Employee was walking beside the shuttle car to get in it when his right forearm struck a piece of rock that was lying on top of the deck near the conveyor area.

EE's left middle finger was caught between (2) crib blocks while installing supplemental roof support.

Employee switched track switch, injuring his back.

Employee was picking up a heintzman jack and a metal strap slide across his left hand.

EE was bent over getting roofbolts out of a bundle, when another EE pulled some roofbolts out of the bundle and one struck him in the mouth.

Employee was shoveling material out from under belt line. He was directly across from another employee. The other employee accidentally threw some material out of his shovel across the belt line and some went into employee's eye.

EE was loading power supply cable in move box. The next day a knot popped outon his left wrist. He started missing work on 5/14/07.

Employee was raising the lid on a battery when his right hand got caught in the handle. The lid continued to open and fell against the other lid smashing the tip of his right middle finger.

EE was cleaning rock off section crusher when a rock smashed his right index finger.

EE was using pork chop clamp & chain to remove belt from storage unit. In the process of pulling the belt the chain broke & pork chop clamp struck him on top of left foot.

Employee was hanging phone line. He had spool of phone line hanging from roof bolt plates with nylon cable hangers. One of the nylon cable hangers broke and the spool fell on his right calf and ankle.

Injured was installing a set of airlock doors when the frame fell striking him in the right arm.

Injured was looking for an oil leak on a Petitto mule, when his leg got caught in the motor coupling. This resulted in a laceration to his left leg.

Employee & 2 co-workers were working on seals. The were building seals in 16' height. Inj EE was on scaffold, 1 co-worker pass lock to inj EE who would pass it up to other. Inj EE felt pull in his stomach.

EE was in process of tighting a hydraulic fitting on a shuttle car, when the fluid came out and struck him in his eyes.

Employee was helping pull equipment cables during a belt move, when he felt a sharp pain in his back. He completed his shift and missed 4 days work.

Employee was using a cement mixture (B-Bond) to seal a cinder block stopping. He had an allergic reaction to the mix causing burns to his right arm.

Employee was helping to install a cable spooling device in a Joy shuttle car when he felt a sharp pain in his left arm. He finished his shift and missed 4 days.

EE was installing belt structure for the longwall belt on headgate #10. He was lifting a piece of 60" top structure when he felt a pain in his right shoulder. He completed the shift and didn't report to work his next shift.

EMPLOYEE WAS SHOVELING BELT. HE WAS JUST GETTING READY TO TIGHTEN DOWN BOLT ON J-HOOK ON BOTTOM ROLLER. ROLLER CAME OFF AND STRUCK HIS RIGHT FOOT (FRACTURED METATARSALS).

Employee was picking up tongue on rail truck and pulled muscles in his back. He worked until Fri, missed Fri and Sat.

Employee was trying to remove rock off of belt and another rock fell behind him hitting the right side of his head & shoulder.

Employee was sorting out structural steel moving around pieces when a beam measuring about 24"x10"x24" fell over on his left foot. He was wearing metatarsal boots, but a piece of the steel caught his foot below the metatarsar ont he side near the ankle.

Employee was removing rock out or chute on the longwall belt when a piece of rock struck his left hand. This resulted in a laceration to his middle finger of his left hand.

EMPLOYEE WAS SHOVELING THE SLOPE BELT WHEN HE BENT OVER & FELT A PAIN IN HIS LOWER BACK.

EE was shoveling around belthead and felt "stinging" in groin area.

EMPLOYEE WAS MOVING A TORCH TO BE USED AT ANOTHER LOCATION WHEN AN EMPLOYEE OF ANOTHER COMPANY (NATIONAL ELECTRIC) DROPPED A PIECE OF ELECTRICAL CONDUIT FROM OVERHEAD, HITTING HIM IN THE LOW BACK & TAIL BONE. DID NOT SEEK MEDICAL TREATMENT AND CONTINUED TO WORK WITHOUT RESTRICTION TILL 10/7/05.

While moving steel under motor, channel came loose striking left ankle.

Pulling in 1 1/2" water line for belt, line got a knik in it, victim was trying to fix kink in line when line hit fire valve causing water to spray him in the face.

Employee injured himself picking up a shuttle car cable.

Trying to put pin back in throw, throw caught index finger. this was track switch 85 lb.

EE tried to stop a roll of belt rope cable that was rolling down a ramp at the wharehouse.

Employee was carrying an oxygen tank; tank slid down and hit his right forearm.

EMPLOYEE WAS IN THE PROCESS OF CONSTRUCTING A ROCK BOX, WHEN HE WAS LIFTING A PIECE OF METAL, HE FELT PAIN IN HIS RT. SHOULDER.

SHUTTLE CAR OPERATOR STOPPED TO HELP WORK ON BOLTER. BOLTER HAD PUMP MTR DOWN. ROCK FELL FROM TOP OF TBAR AND HIT LEFT FOOT

Mine mgt became aware of this accident on 11/5/04 Until this time EE told mine mgt that he was sick.It is company policy to report any accident before leaving the property & have an accident report filled out.He did not comply with company policy.On 11/5/04, he alleged to mine mgt that he had injured his groin while lifting a timber.

EE was trying to get a rock out of the drag chain on his shuttle car, when he lifted the rock, he felt pain in his groin.

Shuttle car hung on feeder. used scoop to pull shuttle car off feeder. while pulling chain broke striking victim above left eye.

Employee was struck on his hard hat by a block that fell from the mine roof as he was recovering blocks from a stoppping that he had just knocked.

EE WAS PUTTING LARGE SAFETY ROPE ON SUPPLY CAR. ROPE TWISTED AND SHARP BURR CUT HIS ARM.

Unloading timbers from a flat car when one of the timbers rolled down and mashed his finger tip.

Employee was dropping bottom rollers out of brakets to put shieve cable on top of rollers. Employee bent down to pick up roller and said he started feeling pain in his lower back.

EMPLOYEE WAS PLASTERING OVERCAST, B-BOND FELL OFF GLOVE AND INTO HIS EYE.

Loading belt and twisted knee.

EE did not report this accident to me. Per phone call he does not know time of accident, stated that it started hurting, also he told me that he did not report this as a accident, i found out per phone call from worker's comp concerning a WC3 report.

EMPLOYEE WAS HANGING WATER LINE WHEN A PIECE OF DRAW ROCK 26" LONG BY 24" WIDE BY 1/4 TO 3" THICK FELL FROM BETWEEN BOLTS, STRIKING HIM ON THE HEAD AND LEFT SHOULDER.

EE'S HAND WAS CAUGHT BETWEEN 6" AND CAR WHEN ANOTHER EE PUSHED LINE ONTO CAR.

WATER HOSE HIT EE IN BACK AND RESULTED IN BACK AND GROIN INJURY.

EMP WAS FITTING PIPE VERTICALLY. TACKED THE PIPEAT THE TOP. TACK BROKE CATCHING FINGER BETWEEN PIPE.
