Miner was assisting in dry plant with return rollers for the 3101 belt. As miner was passing the roller EE grabbed it in a pinch point area. When grabbing the roller the metal cut EE's glove and hand.

The employee was working on the CC1 belt and picked up a wrench to pass it to a coworker positioned above EE. While passing the wrench, it struck the machine guard, causing the wrench to rebound and hit the employee in the mouth. The employee sustained an injury to the mouth as a result of the impact. Employee was seen by Dentist 7/28/26 a root canal was performed today

An employee was assisting in replacing slurry pump PU6. While hoisting the new pump into place with a telehandler and chain hoist, it became stuck. The employee attempted to reposition it by placing EE's right-hand underneath. When the bind released, the pump shifted, crushing EE's right ring finger between the pump and the skid.

Employee smashed EE's hand between flange on discharge suction pipe

Maintenance team were unloading a beam from telehandler forks and placing beam onto catwalk, Back was hurt in the Process.

Employee was helping change metal screens on a T190 McCloskey. Employee was trying to fit a U-bolt in place, to hold the screen in place. (Bottom deck Screen tensioner) The U-bolt plate slipped down smashing employee's finger between the screen plate and the U-bolt.

While connecting conveyor sections, section DF contacted section DE causing section to slide off the dunnage it was resting on. Employee's foot was pinned. The movement continued to slide across the steel toed portion and came to rest on employee's foot, causing injury. Employee was given first aid on site and transported to ER where employee was treated and released.
