Leer South Mine Coal

An unintentional roof fall had occurred at 4 Zone track in the #3 entry 3 to 4 block. No equipment was damaged with no injurie to persons. The area was dangered off with additional support set. The secondary lifeline was rerouted before work was resumed. No ventilation controls were affected at any time during this.

While cribbing track that was lifted with a jack, employee stepped over track to get a crib when the jack fell causing the rail to impact EE's left calf causing small abrasion and a broken fibula.

EE was assisting with final seal prep cleaning the bottom with a shovel when a piece of rock fell from the top and struck their hand against the shovel handle, causing a laceration. Injury was not the result of a training or compliance issue. 7000-1 was initially filled out 5/12/25, but the "Submit" button was inadvertently not clicked, thus this late submission.

Employee was working on the 6879 CM. EE had the light cover lifted and were working on the trailing cable junction box for permissibility purpose. An EE had turned their head to speak with another EE as that EE was walking by when the lid came down smashing IE's hand/ finger between the lid and top of the junction box cover.

A contractor who was walking across the parking lot from EE's parked car to the mine's training center when EE was struck by the car of an employee. The employee was arriving at work to begin EE's shift and searching for a parking space at the time of the accident.

Employee sustained injuries while trying to secure a miner cable into a multi-hook hanger that was beyond EE's reach. The employee attempted to jump to place the cable into the hanger but missed. EE's hand became caught between the miner cable and an adjacent roller hanger, leading to a laceration and a metacarpal fracture of EE's right hand.

EE was assisting in the changing of a ripper head motor on a C/M. EE was hitting a hose connection and inadvertently hit left thumb with a hammer. EE refused treatment, on 10/17/2024 EE decided on EE's own to visit the ER. It was determined by X-ray that EE's left thumb was fractured.

While operating a scoop, the injured person needed to adjust self in the seat. EE grabbed the canopy with the right hand and accidentally caught the palm of their hand between the rib and the canopy. EE was then transported to the hospital for treatment, which included surgery to set the bone in EE's pinky finger and repair the soft tissue damage.

The Mine Elevator here at Leer South Mine went down due to a faulty brake switch that had a bolt loosen which caused the micro-switch to short out. No individuals were on the elevator at the time of the incident. The elevator was repaired and put back into service at 6:28 p.m.

While bolting, the injured individual began drilling a hole. After penetrating the first layer, a section of rock loosened and fell down the drill steel. When it hit the drill pot, it broke, causing a 3-foot piece to drop onto the platform and land on EE's right foot, resulting in a diagnosis of a fractured small toe.

Employee was helping move a ventilation fan when the clean out portion of the tubing came loose and caused the employee's wrist to bend back causing a fracture of the left wrist.

The UPS ( Uninterrupted Power Supply ) to the Galloway Fan Motor failed causing loss of power to motor.

While dismantling the monorail on the longwall tailpiece, the monorail slipped from the injured person's left hand and struck the tip of their right-hand pinky finger. This resulted in a crushing injury to the fingertip and the fingernail, necessitating six stitches to close the wound. The injured individual was released without any restrictions.

While tramming C/M with the boom angled towards them, injured walked up to the boom to adjust the cable when the miner came towards them pinning them between the boom and the rib, Injured was taken to medical facility where they were diagnosed with a fractured pelvis.

A gas inundation occurred on the TG9 section when the Continuous Miner intersected a planned breach of the well leg. No injuries occurred with no damage to any equipment, escapeways, or ventilation controls.

On 6/23/24, while using channel locks to pull a belt across a belt sled, injured twisted right knee and fell to the ground when the channel locks slipped from the belt. EE finished the shift, the next day sought medical evaluation from primary care doctor. MRI found a torn meniscus and ACL ligaments. *On 7/5/24, Leer South decided to put EE off work due to potential further injury.

A face ignition occurred on the TG9 section in the #1 entry at 26 1/2 block while intersecting the #9 gas well leg. All precautions from the well plan were in place with no injuries, damage to equipment,escapeways, or ventilation controls affected. Actions plans were put in place for corrective measures.

While placing a pallet of track sand onto the elevator with a motorized dolly, dolly made contact with the elevator door knocking it off track. Elevator was down from 07:15 until 09:00pm.

Main mine fan developed a vibration, steadily got worse. Leer South made the determination to shut if off and correct the issue. Inspected and found the bolts for the main fan motor loose, tightened and restarted the fan. down at 5:44pm Started fan back at 6:18pm.

While pulling machinery, chain that scoop winch rope was attached to broke, allowing winch rope to fly back striking the employee in the right side of the head.

On 1/12/24 the elevator at Leer South went down with an employee on it. The employee was removed within 1hr & 20min. The elevator was down from 235pm on 1/12/24 until 158pm on 1/13/24. The main ABB drive failed on the elevator and had to be replaced.

EE was repairing a high stopping that was damaged from an earlier explosives detonation. The stopping started to fall towards EE, and EE reacted by jumping off the ladder. EE landed wrong and EE's right ankle turned outwards. EE was transported outside where we iced and bandaged EE. On 1-8-2024 EE was taken to a medical facility and after x-rays was diagnosed with a broken ankle.

Installation of a circuit breaker had began in a power panel with power still energized, Electrician began removing a screw from inside the panel when the screw fell and made contact with a live circuit.

Main Slope hoist hoisting cable showed enough wear that it was time to replace. Hoist was down until 6:00pm that day.

The mine lost power in turn causing the main mine fan to become inoperable. The current server for the automatic switches on the high voltage capacitor bank malfunctioned. The incident occurred at 9:04am and was restored at 9:31am. Crews were withdrawn from the mine after the fan power was not restored within 15 minutes and the power circuits were de-energized.

An unintentional roof fall has been discovered on the King Knob Panel in the #5 entry at #5 block on 8/24/23. No injuries occurred due to this incident with no damage to any equipment, ventilation controls, or escapeways. The air course examiners travel is also unobstructed as well. The dimensions of the fall measured 27'L x 18'W x 10'H. All approaches have been dangered off.

The Main Mine Elevator was down due on 8/20/23 due to an inoperative top door lid switch. No employees or individuals were on the elevator at the time of the incident. The elevator was repaired and returned to service immediately. It was down for a total of 70 minutes.

The main mine elevator was inoperative from 6:55AM to 10:30AM on 08/20/2023 due to a main front door switch failure. No employees or individuals were on the elevator at the time of the incident. The elevator was repaired and returned to normal service.

During inspection of the hoisting rope it was discovered that over lapping had occurred during operation. This was discovered on 08/19/2023 and the hoisting rope was changed out on 08/20/2023.

During the time that the main mine elevator was down, while testing the brakeman car it was discovered that there was a faulty wheel speed sensor. The elevator became operational before the brake car was repaired. Later the brakeman car was repaired and put back into service. There were no injuries involved during this incident.

Due to a malfunction of a door switch, 21 men became entrapped on the Main Mine Elevator. ICE elevator services were notified and the switch was repaired with no injuries to anyone involved.

An unintentional roof fall was discovered on the Slope Mains Panel in the #4 entry 12-13 block. No injuries occurred with no damage to any equipment, ventilation controls, or escapeways. All approaches have been dangered off with additional roof support set. The fall dimensions are 6 1/2' H x 20' W x 50' L.

An unintentional roof fall was discovered in the West Mains panel on 52 zone in the #8 entry at 27 block. No injuries occurred with no damage to any ventilation controls, equipment or escapeways. The dimensions of the fall measured 8'Hx20'Wx20'L. All approaches have been dangered off with additional roof support set.

An unintentional roof fall was discovered on the 6HG section in the #2 entry at 20 blk. No injuries occurred with no damage to any equipment. An action plan has been out into place to clean this area. The mine has reviewed this plan with all crews working in the immediate area. Additional support has been set. Fall dimensions were 9'H x 20'L x 20'W. Ref # 03809526

An unintentional fall occurred on the LW face at the #5 blk. from shield 1 - 9. An action plan was put into place to safely finish mining this area. No injuries occurred with no damage to any equipment. Additional cribs were installed along with beams over the stage loader outby the potted area in the #1 entry. The dimensions of the fall were 4'Hx8'Wx40'L. Ref # 03809437

An unintentional roof fall was discovered by a Fireboss in the SW Mains #1 panel in the #3 entry at 23 blk. No injuries occurred with no damage to any equipment, ventilation controls, or escapeways. Fall measurements were 18'Wx25'Lx10'H. All approaches have been dangered off with additional roof support set. Reference # 0372627.

An unintentional roof fall was discovered on the North Mains#1 panel. This fall measured 6'H x 16'W x 50"L. Located in the #1 entry at 31 blk. No injuries occurred as a result with no damage to any equipment, ventilation controls or escapeways. All approaches have been dangered off with additional roof support set as well.

Main Drive Control for main hoist motor is malfunctioning causing it to shudder at times during operation. This issue does not cause a safety malfunction but mechanism is not operating properly so we have locked the unit out until this is repaired.

A Miner was dumping Muck Buckets on the Surface. When the bucket was flipping back over after dumping the miner's finger was caught between the bucket bell and bottom bell stop.

An unintentional roof fall was discovered in 7 Zone @ 24 blk in the #2 entry. No injuries occurred , with no damage to any ventilation controls, escapeways, or equipment. This fall measured 30L X 20W X 7H. All approaches have been timbered and dangered off. This is located near spad # L1187.

Fire coming out of P/C at the mule train of the longwall, immediate response contain the issue to just the P/C.

Employee was in the process of dumping a muck bucket when left forearm was caught in the counter weight area and pinched.

While mucking a miners left hand was caught between the loaded bucket and backhoe. This occurred as the bucket was being lifted and moved. The miner slipped in the mud causing left hand to enter the red zone between the edge of the backhoe hydraulic tank and the swinging muck bucket.

On February 2, Employee was assisting the unloading of a motor out of a supply car when EE grabbed the rope at the sheave wheel as another employee began operating the winch, 'bumping' the control. After the control was 'bumped', EE yelled stop, & indicated to the other person that a finger was pinched. Resulting as an amputation to the first knuckle of the index finger.

Individual was injured trying to move a piece of coal off the back board that measured 56 inches long by 28" wide which struck EE once it hit the chain. This caused it to flip up hitting EE's face causing EE to fall backwards hitting the back of head on the shield.

An unintentional roof fall was discovered on 11 Zone panel in the #10 entry at 6 block. This fall measured 8'H X 20"W X 20'L. No injuries occurred with no damage to any equipment, ventilation controls, or escapeways. All approaches have been dangered off with additional roof support set. Reference # 03607983.

Ref. #03568534 Location-#6 entry @ 30 blk. An unintentional roof fall was discovered on the SW Mains 1 (Old 4 Zone) panel. No injuries occurred with no damage to any ventilation controls, equipment or travelways. The fall measured 20'Wx5'Hx20'L. All approaches have been dangered off with additional support set.

An unintentional roof fall was discovered on the D1 (5 Zone) panel at 35 blk. between the #2 and #3 entries. This fall measured 5'Hx18'Wx50'L. No injuries occurred with no damage to any ventilation controls, equipment, or escapeways. Additional support has been set with all approaches dandered off. Reference #03549425.

An unintentional roof fall occurred on 24 Zone airway in the #1 entry at 37 blk. This was one block outby spad #138. No injuries occurred due to this incident and no damage to any equipment or ventilation controls as well. The area has been timbered with all approaches dangered off. Reference #03543001.

While attempting to change the batteries, personnel were trying to lift the new batteries with the scoop bucket and place cribs under the batteries. Injured placed their hand under the batteries to remove debris when the batteries slid on the metal scoop bucket. Injured was diagnosed to have a fracture of the second knuckle on the right ring finger.

WHILE PUSHING THE MONORAIL WINCH THROUGH THE AIR LOCK DOORS TO THE BELTLINE, INJURED PLACED RIGHT HAND BETWEEN THE MOTOR GUARD AND THE FRAME OF THE AIRLOCK DOORS. THE WINCH SHIFTED TOWARD THE DOORS AND CAUGHT INJURED'S HAND.

Head side shearer operator was cutting sandstone top traveling to the head gate. Injured was standing approximately 1-2 shields ahead of the head drum, leaning over the pan line monitoring the cut that was in progress. During this process, EE was struck in the neck (right side) by a piece of the metal from the drum.

Injured was struck by a sheave block while installing prefabricated bridges on HG2 beltline. The sheave block came loose from the anchor point striking the employee in the left leg resulting in a broken femur. The investigation showed that individual was in the red zone of the sheave wheel at the time the anchor broke

An unintentional roof fall was discovered on the 52 Zone airway in the #9 entry at 46-47 blk. This was a continuation of a previous fall that occurred on 12/19/2019. No injuries occurred with no damage to any ventilation, equipment or escapeways. Additional roof support has been set with all surrounding approaches dangered off.

An unintentional roof fall was discovered on 54 Zone haul road @ 8 blk. No equipment, ventilation or injuries resulted as of this fall. The escapeway had been rerouted and shown on all affected maps. All crews have been notified of the escapeway change. All surrounding approaches have been dangered off with additional support set.

Injured was operating Head Side of Shear on Longwall. Injured was leaning with left hand on equipment. A rock was thrown from the Head Drum approx. 15 ft and struck the injured's left pinky finger. Finger was fractured and lacerated.

Injured was bolting. When second steel added to steel already in roof. Injured did not get second steel seated into drill chuck. When up pressure was applied, drill steel slipped off side of chuck striking injured in the rt index finger. Injured continued to work until 1/27/22 when EE was taken to Broaddus Hosp for an x-ray and showed avulsion fracture.

Employee was pulling pallets of dust off of a supply trailer using a ratchet strap that was connected to a roof bolt plate by a 1/4 inch clevis. Employee was unhooking the strap when they were struck by a piece of loose rock that fell from the roof.

Employee was clearing rock out of the walkway of the longwall. While they were performing the task, a rock fell from on top of a shield and made contact with hardhat then landed on right thumb, causing it to be fractured.

While assisting a forklift operator removing block supplies from the supply car, employee placed their hand between 2 lifts of blocks. The blocks being lifted by the forklift shifted on the forks smashing EE's index and middle fingers on the left hand between the blocks.

While assisting other employees with installing head drive guard; the chain hoist used to position the guard came loose striking the employee on the right side of the face. Employee was taken to a hospital where employee was examined and treated for two small lacerations to the face and chin resulting in a total of four stitches. Employee was released to work full duty.

Employee was using channel locks to change bits on the continuous miner and as EE was pushing on the handle, it punctured EE's left hand below the thumb. The injury required 3 stitches. The employee was treated and released to full duty and terminated (resigned) employment with the company on 11/23/2021.

While attempting to uncouple the power supply battery cable plug to the scoop; melted insulation from the coupling was expelled, striking the employee on the right forearm. The employee was taken to hospital then transferred to a burn clinic where they were treated for a second degree burn to the right forearm. The employee was admitted overnight.

While attempting to move a de-watering pump the employee felt pain in EE's groin. After leaving the mine property EE later sought medical attention at Summersville Regional Medical Center where EE underwent surgery to repair a hernia.

As the employee stepped down off the front of the roof bolter, the employee stepped on a rock causing right ankle to roll. The employee was transported to a hospital where employee was diagnosed with a severe sprain to the right ankle. The employee began missing work on 10/11/2021.

The employee was stacking blocks to build a stopping when several blocks fell from the top layers striking EE's left hand resulting in a laceration to the palm requiring three stitches and a fracture of the left thumb.

Employee was positioned on #25 shield when the shield began to lower and pull in. The injured was caught between the hoses on the shield and the pan line. The employee was examined at a hospital and diagnosed with a fractured pelvis.

An unintentional roof fall was discovered on the 24 Zone beltline measuring 9'H at the intersection tapering down to 6'H in the crosscut for a distance of 50' appearing to tie in to an existing fall. No injuries to any individual occurred with this fall.

IE was going through track turn airlock door, when the door slammed shut striking IE's left hand. Causing a laceration to the top of the IE's left hand above the pinky.

An unintentional roof fall was discovered by a fire boss. The fall measured 53'L X 5-6'H. All approaches to the fall have been timbered and dangered off. No escapeway, ventilation or equipment was damaged by the fall.

IE was injured while removing a piece of 8' channel from a trolley cart. When the IE picked up the channel on the down slope side, the channel slid off of the cart striking the IE in the right foot.

Two unintentional roof falls occurred on 24 Zone discovered by a Compliance Officer in the #9 entry. At 34 blk. intersection and between 32-33 blk. Both fell 10' high with a distance of 20' measured at the 32-33 blk. location. All approaches have been dangered off and timbered. No escapeway, equipment, or ventilation was damaged by the fall.

Employee was carrying a header beam for a 6' door. When EE went to set the beam down EE's hands slipped, dropping the beam. The beam contacted the mine floor then bounced on top of EE's left foot. Employee finished the shift and later sought medical attention at the hospital. EE was diagnosed with a contusion to the left foot and taken off work as a precaution.

A face ignition occurred on the 4RHG section in the #1 entry at #4 block inby spad # K2005. The continuous miner was trimming the top when a fire occurred on the mine floor. The CM was backed out 15' and the fire was extinguished by the coal crew in 5 minutes.

While attempting to bleed air pressure from a 6" airline by loosening the coupler; the injured loosened the coupler once. Then while loosening the coupler a second time, the line blew apart striking the employee on the back of the head resulting in a laceration that required 3 staples. The employee was treated at the hospital and released to work full duty.

After changing a roof drill bit on the starter steel, the EE had the steel resting on top of EE's right foot. As EE lowered the drill pot, the pot caught the drill steel as it was coming down and pushed the steel down on top of EE's right foot resulting in a fracture to the 2nd metatarsal. The employee was taken off work after being examined by specialist on 5/28/21.

Employee was using a bar to hold track rail in place, the bar came loose allowing the rail to roll on foot causing injury

A fire in the face on the 5R Tailgate section in the #1 entry between 63 and 64 block occurred. The fire was extinguished in less than 10 minutes.

Employee fell from a ladder to the ground when the overcast wall employee was constructing fell resulting in a fractured hip.

EE was lifting a cradle with a chain and when EE let the slack out it came down and hit EE in the right shoulder.

A face ignition occurred on the 5R Tailgate section in the #3 Entry @ #50-#51 Block. The mining crew was cutting the top when the ignition occurred. The Fire was extinguished by the crew in less than 15 minutes.

An unintentional roof fall was discovered by a mine examiner, The fall measured 18'W x 18'L x 10'H. All approaches to the fall have been timbered and dangered off. No escapeway, ventilation, or equipment was damaged by the fall.

Employee was adjusting the winch rope around a bundle of rebar and it slipped pinching employees finger between the rope and edge of the scoop bucket.

Employee hit left knee on the head of the bolter causing injury

The rope gripper mechanism malfunctioned which caused the elevator to stop, trapping an employee on the elevator.

An unintentional roof fall was discovered by a mine examiner. The fall measured 18'W x 20'L x 6"H. The primary escapeway was blocked by the fall. The escapeway was rerouted to a different entry. The fall is being cleaned up. No equipment or ventilation was damaged by the fall. Continuation of roof fall measuring 18'w x10'L x5' thick occurred on 12-30-2020.

While attempting to move a joint of ventilation tubing out of the haul road, the employee states that EE felt EE's back lock up. The employee sought medical attention on 11/10/20. The employee began missing work on 11/11/20 due to a lower back strain.

An unintentional roof fall was discovered in the #42 zone Emergency Escapeway. The fall measured 18 feet wide by 30 feet long by 6 feet high. The emergency escapeway was blocked from travel. The fall has been timbered off at all approaches and the escapeway has been rerouted. No Ventilation or equipment was damaged during the fall.

On the surface both of the yellow airlock doors at the elevator were opened, creating excessive air pressure on the elevator doors, causing them to be forced out of the tracks. The doors were placed back into the tracks and new track guides were installed.

An unintentional roof fall was discovered by a Mine Examiner that measured 18ft wide by 60ft long by 5ft high. No equipment or ventilation was damaged during the fall. The fall will be cleaned up. The location of the fall was on Track Mains Section in the #1 Entry @ #18-19 Block.

While passing through a mandoor the door closed catching the end of EE's left index finger between the door and the door frame resulting in an avulsion requiring sutures and a fracture to the left index finger. The employee was treated at a hospital on 9-29-20 and following a visit with an ortho specialist on the same day, released to work full duty.

A shuttle car operator was adjusting ventilation controls to allow equipment to run through. During this process a scoop came from the opposite direction catching the EE between the scoop and the front of the car. The EE was transported to Ruby Memorial Hospital where EE was treated for a compound fracture to the left femur and laceration of the left leg.

The employee's left little finger was mashed between a water line coupling and the roof bolter bumper. EE was treated at Broaddus Hospital where EE received two sutures to the little finger. The employee was released to work full duty and returned to work on EE's next scheduled shift 7/25/20.

While tightening lug nuts on a mantrip, the wheel stud broke causing the employee to lose balance and to fall rolling the right ankle underneath EE. EE finished the shift and was taken to the hospital on 7/24/2020 where EE was referred to an orthopedic specialist on 7/28/2020. There EE was diagnosed with a distal fibula fracture and was taken off work.

While troubleshooting a set of scoop batteries, the battery cover lid fell catching right hand between the battery frame and the lid. Taken to hospital for exam where they were referred to a hand specialist on 7/22/2020. Employee was treated for a laceration to the right hand requiring sutures, and was released to work full duty.

A continuation from a previous roof fall (12/21/18) occurred on 5/31/2020. The continuation damaged a belt stopping that was repaired in a timely manner. There were no injuries and no escapeways were blocked by the fall. Floor to roof support have been installed. New stopping have been built and the area has been dangered off.

While removing a belt roller the employee felt a burning sensation in abdomen. EE continued to work this day and finished the shift. On 4-30-2020 the EE was diagnosed with an umbilical hernia at hospital. The employee has a family history of umbilical hernias, also the Dr. attributes age and weight gain to the condition.

While changing out a bottom conveyor roller the chain hoist used to lift the belt failed allowing the belt to drop. EE was struck by the belt causing them to fall against the belt take-up frame resulting in two broken teeth. EE was treated, teeth were repaired. EE was release to work with no restrictions.

The employee was walking outby from the face area toward EE's toolbox. As EE was walking EE's right foot slipped on loose material allowing EE's right leg to extend outward. The employee complained with discomfort to EE's right knee. The employee continued working until 1/14/20 at which time EE was taken off work pending surgery on the right knee.

While positioned at the drilling station of the roof bolter, the employee was going to walk toward the back of the machine. As EE placed hand on the bolter tray to assist with stepping up into the bolter walkway, a rock fell from the roof pinching EE's hand between the rock and the tray. This resulted in a laceration to the left forefinger and two fractures to the left hand.

While roof bolting in high top, EE was lowering the drill head to reposition the drill steel. The bit on the drill steel became caught on the roof mesh causing the drill steel to pull out of the drill chuck. The steel glanced off the machine canopy & struck EE on the upper lip resulting in a laceration that required four sutures to close. EE was released to work full duty.

An unplanned roof fall occurred at this operation on 9/17/2019. The fall did damage a set of scoop batteries but there were no injuries. The West Mains section is being pulled back and will be reset away from the fall area. All work is being done according to the action plan submitted to MSHA.

An unintentional roof fall happened on the West Mains Section in Entry #3 inby Spad #M952 that measured 20 ft. L X 18 ft. W X 10 ft. H. No Ventilation or Equipment was damaged by the roof fall.

While carrying CO cable during a section move, the EE tripped and tried to catch self with EE's right arm. The EE finished EE's shift. EE was taken to Hospital on 9/5/19 where EE was examined and released to full duty. The EE continued to work until 9/19/19 when a follow up visit for MRI results revealed a torn rotator cuff. EE was placed off work 9/19/19.

While assisting another employee with trying to repair the CM top side scrubber cover. Cover fell, pinching the left ring finger. EE was taken to hospital at Philippi, WV where they were examined. X-rays revealed a non-displaced fracture to the left ring finger. The EE was released to work full duty.

While unhooking the cable pull rope from the continuous miner pan, the employee's left forearm became pinched between the cutter head stop and the side of the pan. The employee was transported to a hospital in Morgantown, WV where they were examined. EE was admitted later that day to remove a portion of dead skin caused by the pinch. Prepping for graft.

While assisting roof bolters with rib bolting, the EE stepped off the front end of a walk through bolter rolling left ankle. EE was taken to a hospital where EE was examined & released to full duty. During a follow up visit on 7-9-19 to a medical center EE was diagnosed with a high ankle sprain & a cast was applied to the left ankle.

The employee struck EE's head against the mine roof. Contact with the roof caused EE's hard hat to come off which allowed EE's head to contact the roof. The EE was transported to hospital where EE was examined and treated for a laceration to the top of EE's head which required sutures to close. EE was released to return to work full duty.

While roof bolting a piece of rock fell from the roof striking the employee on the left forearm. The employee was transported to hospital Philippi, WV where employee was examined and treated for a laceration to the left forearm requiring five sutures. The employee was released to work with no restrictions.

The guide wheel on the elevator had broken off. No one was on the elevator at the time of the incident. The guide wheel was replaced and the elevator was back in service at 6:45pm.

Section of forms that was bolted with 3/4 in. bolts broke which allowed form to break open releasing concrete onto the work deck causing damage to work deck

Employee was performing maintenance on a continuous miner. Turned and stepped toward the welding insert and tripped over the miner cable. Employee stumbled forward, shifting weight onto the left leg. Employee finished the shift and worked the next scheduled shift. On 2/22/2019 the employee was taken off work pending the results of an MRI.

While using a cutting torch to cut a bent flight out of a shuttle car chain, a piece of hot slag or hot spark got into the employee's left ear. The employee continued to work until 6/26/19 at which time EE went to the hospital for medical procedure to repair perforation to the left ear drum.

Employee was assisting two other miners hanging the continuous miner cable and waterline. Employee lifted arm up to put a rope hanger around the cable and stated that ee felt something pop in arm.

Employee was pulling a motor from Air compressor using a Come-a-long. When ee felt a sting between shoulder blades. Employee continued working until 2/19/19.

EE was repositioning drill station of the twin head roof bolter. EE swung the drill station ee had gloved right hand resting on the hand hold on the underside of the operators canopy. Canopy came in contact with a roof bolt plate that was not flat against the roof due to uneven roof and the plate cut the 3rd finger of right hand requiring 3 sutures to close the laceration.

EE was helping 2 other employees pull the slack out of the Amsteel rope they were placing around a sheave wheel in order to pull a conveyor belt back together when EE experienced pain in EE's lower back extending down EE's left leg.

The injured along with another were in the process of changing a tire on a 6 person diesel personnel carrier. They were in the process of loosening the lug nuts on the wheel when the 4 way lug wrench came off the nut allowing 1 arm of the wrench to strike the injurer's gloved 5th finger of EE's right hand between the wrench and fender resulting in a laceration requiring 3 sutures.

The injured was working assisting others in removing a belt take up system. EE bent over to move a section of the tum table and felt a burning in the abdomen. EE was seen by a physician on 11/16/18 and was diagnosed with an inguinal hernia that will result in surgery to repair.

EE was in the process of installing belt hangers using the roof bolting machine and 5 foot long roof bolts to secure the belt hangers to the roof when the 4th finger on ee right hand was caught between the drill pot and the ATRS. This resulted in an open fracture and tissue loss to this finger.

EE was walking backward dragging the continuous miner cable and water line when ee tripped over a loop of the slack and fell onto back. Resulted in a contusion to ee right tricep and lower back and ee also hyperextended left knee.

EE was operating a radio remote controlled continuous miner when EE was struck on the top of the head by a piece of roof rock measuring approximately 65 inches long by 21 inches wide by 1 inch thick. Resulted in a fracture to EE's C4 vertebrae.

An unintentional roof fall was discovered by a Mine Examiner on the Lyons Mains Section. The roof fall was in the #6 Entry between spad #s L416 and L462 and measured 82 ft. L x 18' W x 12 ft. H. No people or equipment were damaged or ventilation disrupted.

EE was opening a large (3 ft by 6 ft) mandoor that was part of an airlock. Unknown to ee the door in this airlock was not fully closed and when ee opened the door ee was going to pass through ee felt pain in left bicep and shoulder.

Employee was helping hang CM cable. Felt sharp pain in the right shoulder, left arm and neck. Employee continued working until 2/18/2019.

The injured was using a set of 3/4 ton chain tuggets to secure a load on EE's supply trailer when the pin holding the hook in the chain failed allowing the device to strike EE on the left knee resulting in a bruise.

EE was walking towards the continuous miner and was carrying a bucket of miner bits and when EE stepped onto the miners trailing cable and water line, EE felt pain in the right heel.

EE was walking in the walkway of the walk-through roof bolter when they tripped on a raised portion of one of the floor plates and struck their left knee on the blower motor.

The injured was struck on the head and shoulders by a piece of roof rock that fell out between roof bolts. The rock measured 4ft. 9 inches long, by 2 feet wide and was up to 8 inches thick.

The injured was assisting another mechanic install an air to air cooler on a diesel tractor. As they were lowering the cooler into place manually. Employee felt a sharp pain in the right knee. Employee continued to work at their normal job until 10-26-2018 when they had surgery on their knee.

While using a hand held grinder to remove a weld from a piece of metal the grinder disc caught and the grinder bounced back allowing the rotating disc to strike the injured on the chin resulting in a laceration requiring sutures to close.

An unintentional roof fall occurred on the #2 section outby. There were no injuries or equipment damaged, and no escapeways blocked.

The injured was using a slate bar to pry down loose roof rock when the 4th finger of right hand was pinched between the bar and the ATRS raise arm of the roof bolter resulting in approximately 3/8" of the end of finger being pinched off.

An unintentional roof fall occurred on the 4 RE Panel in the no. 4 entry at Spad K409. The fall measured approximately 18 feet wide by 18 feet long by 10 feet high.

An unplanned roof fall occurred on the #4R pillar section. There were no injuries or equipment damage due to the fall and no escapeways were blocked. The section will be pulled back 9 blks where mining will resume

Employee was helping another mechanic set a shuttle car fender back in place, when employee caught left ring finger under the fender causing a laceration that required 7 sutures to close the wound.

The Rope Ripper Tripped on the Mine Elevator causing elevator to stop operating. There was an employee on the elevator at the time of the occurrence. Employee was stuck on elevator from 8:47 p.m. to 9:50 p.m. for approximately 1 hour and 3 minutes (63 Minutes).

The mine experienced a possible face ignition on the 2 Left Section MMU 002. The ignition occurred at the face of the #6 entry approximately 103 feet inby spad number K143.

An unintentional roof fall was discovered during the weekly airway exam. In haste to report the fall, the MSHA hotline was notified prior to the investigation. Upon investigation, it was determined that the roof fall fell under PPL P12-V-03 and was not considered reportable as the fall was in an entry not normally traveled with a common traveled entry in a single air course.

EE was working with 2 other employees hanging the continuous miner cable when one of the two struck the EE with hand in the groin.

EE was setting at rest in a covered minetrip when another employee reacted to a loud noise and swung and struck EE in the mouth with the hand held radio they had in their hand, resulting in 2 broken teeth to the EE which had to have dental repair. The incident was not reported to mine management until 11-29-2017.

EE was using an 8 lbs. sledge hammer to break up a rock that was on top of the continuous miner, when they lost grip of the hammer as they were swinging it, resulting in pain to the right hand as they struck the rock with the hammer. EE did not miss any work nor have any restrictions until 10-4-2017, when they had surgery and were placed on restricted duty.

During the 8 hour exam an unintentional roof fall was discovered on the No. 2 section in the number 6 entry just inby spad J111. The fall measured 18 feet wide by 10 feet high by 66 feet long.

The injured was in the process of closing a set of equipment air lock doors when EE was struck by the door.

An unintentional roof fall occurred while retreat mining was in progress on the # 3 left section in the # 4 entry at the # 32 block.

On May 25, 2017 we received a letter from MSHA notifying us that employee participated in a medical examination that indicated employee has a sufficient degree of pneumoconiosis.

The principal injured was walking the return air course in Sulfur Mains when they heard the mine roof give a break away from them. Their reaction to "run" resulted in them striking their head (hard hat) on a crib resulting in pain in their neck. They continued to work until 5-17-17.

Principal injured was attempting to remove a stuck drill steel from the mine roof by raising the drill head to the roof to align the wrench with the stuck drill steel when employee's right thumb was caught between the wrench and the T-3 channel strap employee was bolting to the mine roof.

The injured was operating the radio remote controlled continuous miner when the coal rib, at back, "popped" off the rib striking ee on the lower legs. The rib measured 9 feet long, by 5 to 7 inches thick and 6 feet high. This resulted in a fracture to right ankle.

EE was watching the 2 inch water line hose as it was being pulled by a scoop, when the coupler on the line caught on a rib corner. The line stretched tight and the coupler came apart allowing the end of the water line to strike the injured on the right leg just below the knee resulting in a fracture to both the fibula and tibia bones.

EE had drilled into the mine roof 5.5 feet with 2 stackable drill steels, EE was attempting to place the third stackable steel into the roof bolter drill pot when the 2 that were in the hole dropped out striking and pinching the little finger on EE's left hand between the steel EE was holding and the ones that dropped out of the roof. Resulted in a laceration requiring 4 sutures.

While the injured was walking the alternate escape way (belt entry) out of the mine as part of the quarterly escape way drill, EE felt/heard a "pop" in EE's left knee. EE continued to work up until 7-27-16 when EE had surgery to repair a torn meniscus.

The Brakes locked up on elevator causing slack in the Ropes then causing Ropes to come off of sheave wheel then causing damage to the Ropes.

An unplanned roof fall was discovered by a mine examiner on #1 section, #5-6 intersection, @ #12 block. The fall measured 70'L by 18"W by 14'H.

A Mine examiner discovered an unplanned roof fall on the #1 section; #6 entry; #10 block; spad #G286 measuring 11' high x 20' long x 20' wide

While conducting on-shift exam of the pumps in this area of the mine, the examiner discovered an unintentional roof fall in the No. 3 entry of 26 zone one break outby spad C439. The roof fall measured 14 feet wide, 25 feet long & 5.5 feet high. A "wild" coal seam and irregular roof was present at the anchorage zone of the 5 foot fully grouted torque tensioned roof bolts installed.

While 1 person was riding the elevator it stopped due to the heater in the control room being turned on causing the temperature in the room to exceed 110 degrees F.

While 2 persons were riding the elevator it stopped due to the governor over speed trip. The switch was reset and the elevator was put back into service.

An unplanned roof fall was discovered by a mine examiner in the #27 zone; #24 block intersection. The fall measured 30'L by 18'W by 5'H

While roof bolting the injured was drilling a hole into the mine roof when a portion of roof broke around the drill steel. The rotation of the drill caused the rock to spin around the steel. EE stopped the rotation and the rock broke and a portion of it struck EE on the forehead above the left eye. The rock caused a laceration and small portions had to be surgically removed.

While riding as a passenger in a rubber tired mantrip, the mantrip hit a bump in the road jarring the EE resulting in pain to the middle/upper back. This incident was reported on 6/16/2015 and the EE did not begin missing work until 7/6/2015.

An unplanned roof fall was discovered by a mine examiner in the 27 zone at 25 block, Spad No. E842. The roof fall measured 18' L X 18' W X 5' H.

Employee's right arm was caught between the feeder and the feeder trailing cable when he was attempting to trip the machine's service breaker. The cable had been caught by a rock in the feeder and became tight at the point he reached for the breaker switch. The injury resulted in a fracture to the radius and ulna bones.

The elevator was in route from the mine bottom to the top when it stopped. The track of the door had some dirt in it so it was cleaned and the access roof door was found to be ajar and it was closed and the elevator worked following these findings.

An unplanned roof fall was discovered on 1/5/2015. The fall was located in the South West Mains 1 in the #6 entry; #17 to 18 block; inby spad #995. The fall measured 18'W X 75'L X 6'H. Weak laminated shale roof that was layered with sand streaks and deteriorated over time.

Unplanned roof fall was discovered on 12/30/14. The fall was located in the 1 right Southwest Mains 4 Entry #1 inby spad #C204

A roof fall was discovered in 28 zone 1A panel, #15 block, spad #E280 during the weekly examination.

A hoisting accident happened on the slope supply track. 5 cars 1 of which was loaded with dust was being pulled out of the mine when the car hooked to the hoist car became uncoupled which resulted in the donut chain and the safety chain breaking letting the entire trip of cars to travel back down the slope approximately 125' before derailing in the rib. No injuries.

Employee was erecting a hollow block ventilation stopping in 7 foot of height when the stopping fell onto him resulting in a laceration to his face requiring 6 sutures.

An unintentional roof fall was discovered during a weekly airway exam. The fall was located in the SW Sub mains 1, 45 feet inby spad D421 in the number 9 entry. The fall measured 42 feet long, 18 feet wide and 7 feet high.

The injured was pulling on a piece of top belt structure with his hands to remove it from the walkway it was laying in when he felt pain in his left shoulder. This resulted in a tear to 2 muscles in his shoulder which required surgery to repair.

An unplanned roof fall occurred on 7/7/2014 in an intersection in Old 8 Zone 12 Block @ Spad #9259.

Move crew had set feeder about 30-40 min. earlier & as they were removing items from the load end of feeder the injured's foot was under the load end when it settled into the coal bottom & onto the top of his left foot which created 2 non displaced fractures that did not require immobilization. He was wearing metatarsal boots at the time which lessened severity of the injury.

An unplanned roof fall occurred on 6/27/2014 on the #3 section in the #3 entry at the #17 block at spad #D634. The fall covered up one of our MRS (Mobile Roof Support) units but did not damage the unit.

EE was attempting to cut open a Bulk Bag of Rock Dust. He was holding onto the top of the bag with his Left Hand, and using his Right Hand to operate the safety utility knife. he made a quick upward motion with the knife to cut the bag open, and did not realize his Left Wrist was in the Line of Fire of the knife. He punctured the blade into his Left Wrist, 3 sutures to close wound.

As the injured was stepping out of the operators compartment of a diesel tractor, the toe of his foot caught under the seat causing him to trip and fall forward out of the operators compartment. In an attempt to catch himself he struck his hands out and as a result, hyperexteded the thumb on his right hand causing a fracture to the metacarpal bone.

Employee participated in a medical examination that indicates he has a sufficient degree of pneumoconiosis.

Employee was hosing out the bath house, when the hose became tangled and stuck; he pulled on it and felt pain in his groin area. He was diagnosed with 2 hernias which required surgery to repair.

There is an old fall about 80' inby this fall. The area outby this old fall was cribbed due to adverse roof. It looks as though this area may fall and the 2 falls will join into 1 fall. Fall dimensions are 80'L X 18'W X 6'H. Fall area SW Mains #3 #10 entry spad #6863

An unplanned roof fall occurred on the SW Mains, #5 Entry, #10 blk, spad #1652. There were no injuries or equipment damaged due to the fall and no escape ways or air courses affected.

An unplanned roof fall occurred in the #9 zone worked out area. The fall was discovered on 1/2/14 by the shift foreman. There were no injuries or equipment damage due to the fall and no airways were obstructed.

The injured was placing a box of unused roof bolter bits onto the mantrip and felt pain in his right shoulder.

The injured was roof bolting when he was struck on the left hand by head coal causing a laceration to the little finger of this hand requiring 3 sutures to close. He was wearing protective metacarpal gloves at the time.

Unplanned roof fall occurred on 7/18/2013 in the SW Mains 3. There were no injuries or equipment damage due to the fall and there were no escape ways blocked or ventilation disruptions. The area will be dangered off and it will be cleaned up at a later date.

The elevator would not function as the ceiling door switch was not activating to allow the elevator to function, as the door was not completely closed. The door closed. The elevator functioned, no damage found.

The surface supply track hoist rope cable became kinked when the system was placed in automatic to lower supplies into the mine and the track switch on the surface was left thrown for the surface supply yard and not the slope track. This allowed the hoist rope to overlap and kink in the slack that was created. The overlap switch was found to be bad.

Employee was handed a block, by another employee while building a stopping. Employee grabbed block, and turned around to place block on a partially built stopping. By doing this he struck his hand on the edge of an already placed block on the stopping, causing a laceration to his left middle finger which requited 5 sutures to close.

Employee was helping install truss bolts when he reached up to free a connector that was fouled against the roof when the truss became tight catching his left hand between the truss and the mine roof causing a laceration to his left middle finger that required sutures.
