris from the CM fell on employee and rolled behind the employee's safety glasses allowing dirt in the left eye. The employee was issued a prescription, antibiotic, for the left eye.

Employee was loosening a pan lid and the lid shifted, cutting a finger. The finger required sutures.

Employee was cranking the suction line up and the crank slipped out of EE's hand and it struck EE on the back of EE's right hand between the thumb and index finger. EE received a fracture to EE's hand.

As bolter cable was reeled up, employee was struck by cable on right knee. EE had micro surgery on 9-1-16 and became reportable at that time. EE did not miss any work and is currently on restricted duty.

Taking down kennedy stopping, cut pinky finger through glove, received 4 stitches.

Picked up jack stand to move it and caused pain to lower back. EE continued working regular job until ee went on restricted duty April 29, 2016.

Employee was taking down Kennedy Panels and cut his right index finger on the panel. He received 4 stitches in his finger.

While examining, the employee was crossing over a belt crossover and strained his calf muscle while descending.

Employee was pulling on the battery nip for a coal hauler when he felt a pain in his lower back. He started missing work on 7/13/15.

Employee was recovering belt structure when he felt a sharp pain in his lower back.

Employee was loading battery cells onto a ride with a boom lift. He pulled on the strap to swing the cell onto the ride when he felt something pull in his left shoulder. The employee stayed at work until he had surgery on 6/29/2015.

Employee was putting a bolt in a hole he had drilled when he felt his shoulder pop and felt a sharp pain.

Employee was climbing down ladder and his hand slipped, straining his arm. He started missing work on 1-17-2015.

While hanging face curtain Employee got his finger hung on a nail. Required 5 stitches

Employee is a contract laborer, he was taking down high voltage cable when he got a cable hanger in his left hand.

Employee was taking down the miner cable, when the cable fell it hit him in the left knee. Employee had surgery on 03/11/14, causing him to become a lost time.

Two employees were installing plastic water line. One was holding the coupler end of the pipe. The other employee shoved the two pieces of pipe together. The employee that was holding the pipe get his hand caught between the coupler and the pipe. This caused a two inch laceration on the palm of his right hand.

Employee was checking the cable on CM6283. As he was pulling the cable loops from the slack pile, he felt a sharp pain across his stomach area.

Employee was working on a Coal Hauler when he twisted to pick up a tool and felt a sharp pain in his lower right side of his back. Employee stayed on the job until 3/14/2013 when he went to Doctor.
