Process plant metallurgical technician was carrying samples in 5 gallon buckets when their back was strained.
Drill mechanic's right pinky and ring finger were injured when a shock sub fell from the rigging contacting EE's hand. The mechanic was in the process of changing out the shock sub for the drill.
The operator was setting up to load/charge a round when the emulsion hose became stuck underneath the primer box lid. When the operator leaned over and moved the hose out from under the lid, the lid dropped down and pinched EE's testicles between the lid and the primer box.
Blast drill technician was assisting in loading a new hammer bit onto the deck of the drill when the hammer slipped pinching EE's finger causing injury.
Process operator received a right pinky injury when a pipe wrench fell from a worktable. This incident caused a restricted injury.
While installing a heavy iron liner into the mixer at the batch plant, the employee suffered a muscular injury in their left bicep.
A maintenance technician was performing work on an UG haul truck in the maintenance shop (located on surface). The mechanic needed to remove a rock that had fallen onto the top deck of the truck. While removing the rock, the rock slid out of their hand, causing a laceration to right thumb. The operator was wearing nitrile gloves.
A maintenance mechanic was troubleshooting a hydraulic leak on a Cat D10 dozer under the floorboard area. During this inspection, pain was felt in the knee that resulted in restricted workdays.
Employee was loading a rock crusher with excavator and noticed crusher was jammed. The foreman turned crusher off, and operator waited for the machine to stop. Then, tried to adjust the jammed rocks with their hands. The employee used smaller rocks to push on the larger rock to try to free it. The larger rock(s) shifted and employee's hand was caught between crusher frame and rock.
While washing the cable bolter, the operator had some grout come off the equipment and splash into their left eye. The operator flushed eye with a bottle of water and continued working. An hour later they told supervisor, and stated that they had no residual issues or pain. The following afternoon, they woke up for their shift and the eye was red and weeping.
While loading bolts from a pallet to a truck, the operator twisted an ankle but continued working and proceeded to move the truck to a new area. While at this new location, employee was removing the banding from a bundle of bolts, and the front stack of bolts fell onto the same ankle/foot, causing a sprain.
While trouble shooting a hydraulic leak on a blast drill, a maintenance technician was injected with hydraulic oil in their left index finger.
A process maintenance worker was lifting the corner of a 1/4 inch plate of steel to allow the forklift to pick up the plate when worker felt pain in EE's back.
The miner was charging a hole in the back and was using a spider plug/pilgrims hat to plug the hole. When EE inserted the plug and pulled the PVC pipe out, a rock was knocked loose and hit the miner in the face.
A process operator received lime burns to abdomen, back, and face when they came into contact with lime. The operator was removing a bolt on a flange at the lime slaker.
A process employee was placing a conveyor guard back into position when the guard slipped smashing finger. The injury resulted in a fractured left ring finger.
Ergonomics, twisting and pulling, rocks on the ground
Haul truck operator felt pain in the hip while dismounting the Komatsu 730 haul truck. The operator has been placed on restricted duty.
An underground operator was manually loading a stocking deck with 8-foot split sets when EE felt a pain in their back. The operator was picking up the split sets and then turning to place them on the deck when EE felt the pain.
A drill tech caught their finger between a blast drill bit and the deck of the drill while attempting to sit the bit on the deck.
A maintenance technician was working on a Epiroc D65 drill installing a new drive line when EE's foot became entangled in hydraulic lines. While trying to free their foot, EE felt a pop in their knee. This resulted in a sprain in the knee and aggravated a previous injury that did not happen on site.
Drill operator was exiting the cab when EE's finger became caught in the door latch causing a small laceration resulting in medical treatment of 4 sutures.
Worker was lifting a 35 lb. box when worker felt pain in back and stomach. A physician has placed worker on light duty.
Employee got a foreign object in EE's eye when EE wiped EE's face and had metal shavings on the sleeve of EE's shirt. The employee was unable to flush the object out of EE's eye and was transported to a medical facility where it was removed and EE was prescribed antibiotics.
6020B bucket pin (weighing about 1000lbs) fell off the back of the technician's truck striking them in the left foot. The pin was in a sling, but resting on the truck bumper when it slipped. Pin fell approximately 3 feet. Tech sustained a crush injury to their left foot.
500 area operator was going to swap pump at the caustic tank, due to issues with the main pump. Operator closed the suction and discharge valves for the pump 500-PP-030. When the operator opened the suction and discharge valves for 500-pp-031, the drain valve broke causing caustic to come in contact with the operators left leg and right forearm.
The maintenance technician was lying down, replacing a hydraulic temp sensor on an excavator. Upon removal of the sensor, hydraulic oil poured onto the technician's face and safety glasses. The employee removed self from the area and wiped the oil from face. While doing so, oil got into the corner of the employee's left eye and began to burn.
Miner cleaning dry lime from a silo, unknowingly got dry lime inside their boot, resulting in second degree chemical burns to the miner's left foot, some time later. The miner reported the injury two days after performing the work. They were assessed on site and transferred to a nearby medical facility for further assessment and treatment.
While an employee was moving a pump a wire fray on the bull hose poked EE in the finger through the glove. EE reported the incident to management on EE's next shift. On 2/2, EE's day off EE was taken to the clinic, infection had set in, the infection was surgically removed, and EE was given antibiotics. EE was released from the clinic on light duty, and EE returned to work.
After changing innertubes on drill steel, the employee was carrying the bad innertube to our scrap rod pile. While carrying one of these bad innertubes the employee's left knee buckled and made a loud popping noise.
A blast drill operator fell approximately 4 feet to the ground, from the drill platform. The operator had leaned against the handrail while moving sample bags when it broke, resulting in the handrail and operator falling to the ground.
Driller lacerated wrist on cotter pin securing the pivot pin of the ladder to BD13 drill. The driller was using the ladder to exit the machine, when EE's hand caught the sharp edge of the pin. The driller notified the supervisor, was taken to the first aid room, then to a medical facility, where EE received 6 sutures and was released to normal duty.
Employee felt pain in lower back and arm while lifting a breakout wrench to disassemble a DTH Hammer
Employee was was pulling a water hose and felt discomfort in the lower back. Employee was seen by the doctor on 3/31/22 and given restricted duty.
Employee strained the back on the right side after stacking a tire on the pallet for storage. The injury did not constitute medical treatment until October 6th when EE was prescribed physical therapy to treat the strain.
An employee was helping carry a submersible pump from a standing metal table to the bed of LV022 (Ford Truck). The employee felt a pain in EE's back. The employee stated the back did not start hurting until the pump was sat down and the pressure was relieved off EE's back.
Process mechanics were changing two discs on the ISA mill that were worn. In the process of removing the third hub, a 2" nylon strap secured around the hub snapped and impacted the mechanic on EE's forearm just above EE's wrist.
Lifting pipe and pipe slipped and caught finger
While attempting to tandem pump in the snake pit a drain valve seal failed and the pipe shifted and hit a mine tech. The mine tech was injured. Left ankle was swelling, the MRT and EMS started first aid and transported mine tech to the hospital.
Operator was doing housekeeping around the Drill and Blast Conex area. Operator was in the process of stacking small hammers on a pallet. As Operator laid a hammer on the stack of hammers, the top hammer rolled down and pinched EE's hand between the hammer below.
Lifting a section of grating as it was slid out of the way the right hand middle finger was caught as a result the tip finger bone was fractured.
Employee was pulling self up on drill. As employee was pulling self up, employee felt a pop in abdomen
During the process of installing a HDPE pipe a worker's foot became stuck in mud. While EE was pulling it out EE strained right knee.
While adjusting the tongs on a forklift, employee got right index finger caught between a tong and the forklift frame. Employee suffered a laceration and wound burst to the index finger.
While installing stainless steel jacketing on a tank, the jacket slipped, hitting the employee's glove, causing a laceration to employee's finger.
Maintenance technician was changing filters on an excavator, and dropping them off the machine into an open drum. After the technician let one go, a second employee reached out with hand to push the dropped filter into the drum. This employee's finger was pinched between the falling filter and a one already in the drum, causing a laceration.
While lifting the tongue of a Godwin Pump trailer, employee suffered a thoracic back strain. Employee was treated by a physician and taken out of work for a few days to recover.
Employee was working to thaw frozen Milk of Lime Feed Hose. The header was not locked at the pump and upon clearing the ice from the hose, the employee cracked open the ball valve to check for flow. This resulted in Milk of Lime being sprayed in the face and chest.
Injured employee and coworker were removing steel from a drill mast to the ground. Employee took a step and felt a "pop" and pain in the right hip.
The employee was doing housekeeping. Employee called out that employee would need help moving a box. The employee then proceeded to lift the box before help arrived. While lifting the box the employee strained lower back.
