EE was working underneath the cone discharge, completing housekeeping activities where EE wrenched or tweaked EE's back causing injury.

Two employees were loading a screen into the back of a service truck when one got EE's finger between the screen and the tailgate causing a crushing type injury

Employee was clearing rocks from the dump site going to the primary plant. Was struggling with loading a rock, so EE picked the rock up and carried it over to the dump. Rock slipped from the employee's grip landing on EE's right foot.

Employees were trying to lower the gates inside the rolling rock box. To lower the gate, 15 bolts needed to be removed. When the last bolt was removed, the gate slipped and landed on the foot of one of the employees.

Was going to help another employee lift an oxygen bottle into the back of a service truck and felt a pull in EE's abdomen. Reported to supervisor and denied observation. Stated EE would let them know if the condition worsens. Came to supervisor at a later date and requested to be seen by the doctor. Doctor placed on restrictions of not lifting more than 25 pounds for 7days.

Cable and hook was wedged between two rollers. Miner pulled the cable out and the hook struck EE in the mouth resulting in a chipped tooth.

While working on conveyor belt maintenance, IP was using a clamp that slipped off and cut EE's right thumb resulting in two stitches.

Operator was using a hose off the water truck to wash down the tracks. When they pulled on hose to move to a different position, they felt a pop in their right shoulder.

Driller was climbing into the cab of the drill, which was parked at a slight angle, when the door swung shut pinching a fingers on left hand causing a laceration.

While shoveling by the mainline 2, the miner noticed a pain on EE's right side, 3 inches below EE's armpit.

Operator when entering a loader, pinched finger between the door and the door frame. Operator refused treatment on first day. Went in for treatment on 5/2/2023. Diagnosed with fracture of the fingertip.

Employee was moving rock around and out of a 7' standard crusher after it had plugged. Rock shifted and pinched finger between rock and metal cone.

Employee was removing chute covers from Ode wash plant when the cover rotated and struck their forearm.

Troubleshooting a hydraulic clamping system on a Symons 5-1/2 Cone Crusher. The pump had been run out of oil due to a leak. The pump was off, at this time they removed three lines inside of the unit to release air. The lines were not properly installed, resulting in bypassing of the relief valves. This resulted in lacerations to EE's right hand.

Shoveling the Tunnel and was shoveling material back onto the belt when felt a pop in their back.

While unloading acetylene tank from truck the tank slipped and fell on EE's finger causing a fracture.

It was a flat area near our roadbase production area where we were moving a conveyor head section.

EE was unhooking chains attached to an item that had been picked and set in place by the crane. As EE was pulling the chains, EE said EE felt a strain in EE's back. EE's back was sore and stayed home to rest for two days following the incident before going to the clinic.

Employee was lifting a section of pipe, employee was lifting with another employee when back started to hurt. Also moved some 5 gal buckets which made it worse.

EE was going up the stairway and EE scraped finger, on the top behind the fingernail, but did not think it was serious and did not report the injury at that time.

Lifting a turbo using an A frame hoist, while picking up the shut felt a pain in EE's arm.

Employee was lifting a railroad tie to hold down a tarp covering a crusher parts, when letting it down it smacked a finger between the railroad tie and crusher.

Was shoveling in the tunnel and sprained their back when twisted to put material onto conveyor.

Employee was picking rock off of the rip-rap pile when a rock higher up rolled down and hit their pinky finger.

Employee was moving a pallet to take inside the saw shed when the employee heard shoulder pop.

Adjusting nut on crusher tramp release cylinder bolts. Trying to loosen net and felt a pain in arm.

A tube was being fed through the catwalk with a loader, with two employees guiding it on the catwalk. While lifting the tube on shoulder, one employee felt pain in shoulder. Upon investigation, the employee felt employee put too much weight on one shoulder and was not lifting properly.

EE was standing behind the employee trying to remove a worn tooth on the loader bucket. A small piece of the tooth came off hitting EE in the left cheek area below the safety glasses.

Two employees were lifting a piece of garbage metal approximately 80LBS off a service truck while doing this employee felt a pulled a muscle in stomach which then became a hernia

Putting all screens in on the bottom deck of the wash plant in a small area and felt a pain in right shoulder

Repairing a screen and cut right thumb which ee needed stitches.

EE and co-worker were working on the tail pulley shaft for the mainline stacker. They had taken a come along and attached it to the shaft of the tail pulley and pulled the shaft backwards. When EE went to remove the bearing from the shaft the bearing turned and crushed EE's finger in between the bearing and the tail pulley adjuster.

Employee was carrying a piece of steel 4ftx2ftx3/8 thick up a flight of stairs. EE turned on the landing of the stairs and felt like ee may have pulled a muscle. EE wasn't too concerned about it at that time. EE texted the next day that EE couldn't put any pressure on leg.

Moving 4' cylinder and smashed finger between shelf and cylinder.

A flat rock approx 6" wide and 20" long bridged across the step box end. The employee attempted to dislodge the rocks. EE's left hand was struck by a rock from up above in the chute. The natural reaction was to pull EE's and back which caused the fall. There was no property damage.

As EE was putting on harness below the cat walk of the McCloskey R155. Co-worker was climbing into the hopper from the cat walk, to shovel. Co-worker threw the shovel towards the hopper and missed. When co-worker missed, the shovel fell back down and hit EE on the brim of hard hat and glanced off of neck.

Repair work was being done to an HP400 Crusher. EE was working inside crusher cavity and climbed out to help work on the top side. While climbing out EE felt a pull/prick on left side of EE's lower back and leg

Doing mainframe liner change hit left knee on cateye when moving to climb out of crusher

Employee was setting a stone on a feeder conveyor of the splitter and pinched his finger tip between the stone and the conveyor frame.

Removing urethane screen panel from Hewitt Robins screen, twisted his lower back in a way that caused pain

Employee was lifting a 2 gallon jug of oil from the back of pick up and felt a pop in his elbow.

Employee was picking mud and tried to grab a larger piece and said he heard and felt a pop in his shoulder.

The employee was removing a u-clamp from a vehicle when his finger was pinched between impact and jack stand. He was taken to a clinic and had two stitches placed in his finger.

The employee was taking a return roller off a conveyer. He lifted it and thought he had control of the roller. But then he felt a pop and a sharp pain in his left forearm. Diagnosed as soft tissue injury of forearm.
