Laceration to (R) middle finger while unloading hydro bags. Gloves were not worn.
Miner was taking hose off of exhausted hydro bag, when second water bag fell and rolled out of cut, causing injury to top of left hand. Cut and tendon damage, and some broken bones.
Employee inflating airbag to move wall and natural seam in rock shifted rock causing a tear in bag and air released through tear blowing dust/dirt in employees eyes.
Employee was assisting with the move of a 2.5" X 4' steel pipe when it fell and struck him on the right shoulder.
He was setting up wire saw when a wall was being pulled down and a pc came loose once wall fell and struck him in the elbow
Employee was pulling on hose and felt a strain in his back.
Employee was pulling on a diamond wire to free it after it had become frozen over night. During the process felt pain in lower abdomen. Visit to Doctor confirmed that it was a hernia.
Laceration to the left hand 5th digit (pinky finger). Employee was plastering around granite block for placement on wire saw. Employee's pinky finger hit edge of granite rock, and received a cut. First aid was performed at office, and employee then went to clinic to see if he needed stitches. Stitches were placed, and employee returned to work.
Employee was unplugging a cord and sprained his wrist.
Employee was helping lift an electrical cable over pins on a wire saw, and felt a pull in his groin. He finished his on Friday May 3rd, came in Monday May 6th, and decided his groin hurt more, so we sent him for medical attention. It was determined he had a groin strain.
Employee was wearing safety glasses when a piece of rust/dirt fell under the top of the glasses and went under his top eyelid. After attempting to clear it out with eyewash solution, was unable to. We sent him to Urgent Care Outpatient Facility. They numbed his eye, and removed the foreign material without incident. Employee returned to work, with no restrictions.
As employee was moving a centralizer for a drill with a chain using an overhead crane when he grabbed the chain to steady the load and the chain turned.
Employee was lining up an I-beam when it slipped and hit his finger.
As the employee was walking away from a granite block he was splitting it opened up and a piece struck his left foot.
Loading drill steel he felt pain in his right shoulder.
He was trying to whip a power cord off a pin installed in quarry wall.
Aggravation to feet from previous incident, 9/6/11 dr note to restrict to sit work, worked 9/7/11 sitting, Family dr appt 9/8-12/11. returned 9/13/11 and no sit work available. Out till dr releases.
5 employees were setting a staging - when the staging was set down, one employees foot was underneath the edge, causing a crush type injury to the foot.
He was putting a jackhammer into a parts washer. Back strain.
EE told his Quarry Manager that he felt he had strained his back the previous Friday, June 11th, while pulling 2inch electrical cable. The quarry manager immediately set up an appointment to go to a care center to be evaluated at that time. He had a no loss of work injury, according to documents from the Care Plex, occupational Health Facility, in PA.
Employee pulling on wire for wire saw & felt a pain in lower abdomen.
Reached between two pieces of stone to retrieve steel plate and one stone shifted pinching finger.
Employee was moving a steel cable.
As employee was removing plug wedges from stone, the stone opened up and the employee struck his forearm on the edge of the stone, causing a laceration which required sutures to close.
Employee reached out to pull fall block towards him and block was still lowering, which pulled left hand into fall block sheave, causing amputation to four fingers on left hand. Fingers were to badly damaged to be reattached.
While putting a drill rod in the shank - the shank was turning and the rod caught his hand and twisted it causing ligament strain to wrist, elbow and shoulder.
He was sitting on rock, drilling a dog hole, & when he got downoff the rock, one foot went between 2 rocks & when he turned, he twisted right knee. Taking meds for inflamation.
A piece of granite rolled onto employee's foot and struck his instep when a ladder was moved causing fractures to four metatarsels.
While pulling a suction hose out of the water, the hose twisted and his hand caught in the check valve. He felt a "pop" in his left hand at that time. He did not go to the doctor until 10/02/2006. Inflamed tendons in the palm of the left hand.
Setting up a line to break with shims and wedges. The shim had a sharp edge and cut employee's hand as he set it in drill hole.

Employee was lifting a large hydraulic jack when he felt pain in his lower back.
Employee was shoveling snow to clear work area then he felt pain in his arm later.
EE WAS TRYING TO MOVE A DIAMOND WIRE SAW CONTROL BOX THAT WAS FROZEN IN THE GROUND. WHEN EE PULLED ON THE BOX, FELT PAIN IN BACK
Employee lifted socket on the end of a steel cable and felt pain in groin area.
EMPLOYEE FELT PAIN IN HIS BACK WHEN HE TRIED TO PULL A TRANSMISSION JACK OUT OF A STORAGE AREA.
Employee bent over to pick up pump and his knee popped. EE did not tell anyone until 9-9-04 at 8:00 am. I took him to the doctor at 1:30 pm and ee is on light duty.
Moving ladder when it slipped, striking ee on rt forehead causing a laceration that took 5 stitches.
EE WAS HOOKING CHAIN TO PAN WHEN CHAIN TIGHTENED UP AND WRAPPED AROUND HIS HAND - CAUSING IT TO BE BRUISED.

WHILE LIFTING A PIECE OF DRILL STEEL EMPLOYEE FELT PAIN IN HIS BACK.
AS ANOTHER EE PUSHED A STONE OVER A SHORT LEDGE, THE STONE CAUGHT AND ROLLED TOWARDS AN EMPLOYEE AND THE STONE STRUCK THE EMPLOYEE IN THE KNEE AREA OF HIS LEFT LEG.
EE WAS PULLING ON CRANE CABLE WHEN BOOM WAS HOISTED CATCHING HIS HAND BETWEEN CRANE CABLE AND CRANE BLOCK.
EE WAS WORKING ON LEDGE & TWISTED HIS RIGHT LEG & HIS KNEE.
HE WAS USING A PRESSURE WASHER & ON HIS OWN DECIDED TO WASH OFF HIS GLOVES. HE HELD HIS HAND AT CLOSE RANGE TO VALVE & PULLED TRIGGER TOO HARD PUNCTURING HIS FINGER THROUGH HIS GLOVE.
PICKING UP ROCK WITHOUT GLOVES & CUT FINGER.
HE WAS PICKING UP GRANITE PIECE AND CUT HIS HAND LEFT THUMB AREA.
