Operator was dismounting D-6 dozer when EE's foot slipped from the blade arm due to wet and muddy conditions causing EE to hit EE's left knee on the arm. Operator had just returned to work from a double knee replacement and the impact on the knee caused the incision/scar area to reopen.

Mostrando 50 de 263Operator was having an overheating issue with the equipment. They shut down the machine to investigate. They opened the coolant reservoir which had somehow been pressurized. As a result, the hot coolant burned the left arm of the operator. The extent of damage is limited to their left arm.
Miner was attempting to relocate a 4" pump discharge line when EE injured their lower back. Later determined to be a muscle strain.
Maintenance Tech was performing maintenance on a D-8 Bulldozer. Miner slipped when trying to cross over the blade to add oil and fell on the blade brace. Miner injured left knee from the fall.
Miner had just completed adding oil to the Hitachi ZX450 excavator when EE stepped down onto the track. After stepping down onto the track. Then, slipped on the track, fell against the catwalk of the machine. EE completed EE's entire shift. It was not until the end of shift EE noted pain in EE's rib area. EE did not report the accident until the following morning (5/6/2022).
EE tried to start the pump located in the rear compartment on the passenger side of the fuel truck and hit the back of EE's hand on a bolt on the mud flap located beside EE cutting hand. The cut EE received from this bolt required 10 stitches in the right hand.
Employee was doing a pre-shift inspection on 290 Volvo excavator. While standing on machine tracks, slipped and fell to uneven ground. Tracks are 30" high.
The employee grabbed the steering wheel to get into the truck. His shoulder popped out of place. The employee then let go of the steering wheel and fell to the ground. He began missing work on 2/7/2014.
The employee stated that he was working on a motor that was suspended above a walkway. The motor fell, striking the employee's foot and collapsing the walkway.
EMPLOYEE WAS PICKING UP A BAG CONTAINING COAL USING HIS LEFT ARM WHEN HE STRAINED A MUSCLE IN HIS LEFT ARM.
The employee stated that he was walking on a barge when the barge cable recoiled and hit him under the right arm, knocking him into the loaded barge. The employee did not begin missing work for this injury until 1/14/2010.
While trying to dislodge the frozen pulling lashing from the loaded barge, something ripped loose and popped in my right shoulder socket.
Employee was pulling a barge down river when he felt a pain in his lower back.
Injured was operating fletcher twin boom roof bolter. Drill steels dropped out of roof and hit EE in middle finger of right hand.
While mining #6 lift to rt. in #2 hg 5-6 bl. S. Main D-F panel. Miner was backing out of left and top fell in, covering up about 10' of the cont. miner. Area posted, retrieved C.M. Cover 250'-area bolted with 6' resin bolts-called MSHA Hotline.
While running a miner, a rock fell and struck his left leg.
While pillaring #11 entry, #7 lift, the roof fell in covering up the continuous miner with approx. 6' of rock. Cover of approx. 300'. Area bolted with 6' fully grouted resin bolts & header boards. Sub main sect. (19 belt). #11 heading outby spad #4409.
While removing a wheel unit from shuttle car, employee had a breaker bar on a wrench. He stood on the breaker bar which slipped off the wrench and struck him on the leg.
Injured twisted knee while running roof bolter. Normal work while bolting. Not sure how knee became injured. Right knee has ligiments torn. MRI scheduled 3/25/08
A roof fall occurred at #20 belt head on E-panel submains 1-North. Laminated shale rock fell from 6'-10'-12'. Area bolted with 6' resin bolts & header boards & 12' cable bolts with monster mats. Cover 415' sect. was moved outby affected area. Cribbed & dangered off. Called hotline.
A roof fall was discoverd at #13xc, #8 entry in the south E panel. No equipement was trapped, ventilation was not affected, the passage of men or material was not affected. The fall was 17ft wide, 17ft long and 7ft high. The intersection was bolted wirh 5ft fully grouted bolts
Employee was pulling a 25 HP motor on a sloped catwalk. The motor started to slide backwards. The employee jerked on the motor. The motor pulled and strained his muscle in his right shoulder. Rest of shoulder was ordered.
EE was bending a 6' bendable resin roof bolt and slipped on pavement and injured right shoulder while leaning forward into the roof bolt.
A roof fall was discovered in the #3 main 2500' outby the section in a parallel entry. Fall location is #3 main #5 xc #10 entry. No impairment or damage to persons, equipment or systems.
Cutting ties off plastic lid of 5 gal. bucket of plaster with utility knife and cut hand in thumb area of left hand.
EE was burning a jig elevator bucket that was stuck in the chute. A piece of slag entered under his safety glasses and burned his lower left eye lid. EE rubbed his eye enough to scratch them. Drops were applied to his eyes. He missed one day, 1/3/08.
Injured man was roof bolting on a twin boom bolter when a piece of rock fell between the ATRS and the canopy striking him on the back of his left lower leg and foot.
Piece of rock approx 1 1/2 feet long 3in thick. Fell from between roof bolts in fully bolted area and hit injured in the back.
Employee was looking for a suction hose "6". He stepped on a patch of ice and fell. He stabbed his middle finger on his right hand. He struck his left knee on a pallet. He tore ligament in his left knee when he fell.
Was servicing front end loader and was standing on steel brace on front end bucket area to reach high area were fittings were located. Slipped and fell on his rt. hand.
A roof fall measuring 17'x30'x7' high was discovered during a rock dust surveyor being conducted during an EOI inspection in the south E panel in the intake air course, #8 entry. #10 xc. The area was bolted w/5' fully grouted bolts. The passage of men and supplies, ventilation or escapeways were not involved.
While injured was tramming bolter from one work place to the next work place, he put his arm outside the tram canopy deck area and trammed bolter into rib causing arm to get caught between rib and bolt machine.
Fell in approx. 12' high x 16' w x 18'l area was bolted with 6' fully grouted resin bolts with 6"x12" header boards and 6-12' cable bolts. top laminated shale no water in top at time of fall cover approx. 480' mining over top of pitts transition, area dangered off & will no be cleaned up called hotline
Injury walked into protruding stub end of cable bolt hanging down below roof line and hit nose.
Injured was preparing to rock dust an area with a co-worker and was holding a bag of rock dust and co-worker was slashing bags open with utility knife and cut injured person on right hand.
Injured was walking on section and a protruding cable bolt end caught him on the right ear causing laceration needing stitches.
Miner had loaded shuttle car and miner operator was on knees at end of miner boom along rib line. When shuttle car was pulling out, the s.c. operator swerved s.c. to right and rear of s.c. pulled toward miner operator, catching his feet with its bumper/frame, injuring his left foot.
Employee was greasing the Wasco hilift. He was using a lever action grease gun. A bucket pin was hard to take grease. He was pulling the lever towards him, with the body of the grease gun resting on his chest. Another employee assisted him pulling the lever. The force against his chest cracked a rib on his upper right chest near his sternum.
Roof bolting with Fletcher RR2 twin head bolter, he had 4' resin bolt in hole and it was partially bent. He was pushing on bottom end of roof bolt with his body weight trying to straighten bolt when bolt twisted in hole causing him to fall into the bolt hurting his shoulder area.
When a 15' cut from #4 to #3 entry holed out, the roof for 2 rows of bolts in #4 fell out, along with the unbolted cut. The approachs were cribbed off; at 4:50 pm the fall extended overriding the cribs, falling through the intersection of #4 entry, landing in the head of the cut. The fall area was bolted with 8' fully grouted bolts & 12' cable bolts.
Injured was making a splice on a trailing cable and bent over to pick up roll of tape and twisted lower back.
Was preparing to bolt on twin boom Fletcher truss bolter and maintenance had been preformed on hydraulic system and an oil slick was left in the work area. He had hold of drill steel looking up at roof & slipped in oily area and right hand hit levers and clinched steel clamps around steel catching left hand.
During preshift examination of the North section faces (MMU #003) a fall was discovered in the face cross-cut #1 entry - 2 entry xc, through the intersection of #2 entry, through xc 2 to 3 stopping at the #3 intersection. The laminated shale fell to the sandstone. The area was bolted w/ 5 ft fully grouted resin bolts. The fall stopped at the cable bolts.
Injured was jacking red devil tugits when he slipped causing him to twist left leg injuring knee.
On the north section on unbolted cut in the face of #4 entry, fall at the end of afternoon shift. At 1:30 a.m. a triangle shaped piece of roof at the row of bolts fell out. A total of 6 bolts were dislodged. This happened on the idle shift.
x5-4-6 bld, along 19 felt on sub main-fell in approx. 6' high, 16 w x 30' l area bolted with 6' fully grouted resin bolts and 4' crib set in inter sent. along belt-top laminated 8 hole and had water in top. cribbed off and dangered off a will not be cleaned up. Called hotline-cover approx. 275.
Injured was using blade knife cutting off shuttle car cable to make splice when knife slipped cutting left index finger-had 5 ctitches.
N. Main set. #9 inter. 2 way-fell in approx. 6-12' high-16' l wide, 25' l. bolted with 6' fully grouted resin bolts and 6" x12" header boards. chen was spot bolted with 12' lb cable tenain bolts. Cover 300' mining on top of pillared pits seam. top laminated shale in the streaks of sandstone chea dangered off cribbed off will not be cleaned up called hot line 54pd.
A roof fall occurred in 4 way inter. Fall approx 5'-6'h, 17'w x 40'l. Area bolted with 6' resin bolts & 6" x 12" hole boards. Prior fall area was supported with 4' wooden cribs. Fall is at edge of pittsburgh piller & solid coal area. Called hotline. Over 240' top shale laminated. Sub main #3 1 brk 2blocks outby spad #419. Fall observed
Was splicing trailing cable and cut right index finger with blade knife.
