The miner smashed EE's finger with a rock while clearing a head box on conveyor C21 at the main plant.

The repair crew was replacing main frame liners in Sandvik cone crusher when Miner reached down to apply upward pressure on a liner which had already been tacked in. EE felt a pop and experienced pain in EE's arm.

Operator was mucking mud with excavator (Cat 365) when a hydraulic cylinder next to the cab burst. The window was shattered, and the operator was sprayed with hydraulic oil. Operator went to the hospital where EE was given a return to work without limits. EE chose to take the week off work. No injury, near miss only.

The plant operator was working with EE replacing a bearing at the head pulley of the sand stacker when EE cut EE's index finger on EE's right hand.

Miner was removing old pipe to be discarded and was bitten by a snake on the right hand.

Employee stepped down and felt a pain in EE's left knee.

Employee (plant repairman) was removing welding hose from truck and cut thumb on the body of the truck.

Employee was lying on left side and felt pain in their ribs. Dr. advised lt duty for 1 week.

Equipment greaser was greasing haul truck when ee attempted to align the grease hose from the grease gun into the grease receptacle. Grease was injected into skin when ee activated the grease gun.

Employee injured finger tip while installing pin in bumper of truck.

Employee got hydraulic oil in eye while conducting a pre-shift inspection. Eye did not bother the employee initially, but became irritated a few days later.
