Employee was guiding a draw bar into the coupler and their right middle finger was caught between the draw bar and coupler. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was in the beltline hanging belt structure chain when a piece of rock fell from between the roof screen striking the employee in the bridge of the nose. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

IE was hanging pumpable crib bags and was struck by a falling rock in the right forearm. Injury was not the result of a training or compliance issue.

Using #39 walk through bolter T bar to push roof bolts out of the way and roof bolt after the place change miner had taken a cut out of the roof at 164-165 #1 St Leo. While extending the T bar oil shot out of a broken fitting and got into EE's left eye causing irritation. No problems with equipment or compliance issues, training or PPE.

Employee was operating a shuttle car on the 7 North Mains CM Section. EE stopped to remove a rock from the roadway. When EE tossed the rock in the shuttle car boom, EE felt a "pop" in EE's left bicep. Employee has a torn bicep muscle. EE has training and experience and wearing required PPE at time of accident.

Injured was changing a cradle at 143 block 6 belt and while pulling the old cradle out it rolled over and bent their right thumb around the back of their hand resulting in an unspecified sprain. No problems with equipment or compliance issues, training or PPE.

Employee was repairing airlock doors when the top beam fell, striking employee in the left wrist and hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was mounting a dust box on CM Bolter station when dust box dropped and landed on employees finger. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee injured right biceps while moving a piece of large rock. They worked unrestricted until 8/2/26, began missing work 8/3/26. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

Employee was loading rib straps onto a scoop without gloves and the strap slid through EE's fingers, cutting EE's left ring and small finger. Compliance with rules/regs, PPE mining equip/systems not a factor. Miner proficiency may be a factor.

Injured employee was opening the North Approach door to the 7 North Shaft. As EE turned the latch handle, the pressure on the door caused it to pull away from EE. EE's right little finger was smashed between the door frame and door handle. EE instinctively jerked EE's hand out from between the handle and frame. This resulted in a partial amputation of EE's little finger.

Employee was loading vent tubes and while turning to load another vent tube, employee felt pain in the middle of EE's back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was building a wall and came into contact with J-Tech pumpable crib material. Employee continued to work until 07/10/2026 and then sought treatment. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was pushing bolt into bolt hole when employee felt pinch and pain in left shoulder. Employee continued to work until 07/07/2026 then sought treatment. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was shoveling the 9 Belt Take-up when right hand contacted the Take-up rail resulting in a cut to EE's hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was working on the 7 North Mains Continuous Miner. EE went to step over a mound of coal/dirt and felt several "pops" in EE's upper left hamstring. Employee has been diagnosed with a torn left hamstring. Employee has training and experience. Was wearing required PPE.

Injured employee and co-worker were removing a top belt cradle for replacement. As they were lifting the cradle out from the belt, they failed to maintain control of the of it and it rolled over and smashed EE's left hand between the cradle and the belt bedrail. Injured had swelling and contusion to EE's hand. Has training and experience. Was wearing metacarpal gloves.

Employee was placing tubes over the bologna skin when EE felt pain in lower back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employees were loading roof straps on top of the miner and the injured employee attempted to duck out of the way, when EE was struck in the forehead. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was pushing roof bolt to the top when the roof bolt bent and pinched EE's finger between bolt plate and strap jack. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was installing the door back on the Scoop kitchen when the door slipped from its hinges and pinched the employee's finger. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was placing the tire back onto the scoop when the tire slipped off the axle and landed on the employee's foot and ankle. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

A shaft driller apprentice was helping apply gunite to the shaft ribs on 6/3/26 at approx. 11pm. During the course of this process the miners pants became soaked with the concrete material. When the miner woke up at 1pm on 6/4/26 EE discovered significant concrete burns on both of EE's legs from the knees down.

Employee was Z-Drilling when dropping steels, the steels came apart and fell on employees finger. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Injured employee was getting roof bolting supplies out of a scoop bucket to supply up a roof bolter. As EE picked up a bundle of channel strap plates, EE felt a shocking pain in EE's lower back. Became sore and stiff and was unable to finish EE's shift.

Employee was shoveling on the beltline, when felt pain in shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was changing out draw bar on motor when EE pinched EE's finger. Mining systems was not a factor, compliance with rules and regulations was not a factor. PPE was a factor.

Employee was helping get an air drill unstuck off of a cable bolt, when the drill was free the handle came around and struck EE on the hip. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was changing a piece of belt structure when the frame slid, and EE's finger was caught between bed rail and frame. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.

Employee was installing roof bolts on the right side of the continuous miner. EE had completed drilling a hole and was pushing a roof bolt up in the hole when EE felt a "pop" in EE's right shoulder. No rules or regulations were violated and EE was wearing required PPE. Body failure performing normal job duty.

Employee was bolting when the pusher steel slipped out of EE's hands and hit EE on the head. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was helping unload structure from scoop when EE dropped structure on EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was putting supplies on miner when EE picked up a rib strap when it cut through EE's glove. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was pulling shuttle car slack to hang when EE felt pain on the right side of EE's chest. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was lifting miner cable and water line into hanger when EE felt pain in lower back and leg. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor. Body positioning was a factor.

Employee was exiting a mantrip when EE's right foot was stuck under the seat. When attempting to pull it free EE injured EE's right knee.

Employee was at the miner hanging cable and water line into hanger when EE went to pull down on cable, EE felt pain in EE's shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was helping pull belt off of dolly when the belt grab slipped off belt and struck EE's nose. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was helping work on scoop battery when battery lid came down and hit EE's elbow/bicep. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was loading waterline onto scoop for belt move. When one of the water lines pipes slid off the scoop and struck EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was cleaning on LW face when another employee was attempting to pass EE, that employee slipped and fell and the miner that was cleaning tried to catch EE and injured EE's back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Injured was cutting down roof screen to unload "bagged" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.

Employee was struck by a rock that had fallen from in between the shields, striking EE's left hand, and resulting in a contusion to EE's left pinky and ring finger. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was helping hang miner cable and water line into a roller hanger when they felt pop in their upper arm (bicep). PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was carrying a cradle with 3 rollers down C Belt line and felt pain in the middle of EE's back. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was dragging hoses off the face when they became stuck. When the pull rope broke, the hoses whipped the employee in EE's right arm, resulting in a contusion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was spotting rollers and shoveling belt when EE felt pain in EE's hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was carrying a spool on EE's shoulder through the Golden Ridge Bottom Cut Through, when EE's left thumb was caught between the spool and the roof. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Injured employee was helping hang the continuous miner cable. Lifted it to EE's shoulder and then up into a hanger. Felt pain in EE's shoulder and later after work, EE noticed a small knot and swelling in EE's shoulder. Reported the incident on 3-9-26 and sought treatment that day and began missing work. No rules or regulations were violated and EE Was wearing required PPE.
