Employee was helping lift a piece of feeder chain when he felt a sharp pain in chest and lower back. Employee had suffered a severe burn injury of the upper chest from an earlier accident.
Injured was shoveling on the belts and began experiencing severe lower back pain
Injured was assisting in unrolling 2" plastic pipe for installation. The end of the pipe had been secured to the mine roof by tying it secure. The pipe came loose and struck the injured in mouth causing injury to his right upper lip. Five sutures were required.
WAS LIFTING A SCOOP TIRE FROM A MAINTENANCE JEEP WHEN HE FELT A PAIN IN HIS LOWER BACK.
Injured was lifting on a roof bolter pump motor to turn it around in the scoop bucket when he felt a "pop" in his left abdomen area.
EE WAS CARRYING DESK AT UPSHUR PROPERTIES (ANKER) FOR THE SAGO MINE'S NEW OFFICE FACILITY WHEN HIS HAND WAS CAUGHT BETWEEN OFFICE DESK AND FILING CABINET. THE LACERATION REQUIRED 8 STITCHES.
UNLOADING TOOLBOX OFF OF #4 MANTRIP, FELT PAIN IN LEFT HIP.
Injured was straightening a roof bolt while installing it in the drill hole when he straiined his back
EE called supervisor 3-12-05 that he had pulled a muscle & could return to work on 3/15/05. He also told supervisor he had pulled the muscle lifting a tire.
Injured had just finished assisting in the hanging of the cm and roof bolter cables when he began to experience pain in lower back and right leg.
EMPLOYEE EXPERIENCED A BURNING SENSATION IN HIS LOWER RIGHT ABDOMEN & SWELLING JUST UNDER HIS BELTLINE. SEVERAL X-RAYS & AN M.R.I. WERE TAKEN & A RUPTURE WAS DETERMINED. THE INJURY DID NOT BECOME REPORTABLE UNTIL 5/9/05 WHEN SURGERY WAS PERFORMED TO REPAIR THE RUPTURE.
Injured was rolling a rock out of the roadway when it slipped from his hands and fell back onto his leg
While working on No. 11 Scoop, EE took the lid off of the directional switch box, dropping the lid on his right hand. This resulted in a laceration to his right index finger, requiring 6 stitches. Making this a Medical Reportable Accident. EE did not miss any work.
EE went to supvervisor on 1-8-05 & gave him a dr's excuse to be off til 1-15-05. ee came back on 1-15-05 wihtout a release. he was told he could not come back to work w/o release. ee came backwith a light duty release. hwm does not offer light duty. he then for the first time said that he hurt himself lifting a tire. he has not returned to work.
Individual was on knees installing slides in the continuous haulage rapid haul (lowlow) when he twisted his body in an awkard position straining his back
Was filling a can of water out of fire tap to plaster stopping when a piece of rock 124" by 16" by 4" slid off screen and struck him on right shoulder and neck and knocked down waterline.
INJURED WAS SPLICING BELT. WHEN PULLING ON BELT HE FELT PAIN IN THE RIGHT SIDE OF HIS STOMACH CAUSING A STRAIN.
THE INJURED EE WAS WORKING ON A SPEED REDUCER AT #14 BELT DRIVE. WHILE TURNING THE REDUCER EE GOT HIS HAND CAUGHT IN THE GEARS, RESULTING IN A LACERATION ON THE TOP OF HIS RIGHT HAND, APPROX. 3" LONG.
Injured strained muscles in back while pushing bolt up through resin into bolt hole
Injured had a 21' joint of 4" schedule 80 pipe up on a 2' board (header) to allow him to wrap a cutting guide around it to allow him to mark the pipe for cutting. He was positioned with his back to the board. As he was marking the pipe, it rolled off and crushed his finger on the concrete floor.
Injured was lifting on the end of a 21SC floating axle to reposition it on a supply car.
INJURED STATED THAT HE WAS LIFTING A BUGGY TIRE, LOST HIS FOOTING, SLIPPED AND SPRAINED THE RIGHT SIDE OF HIS BACK. INJURED WENT ON LTA AS OF 8/14.
WHILE LIFTING A BELT ROPE, INJURED STATED HE FELT SOMETHING PULL, ALONG WITH A BURNING SENSATION IN HIS LOWER RIGHT RIB.
THE INJURED WAS REPLACING A WIPER BLADE ON A BELT CLEANER WHEN PART OF THE CLEANER SLIPPED CATCHING HIS LEFT INDEX FINGER, RESULTING IN A LACERATION. THE LACERATION REQUIRED 9 SUTURES.
Injured was attempting to free a stuck return roller when his small finger on his left hand became stuck between the roller and the belt
Twisted left leg, while loading belt structure. he felt his leg begin to " burn " when he twisted to throw belt structure into scoop bucket.
Injured was shoving rock from the top of a continuous miner with his bare hands. His hand slipped and struck a sharp rock causing the injury.
This is a "low coal" operation. As the injured exited the shuttle car in a rolling motion of his body, he heard and felt something snap in his knee.
Injured was moving c.m. cable out of roadway. he was standing bent over reaching to his left and pulling to his right when he felt and heard a pop in his back. EE stated he lay there for approx. 5 min before was able to get up. stated he was still having excrucrating pain.
Installing belt structure & carrying structure, a knot developed in his groin. He felt this was due to an accident he had sometime the 1st week of Feb., as he was helping to install the no. 1 belt into the mine. He had belt structure at the time, felt a sharp pain in his groin and notified his supervisor of this on that day.
INJURED WAS ASSISTING IN THE LOADING OF BELT STRUCTURE RAILS ONTO A SCOOP WHEN HIS FINGER WAS CAUGHT BETWEEN A BELT RAIL AND THE TOP EDGE OF THE SCOOP BUCKET. THIS OCCURRED WHEN INDIVIDUAL HE WAS WORKING WITH THREW HIS END OF THE RAIL TOWARD THE SCOOP CAUSING A WHIPPING ACTION STRIKING HIS FINGER.
THE INJURED ALONG WITH ANOTHER INDIVIDUAL WAS CARRYING A 20 FT LONG BY 8 INCH DIAMETER PLASTIC PIPE ONT HERE SHOULDERS.T HE INJURED WAS INT HE REAR WHEN THE INDIVIDUAL AT THE FRONT FELL DOWN C AUSING THE WEIGHT OF THE PIPE TO DISLOCATE THE INJUREDS RIGHT SHOULDER.
THE INJURED ALONG WITH OTHERS WERE IN THE PROCESS OF INSTALLING EQUIPMENT/VENTILATION DOORS WHEN HE EXPERIENCE PAIN IN HIS LOWER BACK.
INJURED WAS ON HIS KNEES LIFTING A SHORT SECTION OF ROOF BOLTER CABLE TO HANG IT UP TO THE MINE ROOF (SEAM HEIGHT IS APPROX 42") WHEN HE FELT PAIN IN HIS LOWER BACK.
THE INJURED WAS LIFTING 10 LB BAGS OF ROCKDUST FROM THE MINE FLOOR AND PLACING THEM ON THE SCOOP WHEN HE TWISTED HIS RT ANKLE AND FELT PAIN IN HIS LOWER BACK (LEFT SIDE).
INJURED WAS IN THE PROCESS OF INSTALLING A 14 FT LONG CABLE BOLT INTO THE MINE ROOF, HE HAD DRILLED HIS HOLE AND WAS REMOVING A CABLE BOLT FROM TOP OF ROOF BOLT MACHINE. AS HE SLID THE BOLT OFF THE MACHINE IT CAUSED ANOTHER CABLE BOLT TO SRING AND STRUCK HIM AT THE CORNER OF HIS RT EYE, CAUSED A LACERATION WHICH REQUIRED 5 SUTURES.
INDIVIDUAL WAS ALONG SIDE OF THE ROOF BOLT MACHINE BENDING A ROOF BOLT WHEN A ROCK MEASURING 18"LONG BY 12" WIDE BY 5" THICK FELL FROM BETWEEN THE RIB AND ROOF BOLTS STRIKING HIM IN THE BACK.
INJURED WAS KNEELING ABOUT 18" FROM SIDE OF CM AS HE AND THE MAINT FOREMAN WERE PRYING THE PIN OUT OF THE RIB RAIL. WHEN THE PIN CAME OUT, THE RAIL SPRANG OUTWARD, DUE TO BEING UNDER PRESSURE , AND STRUCK HIS LEG.
INJURED HAD USED A PICK HAMMER TO KNOCK AN AIR HOLE IN THE TOP OF A 5 GAL OIL CAN TO VENT IT. AS HE POURED IT INTO THE BOLT MACHINE, HE LIFTED THE CAN OVER THE MACHINE, THE CAN SLIPPED AND HIS FINGER WENT INTO THE HOLE AND WAS CAUGHT AND CUT BY THE JAGGED EDGES.
EE WAS POSITINING HIMSELF BENEATH THE DRILL CANOPY BY "WALKING" ON HIS KNEES WHEN HE TWISTED HISKNEE BY ROTATING HIS BODY ON IT. NOTE: DID NOT BEGIN MISSING WORK UNTIL 7/18/03
EE WAS OPERATING THE CONTINUOUS MINER AND HE REACHED FOR AND GRABBED THE CABLE AND WATERLINE. WHEN HE DID HE FELT PAIN IN HIS LOWER BACK.
INJURED WAS SITTING ON THE BENCH IN THE LOCKER ROOM. HE ACCIDENTALLY SAT ON A CHAIN TO A BASKETTHE CHAIN CAME LOOSE THE BASKET DROPPED AND HIT HIM ON THE TOP OF HIS HEAD.
THE INJURED AND TWO OTHERS WERE LIFTING ON THE WING OF THE SECTION BELT TAILPIECE IN ORDER TO GET THE WING BACK-UP ON TOP OF THE CONVEYOR BELT. THE INJURED WAS STANDING UP ON THE TAILPIECE AND HAD TO BEND OVER TO REACH THE WING. HE EXPERIENCED PAIN IN HIS LOWER BACK AFTER THE LIFT.
EE WAS ATTEMPTING TO REHANG A SECTION OF BELT DRIVE GUARDING (WHICH WAS FOUND BY HIM TO BE LOOSE AT ONE END). WHILE DOING SO, A ROCK CAME OFF THE BELT, STRUCK THE GUARD CAUSING IT TO BE KNOCKE D OUT OF HIS HANDS AND STRIKE HIS LEFT FOREARM.
THE INJURED WAS ORIGINALLY HURT ON 2-4-03 BUT DID NOT BEGIN MISSING WORK UNTIL 3-25-03. HE WAS INJURED AS HE AND 3 OTHERS WERE HANGING THE MINER CABLE AND WATER LINE ACROSS THE NO.6 ENTRY WHEN HE FELT PAIN IN HIS LOWER BACK. HE HAS A HISTORY OF LOWER BACK INJURIES AND HE CONTINUED TO WORK UNTIL 3-24-03 WHEN HE LEFT WORK TO GO TO A DR.
WHILE WALKING UP AN ENTRY THE INJURED STRUCK HIS HEAD ON A LOOSE HANGING ROOF ROCK, CUTTING HIS LEFT EAR. THE CUT REQUIRED 10 SUTURES.
MINER WAS DRAGGING A WASH DOWN HOSE WHEN IT GOT TANGLED. MINER JERKED ON THE HOSE SUFFERING A STRAIN OR HERNIA IN THE LEFT GROIN AREA. DR REMOVED MINER FROM WORK 12-3-02.
THE INJURED WAS HELPING TO INSTALL BELT STRUCTURE (42" STRUCTURE) IN THE NO. 12 BELTLINE, WHEN HE EXPERIENCED PAIN IN HIS LEFT HAND AND ARM.
