The plant operator was working with EE replacing a bearing at the head pulley of the sand stacker when EE cut EE's index finger on EE's right hand.

While working on a new screen deck installation the miner was working to install the springs and EE's left hand was struck with a hammer.

Miner lost balance and fell on stairs at screen tower.

Employee stepped down and felt a pain in EE's left knee.

Employee was burning wood debris in the main pit area. Employee was too close to the flames, resulting in 1st and 2nd degree burns to face and arms.

Employee failed to maintain footing while on track of Cat Excavator, and landed on his buttocks.

Employee had a laceration to the left forehead area after coming in contact with a piece of cut metal.

EE was welding while standing on a ledge approx. 4-5' above the ground. EE slipped on loose material and fell down, breaking his ankle.
