Speed Mine Coal

An unplanned roof fall occurred on 10/18/2025 at approximately 1:16 AM on the 4 right panel of Unit 1. The fall occurred in the intersection of No. 8 entry, crosscut No. 17 and was approximately 20ft X 20ft X 10ft thick.

Injured employee was in the process of closing air lock doors at the mouth of the section when left hand was caught between the doors. This caused a laceration on the left index finger, resulting in 8 sutures.

Employee was injured while in the process of closing airlock doors. Employee was walking over track when EE tripped on the track and attempted to catch self with EE's hands causing a laceration to the left hand, resulting in 5 sutures.

Employee was injured when EE struck self in the face with a ratchet while tightening the gear case mounting bolts on a continuous miner causing a laceration above EE's left eye resulting in 18 sutures.

Employee was injured when their left leg was struck by 6' overcast steps while in the process of assisting another employee in moving the steps.

At approx. 8:40 am it was found that the Coal Fork elevator was not operating. Once inspected it was determined that the shaft was broken on the secondary shiv pulley.

An unplanned roof fall occurred on 10/29/24 at approximately 11:15 PM outby MRS units on the #3 section which is currently retreat mining. This fall occurred above the anchorage of primary support in the #7 entry, #15 crosscut approximately 40 feet inby spad #28161.

Employee was injured when their right leg and foot were struck by a rock that dislodged from the rib. Employee's foot was temporarily placed in a walking boot.

Employee was operating continuous miner when a rock measuring 23" long X 18" wide X 3.5" thick dislodged from the top striking EE in the left shoulder. Employee initially sought medical treatment and was released to return to work. Employee went to a follow-up appointment on 10/31/2024 where EE was placed off work for physical therapy treatment.

Employee was injured when EE's right thumb was caught between two roof bolts while installing monkey faces. 6 sutures were applied to a laceration and EE was diagnosed with a fractured right thumb.

Roof Bolter operator was injured when a glove was caught between the drill steel and the chuck of the drill pot while drilling. EE was examined by a physician and diagnosed with a fractured right thumb.

EE was evaluated for Pneumoconiosis at the Occupational Lung Center in Charleston, WV. EE was diagnosed with a 20% pulmonary functional impairment on 5/15/2024.

At approx. 3:25 pm it was found that the Coal Fork elevator was not operating. Once inspected it was determined that the bearing was busted on the head frame shiv pully.

Continuous Miner operator had just began to load in the face of #9 when a rock fell off the continuous miner striking the EE in the right leg resulting in fractures to the right fibula. Lost workdays were a result of this injury.

Employee tripped while working causing employee to fall and striking right palm of hand below the thumb causing a laceration. Employee received six sutures and returned to work.

Employee states was in the process of bending a roof bolt when it became lodged under the drill pot. While attempting to free the bolt it suddenly sprang upward striking EE in the face causing a laceration to EE's upper lip that resulted in 3 sutures.

Employee was preparing to make a belt splice on the slope belt. In the process of moving the belt cutter to cut the belt, a finger encountered the knife blade of the cutter, and cut their left middle finger. The employee finished the shift then went to the CAMC General to be examined. Employee was given 10 stitches in left middle finger by a physician.

Employee was rolling a scoop tire out of the way when EE slipped, striking EE's head on a high-line scoop insert. Employee was diagnosed with acute low back pain and a facial contusion and placed off work.

Continuous miner operator was taking the last lift out of the block when the corner of the rib fell out striking EE resulting in a dislocated right knee and fractured left ankle. Lost workdays were a result of this injury.

At approx. 9:20pm it was found that the Coal Fork elevator was not operating. Once electricians arrived they determined there was a ground that caused the contactor to go bad. At approx. 1:15am the elevator was back in full operation.

At approx. 8:45am an unplanned roof fall occurred at crosscut 85 in the belt entry on 6 west E Mains belt. This was at Spad # 23891.

On 8/28/2023 9" of rain fell in about 3 hours. This allowed water to run down the box cut into the slope area of the mine and pool up. This area of the mine is the old mine in the seam above where we currently mine at the top of the internal slope located at Dawes.

An unplanned roof fall occurred on 7/27/2023 at approximately 11:45 pm at CC 85 near spad # 23866 on 6 west track.

At 11:20 am on June 1st there was a methane ignition resulting in a mine fire that lasted 20 minutes on Unit 4 miner section (MMU 019-0).

Employee was moving a rock to set a timber when EE felt a pain in EE's right shoulder. Employee initially sought medical treatment and was released to return to work. Employee went to a follow up appointment on 09/26/23 where EE was advised that the MRI that was given on 08/16/23 showed a labral tear of the right shoulder that will require surgical repair in the future.

EE stated they were helping remove a rock from on top of the miner. They grasped the rock placing one hand on each side and pulled the rock to slide it off to the ground. In the process the rock slid down and smashed their fingers between the rock and the metal shield of the miner. EE received 3 stitches in the left index finger.

Teammate stated EE was in the process of replacing bits on the 088 miner. EE had removed the old bit and was pushing the new bit into place when it became stuck so EE took EE's pick hammer and struck the bit to force it in place. After several hits a piece of metal came back and struck EE. On 3/30/2023 EE had the metal removed and received 3 stitches.

Employee stated had just finished using the mine phone at the section tail and started to walk to the face. As employee stepped toward the feeder employee tripped over employee's feet and fell allowing left forearm to come in contact with the flag pin that holds the section tail in place. Employee received 10 stitches and was given a full release back to work.

EE was in the process of changing a boom swing pin on the 401 roof bolter. EE had removed the old pin and set it in the tray of the bolter. As EE was driving the new pin in place the old pin which weighs approx. 33 pounds fell out of the tray smashing EE's left pinky finger. EE received 3 stitches and was diagnosed with a fracture of the pinky finger.

Employee stated employee was helping another employee unload oxygen bottes on Unit 4. After they had finished unloading the injured employee stated shoulder started to hurt. On Sept 5th 2023 employee started physical therapy. Being reported per citation #9559714 under protest to avoid any further enforcement action.

EE stated: was skiving a piece of belt to put a splice on it. In the process of of cutting the belt with the Stanley utility knife, the knife slipped out of the belt and punctured EE's left hand requiring 2 stitches.

Employee stated EE had drilled EE's hole and was in the process of installing the bolt when EE felt EE's shoulder pop and start hurting. Being reported per citation #9559715 under protest to avoid any further action.

Employee stated that EE was in the process of removing rock off of the left C/M Machine on the Unit 2 Section. EE stated that EE was going to attempt to push a rock off the machine so they could move it. In the process of EE pushing the rock off the C/M machine it broke. This caused EE's hand to slip allowing EE's arm to strike the sharp edge of the rock. EE received 6 stitches.

EE stated that EE was in the process of squaring on the end on the belt to make a belt splice during a Pull Back. EE stated that the belt was laying on a board, that was laying across the belt rails and that while EE was cutting from the middle of the belt towards self that the knife slipped, and EE cut self across the right thigh. The employee received 5 stitches

Employee stated employee was carrying a rub rail from the end of the track to the section when employee tripped over a track rail and fell to the ground striking employee's back. Being reported per citation #9559716 under protest to avoid any further enforcement action.

Employee stated employee was operating the miner in #7 entry on Unit 3 when a piece of rock fell from the mine roof striking employee on right shoulder. The rock measured 21"x21"x2-9" thick. Being reported per citation #9559717 under protest to avoid any further enforcement action.

Employee stated EE was putting hyd. oil in the miner on Unit 3. In the process of moving the can of oil from EE's right side to EE's left EE felt pain in EE's groin. Being reported per citation #9559718 under protest to avoid any further enforcement action.

Employee stated that EE was traveling with an inspector and they were going through a mandoor. EE was holding the door for them to go through and after the last went through EE turned to go through. EE's hand slipped off of the handle and the door slipped out of EE's hand causing the door to smash EE's right thumb. Employee received 4 stitches.

Employee stated EE was changing out a piece of 60" top structure on surface #2 belt. EE stated EE was pulling an old piece out of the belt line when it slipped out of EE's hand allowing the structure to smash EE's hand between the rail and the structure. (On 8/25/2022 employee was diagnosed with a broken 5th digit "pinky" finger.)

An unplanned roof fall occurred on 8/19/22 at approximately 20:05 hrs. The fall occurred at crosscut 56 of the slope belt line. No persons were injured as a result of this fall.

Employee stated EE had hurt EE's back several times in the past and on EE's way to the section EE's back started to hurt again. Being reported per citation #9559719 under protest to avoid any further enforcement action.

The Coal Fork Main mine hoist was down from 9:30am until 11am on August 2nd due to a contactor in the hoist house going bad. The contactor was replaced and the hoist is working as of 11am on August 2nd.

Employee stated they had drilled a hole and was in the process of installing a 6' bolt when a piece of rock fell that measured 2" x 18" wide x 2" thick striking them on the back. Employee missed on 07/22/2023 to seek medical observation. Being reported per citation #9559720 under protest to avoid any further action.

Employee stated the were getting out of a scoop on Unit 4. When they put their weight on their left ankle, it twisted to the side and they felt pain. Being reported per citation #9559721 under protest to avoid any further enforcement action

Employee stated they were picking up trash in the scoop road on Unit 1. In the process of picking up a wooden pallet and throwing it in the scoop bucket, they felt pain in their groin. Being reported per citation #9559722 under protest to avoid any further enforcement action.

Employee stated EE was throwing a track switch for the mantrip to go through. As EE got back in the mantrip EE hit EE's knee on the side of the mantrip and felt it pop. Medical repair of EE's meniscus. Being reported per citation #9559723 under protest to avoid any further enforcement action.

Employee stated employee was operating the #1 Emu coming off of the #3 section traveling toward the Coal Fork Bottom to finish shift. Employee stated employee was not paying attention and drove into the cross under guard at the #1A belt line. This is where the track and belt cross. Employee received 6 stitches in face and returned to work.

An ex employee was awarded 15% pneumoconiosis on February 17th 2022. Employee retired on 06/01/2018.

Employee stated was in the process of adding 4" aqua waterline in prep for a belt move. Employee picked up a crib block and used it to strike the end of the waterline and after several hits employee caught a finger between the crib and the waterline. On December 7th employee had the end of the finger removed due to nothing left to protect the bone.

Employee was changing a rotation hose on the 419 roof bolter and the channel locks employee was using slipped off the fitting causing employee's right palm to come in contact with a metal tag on the bolt tray. This caused a 5 inch cut that required 5 stitches.

Employee stated that they were lifting a full bottle of oxygen to place in the holder on the mantrip when they felt something pop in shoulder. Employee was released and worked regular schedule. On 2/22/2022 employee had rotator cuff surgery to repair a tear and was released back to work.

Employee was bolting and wearing knee pads and states that when EE dropped down on knees, EE felt something pop in the right knee.

Employee stated IE and another worker were in the process of replacing the rub rail on the 6927 cm. They set the rub rail on the bracket and was in the process of pushing it on in. IE had finger caught between the rub rail and the frame on the miner. IE received stitches on 8/3/2021 and a skin graft.

Former employee filed pneumoconiosis on 6/1/2021 and on 6/6/2023 was evaluated at the Occupational Lung center in Charleston WV. EE was awarded 10% permanent partial disability on 7/27/2023.

The Employee stated that they were bolting on 416 Fletcher walkthrough R/B, in 6 right on Unit 3, they had set an inside bolt and was swinging out to set to outside bolt, when a piece of the top fell and struck them on the left shoulder, left forearm, and left side of the face.

A roof fall was discovered on 17 rt panel between break 23 & 24. inby spad 21112.

A roof fall was discovered on the active 2-Section between the # 3 and # 5 entries at cross cut 17 just inby spad #24556. The length of the fall is approximately 70' long x 20' wide x 6' thick.

The employee stated: was in the process of installing the last gate shield when a piece of cap coal fell between the shields from the top and struck the employee in the top of the head. The employee was taken to a hospital, where employee was taken off work.

The employee stated the employee was in the process of installing the last gate shield when a piece of cap coal fell between the shields from the top and struck the employee in the right shoulder at the base of the neck. The employee was taken to a hospital, where the employee was diagnosed with a closed C-7 fractured vertebra.

On October 30, 2020, we received reports that an employee was diagnosed with occupational Pneumoconiosis.

On 8/31/2020 we received reports that the employee was diagnosed with occupational pneumoconiosis.

On August 6th 2020 we received reports that employee was diagnosed with occupational pneumoconiosis.

The employee was in the three right crosscut roof bolting when a piece of rock fell striking the employee in the right arm, causing a laceration. The employee sought medical attention at CAMC where is received 28 sutures. The employee return to work the next scheduled shift. This incident if being reported as medical only.

I was notified on 4/20/2020 the employee was diagnosed with black lung.

Employee was awarded 10% pneumoconiosis on July 12, 2021.

The employee was diagnosed with pneumoconiosis.

Employee stated that EE was using a sledgehammer to knock a stopping when the injury occurred, Employee was knocking a stopping and the employee was "choking up" on the hammer handle while swinging into a block stopping causing EE's finger to be caught between the stopping and the hammer handle causing a laceration below the knuckle on employees left index finger

Employee and 2 others were in the process of recovering belt structure. While breaking down the belt structure, they had to remove 6" aluminum water line that was hanging from the mine roof with belt chains. As employee and the other 2 employees was lifting the water line up to remove the chain from 3rd joint they were letting it down it fell and struck employee on the left ankle.

Employee stated that EE was using the canopy of the longwall shield to lift a relay bar, during the procedure the tip of the shield came in contact with a built set of cribs the tip of the canopy slipped off the cribs and came downward in a spring-type motion which caused the employee to be struck by the bottom side of the shield canopy and the top of the pan line.

Employee was cutting belt strings off the bottom rollers on the Surface #2 belt with an OLFA Brand Knife when the knife came in contact with EE's left thumb causing a Laceration that required 3 sutures.

The injured employee and another EE were in the process of re-banding a pallet of 30" crib block. While doing this, there was another empty pallet on top of the crib blocks, the employee attempted to pull the empty pallet off of the crib blocks and EE's left foot slipped and caused the empty pallet to fall, striking the top of EE's right foot.

Employee had walked outby in the track entry from the section to get miner bits that were laying beside the track. As EE returned walking inby to the section EE stepped on a wet rock causing EE to stumble and strike right foot on a rock which resulted in pain to EE's big toe.

EE stated was in the process of assisting with a belt move, EE further stated was attempting to pick up a 60 inch top roller. While attempting to pick up the roller, EE slipped and fell causing right knee to land on a piece of belt structure that was laying on the ground.

The employee stated EE was approaching the bottom of the elevator after inspecting 6-west A belt and track and started to smell smoke. The employee later sought medical treatment and was given a full release. On 10/2/2019 employee sought medical treatment again and was taken off work; this incident is being reported as an LTA.

At approximately 2:10 pm, a fire was discovered on a Polaris EMU that was located at the coal fork elevator bottom area that was not extinguished within 5 minutes.

Employee was installing bolt and got EE's finger smashed in-between the bolt and drill chuck causing injury

The employee was shoveling at six surface tail when EE twisted wrong and felt "pop" in EE's stomach. The employee was taken to CAMC, where EE was diagnosed with a strain to the abdomen. The employee was taken off work. This incident is being reported as an LTA.

The employee was operating a man trip (tub) from the mouth of 2 section to the end of the track when a rock fell from the mine roof, landing on the canopy of the tub. When the rock landed, it broke causing a piece of the rock to come into the operators compartment striking the employee. in the face.

The employee stated EE was mounting a D-9 Dozer when EE's right knee made a popping sound. The employee sought medical attention that day and was given a release. On 7/9/2019 employee sought medical care again and was taken off work.

Employee stated they were in the process of setting bits on the shear when a piece of roof rock swung in striking the employee on the right ankle. The employee sought medical attention was and was referred to a specialist. On July 2nd the employee was diagnosed with a fractured ankle. This incident is being reported at medical only.

The employee stated was in the process of helping build a crib on the longwall pull out when a piece of rock fell, striking the employee on the back and neck area. The employee sought medical attention and was diagnosed with a hairline fracture of the C-1. This incident is being reported as medical due to the hairline fracture.

EE stated EE was in the process of climbing off a scaffold when EE's foot slipped, causing EE to strike the ground. On 10/08/2019, the employee was taken off work. This incident is being reported as an LTA.

The employee stated was in the process of cutting a piece of a lifeline when the knife slipped, causing a small laceration to EE's right index finger. The employee sought medical treatment and received five sutures. This incident is being reported as Medical only.

The employee stated was laying track when the rail slipped off the jack smashing left hand. The employee sought medical treatment at a hospital where employee received three sutures to middle finger and four in ring finger and diagnosed with a fracture of the tip of the finger. This incident is being reported as medical only.

The employee stated was leveling some belt structure when a piece of rock fell striking the employee on the left side of EE's face above the eyebrow, causing a laceration. The employee sought medical treatment and received fourteen sutures. The employee sought medical treatment the following day and was taken off work.

A roof fall was discovered in the bleeders of the old 17 headgate at break 92 to 70 ft outby 94break in the # 3 entry. Size of the fall was 18' wide x 7' thick x 140' long. The area has been dangered off.

The employee stated EE and three other EE's were trying to raise the lid on a tailpiece at break 4 of the longwall belt when it shifted. The employee stated as they were letting it down EE's left middle finger got caught in hinge point, amputating the tip of the finger.

Employee was firebossing the 3 south belt. Went to step over a twenty-four inch piece of poly pipe that was twenty-four inches off the mine floor. Foot slipped in mud causing them to strike the other foot against a 6 inch crib block, resulting in a fracture to a metatarsal bone.

On April 16, 2019, we were notified employee was diagnosed with black lung.

Employee was helping to carry an oxygen tank and felt pain in their back. Employee continued to work until they were put off work by a physician on April 8, 2019.

The employee was diagnosed with pneumoconiosis.

The employee stated EE was in the process of cutting some dust hole when EE's knife EE was using slipped out causing a laceration to EE's arm. The employee sought medical treatment where EE received four sutures. This incident is being reported as Medical only.

The employee stated EE was placing # 728 mantrip back on the rail at break 99 when EE's pinky and ring finger got pinched between the end of the jack bar and the coal rib causing a laceration. The employee sought medical attention where EE received seven sutures. This incident is being reported at medical only.

The employee stated ee was in the process of trying to hang some miner in the number three entry when ee felt a burning sensation in the middle of back. The employee sought medical attention on 3/21/2019 and was taken off work.

Employee stated that EE was standing on top of the tailpiece hanging strings to set the tail piece. As EE leaned over to tie the string EE grabbed a monkey face to brace self. When EE grabbed the monkey face it spun causing EE to stumble back and EE's foot became caught in between the frame of the tail piece injuring EE's right ankle.

TRACTOR TRAILER TRYING TO TURN AROUND AT THE GATE AND SCRAPPED AGAINST THE GATE POLE AND BENT THE POLE THEREFORE BREAKING THE POLE RESULTING IN GUARD BEING HIT IN THE HEAD

Employee stated that while trying to carry a top structure cradle out of the belt entry on the longwall ee felt a pain in back.

Employee was struck in the right foot by a piece of a kettle bottom while trying to install a roof bolt to support it.

Employee is receiving black lung benefits.

Employee was in the process of removing debris off of the head drive on the long wall. When EE turned to walk away a piece of rock fell from the mine roof striking EE in the back of the legs.

Employee was attempting to throw/move a piece of continuous miner cable out of the roadway. While moving the cable EE felt a pain in EE's back.

Employee was riding in a rubber tire mantrip. While traveling under a cross under the employee struck their head on the hanging cross under guard.

Employee was loosening bolts in the continuous miner gear case. While pulling on ratchet, the ratchet slipped striking ee in the forehead causing a laceration.

Employee stated EE was helping remove 60" belt structure from the longwall belt line. As EE was pulling a piece of top structure the employee's feet slipped causing EE to fall backwards. The structure fell on EE's feet twisting EE's left ankle.

Employee was walking through a crosscut when ee stepped in a rut causing to roll ankle.

Employee was holding 2 x 4's. Another employee was using sledge hammer and hit first employee on left hand. Injuring index finger.

Employee was walking past the power center and while walking employee stepped on a ledge causing employee to roll left ankle.

Employee was in the process of installing a bottom roller on the parallel C-Belt. While lifting the roller EE felt a pop in the right shoulder. Employee was referred to a specialist on 10/19/2018 for an MRI and was placed off work.

Employee was in the process of washing a shield hauler with a 1 1/2" fire hose and nozzle. In the process of spraying the hauler, the hose busted just below the nozzle causing the hose and nozzle to fly back rapidly striking the employee in the face causing lacerations and fractures.

Employee stated that they noticed a rock hung between the bottom belt and the belt structure, they began striking the rock with a two foot bolt wrench, while striking the rock, the roller grabbed the wrench jerking it out of their hand causing their hand to slam off the belt structure.

Employee was in the process of watering the batteries on the belt maintenance duckbill mantrip. When they leaned over the batteries to remove the cell caps, the channel locks that they had on their belt came in contact with the frame of the batteries and the positive terminal post causing an electrical arc and the employee experienced flash burns to the face and eyes.

Employee stated that they and the rest of the maintenance crew were setting at the service scoop eating, when a piece of draw rock fell from the mine roof striking EE in the left forearm causing a laceration.

The American Eagle Mine experienced an unplanned roof fall above the anchorage zone in between 76 and 77 breaks on the old 2-South mains in the #7 entry. This is not an escape way and does not alter ventilation. The dimensions are approximately 50' long 18' wide and approx. 8' thick. No injuries occurred during fall.

Employee stated that EE was walking back from the section cache box. While walking back EE stepped on a small rock that had fell from the mine rib. EE felt a sharp pain as if something had hit EE in the calf. After scheduling an MRI in 6/26/2018 it was found that EE had a partial tendon tear and was placed off work.

Employee was in the process of hanging a gob curtain on the head side of the longwall face. While doing so, a rock brow rolled from the rib striking EE in the lower extremities causing a fracture to EE's right ankle.

Employee was assisting in spot bolting a rehab area. In the process of looking for a bolt to spot EE was looking at the mine roof when a piece of draw rock fell from the mine roof striking EE in the upper lip causing a laceration.

Employee stated that EE was assisting another employee carry a piece of 6in aluminum water line to install it. while sliding the water line onto the structure it got stuck, when the other employee pushed it, it came loose smashing EE's hand in between the two joints of water line causing a laceration to the middle finger.

Employee stated that ee was installing a rib bolt on the unit 1 section. While doing so, ee leaned over drill mast to pull drill steel out of the rib. While pulling the steel out ee felt a pop in shoulder and a tingling in arm.

Employee stated was bolting in the 2-left face, ee looked down to adjust drill pod, when ee looked back up at the mine roof to see if adjustments were correct a piece of draw rock fell from the mine roof striking the back side of the ATRS and hitting ee in the face causing a small laceration to the upper lip.

Employee stated that they were working on the parallel A belt motor reducer. EE was holding a wrench in place while another employee tightened the bolts. The wrench slipped off the bolt and smashed EE's right ring finger between the wrench and the frame of the belt head.

Employee stated was assisting other employees to carry a set of overcast steps. While carrying the steps EE was looking down so EE didn't trip over anything. While looking down EE struck head on a roof bolt sticking down from the mine roof causing EE to jar head and neck. This employee finally got in with a specialist on 5/16/18 and was placed off work.

Employee stated that EE was helping the bolt crew tram up a ramp being used to cut a belt channel. EE reached to pull the cable out of the way so they could back up. While doing so EE slipped on a pile of rocks on the incline and put left hand out to catch self when hand came in contact with a jagged pizza pan on a rib bolt causing a laceration.

Employee stated that ee was walking up the intake on the Parallel Mains when ee stepped on a small ledge with left foot causing left ankle to roll. This resulted in a minor fracture in the ankle. This accident is late getting reported due to the employee not being able to see the specialist until 4/24/2018. After the visit the MSHA site was down so it could not be submitted.

While lifting a top belt roller, employee felt pain in lower back.

Employee was in the process of changing a tie rod end on a shuttle car. The rod end was seized so EE put right arm against frame of shuttle car to increase force on wrench. While doing so EE felt a pop and instant pain in right shoulder. This employee returned to work the next day and recently went back to doctor and was placed off work for surgery as of 5/18/2018.

Employee stated that ee was throwing the track switch located on coal fork bottom so another ee could spur up a man trip. Employee stated that ee stepped over the switch and as ee threw the handle it flew out of hand striking left knee causing a bruise.

Employee was attempting to hang a light on the #70 long wall shield using zip ties. While doing so EE was attempting to cut off the excess zip tie with a razor knife and slipped and cut the left hand. Employee received stitches to close laceration.

Employee was assisting another employee getting block out of the return to build a stopping. When employee needed to use the bathroom they stepped into the crosscut and leaned against some block that were plastered to the rib. While doing so the blocks came loose and fell striking them in the lower back.

Employee stated was attempting to untangle miner water line. While doing so the water line came in contact with a wooden track tie leaning against the mine rib knocking it over striking the employee in the hard hat and forehead area.

Employee stated was on knees beside the mule washing the cats out when a piece of rock measuring 32"x44" and 1-4" thick fell from the mine roof striking employee in the left arm and shoulder.

Employee stated that they were taking apart the frame for the storage unit on 20HG. The hauler winch was hooked to one end so they had to pick it up and crib it to unhook the winch rope, while unhooking the rope the coal under the far end of the frame crushed out causing the frame to creel on the crib striking the employee on the right foot.

Employee was fire bossing 6-West B-Mains belt. Employee noticed a bottom roller down and needed to adjust the waterline to gain enough clearance to install a new roller hanger. The water line came loose pinning the right hand between the hanger and the belt structure.

Employee was at 174 brk on parallel C-Belt trying to train the bottom belt, while doing so employee slipped on the wet bottom causing employee to fall into the belt structure. Employee's right arm became caught between the side can of the top structure and the top belt. Spraining employee's right arm.

Employee stated that employee was cutting up rock with the shear. While doing so, a piece of rock flipped up on top of the pan line. Employee shut off the shear to shove the rock into the chain and while doing so a sharp edge of the rock struck employee's right forearm causing an abrasion/laceration.

American Eagle Mine experienced an unplanned roof fall above the anchorage zone of the primary support at 14 brk in the number 3 entry (spad #22526)on the unit 4 section. The dimensions are 80' long x 20' wide and approximately 8' thick. No injuries are reported due to the fall.

Employee stated that ee was making faces. EE was in the #3 entry face dating up. As ee was walking out a piece of rock fell from the mine roof striking ee in back and head.

Employee stated that ee was attempting to cut a flapper off of the mother drive belt. While doing so, knife slipped causing ee to cut left thumb.

American Eagle Mine experienced an unplanned roof fall above the anchorage point of the primary support at 61 brk in the #3 entry on the old 2-South Mains track entry. The dimensions are approximately 20' long x 20' wide. All approaches have been breakered off and no escape ways were affected.

Employee stated had just got done helping change batteries on the section scoop. After doing so, as employee was walking back towards the face employee stepped in a hole and twisted right ankle.

Employee stated that ee was standing beside the service scoop looking for something. While doing so, another employee struck a bit lug on the miner with a hammer causing a piece of metal to chip off hitting ee in the right forearm.

American Eagle Mine experienced a roof fall in the primary escape way for the unit 3, unit 4, and the long wall. The fall occurred in between 78 break(spad#12559) and 79 break(spad#12568)in the #6 entry on the old 2-South mains.

Employee stated that they were attempting to pull bolter slack off of the reel. While doing so the bolter moved forward pinching their left index finger between the cable and the machine.

Employee was picking up a prop setter post and felt a sharp pain in lower back.

Employee stated that employee was prying on a hose staple with a slate bar and while doing so the bar slipped striking employee in the forehead causing a laceration over employee's right eye.

Employee stated that they were loadings pans onto a flat car with a shield hauler. The pan was hanging over on the offside of the car. Employee stepped forward to hook up the winch rope, while doing so their leg was caught between the hauler fork and the flat car. Accident happened on 9/17/2017 but employee did not see specialist until 9/27/2017 to deem the accident reportable.

Employee stated walked to the face to see how much was left in the cut. While doing so employee noticed a bad rib and notified miner man to move. Employee put employee's hand on rib to see if it was loose and upon making contact the rock cut trough employee's glove causing a laceration in employee's hand requiring 6 stitches.

The elevator at the dawes portal began having mechanical issues and was briefly taken out of service at 6:55 pm for repairs.

Tripped while carrying a belt cleaner in poor footing conditions at a conveyor transition area. The object struck the right hand small finger breaking the finger at the tip.

An unplanned roof fall that was above the anchorage zone of the primary support has occurred in the #5 entry at 26 break on the 1-North D panel. The dimensions are 20' long by 90' wide and estimated to be 8' high.

Employee stated that while EE was attempting to drag a piece of monorail with a rope hanger, the hanger slipped off causing EE to fall and injure right arm.

Employee stated: was attempting to hang a piece of curtain in the #8 entry, while doing so EE slipped on a rock causing EE to fall into the rib injuring right shoulder.

Employee was operating #550 scoop traveling outby in the #3 entry in route to get supplies, while going through a set of fly pads the employee struck the battery end of 532 scoop. Employee felt a pain in the neck but continued to go get supplies where they felt a burning sensation in their neck accompanied by pain.

Employee was installing the last row of bolts in the face, the face had gob and was low so they had to bend bolt to install it. While shoving bolt in the hole with drill pod the employee attempted to drop the drill pod and place the bolt in the chuck and the bolt popped loose catching the employees hand between the bolt and drill chuck.

Employee stated that ee was trying to pull the tail apron back on the boom of the miner with chain ratchets, while the apron was pulled back ee was attempting to pry out the retaining spring. While doing so, the bar slipped smashing right pinky finger.

An unplanned roof fall that was above anchorage zone of the primary support occurred in the #6 entry at 18 break on the #1 section. The dimensions of the fall are 30 feet long, 50 feet wide, and estimated to 8 feet high.

Employee stated that employee was cutting a piece of new belt, when the blade of employee's knife broke causing employee's right pinky finger to be cut.

Employee stated that they were packing miner bits from the #2 and #3 entries down to the end of the track on the unit #3 section. While doing so, the employee slipped in the mud causing them to twist their left knee.

Employee stated that while EE was lifting a 20 ft X 6 inch Victaulic water line up onto a mantrip, EE felt something pull in the left shoulder. EE and co-worker were the only two that were there lifting the material and picking up trash.

Employee stated that employee was installing a 16 FT. Cable Bolt in the Last Open Cross Cut of the #5 Entry. While trying to retrieve employee's drill still out of the hole it came loose falling out of the hole and striking employee on the left index finger causing a laceration. Employee was wearing proper PPE at time of the incident.

Employee stated was in the process of connecting the links of a CM chain and was reaching to pick up hammer. Employee's belt wearable SCSR caught a chain link and slid the link off of the boom and onto hand. This incident resulted in a fracture to thumb.

Employee stated that while taking down the CM Cable from a roof bolt plate, the hook sprang off of the lip of the plate and swung around striking employee in the nose. The hook ended up lacerating employee's nose, which required 11-12 stitches. Height in the area was 52-53 inches, cable was under some tension, employee was dropping cable by self.

Employee stated that EE bent a bolt against the t-bar of the roof bolter to get it up into the place so the bolt could be installed. When the employee got the bolt started up into the hole EE was going to attempt to bend it back to straighten it up. In the process of EE bending the bolt back EE felt a pain and EE's shoulder popped.

An unplanned roof fall that was above the anchorage zone of the primary roof support occurred in the number 10 entry on the number 1 section of the American eagle mine. 70Lx20Wx8H. There were no injuries associated with the fall.

Employee stated was operating the #323 Shuttle Car on the Unit 3 section. In the process of tramming shuttle car the employee ran over a rock in the roadway which caused employee to strike their head against the canopy of the shuttle car. This caused the employee to feel a pain in neck.

Employee called stating fell at work and twisted right knee

Employee was assisting another employee to remove a mandoor from a stopping. They knocked the blocks around the door out and the employee was going to attempt to pull the door out. In the process of the employee pulling the door out a cinder block fell from the above where the door was positioned and struck the employee on the hand.

Employee stated that they walked outby to get a solid piece of curtain to hang over the fly pads between the #8 - #9 entry on 1 North. Employee noticed a piece of rock that needed scaled down and went to the bolter and got a slate bar. In the process of scaling the rock some additional rock fell out and the employee slipped landing on their left arm.

Employee was walking from the head side to the tail side on the longwall face. EE noticed that there was a 2 foot piece of belt stuck between the #31 shield and #34 shield. The employee was cutting the belt in smaller pieces and moving it out of the walkway on the longwall face. In the process of pulling on a piece of belt the employee felt a pain in lower back.

Contractor was helping to move supply doors which were located on 20 HG A belt. They freed the doors and were removing the masonry block from around the doors. Several blocks were hung along the doors where the waterline and cables went through the block. In the process of taking down the doors one of the blocks fell and struck contractor on the right hand smashing a finger.

Employee was installing a high voltage plug in the back of the section power center. EE was having trouble tightening the plug all the way and EE struck the high voltage plug wrench with a hammer to assist in tightening it. EE accidentally hit hand between the thumb and pointer finger. EE received two stitches.

A contractor was operating a shuttle car on the unit 2 section. In the process of operating the s/c EE hit a ledge in the mine floor causing EE to strike the canopy of the s/c with EE's hard hat. EE filled an incident report but did not say EE was going to the doctor which refereed EE to a specialist. EE rtw on 4/20/2017 notifying the mine sight EE was off due to a strained neck.

Employee was assisting another employee to set a feeder on the unit 3 section. In the process of the employee standing helping to set the feeder the manifold blew off of the 8 inch metal water line. This caused water to hit the employee and for the employee to slip hitting the rib with left elbow. When elbow struck the rib it first struck the corner of a rib bolt plate.

Employee was assisting install a new set of air lock doors on the 20 HG longwall section. EE set the jack against the mine roof and was preparing to adjust the set screws to make the jack secure firmly to the mine roof. In the process of preparing to tighten the set screws the jack slipped and slid down smashing the employees left hand and finger.

Employee was walking through some mud and water that was deeper than ee realized. EE was carrying miner bits in one hand and lunch box in the other. EE foot slipped and ended up twisting knee.

Employee and two other employees were sitting on the side of the scoop eating lunch. After the employees got finished eating lunch they got up and started walking so they could go back to work. They said they heard a pop and a rock fell. When the rock fell it struck the injured employee on the shoulder and the foot. The rock measured 36" long X 12" wide X 2 1/2" thick.

Employee went to portable potty and while stepping out of it, employee said they missed their step and fell on the pavement. Employee said they hit their left elbow on pavement and the back of their hand was swollen.

Employee was attempting to tram the #753 tub out of the track spur when the tub derailed. EE jacked the tub up until it was above the ball of the track rail. When EE got it high enough EE was going to attempt to shove it off of the jack to make it land back on the rail. In the process of shoving the tub over it landed the wrong way and landed on the employee's foot.

Installing wheel onto machine when the wheel got away from them. A slate bar that they were using flew up and struck employee on right shoulder and then right eye.

Employee was trying to lower #205 shield and prepare for transport. The Shield would not lower ee opened the relief valve on the back of the manifold to release the emulsion oil. At this point the shield still did not lower but ee thought all of the pressure was off. EE opened the staple lock to the slave line and it still had pressure resulted in it striking arm.

At Approximately 12:55pm the substation that supplies power to the Coal Fork Portal caught on fire. The cause was unknown.

Employee was helping direct the shield hauler operator. They were moving the head drive cover frame from the head drive. The Shield Hauler Operator set it down and released the slack out to unhook the cable from the winch. The spotter went to unhook the chain and the frame slid over striking foot.

Employees moved the roof bolter outby to replace a damaged roof bolt then hung the CM cable to clean with the scoop. After returning the RB to the face, they realized that something had roofed the CM cable in the top. While walking over to see if it needed to be repaired it blew up and burnt the employees face.

Employee was riding in a diesel mantrip as a passenger, when the trip struck a hanging 6 inch valve and airline. The 6 inch airline entered under the canopy roof and inside the canopy post and pinned the employee against the mantrip frame and breaking employee's left leg Fibula. Apparently the airline was dislodged from the roof from a previous trip ahead of the mantrip.

Employee was trying to take a shield off the reel box on the #322 shuttle car on the 1 North Section. Employee was using a 3/4" inch wrench to try to break the bolt loose. EE was pulling the wrench toward self when this incident occurred. In the process of pulling on the wrench it slipped off causing EE to fall on the ground landing on right hand.

Employee was attempting to change miner bits out on the #219 C/M Machine on the 1 North Section. The bit that the employee was trying to remove was stuck. In the process of striking the bit with a hammer the employee missed the bit and struck self in the hand with a hammer.

An unplanned roof fall has occurred in the #6 entry on the MMU 012-0 1 North Section between 25-26 breaks. Fall measures approximately 18' wide X 60' Long X 6' High. 2 MRS's are located inby the fall. This is a pillar section.

Contractor was helping another employee set up the feeder. EE walked over to the other side to build a crib under the feeder. The employee told the other to raise up & then lower the cats on the feeder. In the process of the employee reaching to move a wedge from the feeder, the employee's thumb became caught between the half header & frame of the feeder.

The employee was in the process of jacking up a mantrip back onto track. While using a regular lifting jack during the process of jacking up the mantrip the jack kicked out causing the jack bar to strike the employee in the facial area.

Employee was watching the C/M machine cable as the C/M machine was advancing to another entry on the 17 HG section. EE had EE's back turned away from the roof bolter cable which was laying on the ground. When the roof bolter operator engaged the cable reel on the roof bolting machine the cable struck the employee on the back of EE's feet and caused EE to fall on EE's back.

Injured employee was in the process of moving bolter cable when ee was shocked by it while handling it in mud. Employee sought medical treatment and was given a full release.

The Dawes elevator failed to operate at 2:45 PM on 09/09/2016 due to a buffer switch being grounded. They got the issue fixed at 4:55 PM and it started working again. There were no employees on the elevator when this incident occurred.

An unplanned roof fall that impedes travel of the track / secondary escape way occurred at 25 crosscut on the 4 South track. The width of the fall was 45 feet wide. The length of the fall was 20 feet long. The thickest point of the fall was 2 1/2 feet thick.

Employee was assisting in putting a metal guard over a hydraulic jack on the take up on the 1 North B Belt Drive. While sliding the guard into place it moved unexpectedly and slid off the frame and caught the injured employees hand between the guard and the frame. This caused lacerations to both middle and ring fingers on EE's left hand.

Employee was operating the #419 roof bolting machine on the 1 North Section. EE was bolting in the #5 left entry. In the process of EE starting to drill a hole in the top to install a roof bolt, a piece of rock that measured 26"X10"X8" fell from the mine roof and struck the injured employee in the right hand.

An unplanned roof fall occurred outby the face of #6 entry on Section 2, just inby survey spad #21009. The fall measured approximately: 45 to 50 ft in length, 16 ft width, and 6 to 8 ft in height.
