At approximately 8:35pm the miner operator was fatally injured when he caught himself between the tail of the miner and the coal rib.
While bolting, a rock measuring 12 inches by 6 inches by 3 inches fell out and struck the employee in the right wrist. Employee stayed on the job until 7/11/2013 when his DR. gave him larger restrictions.
Employee was cutting the belt to make a splice. The blade on the belt knife broke and the knife was pulled into his left leg causing a 1 1/2 to 2" laceration
Employee was working on a Coal Hauler when he twisted to pick up a tool and felt a sharp pain in his lower right side of his back. Employee stayed on the job until 3/14/2013 when he went to Doctor.
Employee was drilling a hole to install a roof bolt, when he saw a rock starting to fall. He turned around hitting his arm on the boom of the roof bolter, causing a 3 in laceration on his right forearm. The cut required stitches.
Golf cart #1 would not tram and employee stopped to give his golf cart to the individual that had golf cart #1. The mechanics stopped at the same time and was visually inspecting it when they observed mud and water on wiring. The cart stared tramming on its own and caught employee on the left leg.
Hanging Miner cable employee felt pressure in his lower back like someone standing on his shoulders. Employee Stayed on the job until 10-01-2012 when his Doctor gave him a work slip.
Employee was cleaning cab of scoop out when a rock measuring 16"x24"x8" hit him in the right side of his rib cage.
While hanging high voltage cable a section of the cable started to drop out of HV hanger and employee attempted to catch the cable. He felt a pain in his right shoulder. The employee stayed on the job until 4/17/2013 when his doctor took a MRI of his shoulder.
Employee was putting a cable back into the machine while he was pushed it into the compartment he cut the top of his right pointer finger on a hose clamp.
Mechanic was changing out the canopy lift cylinder on a roof bolter. He chained the canopy to the roof on a chain hanger and cut the pins out of the cylinder, they applied heat to the cylinder eye and applied down pressure with the boom of the bolter and then hit it with a sledge hammer. The cylinder broke loose and the miners foot was under the drill pot of the bolter.
While moving a new miner to the section the employee and another mechanic was pulling up the cable slack when his foot got entangled in the loop of the cable causing him to fall.
While loading belt material into a coal hauler the miner stepped out of the bake of the car (aprox. 6" step) and he rolled his left ankle.
miner was struck in the forehead while attempting to removing a belt splice pin using a claw hammer, he hit the claw hammer with another hammer causing a piece of metal to chip off and hit him in the forehead just above his safety glasses.
Man was installing a roof bolt after drilling the hole and got his pointer finger on his right hand caught between the wrench and the washer on the bolt, he missed the hole and put pressure on the bolt against the roof.
Employee was lifting a piece of screen sire and felt a twinge in his neck and down his left shoulder.
Employee was walking to retrieve tools from tool sled when he stepped in a hole and rolled his left ankle.
At approximately 10:40pm in entry #6 between #48 and #49 crosscut of the Main West fell measuring approximately 60ft long and varying in height from 1ft to 5ft.
Employee was hand dusting face area when he stepped on a piece of wire and stuck it in his right foot.
Employee was slicing an apple with a razor knife and the knife slipped cutting the palm of his left hand. stitches were applied.
While raising a lid on the battery of a rock duster the employee was attempting to put the bar in place that holds the lid up for charging, when the lid fell pinching the index finger of his left hand between the battery and the lid.
Employee was putting up screen wire in our Main West travel road with a double boom roof bolter. He missed the hole when trying to insert the roof bolt and the bolt bent. He then accidentally hit his rotation, instead of the lowering lever, causing the bent bolt to swing around and hit his right wrist.
Repairman was repairing a feeder conveyor chain. He was driving the keeper back on the pin when a piece of the tool he was using as a punch flew off and went into his left lower thigh. He recieved 3 stitches and has not missed any work due to this accident. The piece of steel that went onto his leg was approx. 1/2" X 1/4" X 1/4".
Employee was assembling belt structure for a belt move and felt a pain in his right shoulder. He missed his first day of work over the accident on 12/20/2010.
Examiner strained his groin and back while pulling a piece of curtain out of the #1 entry in the 1st West faces that had rock on top of it.
Employee was setting a prop in #8 entry of the 2nd E. Panel. A piece of rock fell out of the top hitting him in the back of the head. This didn't hurt him but, when he twisted around to get away from the rock he twisted his knee which caused pain in his right knee. Report was filled out but didn't do anything until now. He is having surgery on 3/29/11.
An examiner reported a loose rock to the section face boss. The face boss went and got a pry bar to pull it down with, when he pulled the rock down it landed on its end and tipped over, striking the boss on his left knee. This happened between the #1 and #2 entries in the last open cross-cut of the 2nd East panel.
Section foreman was putting up sights in #2 entry of the 1st East panel and stepped in a hole and twisted his knee. He worked until 3/18/2010 and his knee is going to require surgery.
A rock fell from the roof between the roof bolts in the #4 entry of the 1st E panel striking the employee in the back. This caused him to fall and twist his left ankle. He went to the Dr. during our Christmas vacation and is unable to return to work as scheduled on 1/4/10. Due to the ankle injury.
An unintentional roof fall occurred in the 1st East Panel (001-0) off the Main South Entries in #2 entry at #1 cross cut at the +120 foot mark. This fall was located in the center of the intersection approximately 18' wide by 18' long and approximately 30 to 36 inches high. MSHA was notified within the prescribed time frame.
Backing the miner out of a cut, he stopped the miner to put a loop of a cable onto the tail, the cable strap was to short to reach the hook on the tail, he swung the tail over, smashing his right forearm between the coal rib and the miner tail.
Two mechanics were trouble shooting a breaker and plug on the transformer with a multi-meter. The breaker had lost power several times, when the mechanic was checking the plug, with the meter, it went phase to phase.
