Operating a Eucilid R-50 Haul Truck down a haul road after dumping a load of material and was unaware that the dump body was still in the raised position. The headboard came in contact with a derrick guy wire and caused the truck to overturn.

Employee was assisting with the move of a 2.5" X 4' steel pipe when it fell and struck him on the right shoulder.

As employee was moving a centralizer for a drill with a chain using an overhead crane when he grabbed the chain to steady the load and the chain turned.

As employee was connecting a water hose he lost his footing and he fell down.

As the employee was walking away from a granite block he was splitting it opened up and a piece struck his left foot.

Employee stretched to reach a control and strained a calf muscle.

While pulling a suction hose out of the water, the hose twisted and his hand caught in the check valve. He felt a "pop" in his left hand at that time. He did not go to the doctor until 10/02/2006. Inflamed tendons in the palm of the left hand.

Employee returning from lunch break off the property twisted his ankle getting out of his vehicle at the work site.

EE WAS INFLATING A TIRE WITH AN AIRLINE WHEN THE VALVE SLIPPED OFF AND DIRT BLEW INTO HIS EYE.

THE EE WAS PRYING A PIECE OF GROUT AND FELT PAIN IN GROIN AREA.

WHILE SHARPENING DRILL BITS THE EMPLOYEE FELT A PAIN IN HIS RIGHT WRIST.

SHOVELING GROUT, WHILE DOING THIS HE FELT PAIN IN HIS WRIST.

AS INJURED EE AND ANOTHER PERSON WERE CHANGING A PUMP IN A SUMP THE INJURED EE'S BOOT GOT STUCK IN MUD AND HE FELL FORWARD. THIS CAUSED THE LEFT KNEE TO BE STRAINED.

EMPLOYEE WAS CUTTING HARDWOOD WEDGES ON A BENCH SAW. PIECE OF WOOD WAS KICKED BACK STRIKING EMPLOYEE ON NOSE.

EE LIFTED A SMALL STEEL RACK (ABOUT 30 LB.) AND TWISTED HIS BODY AS HE PUT IT ON A SHELF. HE FELT PAIN IN THE MIDDLE OF HIS BACK AFTERWARDS.

EE WAS STANIDNG IN A CORNER OF THE QUARRY ABOUT 450' FROM A BLAST. SHOT WAS COVERED WITH A MAT BUT A SMALL PIECE OF FLYROCK WENT UP AND CAME DOWN, STRIKING EE IN HARD HAT AND SAFETY GLASSES. H AT BROKE AND GLASS FRAME IMPACTED HEAD RESULTING IN CUT OVER EYE AND CORNEAL ABRASIONS.

AS EE WAS BREAKING A PIECE OF DIMENSION STONE A HIDDEN SEAM OPENED AND A PIECE OF STONE FELL ANDHIT HIS FOOT.

AS EMPLOYEE WAS BREAKING A PIECE OF STONE WITH PLUG WEDGES THE STONE OPENED UP AND FELL AGAINST EMPLOYEES SHIN CAUSING SCRAPES ALONG HIS RIGHT SHIN.

GROUT WEDGED IN DUMP BODY AS HE WAS DUMPING IN DIMENSION YARD. THIS STOOD TRUCK ON END, GROUT THEN DISLODGED SENDING TRUCK TO GROUND. EE WAS WEARING HIS SEAT BELT BUT STILL SUSTAINED FRACTURE TO SPINE WHEN VEHICLE CAME DOWN.

EE WAS INSTALLING GEAR BOX ON DRILL USING AN OVERHEAD CRANE TO LIFT GEAR BOX. AS GEAR BOX CONTACTED DRILL MAST, THE CHAIN WENT SLACK AND GEAR BOX TOPPLED OVER. AS IT FELL, IT PUT WEIGHT BACK O N CHAIN AND EE''S FINGERS WERE CAUGHT BETWEEN CHAIN AND GEAR BOX. PARTIAL AMPUTATION OF RING AND SMALL FINGER OF LEFT HAND.
