The employee was near a screw conveyor on the 3rd floor of the finish mill 2 building. At some point their hand and arm became entangled in the screw conveyor. Emergency response was called immediately and the employee was transported to a hospital. The hospital later reported that the employee passed away.

Employee was operating a pick-up truck on the -330 bench. A haul truck turned onto the -330 bench. The haul truck operator indicated they did not see the pick-up truck resulting in fatal injuries.
A contractor was servicing the Alimak elevator at the fourth level of the pre-heater tower. The contractor was on top of the elevator and a mine employee was inside the elevator assisting. The person inside the elevator heard a clicking sound and the elevator started a downward descent. The contractor on top of the elevator received crushing injuries when the elevator moved.
Employee pulled aluminum boat over three electric conductors in dredge pond. A patch on the bottom of the boat cut into one conductor and electrocuted the employee. The employee died as a result of the accident.
The employee was present while two contract workers were performing a repair on a centrifugal water pump. During the repair, an explosion occurred. Heat may have been applied during the repair. The employee suffered fatal injuries.
The employee was moving manlift to the ground and became trapped between the I beam and the control panel. Condition of the controls in the basket is suspect.
EE WAS WORKING WITH A CREW UNDERNEATH THE TABLE OF THE RAW MILL. HE LEFT THE WORK AREA AND HAD TRAVELED APPROXIMATELY 100 FEET DOWN AN ADJACENT DUCT WHEN HE FELL INTO A PILE OF FLY ASH INSIDE THE DUCT.
TIRE SERVICE TECH WAS POSITIONING A 50 TON DUMP TRUCK TIRE WHEN THE TIRE FELL FROM THE BOOM CLAMP TRAPPING TECH BETWEEN TIRE AND BOOM TRUCK BED AND FRAME. MINOR DAMAGE TO THE BOOM TRUCK BUT FA TALLY INJURING THE TIRE TECH. THE AREA WAS AN OPEN FIELD AREA, WEATHER WAS CLEAR, AREA WAS DRY, DAYLIGHT HOURS.

INDIVIDUAL WAS MASHED BETWEEN AN ELEVATOR AND THE STRUCTURAL STEEL ON LEVEL L-3 PREHEATER TOWER.
INJ CONTRACTOR WAS ON THE GROUND, OUTSIDE HIS VEHICLE JUST AFTER HIS TRUCK WAS LOADED. JUST BESIDE THE VICTIM'S TRK ANOTHER CONTRACTOR BEGANBACKING UP TO BE LOADED. THE MOVING VEHICLE WASN OT EQUIPPED WITH AN OPERABLE BACK-UP ALARM. THE DRIVER STATED THAT HE HAD SEEN VICTIM OUTSIDE HIS TRK ON THE BACK LOOKING AT THE ROAD, BUT DID NOT SEE HIM ON THE GROUND. HE SAID HE FELT A BUMP
