Miner was attempting to relocate a 4" pump discharge line when EE injured their lower back. Later determined to be a muscle strain.

EE tried to start the pump located in the rear compartment on the passenger side of the fuel truck and hit the back of EE's hand on a bolt on the mud flap located beside EE cutting hand. The cut EE received from this bolt required 10 stitches in the right hand.

The employee stated that he was working on a motor that was suspended above a walkway. The motor fell, striking the employee's foot and collapsing the walkway.

EMPLOYEE WAS PICKING UP A BAG CONTAINING COAL USING HIS LEFT ARM WHEN HE STRAINED A MUSCLE IN HIS LEFT ARM.

While trying to dislodge the frozen pulling lashing from the loaded barge, something ripped loose and popped in my right shoulder socket.

Employee was pulling a barge down river when he felt a pain in his lower back.

Employee was pulling a 25 HP motor on a sloped catwalk. The motor started to slide backwards. The employee jerked on the motor. The motor pulled and strained his muscle in his right shoulder. Rest of shoulder was ordered.

Roof bolting with Fletcher RR2 twin head bolter, he had 4' resin bolt in hole and it was partially bent. He was pushing on bottom end of roof bolt with his body weight trying to straighten bolt when bolt twisted in hole causing him to fall into the bolt hurting his shoulder area.

Injured was making a splice on a trailing cable and bent over to pick up roll of tape and twisted lower back.

Was putting tension on cable bolts and tension unit had metal burr on it and caught rt index finger. Made 1" laceration.

Injured was bending a resin roof bolt to put in roof and pulled muscle in lower back.

Was bending a roof bolt that was part way in hole, he slipped on muddy wet pavement & pulled groin muscle.

Injured was picking up heavy object from floor level in warehouse and heard right knee pop.

Was breaking metal band on bundle of roof bolts. When band broke open he was cut on right ring finger.

Injured person was pulling shuttle car cable to a new location on section when injured stomach muscle's. Diagnosed as hernia. This person worked light duty until 6/12/07-day of surgery.

Was putting a bucket of mortar over a stopping wall. Being submitted due to Part 50 Audit.

Employee was removing 12.00 x 24 tire from a truck. He tried to lift a tire, he strained his lower back, left side. He worked the rest of the day. On Monday he had severe pain in his back and did not report to work. On Tuesday he came to work but could not punch in. He went to the chiropractor.

Pulled a muscle in his back (left side) while lifting CM cable & water hose.

Employee was unloading bucket of 36" used belt structure from scoop bucket and pulled muscle in back lifting 1 pc of structure.

Picked up a piece of belt and pulled back muscles.

Installing 6" yellow mine water line. Put 2" shut off valve on end. Filled line with mine water. Opening 2" water shut off valve to purge line of rocks and debris. First coupler outby failed and jetted 6" line inby. EE's hand was on valve causing hard jerk on shoulder.

Individual was bending a roof bolt, getting ready to install. EE lost footing, bearing plate cut back of left hand. 4 stitches

EE was pulling up sticks on silt fence. When he pulled something on his lower back.

WHILE EE WAS GOING THROUGH A MANDOOR, HE FELT PAIN IN THE LEFT SIDE OF HIS STOMACH. DIAGNOSED AS HERNIA - SURGICALLY REPAIRED ON 1/28/05.

Employee picked up a 4 ft section of 6" schedule 80 pipe, twisted his back and pulled a muscle in his lower back.

WHILE CLEARING LOOSE ROCK OFF OF MINER, EE DROPPED ROCK AND CUT FINGER.

While trying to remove a large piece of slate off of belt EE felt a tear in right side of abdomen.

EE WAS REPLACING B-1 SCREEN PANEL WHEN THE PANEL BECAME WEDGED IN BOTTOM DECK. EE JERKED ON SCREEN, STRAINING HIS BACK, LOWER BACK. EE CONTINUED WORKING FOR THE FOLLOWING 2 WEEKS. ON 7/25/04 EE WENT TO THE HOSPITAL - PAIN HAD INCREASED TO THE POINT OF UNBEARABLE. X-RAYS WERE TAKEN: SHOWED BULGING DISC IN L-3. FIRST DAY MISSED WAS 7/26/04.

EE WAS WATERING BATTERIES ON #3 SECTION SCOOP. HAD LID WEDGED AGAINST TOP. LID FELL DOWN SMASHING & CUT RIGHT THUMB.

FOUND ROCK ON JABKO CABLE 4 BELT 1 1/2 X-CUTS OUTBY 4160 X-OVER SIZE 4 TO 4 1/2' LONG 2 1/2'WIDE 5"THICK TAPERING TO 1". INJURED LIFTED ROCK TO ROLL OUT OF WAY WHEN IT BROKE ON LIFTING END FAL LING ON VICTIMS'S RIGHT FOOT.

EMPLOYEE WAS HELPING REGULAR SCOOP MAN PLUG UP BATTERIES WHEN HE TURNED AROUND GOT HIS FOOT CAUGHT IN MUD CAUSED HIM TO TWIST AND FALL RESULTINGIN INJURY.

EMPLOYEE WAS KNEELING DOWN MOVING MINER CABLE OVER TO RIB, AND STRAINED A MUSCLE IN UPPER BACK.

EE WAS HELPING BELTMAN TO PUT TOP BELT ROLLER BACK UP ON BED RAILS WHEN HE PINCHED HIS RIGHT MIDDLE FINGER BETWEEN BED RAIL AND BELT ROLLER STRUCTURE.

EE STOOD UP AND HIT HEAD ON PIPE.

EMPLOYEE WAS LOADING BELT STRUCTURE INTO A SUPPLY CAR. WHEN HE TWISTED HIS BACK ABOUT 7:00 PM CONTINUED TO WORK THE REST OF THE SHIFT. EMPLOYEE NEVER SAID ANYTHING ABOUT IT TO HIS FOREMAN AT THE TIME OR AFTER THE SHIFT. IT WAS REPORTED TO ME AT 3:00 PM ON 9-13-01. HE HAS NOT REPORTED SINCE.

EE WAS LIFTING PUTTING BELT STRUCTURE IN AND PULLED MUSCLE IN LOWER BACK.

HE TWISTED TO PICK UP ROOF BOLT TO PUT PLATE ON IT & HURT BACK.

EMPLOYEE WAS CLEANING WET MATERIAL FROM SHOOT, WHEN A ROCK FELL FROM THE FEEDER BIN STRIKING THE EE ON THE RIGHT HAND CAUSING INJURY TO THE RIGHT THUMB AND HAND.

WAS OUTSIDE OF MINE CLIMBED OUT OF SCOOP AND SAID BACK HURT.

MAN WAS PICKING UP MINER CABLE TO MOVE OUT OF ROAD WHEN HE PULLED THE MUSCLE IN BACK.

WAS LIFTING 5X7"X16' X-BARS ONTO CAR. STRAINED BACK. DID NOT NOTIFY MANAGEMENT UNTIL 11/13/98.

SHOVELING BELT, TWISTED AND HURT BACK.

LIFTING BELT TO INSTALL ROLLERS HURT LOWER BACK

WHEN MAN WAS PICKING UP BAG OF ROCK DUST GOT SHARP PAIN IN LOWER BACK.

WAS LIFTING MINER CABLE UP TO TIE UP AND STRAINED HIS BACK AND SIDE.

HE HAD PARKED HIS SC TO HELP HAND MINER CABLE. HE HAD TAKEN HIS GLASSES OFF TO CLEAN THEM WHEN ANOTHER SC OPERATOR ASKED FOR A HAND WITH CABLE. HE THREW HIS GLASSES INTO HIS SHIRT AND GRABBED AHOLD OF CABLE.

WAS PULLING ON A PIECE OF 4" FLEX DISCHARGE LINEHURTING HIS BACK.

WORKER WAS HANDLING OLD BELT STRUCTURE ON 3 BELT LINE, HE CUT HIS FINGER ON A SHARP PLACE ON ROLLER.

WAS LIFTING A ELECTRICAL TRANSFORMER AND PUTTING IT IN A STARTER BOX AND HURT HIS BACK.

WHILE WASHING IN THE SHOER, EE GOT SOAP IN HIS EYE.
